Abstract
Bei dieser Vorrichtung und dem zugehörigen Verfahren ist eine Gehäuseeinheit mittels einer mit dieser verbundenen steuerbaren Befestigungseinheit derart ausrichtbar, dass die in der Gehäuseeinheit angeordnete mit der Röntgenquelle verbundene Blendeneinheit entsprechend einer Positioniervorgabe ausrichtbar ist.

Beschreibung
Vorrichtung und Verfahren für ein Diagnosegerät Zur Diagnose und Behandlung werden medizinische Geräte bei¬ spielsweise mit Ausprägungen für Radiographie, Fluoroskopie und Angiographie verwendet. Um die Röntgenbelastung für den Patienten jeweils so gering wie möglich zu halten, wird ein Röntgenfenster jeweils auf den zu untersuchenden Bereich der auch als Region of Interest bezeichnet werden kann festge¬ legt. Die Begrenzung des Röntgenfensters kann mittels einer mit Blendenlamellen ausgebildeten Blendeneinheit erfolgen. Die Positionen der Blendenlamellen der Blendeneinheit und insbesondere eine Drehung der gesamten Blendeneinheit kann manuell oder mittels ansteuerbarer Elektromotoren die an den Lamellen der Blenden und/oder direkt an der Blendeneinheit angeordnet sind durchgeführt. Diese Ausstattung, insbesondere die Drehung der gesamten Blendeneinheit am Ausgang des Rönt- genstrahlenkopfes bringt jedoch den Nachteil mit sich, dass das den Röntgenstrahlerkopf und die gesamte Blendeneinheit umschließende Gehäuse für die zu bedienende Röntgenquelle ein großes Volumen einnimmt. Ein zusätzlicher Nachteil liegt im eingeschränkten Händling des großen Gehäuses. Neben dem Nachteil einer eingeschränkten Positionierungsmöglichkeit des Ge- häuses der Röntgenquelle mit bewegbarer Blendeneinheit bringt den weiteren Nachteil mit sich, dass die Mechanik für die Aufhängung und Steuerung der Röntgenquelle grösser dimensioniert werden muss. Der Erfindung liegt die Aufgabe zugrunde, eine weitere Ausge¬ staltung eines mit Röntgenquelle und Blendeneinheit versehe¬ nen Gehäuses einer Röntgenquelle anzugeben.
Die Aufgabe wird durch die im Patentanspruch 1 oder 8 angege- benen Merkmale gelöst.
Diese Vorrichtung und das dazugehörige Verfahren weist eine an einer positionierbaren Befestigungseinheit angeordnete
Gehäuseeinheit mit einer Röntgenquelle und einer der Röntgen¬ quelle nachgeordneten Blendeneinheit auf, wobei eine Steuer¬ einheit vorgesehen ist und zur Ausrichtung der Blendeneinheit die positionierbare Befestigungseinheit derart von dieser an¬ gesteuert wird, dass die Blendeneinheit entsprechend einer Vorgabe ausrichtbar ist.
Der Gegenstand der Erfindung bringt neben dem Vorteil, dass die Blendeneinheit sowie das die Röntgenquelle aufnehmende Gehäuse klein, dass heißt raumsparend dimensioniert werden kann, den weiteren Vorteil mit sich, dass zusätzlich zur größeren Bewegungsfreiheit des Gehäuses mit Röntgenquelle und Blendeneinheit eine einfachere Aussichtung auf einen dazu korrespondierenden Röntgendetektor ermöglicht wird.
Der Gegenstand der Erfindung bringt den Vorteil mit sich, dass aufgrund der Gewichtsreduktion bei der Ausgestaltung der Blendeneinheit die Mechanik für die Bewegung des Gehäuses mit Röntgenquelle und Blendeneinheit feingliedriger dimensionier¬ bar ist.
