Abstract
Description
また本発明は、内視鏡用クリップ装置を用いて、体内留置用クリップを体内に取り付ける方法に関するものである。
そこで本発明は、体内留置用クリップを容易に取り外すことのできる内視鏡用クリップ装置を提供することを目的とする。
また本発明は、内視鏡用クリップ装置から体内留置用クリップを容易に取り外すことのできる、体内留置用クリップの取り付け方法を提供することも目的としている。
また、本発明の内視鏡用クリップ装置は羽根部の幅が小さいため、内視鏡用クリップ装置から体内留置用クリップを取り外す際に、羽根部に負荷される引張荷重を低減することができる。
さらに本発明の体内留置用クリップの取り付け方法は、体内留置用クリップで病変等の留置対象物を保持した後、体内に留置する段階で挟持部を内筒体に対して遠位端側に押し出す動作が不要であるため、挟持部から体内留置用クリップを容易に取り外すことができる。
本発明の内視鏡用クリップ装置は、外筒体と、外筒体内に配置されている内筒体と、内筒体内に収められた線状物と、線状物の遠位側に接続されて、体内留置用クリップを保持するための挟持部と、を含む内視鏡用クリップ装置であって、挟持部は、近位側において互いに近接している2つの挟持基材を有しており、挟持基材の遠位端には、挟持基材に対して内方に形成されている羽根部が設けられて、挟持基材よりも羽根部のヤング率が小さい。本発明の内視鏡用クリップ装置の羽根部はヤング率が小さく弾性変形し易いため、挟持部によって保持された体内留置用クリップと挟持部とが離間する方向に引張荷重を負荷すれば、内視鏡用クリップ装置から体内留置用クリップを容易に取り外すことができる。
挟持部20は体内留置用クリップ100を保持するものであり、図3(a)および図3(b)のように挟持部20は近位側において互いに近接している2つの挟持基材30a,30bを有している。挟持基材30a,30bの遠位端32a,32bには、挟持基材30a,30bに対して内方(図3ではX方向)に形成されている羽根部40a,40bがそれぞれ設けられており、挟持基材30a,30bの近位端31には接続部材55を介して線状物50が接続される。
ここで、挟持基材30a,30bの区間とは、挟持基材30a,30bの遠位端32a,32bから近位端31までの道のりを指す。
図5は、本発明の実施形態に係る内視鏡用クリップ装置10の近位側の平面図を表す。図5において、第1ハンドル90は最も近位側に配置されており、次いで第2ハンドル91が配置されている。また、第1ハンドル90を外筒体70に対して近位側に移動させると、これに伴い内筒体80が近位側に移動する。また、第2ハンドル91を外筒体70に対して近位側に移動させると、これに伴い線状物50および挟持部20が近位側に移動する。このように第1ハンドル90および第2ハンドル91が設けられれば、内筒体80や線状物50の移動操作がし易くなる。
クリップ装置10を用いたクリップ100の取り付け方法について、図7~11を用いて詳細に説明する。本説明に使用するクリップ装置10は、外筒体70、内筒体80、線状物50、挟持部20を含むものであり、挟持部20は、近位側において互いに近接している2つの挟持基材30a,30bを有しており、挟持基材30a,30bの遠位端には羽根部40a,40bが設けられている。羽根部40a,40bは挟持基材30a,30bの幅の半分以下の幅を有し、挟持基材30a,30bよりもヤング率が小さい。また、クリップ100は、腕部120a,120bと、腕部120a,120bに対して内方に形成されている先端部123a,123bを有しており、クリップ100の外側には締結具110が設けられている。
まず、クリップ装置10とクリップ100との位置合わせを行う。第2ハンドル91を遠位側に押し出し、図7に示すようにクリップ装置10の挟持部20の遠位端を内筒体80の外に露出させる。具体的には線状物50および挟持部20を遠位側に移動させると、内筒体80の内腔によって内方(図3のX方向)に押力が付与されていた羽根部40a,40bおよび挟持基材30a,30bは外方に互いに離間していき、挟持部20の開度は徐々に増していく。
次に、挟持部20を内筒体80内に収納するために、第2ハンドル91を近位側に引き込む。開状態の挟持部20は、挟持基材30a,30bの近位側の縁部が内筒体80の内側面に当接している。線状物50とともに挟持部20が近位側に引き込まれると、遠位側の縁部が内筒体80に当接する。内筒体80から露出している挟持部20の部分が少なくなるにつれて羽根部40a,40bの開度は低下するため、羽根部40a,40bによって体内留置用クリップ100は保持される。なお、体内に留置する前段階においてクリップ100が閉状態になることを防ぐため、挟持部20の内筒体80内への引き込みは体内留置用クリップ100の遠位側までにとどめておく。
