Abstract
本発明は、ヒトPTH(1-34)(テリパラチド)を有効成分とし保存安定性に優れた液体医薬組成物を含む医薬製剤及びその製造方法を提供する。 本発明は、容器と、前記容器中に充填された、配列番号1に示すアミノ酸配列からなるヒトPTH(1-34)、糖及びアミノ酸を水中に含む液体医薬組成物とを含む、医薬製剤に関する。本発明はまた、前記医薬製剤の製造方法であって、前記糖及び前記アミノ酸を水中に含む水溶液中に、前記ヒトPTH(1-34)を加えることを含む、液体医薬組成物を調製する調製工程と、前記液体医薬組成物を容器に充填する充填工程とを含む方法に関する。
Description
本発明は、容器に充填されたヒトPTH(1-34)(テリパラチド)を含有する液体医薬組成物を含む医薬製剤に関するものである。
ヒトPTH(1-34)(テリパラチド)はヒトパラチロイドホルモン(ヒトPTH)のN末端の1-34残基からなるポリペプチド(アミノ酸配列を配列番号1に示す)であり、骨粗鬆症の治療薬として知られている。テリパラチド酢酸塩を有効成分とする注射剤として旭化成ファーマ株式会社から「テリボン皮下注用56.5μg」として販売されている。「テリボン皮下注用56.5μg」は凍結乾燥製剤であり、使用に際して用事溶解を必要とするため、作業が煩雑であり、医療従事者の採取容量ミスの可能性もある。そこで、医療現場ではテリパラチド製剤の液剤化が望まれている。
特許文献1では、ヒトPTH(1-34)、医薬として用いるためのポリオール安定化剤、および、組成物のpHを3より大きく7までの範囲内に維持するための緩衝剤を含む、ヒト患者に非経口投与可能な状態の保存安定性のある無菌溶液の形の医薬組成物が開示されている。そして、ポリオール安定化剤としてマンニトールが開示されている。
特許文献2では、露光下でのヒトPTH(1-34)水溶液注射剤の保存方法であって、該方法は、該水溶液注射剤のpHを3~5とし、且つ安定化剤としてメチオニンをヒトPTH(1-34)の1重量部に対して40~4,000重量部の割合で含有させることを特徴とし、但し該水溶液注射剤が60万Lux・hrの条件で露光された際の当該注射剤中のヒトPTH(1-34)の残存率が87%以上である、前記保存方法が開示されている。
特許文献1では、前記無菌溶液の形の医薬組成物中のヒトPTH(1-34)濃度として100μg/mL~500μg/mLが記載されているが、この濃度は、既存製品中のテリパラチド濃度(56.5μg/mL)よりも高いため、特許文献1に記載の前記無菌溶液の形の医薬組成物は注射剤としての実用性に乏しいものであった。
特許文献2の実施例1、2では、1mL中に約30μgのヒトPTH(1-34)と、約20mgのDL-メチオニン、L-メチオニンを含有する水溶液注射剤が開示されており、この水溶液注射剤は40℃での露光下での保存安定性が高いことが開示されている。一方で、メチオニンのヒトへの一日の投与の許容量は皮下注射の場合は0.2mg/kg体重、静脈内注射の場合は0.1mg/kg体重とされており(医薬品添加物事典2016(薬事日報社刊))、体重60kgのヒトに対する一日の投与の許容量は、皮下注射の場合は12mg、静脈内注射の場合は6mg、である。このため、特許文献2の実施例1、2に記載の水溶液注射剤の1mLを体重60kgのヒトに皮下注射又は静脈内注射で投与する場合、メチオニンを約20mg投与することになり、皮下注射又は静脈内注射の1日の許容量を超えてしまうという問題がある。
本発明者は、ヒトPTH(1-34)(テリパラチド)を有効成分とし保存安定性に優れた液体医薬組成物を含む医薬製剤及びその製造方法を提供することを目的として鋭意検討した結果、以下の発明を完成するに至った。
(1)容器と、前記容器中に充填された、配列番号1に示すアミノ酸配列からなるヒトPTH(1-34)、糖及びアミノ酸を水中に含む液体医薬組成物とを含む、医薬製剤。
(2)前記糖がマンニトール、ソルビトール、及び、精製白糖から選択される1種以上である、(1)に記載の医薬製剤。
(3)前記アミノ酸がメチオニンである、(1)又は(2)に記載の医薬製剤。
(4)前記液体医薬組成物が前記メチオニンを1~10mg/mLの濃度で含む、(3)に記載の医薬製剤。
(5)前記水がpH5.0以下の緩衝水溶液である、(1)~(4)のいずれかに記載の医薬製剤。
(6)容器と、前記容器中に充填された、配列番号1に示すアミノ酸配列からなるヒトPTH(1-34)、糖及びアミノ酸を水中に含む液体医薬組成物とを含む、医薬製剤を製造する方法であって、
前記糖及び前記アミノ酸を水中に含む水溶液中に、前記ヒトPTH(1-34)を加えることを含む、液体医薬組成物を調製する調製工程と、