Der Gegenstand der Erfindung bringt den Vorteil mit sich, dass das Gehäuse der Röntgenquelle kompakt gestaltbar ist. Nachfolgend wird der Gegenstand der Erfindung anhand von schematischen Zeichnungen erläutert. Es zeigen:

Bei dieser Vorrichtung und dem zugehörigen Verfahren ist eine Gehäuseeinheit mittels einer mit dieser verbundenen steuerba-
ren Befestigungseinheit derart ausrichtbar, dass die in der Gehäuseeinheit angeordnete mit der Röntgenquelle verbundenen Blendeneinheit entsprechend einer Positioniervorgabe ausge¬ richtbar ist.
Figur 1 zeigt schematisch einen auf einer Liege L plazierten Patienten 0 bei dem beispielsweise im Bereich des Oberarms eine Röntgenaufnahme anzulegen ist. Dieser Bereich im Oberarm des Patienten ist mit einer Region of Interest ROI markiert.
In Figur 2 ist der auf der Liege L plazierte Patient 0 mit dem markierten Region of Interest ROI und einem möglichen Röntgenfeld das auch als Einblendungsfeld EBF bezeichnet wer¬ den kann abgebildet. Zu sehen ist, dass eine weitaus größere Fläche im Bereich des Oberarms des Patienten 0 mit Röntgen¬ strahlen durchleuchtet werden würde.
In Figur 3 ist das Röntgen- bzw. Einblendungsfeld EBF ent¬ sprechend einem zu untersuchendem Bereich, dass auch als Re- gion of Interest ROI bezeichnet, angepasst werden kann. In dieser Figur ist ebenfalls der Ablauf zur Erfassung des Regi¬ on of Interest ROI mittels einer gezielten Blendeneinstellung angedeutet. Das Röntgenfeld EBF wird entsprechend der Größe des Region of Interest ROI durch entsprechende Blendenlamel- len verringert. Die Blendenlamellen sind jeweils in der Blendeneinheit BE angeordnet. Eine Positionierung der Blendenla¬ mellen, beispielsweise in seiner x- und/oder y-Richtung, kann manuell oder durch kleinste platzsparende Elektromotoren er¬ folgen. Das auf die Breite des Region of Interest ROI be- grenzte Einblendfeld EBF muss in einem weiteren Schritt ent¬ sprechend der Orientierung des Region of Interest ROI ausge¬ richtet werden. Entweder wird das begrenzende Einblendfeld EBF beispielsweise um eine Kante von diesem entgegen dem Uhrzeigersinn manuell, semimanuell oder elektronisch gedreht bis eine Übereinstimmung zwischen dem Einblendfeld EBF und dem
Region of Interest ROI erreicht ist. Eine weitere Vorgehens¬ weise wäre, dass das Einblendfeld EBF beispielsweise in y- Richtung verschoben wird bis das Zentrum des Einblendfeldes
EBF mit dem Zentrum des Region of Interest ROI übereinstimmt. Danach erfolgt eine Drehung des Einblendfeldes EBF in oder gegen den Uhrzeigersinn. Die Drehung des Einblendfeldes EBF erfolgt durch eine Drehung des gesamten Gehäuses in dem die Röntgenquelle und die Blendeneinheit angeordnet ist. Die Be¬ wegung der Blendenlamellen zur Anpassung des Einblendfeldes EBF an den Region of Interest ROI kann manuell, semimanuell oder elektronisch erfolgen. Eine Drehung der Blendeneinheit erfolgt durch Bedienung/Ansteuerung des für die Ausrichtung der Röntgenquelle vorgesehenen Mechanik und die diese steu¬ ernden Elektromotoren. Zu der Ausrichtung der Röntgenquelle wird eine dafür vorgesehene, hier nicht explizit abgebildete Recheneinheit mit Steuereinheit verwendet. Über eine mit der Recheneinheit verbundenen Eingabeeinheit kann mittels eines Bildschirmes ein Ausrichtvorgang visualisiert und die in der Recheneinheit integrierte Steuereinheit Steuersignale für die Einstellung von Blendenlamellen und eine Drehung der Röntgenquelle mit der Blendeneinheit BE initialisiert und steuernd durchgeführt werden.