外筒体70内にクリップ100および内筒体80が収納されたクリップ装置10を内視鏡の鉗子口から挿入し、外筒体70の遠位端を患者の病変付近まで到達させる。このとき、術者は内視鏡から取得した映像で病変の位置や病変の状況等を観察しながら、第1ハンドル90および第2ハンドル91を軸方向に回転させて、病変に対するクリップ100の先端部123a,123bの保持方向を調整する。外筒体70に対して、第1ハンドル90および第2ハンドル91を遠位側に一緒に押し出すと、図9に示すように挟持部20は内筒体80に収納された状態でクリップ100の一部が外筒体70から露出する。つまり、外筒体70の内腔形状によって拘束されていたクリップ100の腕部120a,120bが外筒体70から露出されるのに伴い、クリップ100の開度は次第に大きくなる。
最後に、クリップ装置10からクリップ100を取り外して、体内にクリップ100を留置する。クリップ装置10の挟持部20を近位側に引き込むために外筒体70に対して第2ハンドル91を近位側に移動させる。そうすると、羽根部40a,40bに対して引張荷重が負荷されるため、挟持基材30a,30bと羽根部40a,40bのなす角度θは大きくなり、羽根部40a,40bが外方に開く。このため、図11に示すようにクリップ装置10の挟持部20で保持していたクリップ100が羽根部40a,40bを乗り越えてクリップ装置10から取り外される。本発明の体内留置用クリップ100の取り付け方法は、クリップ100によって病変等の留置対象物を保持した後、クリップ100を体内に留置する段階で挟持部20の内筒体80に対して遠位端側に押し出す動作が不要である。すなわち、クリップ100で留置対象物を保持した後は挟持部20を近位端側に引き込む操作のみを実施すればよいため、クリップ装置10からクリップ100を容易に取り外すことができる。なお、本発明のクリップ装置10の羽根部40は、挟持基材30よりも幅が小さく、また挟持基材30よりもヤング率が小さいため、弾性変形しやすいものでもある。

- 外筒体と、該外筒体内に配置されている内筒体と、該内筒体内に収められた線状物と、前記線状物の遠位側に接続されて、体内留置用クリップを保持するための挟持部と、を含む内視鏡用クリップ装置であって、
前記挟持部は、2つの挟持基材を有しており、
前記挟持基材の遠位端には、前記挟持基材に対して内方に形成されている羽根部が設けられて、前記挟持基材よりも前記羽根部のヤング率が小さいことを特徴とする内視鏡用クリップ装置。 - 外筒体と、該外筒体内に配置されている内筒体と、該内筒体内に収められた線状物と、前記線状物の遠位側に接続されて、体内留置用クリップを保持するための挟持部と、を含む内視鏡用クリップ装置であって、
前記挟持部は、2つの挟持基材を有しており、
前記挟持基材の遠位端には、前記挟持基材に対して内方に形成されている羽根部が設けられて、該羽根部は、前記挟持基材よりも幅が小さいことを特徴とする内視鏡用クリップ装置。 - 一方の前記挟持基材の先端に設けられる前記羽根部と、他方の前記挟持基材の先端に設けられる前記羽根部が互い違いに配置されている請求項1又は2に記載の内視鏡用クリップ装置。
- 前記羽根部は、前記挟持基材に対して90度よりも大きく180度よりも小さい角度をなしている請求項1~3のいずれか一項に記載の内視鏡用クリップ装置。
- 前記挟持基材と前記羽根部は、一体的に形成されている請求項1~4のいずれか一項に記載の内視鏡用クリップ装置。
- 前記羽根部の材料がNi-Ti合金又はステンレス鋼である請求項1~5のいずれか一項に記載の内視鏡用クリップ装置。
- 前記体内留置用クリップには、前記内筒体の内径よりも外径が大きく、軸方向に移動可能な締結具が設けられている請求項1~6のいずれか一項に記載の内視鏡用クリップ装置。
- 前記内筒体の近位側に接続されて、前記外筒体に対して軸方向に移動可能な第1ハンドルと、
前記線状物の近位側に接続されて、前記外筒体に対して軸方向に移動可能な第2ハンドルと、が設けられる請求項1~7のいずれか一項に記載の内視鏡用クリップ装置。 - 前記羽根部の長さが前記内筒体の内径の半分以下である請求項1~8のいずれか一項に記載の内視鏡用クリップ装置。
- 前記羽根部は前記挟持基材よりも肉厚が小さい請求項1~9のいずれか一項に記載の内視鏡用クリップ装置。
- 前記挟持基材の少なくとも一部区間には、前記挟持部の内方に凸となる湾曲部が形成されている請求項1~10のいずれか一項に記載の内視鏡用クリップ装置。
- 外筒体と、該外筒体内に配置されている内筒体と、該内筒体内に収められた線状物と、前記線状物の遠位側に接続されて、体内留置用クリップを保持するための挟持部と、を含み、前記挟持部は、2つの挟持基材を有しており、前記挟持基材の遠位端には、前記挟持基材に対して内方に形成されている羽根部が設けられている内視鏡用クリップ装置の体内留置用クリップの取り付け方法において、
前記体内留置用クリップで留置対象物を保持するステップと、
その後、前記線状物を、前記内筒体に対して遠位端側に押し出すことなく、近位端側に引き込むことにより、前記体内留置用クリップが前記羽根部を乗り越えて、前記体内留置用クリップが前記挟持部から取り外されるステップと、を有する体内留置用クリップの取り付け方法。