本発明の医薬製剤は、ヒトPTH(1-34)(テリパラチド)を有効成分する液体医薬組成物を安定に保存することができる。
本発明の医薬製剤の製造方法によれば、製造中及び製造された製品の保存中のヒトPTH(1-34)(テリパラチド)の劣化を抑制することができる。
<有効成分>
ヒトPTH(1-34)は、配列番号1に示すアミノ酸配列からなるポリペプチドである。配列番号1に示すアミノ酸配列からなるポリペプチドは、医薬として許容される塩の形態であるものも包含する。医薬として許容される塩としては酢酸塩が好ましい。酢酸塩の形態のヒトPTH(1-34)としては、CAS番号99294-7のテリパラチド酢酸塩(分子式C181H291N55O51S2・5CH3COOH、分子量4417.97)が知られている。
ヒトPTH(1-34)は、配列番号1に示すアミノ酸配列からなるポリペプチドである。配列番号1に示すアミノ酸配列からなるポリペプチドは、医薬として許容される塩の形態であるものも包含する。医薬として許容される塩としては酢酸塩が好ましい。酢酸塩の形態のヒトPTH(1-34)としては、CAS番号99294-7のテリパラチド酢酸塩(分子式C181H291N55O51S2・5CH3COOH、分子量4417.97)が知られている。
<医薬製剤の好ましい実施形態>
本発明の医薬製剤は、保存時に、ヒトPTH(1-34)の類縁物質への変性が抑制されており保存安定性に優れている。