In Figur 4 sind das Diagnosegerät und ein innerhalb des Pati¬ enten angeordneter Region of Interest ROI gezeigt. Eine unterhalb des Patienten 0 anzuordnende Detektoreinheit ist nicht explizit dargestellt. Der dargestellte Teil des hier deckengehängten Diagnosegerätes ist ein in einem Gehäuse untergebrachte Röntgenquelle R und eine am Ausgang der Röntgen¬ quelle R plazierte Blendeneinheit BE mit verstellbaren Blen¬ denlamellen. Die Röntgenquelle R und die Blendeneinheit BE können starr miteinander verbunden sein. Ebenso könnte eine starre Verbindung zwischen Röntgenquelle und Blendeneinheit BE entriegelt werden und eine Drehung der Blendeneinheit könnte mit Hilfe eines Flansches erfolgen. Das Gehäuse der Röntgenquelle R ist mit einer Befestigungseinheit B bei¬ spielsweise einem Mehrgelenkarm im Deckenbereich bewegbar an- geordnet. Die Befestigungseinheit B weist in dieser schemati¬ schen Darstellung zum einen an der Decke verfahrbare erste und zweite Bewegungseinheit EBE, ZBE mit denen eine Positio¬ nierung der Befestigungseinheit B in x- und/oder y-Richtung
erwirkt werden kann. An der ersten und/oder zweiten Bewegungseinheit EBE, ZBE ist ein höhenmäßig verstellbares Stativ eingehängt. Zwischen dem Stativ S und dem Gehäuse der Rönt¬ genquelle R ist eine schwenkbare erste Verbindungseinheit EV angeordnet. Mit dieser schwenkbaren ersten Verbindungseinheit EV kann unter anderem beispielsweise ein Verfahren des Gehäuses der Röntgenquelle R durch die erste und/oder zweite Bewe¬ gungseinheit EBE, ZBE derart kompensiert werden, dass ein auf einen Punkt im Region of Interest ROI gerichteter Zentral- strahl Z der Röntgenquelle R dort ausgerichtet verbleibt oder beispielsweise um einen Punkt im Region of Interest ROI ge¬ dreht werden kann. In der Figur 4 ist am Patienten der Region of Interest ROI angedeutet. Zusätzlich ist das bereits der Bereite des Region of Interest angepasste Einblendfeld EBF abgebildet. Gezeigt ist auch der von der Röntgenquelle R aus¬ gehende Zentralstrahl Z (siehe auch Fig. 5) . Eine Überlappung des Einblendfeldes EBF mit dem Region of Interest ROI erfolgt durch eine abgestimmte Ansteuerung der ersten und/oder zweiten Bewegungseinheit EBE, ZBE mit der ersten Verbindungsein- heit EV. Die abgestimmte Ansteuerung bewirkt eine Bewegungs¬ richtung BWR des Gehäuses der Röntgenquelle mit Blendenein¬ heit B unter Beibehaltung der Ausrichtung des Zentralstrahls Z der Röntgenquelle R. Die Bewegung des Gehäuses der Röntgen¬ quelle R mit der Blendeneinheit BE kann bewegungsunterstützt erfolgen. Mittels eines Lichtfensters, dass das Einblendfeld EBF wiedergibt kann der Bediener das Gehäuse mit Röntgenquel¬ le und Blendeneinheit BE in der angezeigten Bewegungsrichtung BWR bewegen, so dass das Einblendfeld EBF sich mit dem Region of Interest ROI überlagert. Dazu kann die Befestigungseinheit B an der das Gehäuse der Röntgenquelle R mit der Blendenein¬ heit BE befestigt ist entweder manuell, semimanuell oder vollelektronisch derart steuern, dass das Einblendfeld EBF mit der Region of Interest ROI zusammenfällt. Mittels der ersten Verbindungseinheit EV kann das Gehäuse der Röntgen- quelle R mit Blendeneinheit BE um eine in der Mitte des Sta¬ tivs S verlaufende Achse geschwenkt werden. Mittels der Steu¬ ereinheit SE können nach Eingabe von gewünschten Positionsda¬ ten bezüglich des Region of Interest ROI und die Größe des
