Abstract
본 발명은, 트라스투주맙 항체 또는 이의 항원 결합 단편; 안정화제; 및 계면활성제를 포함하나, 완충화제를 포함하지 않는 액체 제제 및 이를 제조하는 방법에 관한 것이다.
Description
본 발명은 트라스투주맙 항체 또는 이의 항원 결합 단편을 포함하는, 안정화 액체 제제에 관한 것으로서, 보다 구체적으로는 완충화제를 포함하지 않으면서 트라스투주맙 항체 또는 이의 항원 결합 단편을 안정화시킬 수 있는 액체 제제 및 그 제조방법에 관한 것이다.
환자 편의성을 증진시키기 위한 고농도 항체 의약품 개발 필요성이 부각되고 있다. 예를 들면, Herceptin® SC (120 mg/mL)이 시판된 상태이다. 이들 고농도 항체 의약품은 완충화제를 포함하고 있다. 예를 들어, Her2 양성 유방암 치료제로 잘 알려진 Herceptin® SC (트라스투주맙(trastuzumab)) 또한 완충화제를 포함하고 있다. 구체적으로, Herceptin® SC은 액상 제형 항체 약물로 120 mg/mL 트라스투주맙, 20 mM 히스티딘, 8% 트레할로스, 10 mM 메티오닌 및 0.04% 폴리소르베이트 20으로 구성되어 있고, 안정화 조건에서 pH 5.5를 갖는다.
그러나, 히스티딘과 같은 일부 완충화제는 스트레스 조건에서 자가-분해되기 쉬워서, 항체 의약품에 안정성에 영향을 미친다고 알려져 있다. 또한 인산의 경우, 동결 환경에서 pH를 유지하지 못하여 항체의 안정성에 영향을 미칠 가능성이 높다고 알려져 있다.
그러므로, 트라스투주맙 항체 의약품의 안정성에 영향을 미칠 수 있는 완충화제를 포함하지 않고도 다양한 스트레스 상황에서 항체 단백질을 안정화시킬 수 있는 제제의 개발이 여전히 요구된다.
본 발명의 일 양상은, 완충화제를 포함하지 않아 트라스투주맙 항체 의약품의 안정성을 보다 향상시킬 수 있으면서, 트라스투주맙 항체의 안정성을 유지하는데 바람직한 pH를 유지하는 트라스투주맙 항체를 포함하는 안정한 액체 제제를 제공하는 것이다.
본 발명의 다른 양상은 상기 액체 제제의 제조 방법을 제공하는 것이다.
본 발명의 일 양상은, a) 트라스투주맙 항체 또는 이의 항원 결합 단편; (b) 안정화제; 및 (c) 계면활성제를 포함하고, 완충화제를 포함하지 않는 안정한 액체 제제를 제공한다.
본 발명의 다른 일 양상은, 용매에 안정화제를 첨가하여 혼합 용액을 제조하는 단계로서, 상기 혼합 용액은 완충화제를 포함하지 않는 것인 단계; 상기 혼합 용액에 트라스투주맙 항체를 첨가하는 단계; 및 상기 트라스투주맙 항체가 첨가된 혼합 용액에 계면활성제를 첨가하는 단계를 포함하는 안정한 액체 제제를 제조하는 방법을 제공한다.
본 발명의 일 양상에 따른 액체 제제는, 스트레스 조건에서 자가-분해되기 쉬워 트라스투주맙 항체 의약품에 안정성에 영향을 미친다고 알려진 완충화제를 포함하지 않고도, 트라스투주맙 항체의 안정성을 유지하는데 바람직한 pH를 유지하고, 다양한 스트레스 조건에서 안정성을 확보할 수 있다. 또한, 상기 제제는 고농도의 트라스투주맙 항체를 포함하는 액체 제제이므로, 다양한 투여 형태로서 사용될 수 있다.