本発明の医薬製剤に使用することができる容器としては、液体医薬組成物を収容することができるものであればよいが、例えば、バイアル、アンプル、シリンジ等の各種形態の容器が使用できる。前記容器の材質は特に限定されないが、例えば、ガラス、ポリエチレン、ポリプロピレン、シリコン、ステンレス鋼等が挙げられる。前記容器は、より好ましくは、ガラス、ポリエチレン、ポリプロピレン又はシリコンを、少なくとも液体医薬組成物と接触する表面に含む容器が好ましい。前記容器は遮光された容器であることが好ましい。前記容器がガラス容器である場合、少なくとも液体医薬組成物と接触する表面が、SiO2被膜で被覆(シリコート加工)された表面であることで、保存中のヒトPTH(1-34)の類縁物質への変性が更に効果的に抑制されるため好ましい。
本発明の医薬製剤では、前記容器中に、配列番号1に示すアミノ酸配列からなるヒトPTH(1-34)、糖及びアミノ酸を水中に含む液体医薬組成物が充填され封入されたものである。容器は密封されており、容器内の気相部分の酸素濃度は、好ましくは5%以下、より好ましくは4%以下、より好ましくは2%以下、さらに好ましくは1%以下であり、特に好ましくは0.5%以下である。このような低い酸素濃度は、容器中に前記液体医薬組成物を充填し、気相部分を窒素、アルゴン等の不活性ガスにより置換した後、前記容器を密封することで達成できる。
以下に、前記液体医薬組成物の好ましい実施形態について具体的に説明する。
前記液体医薬組成物中のヒトPTH(1-34)の濃度は特に限定されないが、希釈を必要とせず注射剤として直接ヒトに投与できる濃度であることが好ましく、フリー体のヒトPTH(1-34)(テリパラチド)に換算した濃度として、例えば1~500μg/mLであり、好ましくは1~150μg/mLであり、例えば56.5μg/mLである。
糖としては、医薬として許容される糖であれば特に限定されず、単糖、二糖、又は三糖以上の多糖であることができる。糖は1種の糖からなっていてもよいし、2種以上の糖の混合物であってもよい。糖は、前記液体医薬組成物の調節する等張化剤としての機能に加えて、ヒトPTH(1-34)の類縁物質への変性を抑制する機能を有する。糖の具体例としては、マンニトール、ソルビトール、精製白糖、マルトース、グルコース等が例示でき、好ましくは、マンニトール、ソルビトール及び精製白糖から選択される1種以上であり、最も好ましくはマンニトールである。マンニトール、ソルビトール及び精製白糖から選択される1種以上の糖は、ヒトPTH(1-34)の類縁物質への変性を抑制する機能が特に高いため好ましい。糖は、D体、L体、D体とL体との混合物のいずれであってもよい。前記液体医薬組成物中での糖の濃度は特に限定されないが、希釈を必要とせず注射剤として直接ヒトに投与できる濃度であることが好ましく、例えば0.5~500mg/mL、好ましくは5~100mg/mLである。糖がマンニトールである場合には、その濃度として52.1mg/mLが例示できる。
アミノ酸は、医薬として許容されるアミノ酸であれば特に限定されず、例えばメチオニン、リシン、アルギニン、ヒスチジン、トリプトファン、オルニチン等が挙げられ、特に好ましくはメチオニンである。アミノ酸は1種のアミノ酸からなっていてもよいし、2種以上のアミノ酸の混合物であってもよい。アミノ酸は、ヒトPTH(1-34)の類縁物質への変性を抑制する機能を有する。特に、アミノ酸を糖と共存させることで、両物質が単独でも有しているヒトPTH(1-34)の類縁物質への変性を抑制する機能が相乗的に発揮されるため、アミノ酸を単独でヒトPTH(1-34)の保存剤として用いる場合と比較して、アミノ酸の使用量を大幅に低減することができる。この作用は、アミノ酸がメチオニンである場合に特に顕著である。アミノ酸は、L体、D体、L体とD体との混合物のいずれであってもよい。前記液体医薬組成物中でのアミノ酸の濃度は特に限定されないが、希釈を必要とせず注射剤として直接ヒトに投与できる濃度であることが好ましい。アミノ酸がメチオニンである実施形態では、前記液体医薬組成物中でのメチオニンの濃度は、好ましくは1~10mg/mLであり、より好ましくは、1~5mg/mLであり、例えば2mg/mLである。このようなメチオニン濃度がこのような低濃度の液体医薬組成物は、有効量のヒトPTH(1-34)をヒトに投与した場合でも、メチオニンの投与量としてはヒトへの1日の許容量(皮下注射の場合は0.2mg/kg体重、静脈内注射の場合は0.1mg/kg体重)を下回ることが容易であるため好ましい。
前記液体医薬組成物中の水は医薬として許容される水であればよい。前記液体医薬組成物中の水は、医薬として許容される、pH5.0以下の緩衝水溶液の形態であってもよい。pH5.0以下の緩衝水溶液としては酢酸緩衝液、リン酸緩衝液、クエン酸緩衝液等が例示でき、好ましくは、酢酸緩衝液又はリン酸緩衝液である。酢酸緩衝液、リン酸緩衝液又はクエン酸緩衝液の濃度としては6.0~21.0mMが例示できる。pH5.0以下の緩衝水溶液中では、ヒトPTH(1-34)が安定に保持され易い。pH5.0以下の緩衝水溶液は、より好ましくはpH3.0~5.0、より好ましくはpH3.5~5.0、特に好ましくはpH4.0~4.5の緩衝水溶液である。
前記液体医薬組成物の、特に好ましい実施形態としては、pH5.0以下の酢酸緩衝液中に、ヒトPTH(1-34)、D-マンニトール及びL-メチオニンを含有する液体医薬組成物である。前記特に好ましい実施形態において、ヒトPTH(1-34)はテリパラチド酢酸塩であることが更に好ましい。前記特に好ましい実施形態において、ヒトPTH(1-34)の濃度は、フリー体換算で、56.5μg/mLであることが更に好ましい。前記特に好ましい実施形態において、D-マンニトールの濃度は52.1mg/mLであることが更に好ましい。前記特に好ましい実施形態において、L-メチオニンの濃度は2mg/mLであることが更に好ましい。
本発明の医薬製剤中では、前記液体医薬組成物は保存安定性に優れており、例えば、25℃で1ヵ月保存したときのヒトPTH(1-34)に由来する類縁物質の総量が、原料として用いたヒトPTH(1-34)に対して5%以下であることができる。
本発明の医薬製剤中では、前記液体医薬組成物は保存安定性に優れており、例えば、25℃で1ヵ月保存したときのヒトPTH(1-34)に由来する類縁物質のうち実施例で定義する相対保持時間0.76の類縁物質の量が、原料として用いたヒトPTH(1-34)に対して0.5%以下であることができる。