Einblendfeldes EBF eingegeben werden. Die Steuereinheit SE erzeugt hierzu für die erste und zweite Bewegungseinheit EBE, ZBE sowie für die erste Verbindungseinheit EV nötigen Steuer¬ daten um die Ausrichtung der das Gehäuse aufnehmenden Rönt- genquelle R mit Blendeneinheit BE durchzuführen. Die Ausrich¬ tung kann in der Weise erfolgen, dass ein im Ausgangspunkt auf das Region of Interest ROI gerichteter Zentralstrahl Z der Röntgenquelle R in seiner Ausrichtung auf den Region of Interest ROI verbleibt. Die Blendeneinheit BE rotiert somit um den Zentralstrahl Z. Über die angedeutete Recheneinheit RE kann über eine Eingabeeinheit EE und einem auf dem Bildschirm BS abgebildeten Diagnosegerätes die augenblickliche Positio¬ nierung des unter anderem die Röntgenquelle und die Blendeneinheit aufnehmenden Gehäuses angezeigt sowie die Ausrichtung des Gehäuses simuliert bzw. schrittweise durchgeführt werden. Nach Angabe und Ausrichtung sowie der einzustellenden Größe des Region of Interest ROI können die für die Befestigungs¬ einheit B und die erste Verbindungseinheit EV sowie für die Einstellung nötigen Steuersignale in der Steuereinheit SE ge- neriert und eine exakte Positionierung des auf den Region of Interest ROI auszurichtenden Einblendfeldes EBF durchgeführt werden .
In Figur 5 ist das in Figur 4 schematisch dargestellte Diag- nosegerät in seiner Endposition gezeigt. Nach Ausrichtung des Einblendfeldes EBF der Röntgenquelle R auf den Region of Interest ROI kann beispielsweise eine Röntgenaufnahme ange¬ legt werden. Nicht die Blendeneinheit DE selbst oder z.B. ein Gelenk zwischen Röntgenstrahlröhre R und Blendeneinheit BE sondern die Komponenten der Befestigungseinheit B führen die Einzelschritte zur Rotationsbewegung und Ausrichtung des Einblendfeldes EBF aus. Die Komponenten der Befestigungseinheit B bewegen sich derart, dass die Blendeneinheit BE entspre¬ chend der Vorgabe eines Region of Interest ROI ausgerichtet wird. Dies bringt den Vorteil mit sich, dass Steuerungsmit¬ tel, insbesondere Elektromotoren zur Durchführung einer Verschiebung und/oder Drehung der gesamten Gehäuseeinheit in der unter anderem die Röntgenquelle R und die Blendeneinheit BE
angeordnet sind durchgeführt werden. Dies bringt zudem den weiteren Vorteil mit sich, dass das die Röntgenquelle R und die Blendeneinheit BE aufnehmende Gehäuse vorteilhaft verkleinerbar ist, so dass die Drehung bzw. Positionierung oder Ausrichtung der Blendeneinheit BE über Glieder der Befestigungseinheit B erfolgen kann. In einer weiteren Ausgestaltung der Erfindung kann die Ausrichtung der Blendeneinheit BE über eine Fernsteuerung oder über Gesten die mittels einer Kamera vom Bediener des Diagnosegerätes analysiert und in Steuersignale umgesetzt werden erfolgen.

1. Vorrichtung mit einer an einer positionierbaren Befesti- gungseinheit (B) angeordneten Gehäuseeinheit mit einer Rönt¬ genquelle (R) und einer der Röntgenquelle (R) nachgeordneten Blendeneinheit (BE) ,
dadurch gekennzeichnet,
dass eine Steuereinheit (SE) zur Ausrichtung der Blendenein- heit (BE) vorgesehen ist, wobei die Steuereinheit (SE) zur Ausrichtung der Blendeneinheit (BE) die positionierbare Be¬ festigungseinheit (B) derart ansteuert, dass sich die Blen¬ deneinheit (BE) entsprechend einer Vorgabe ausrichtbar ist.