이하, 본 발명을 하기 실시예에 의하여 더욱 상세하게 설명한다. 단, 하기 실시예는 본 발명을 예시하기 위한 것일 뿐, 이에 의해 본 발명의 범위가 한정되는 것은 아니다.
제조예: 실시예 및 비교예의 제조
실시예 1 내지 5의 제제를 하기와 같이 제조하였다.
먼저, 표 1의 조성으로 트라스투주맙(5 mM 히스티딘 버퍼(pH 6.0), 40 mg/mL, Samsung Bioepis) 및 계면활성제를 제외한 완충화제 및 안정화제를 폴리카르보네이트 재질의 5L 바오테이너(biotainer) 용기 중의 멸균 증류수에 첨가하여 5L 완충 용액을 제조하였다. pH 6.0의 5 mM 히스티딘 버퍼 중 40 mg/mL 트라스투주맙 용액 3mL를 투석 카세트(MWCO 10 kDa)(Slide-A-Lyzer cassette, Thermo Fisher Scientific)에 넣은 다음, 이 트라스투주맙 용액이 포함된 카세트를 상기 완충 용액 1,600mL가 포함된 2L 비커에 넣어 투석이 이루어지게 하여, pH 6.0의 5 mM 히스티딘 버퍼를 상기 완충 용액으로 교환하였다. 그 후, 트라스투주맙을 120 mg/mL 이상으로 농축하였다. 농축은 투석된 트라스투주맙 함유 용액을 컷오프 분자량이 30 kDa인 Amicon Filter를 통과시켜 이루어졌다. 마지막으로, 상기 농축된 트라스투주맙 용액에 상기 제조된 완충 용액 중에 20x 농도 즉 0.8 w/v%로 제조된 계면활성제를 1x 농도가 되도록 첨가하였다. 비교예 1의 조성 또한 표 1에 기재하였다.

시험예 1: 완충화제 불포함 제형의 pH 안정성 확인
상기 제조된 실시예가 완충화제를 포함하지 않고도 다양한 스트레스 조건에서 바람직한 pH를 유지할 수 있는지 확인하였다. 구체적으로, 실시예 1 내지 5 및 비교예 1의 제제를 온도 스트레스, 동결-해동 스트레스, 및 교반 스트레스에 노출시킨 후, 액체 제제의 pH를 측정하였다. 비교예1은 헤셉틴 SC 제형(Herceptic SC formulation)으로서 20 mM His 포함하는 제제이다.
상기 온도 스트레스는, 실시예 1 내지 5 및 비교예 1의 제제 각각 1 mL를 폴리프로필렌 재질의 1.5 mL 마이크로튜브(microtube)(Axygen 사)에 넣고, 상기 튜브를 안정성 항온기(JEIO TECH 사)에서 40℃의 온도 스트레스 조건에 4주간 노출시켰다. 구체적으로, 상기 튜브를 상기 안정성 항온기 내에 넣고 온도 40±2℃ 및 상대 습도 75±5%의 조건에서 4 주간 두었다. 4주간 보관된 제제에 대하여 pH를 측정하였다.
또한, 동결-해동 스트레스는, 실시예 1 내지 4 및 비교예 1의 제제 각각 1 mL를 폴리프로필렌 재질의 1.5 mL 마이크로튜브(microtube)(Axygen 사)에 넣고, 상기 튜브를 -70±10℃로 유지되는 딥프리져(deep freezer)(Thermo scientific 사)에 넣고 18시간 유지하여 동결 및 동결 상태로 유지한 후, 상기 튜브를 꺼내 상온에서 1시간 방치하여 해동 및 해동 상태로 유지하였다. 이 과정을 5회 반복하였다. 최종적으로 해동된 제제에 대하여 pH를 측정하였다.

시험예 2: 동결-해동 스트레스에서 완충화제 불포함 제제의 안정성 확인
상기 제조된 실시예가 동결-해동 스트레스 조건에서 안정성을 유지할 수 있는지 크기 배제-고능성 액체 크로마토그래피(Size-Exclusion HPLC, SE-HPLC)를 이용하여 확인하였다.
동결-해동 스트레스 조건은 상기 시험예 1에 기재된 바와 같았다. 표 3은 그 결과를 나타낸 것이다. 제제 당 3 반복으로 제조된 제제를 각각 분석하여 얻어진 데이터를 구하였다. 제제에서 항체가 안정성을 상실하는 경우, 응집(aggregation)이 유발되어 고분자량(high molecular weight; HMW) 물질의 비율이 증가하거나 항체의 분해 또는 이황화 결합 불안정화 등에 의해서 단량체 비율 또는 저분자량(low molecular weight; LMW) 물질의 비율이 감소하는 것으로 관찰될 것이다.
그 결과, 실시예 1 내지 5 모두, 동결-해동 스트레스 조건에서 비교예 1과 유사한 안정성을 가짐을 확인하였다.

- 청구항 18에 있어서, 상기 안정화제는 10 내지 200 mM의 아르기닌 및 1 내지 20 mM의 메티오닌인 것인 액체 제제.
- 청구항 18에 있어서, 상기 pH는 5.5±0.2인 것인 액체 제제.
- 용매에 안정화제를 첨가하여 혼합 용액을 제조하는 단계로서, 상기 혼합 용액은 완충화제를 포함하지 않는 것인 단계;상기 혼합 용액에 트라스투주맙을 첨가하는 단계; 및상기 트라스투주맙이 첨가된 혼합 용액에 계면활성제를 첨가하는 단계를 포함하는 안정한 액체 제제를 제조하는 방법.