ここで、ヒトPTH(1-34)、糖、アミノ酸、水、容器、医薬製剤及び液体医薬組成物の好ましい実施形態については医薬製剤に関して上記の通りである。前記水溶液中での糖又はアミノ酸の好ましい濃度は、液体医薬組成物における糖又はアミノ酸の好ましい濃度と同様である。
前記調製工程では、糖及びアミノ酸を水中に含む水溶液を予め用意し、その中にヒトPTH(1-34)を加えることにより、ヒトPTH(1-34)の劣化を最小限に抑制することができる。
前記調製工程は、オゾンを殆ど含まない窒素雰囲気下で行う必要はないが、オゾン濃度の低い雰囲気下で行うことが、前記調製工程中でのヒトPTH(1-34)の劣化が抑制されるため好ましい。
前記調製工程及び前記充填工程では、前記水溶液又は調製された前記液体医薬組成物は、不活性ガスでバブリングしながら取り扱うことが好ましい。
前記充填工程において、前記液体医薬組成物を容器に充填する際は、容器内の気相部分を不活性ガスで置換して封入することが好ましい。
<保存試験の手順>
1mLあたりテリパラチド酢酸塩60.6μg(テリパラチドとして56.5μg)と、各実験で説明する他の成分とを調合して調製した水溶液製剤1.2mLを、バイアル中に充填し、気相を不活性ガスで置換し、接液部にテフロン(登録商標)ラミネートを有するゴム栓で封入したものを、保存試験用試料とした。
1mLあたりテリパラチド酢酸塩60.6μg(テリパラチドとして56.5μg)と、各実験で説明する他の成分とを調合して調製した水溶液製剤1.2mLを、バイアル中に充填し、気相を不活性ガスで置換し、接液部にテフロン(登録商標)ラミネートを有するゴム栓で封入したものを、保存試験用試料とした。
前記水溶液製剤を調製する際の原料の調合とバイアルへの充填は、特に明示しない限り、室温(22℃)にて空気雰囲気下で行い、原料の調合から充填の完了までを約5時間かけて行った。原料の調合の際は、先に、テリパラチド酢酸塩以外の成分を調合し、その後に、テリパラチド酢酸塩を調合した。原料の調合は、水又は水溶液中を窒素でバブリングしながら非遮光条件下で行った。バブリング及びバイアル内の気相を置換するための不活性ガスとしては、特に明示しない限り、窒素を用いた。
保存試験用試料を、各実験で説明する温度、時間条件で保存した。保存試験を行った安定性試験機内は遮光されている。
各条件で保存した保存試験用試料中のテリパラチドの類縁物質の含有量を、後述する手順で測定した。
原料として投入したテリパラチド酢酸塩のテリパラチド換算重量(56.5μg)に対する、各条件で保存後に測定された保存試験用試料中の類縁物質の総重量の割合を、百分率で表したものを、総類縁含有率(%)とした。
封入する直前の前記水溶液製剤についても、同様の手順で総類縁含有率(%)を求めた。
封入する直前の前記水溶液製剤中の総類縁含有率(%)をAIN(%)とし、各条件で保存後の保存試験用試料中の総類縁含有率(%)をA保存条件(%)とした。
AIN(%)は、前記水溶液製剤の調合と充填の段階で生じた類縁物質の割合を示す。
AIN(%)からA保存条件(%)への変化幅は、各条件での保存中の類縁物質の増加幅、すなわち劣化度合いの指標となる。
更に、各条件での保存後のテリパラチド残存率(%)を次式により算出した。