2. Vorrichtung nach Patentanspruch 1,
dadurch gekennzeichnet,
dass die Steuereinheit (SE) zur Ausrichtung der Blendeneinheit (BE) die positionierbare Befestigungseinheit (B) derart ansteuert, dass unter Beibehaltung der Ausrichtung des Zent- ralstrahls (Z) der Röntgenquelle (R) entsprechend der Lage eines Untersuchungsgebietes (ROI) ein auf dieses ausrichtba¬ res Einblendfeld (EBF) mittels der positionierbaren Befesti¬ gungseinheit derart gedreht wird, dass eine Übereinstimmung zwischen dem Untersuchungsgebiet (ROI) und dem Einblendfeld (EBF) erreichbar ist.
3. Vorrichtung nach einem der vorhergehenden Patentansprüche,
dadurch gekennzeichnet,
dass die positionierbaren Befestigungseinheit (B) aus mindes¬ tens einer an der Decke eines Raumes angeordneter einer Ebene verfahrbaren ersten und zweiten Bewegungseinheit (EBE, ZBE) gebildet ist.
4. Vorrichtung nach einem der vorhergehenden Patentansprüche,
dadurch gekennzeichnet,
dass die positionierbare Befestigungseinheit (B) ein Mehrge- lenkarm ( EBE, ZBE , EV) ist.
5. Vorrichtung nach einem der vorhergehenden Patentansprüche,
dadurch gekennzeichnet,
dass an der ersten oder zweiten Bewegungseinheit (EBE, ZBE) ein in seiner Länge variierbares Stativ (S) angeordnet ist und am freien Ende des Statives (S) eine mindestens mit einen Freiheitsgrad ausgebildete erste Verbindungseinheit (EV) eine Verbindung zu der Gehäuseeinheit mit der darin angeordneten Röntgenquelle (R) und der Blendeneinheit (BE) aufweist.
6. Vorrichtung nach einem der vorhergehenden Patentansprüche,
dadurch gekennzeichnet,
dass die Blendeneinheit (BE) verschiebbare Blendenlamellen zur Begrenzung des Einblendfeldes (EBF) aufweist.
7. Vorrichtung nach einem der vorhergehenden Patentansprüche,
dadurch gekennzeichnet,
das Röntgenquelle R und die Blendeneinheit zueinander fixier¬ bar sind.
8. Verfahren mit einer an einer positionierbaren Befesti- gungseinheit (B) angeordneten Gehäuseeinheit mit einer Rönt¬ genquelle (R) und einer der Röntgenquelle (R) nachgeordneten Blendeneinheit (BE) ,
dadurch gekennzeichnet,
dass zur Ausrichtung der Blendeneinheit (BE) die positionier- bare Befestigungseinheit (B) derart angesteuert wird, wobei die Blendeneinheit (BE) entsprechend einer Vorgabe ausricht¬ bar ist.
9. Verfahren nach Patentanspruch 8,
dadurch gekennzeichnet,
dass zur Ausrichtung der Blendeneinheit (BE) die positionierbare Befestigungseinheit (B) derart angesteuert wird, dass unter Beibehaltung der Ausrichtung des Zentralstrahls (Z) der Röntgenquelle (R) entsprechend der Lage eines Untersuchungs¬ gebietes (ROI) ein auf dieses ausrichtbares Einblendfeld (EBF) mittels der positionierbaren Befestigungseinheit derart gedreht wird, dass eine Übereinstimmung zwischen dem Untersu¬ chungsgebiet (ROI) und dem Einblendfeld (EBF) erreichbar ist.
10. Verfahren nach einem der vorhergehenden Patentansprüche, dadurch gekennzeichnet,
dass die positionierbare Befestigungseinheit (B) in der Ebene verfahrbar ist.