<類縁物質の総量の測定>
類縁物質の測定は以下の条件で行った。
試料溶液:各条件で保存した保存試験用試料のテリパラチド約56.5μgに対応する量(本品1mLに相当)をとり、溶解液(塩化ベンザルコニウム3gを硫酸緩衝溶液に溶かし、全量を2000mLとした液)80μL及び液体クロマトグラフィー用アセトニトリル120μLを加えて試料溶液とする。
検出器:紫外吸光光度計(測定波長:214nm)
カラム:内径4.6mm、長さ15cmのステンレス管に3.5μmの液体クロマトグラフィー用オクタデシルシリル化シリカゲルを充填したもの。
カラム温度:40℃付近の一定温度
移動相A:0.05 mol/L 硫酸塩緩衝液/液体クロマトグラフィー用アセトニトリル混液(9:1)

内面をSiO2被膜で被覆(シリコート加工)したホウ珪酸ガラス製バイアル(CS-5シリコート加工、CS-2シリコート加工)を用いることで、保存期間中のバイアル内でのテリパラチドの安定性が高まり類縁物質の生成が抑制されることが確認された。また、2mLのバイアルを用いるほうが、5mLのバイアルを用いるよりもテリパラチドの安定性が高まり類縁物質の生成が抑制される傾向が認められた。
以下の実験では、特に明示しない限り、バイアルとして、5mL容量の内面をシリコート加工したホウ珪酸ガラス製バイアル(CS-5シリコート加工)を用いた。
<実験2>
水溶液製剤として、水中に、60.6μg/mLのテリパラチド酢酸塩(テリパラチドとして56.5μg/mL)を含み、氷酢酸を添加してpH4.5に調整した水溶液製剤を用いた。

以下の実験では、特に明示しない限り、ゴム栓として塩素化ブチルゴムを材料とするゴム栓を用いた。
<実験3>
水溶液製剤として、水中に、60.6μg/mLのテリパラチド酢酸塩(テリパラチドとして56.5μg/mL)、及び、0.17mMの氷酢酸を含み、適量の塩酸又は水酸化ナトリウムを添加してpHを3.5、4.0、4.5、5.0又は5.5に調整した水溶液製剤を用いた。
水溶液製剤として、水中に、60.6μg/mLのテリパラチド酢酸塩(テリパラチドとして56.5μg/mL)、及び、0.17mMの氷酢酸を含み、適量の塩酸又は水酸化ナトリウムを添加してpHを3.5、4.0、4.5、5.0又は5.5に調整した水溶液製剤を用いた。
保存試験用試料の保存条件は、5℃で3カ月間、10℃で2週間、1カ月間及び3カ月間、25℃で2週間及び1カ月間、40℃で1週間及び2週間とした。

pHが4.0又は4.5である場合に保存期間中のテリパラチドの総類縁含有率の上昇及びテリパラチド残存率の低下が抑制され、テリパラチドが安定化されることが確認された。特にpH4.5の場合にテリパラチドが安定化される効果が高い。
水溶液製剤のバイアルへの充填までのテリパラチドの劣化を反映する開始時の総類縁含有率(図3のIN)に関してはpH間で差異はない。

「酢酸緩衝液(6.8mM)」試験区は、6.8mMの酢酸緩衝水溶液中に、60.6μg/mLのテリパラチド酢酸塩(テリパラチドとして56.5μg/mL)を含み、適量の塩酸又は水酸化ナトリウムを添加してpHを4.5に調整した水溶液製剤を用いた試験区である。
「氷酢酸+NaOH」試験区は、水中に、60.6μg/mLのテリパラチド酢酸塩(テリパラチドとして56.5μg/mL)、6.8mMの氷酢酸を含み、適量の塩酸又は水酸化ナトリウムを添加してpHを4.5に調整した水溶液製剤を用いた試験区である。
「酢酸緩衝液(20.4mM)」試験区は、20.4mMの酢酸緩衝水溶液中に、60.6μg/mLのテリパラチド酢酸塩(テリパラチドとして56.5μg/mL)を含み、適量の塩酸又は水酸化ナトリウムを添加してpHを4.5に調整した水溶液製剤を用いた試験区である。
「クエン酸緩衝液(6.8mM)」試験区は、6.8mMのクエン酸緩衝水溶液中に、60.6μg/mLのテリパラチド酢酸塩(テリパラチドとして56.5μg/mL)を含み、適量の塩酸又は水酸化ナトリウムを添加してpHを4.5に調整した水溶液製剤を用いた試験区である。
「リン酸緩衝液(6.8mM)」試験区は、6.8mMのリン酸緩衝水溶液中に、60.6μg/mLのテリパラチド酢酸塩(テリパラチドとして56.5μg/mL)を含み、適量の塩酸又は水酸化ナトリウムを添加してpHを4.5に調整した水溶液製剤を用いた試験区である。
保存試験用試料の保存条件は、5℃で3カ月間、10℃で2週間、1カ月間及び3カ月間、25℃で2週間及び1カ月間、40℃で1週間及び2週間とした。

「精製白糖」試験区は、6.8mMの酢酸緩衝水溶液中に、60.6μg/mLのテリパラチド酢酸塩(テリパラチドとして56.5μg/mL)と、97.9mg/mLの精製白糖を含み、適量の塩酸又は水酸化ナトリウムを添加してpHを4.5に調整した水溶液製剤を用いた試験区である。
「D-ソルビトール」試験区は、6.8mMの酢酸緩衝水溶液中に、60.6μg/mLのテリパラチド酢酸塩(テリパラチドとして56.5μg/mL)と、52.1mg/mLのD-ソルビトールを含み、適量の塩酸又は水酸化ナトリウムを添加してpHを4.5に調整した水溶液製剤を用いた試験区である。
「マルトース一水和物」試験区は、6.8mMの酢酸緩衝水溶液中に、60.6μg/mLのテリパラチド酢酸塩(テリパラチドとして56.5μg/mL)と、51.0mg/mLのマルトース一水和物を含み、適量の塩酸又は水酸化ナトリウムを添加してpHを4.5に調整した水溶液製剤を用いた試験区である。
「D-グルコース」試験区は、6.8mMの酢酸緩衝水溶液中に、60.6μg/mLのテリパラチド酢酸塩(テリパラチドとして56.5μg/mL)と、51.0mg/mLのD-グルコースを含み、適量の塩酸又は水酸化ナトリウムを添加してpHを4.5に調整した水溶液製剤を用いた試験区である。
保存試験用試料の保存条件は、5℃で3カ月間、10℃で2週間、1カ月間及び3カ月間、25℃で2週間及び1カ月間、40℃で1週間及び2週間とした。

等張化剤としてD-マンニトール、精製白糖又はD-ソルビトールを用いた試験区では、40℃で保存期間中の総類縁含有率の上昇及びテリパラチド残存率の低下が、等張化剤としてマルトース一水和物又はD-グルコースを用いた試験区と比較して小さく、保存安定性が高いことが確認された。
水溶液製剤のバイアルへの充填までのテリパラチドの劣化を反映する開始時の総類縁含有率(図5のIN)に関しても試験区間で差異は無かった。

「L-メチオニン(2mg/mL)」試験区は、6.8mMの酢酸緩衝水溶液中に、60.6μg/mLのテリパラチド酢酸塩(テリパラチドとして56.5μg/mL)と、52.1mg/mLのD-マンニトールと、2mg/mLのL-メチオニンとを含み、適量の塩酸又は水酸化ナトリウムを添加してpHを4.5に調整した水溶液製剤を用いた試験区である。
「L-メチオニン(10mg/mL)」試験区は、6.8mMの酢酸緩衝水溶液中に、60.6μg/mLのテリパラチド酢酸塩(テリパラチドとして56.5μg/mL)と、52.1mg/mLのD-マンニトールと、10mg/mLのL-メチオニンとを含み、適量の塩酸又は水酸化ナトリウムを添加してpHを4.5に調整した水溶液製剤を用いた試験区である。
保存試験用試料の保存条件は、5℃で3カ月間、10℃で2週間、1カ月間及び3カ月間、25℃で2週間及び1カ月間、40℃で1週間及び2週間とした。

D-マンニトールとL-メチオニンとを併用する場合、L-メチオニンの濃度が1mg/mL、2mg/mL、10mg/mLのいずれでも、テリパラチドの劣化の抑制及び類縁物質の生成の抑制に関して同等の効果が認められた。
<実験7>
水溶液製剤の調製時の雰囲気中のオゾン濃度とメチオニンの添加の有無の、テリパラチドの類縁物質の生成への影響を調べるために以下の試験を行った。

「0.01ppm(メチオニン有)」試験区及び「0.01ppm(メチオニン無)」試験区は、水溶液製剤の原料の調合から充填までの約5時間の工程を、オゾン濃度が0.01ppmの空気雰囲気下で行った試験区である。
「0.1~0.5ppm(メチオニン有)」試験区及び「0.1~0.5ppm(メチオニン無)」試験区は、水溶液製剤の原料の調合から充填までの約5時間の工程を、オゾン濃度が0.1~0.5ppmの空気雰囲気下で行った試験区である。
「0ppm(メチオニン有)」試験区、「0.01ppm(メチオニン有)」試験区及び「0.1~0.5ppm(メチオニン有)」試験区は、いずれも、水溶液製剤として、6.8mMの酢酸緩衝水溶液中に、60.6μg/mLのテリパラチド酢酸塩(テリパラチドとして56.5μg/mL)と、52.1mg/mLのD-マンニトールと、2mg/mLのL-メチオニンとを含み、適量の塩酸又は水酸化ナトリウムを添加してpHを4.5に調整した水溶液製剤を用いた。
「0ppm(メチオニン無)」試験区、「0.01ppm(メチオニン無)」試験区及び「0.1~0.5ppm(メチオニン無)」試験区は、いずれも、水溶液製剤として、6.8mMの酢酸緩衝水溶液中に、60.6μg/mLのテリパラチド酢酸塩(テリパラチドとして56.5μg/mL)と、52.1mg/mLのD-マンニトールとを含み、適量の塩酸又は水酸化ナトリウムを添加してpHを4.5に調整した水溶液製剤を用いた。

テリパラチド水溶液製剤を調製するための、原料の調合から充填までの約5時間の工程を、同じオゾン濃度の雰囲気中で行う条件下では、保存試験開始時の総類縁含有率(図7、表9のIN)が、メチオニンを配合しない場合(メチオニン無)と比較して、メチオニンを配合する場合(メチオニン有)に顕著に低いことが確認された。
保存試験開始時の総類縁含有率は、水溶液製剤のバイアルへの充填までのテリパラチドの劣化を反映する。水溶液製剤中のメチオニンにより、水溶液製剤の調製時の雰囲気中のオゾンによるテリパラチドの劣化が抑制されて類縁物質の生成が抑制されることが示された。
保存期間中の各試験区の水溶液製剤における、テリパラチドの類縁物質のうち、HPLCでの相対保持時間が0.76の類縁物質(RRT0.76)、及び、HPLCでの相対保持時間が0.98の類縁物質(RRT0.98)の、原料テリパラチドに対する含有率を、それぞれ、図8、図9に示す。
図8から、テリパラチド水溶液製剤の調製を同じオゾン濃度の雰囲気中で行う条件下では、保存試験開始時の類縁物質RRT0.76の含有率(図8のIN)が、メチオニンを配合しない場合(メチオニン無)と比較して、メチオニンを配合する場合(メチオニン有)に顕著に低いことが確認された。類縁物質RRT0.76は、テリパラチドのメチオニン残基が変性した類縁物質であると推定されており、メチオニンの添加は、テリパラチドのメチオニン残基のオゾンによる変性を抑制することが推定される。
<実験8>
水溶液製剤として、6.8mMの酢酸緩衝水溶液中に、60.6μg/mLのテリパラチド酢酸塩(テリパラチドとして56.5μg/mL)と、52.1mg/mLのD-マンニトールと、2mg/mLのL-メチオニンとを含み、適量の塩酸又は水酸化ナトリウムを添加してpHを4.5に調整した水溶液製剤を用いた。

- 容器と、前記容器中に充填された、配列番号1に示すアミノ酸配列からなるヒトPTH(1-34)、糖及びアミノ酸を水中に含む液体医薬組成物とを含む、医薬製剤。
- 前記糖がマンニトール、ソルビトール、及び、精製白糖から選択される1種以上である、請求項1に記載の医薬製剤。
- 前記アミノ酸がメチオニンである、請求項1又は2に記載の医薬製剤。
- 前記液体医薬組成物が前記メチオニンを1~10mg/mLの濃度で含む、請求項3に記載の医薬製剤。
- 前記水がpH5.0以下の緩衝水溶液である、請求項1~4のいずれか1項に記載の医薬製剤。
