Definitions
- the present invention relates to a superstructure used for an implant.
- the implant has a pure titanium or titanium alloy fixture that is implanted in the jawbone and an upper structure that is connected to the fixture.
- the superstructure includes a framework, artificial teeth joined to the framework, and a connecting member joined to the inner lower portion of the framework to connect the framework to the fixture.
- a noble metal alloy such as gold or platinum or a material obtained by directly processing a framework material is used, and a gold alloy is more preferable from the viewpoint of corrosion resistance, hardness, and workability.
- porcelain, hard resin, etc. are used for the artificial tooth, porcelain is more preferable from the viewpoint of wear resistance, cleanability, and self-cleaning properties.
- the implant is provided in the patient's oral cavity as follows. First, the fixture is implanted in the patient's jawbone. Next, after joining the framework and the connecting member, the superstructure is formed by forming artificial teeth on the framework. When the artificial tooth is made of porcelain, the raw material powder is applied to the outer surface of the framework, and the artificial tooth is formed by firing at a high temperature. Next, the superstructure connecting member is fastened to the fixture planted in the oral cavity of the patient with a screw.
- a framework made of a cobalt-chromium alloy has a problem in that the accuracy of the joint portion is difficult because an oxide film is generated on the surface when porcelain is fired.
- the cobalt-chromium alloy may be oxidized (corroded) during welding, which may reduce accuracy.
- the welding area is small and only the surface layer is welded, the joining is insufficient, and the joining between the framework and the connecting member may be lost during use in the oral cavity.
- air bubbles are generated in the joint portion during welding, when the raw material powder is applied and fired after joining to form an artificial tooth, air may be released from the air bubbles and a hole may be formed in the artificial tooth.
- the present inventors joined cobalt chromium alloys and gold alloys by brazing and evaluated the strength (see Non-Patent Document 1), but brazed the cobalt chromium alloy and the gold alloy to evaluate the strength. I could't do it. Therefore, at present, an abutment in which a framework made of a cobalt chromium alloy and a connecting member made of a gold alloy are brazed has not been realized.
- An object of the present invention is to provide a superstructure using a cobalt chromium alloy.
- the present invention is a superstructure made of pure titanium or a titanium alloy and connected to a fixture that is implanted in a jawbone. And a connecting member that is made of a gold alloy and is joined to the inner lower portion of the framework and connects the framework to the fixture.
- the framework and the connecting member are made of gold and silver. It is characterized by being integrated by brazing with a brazing material comprising a gold alloy.
- the superstructure of the present invention is firmly joined by brazing a framework made of a cobalt chromium alloy and a connecting member made of a gold alloy with a brazing material made of a gold alloy containing gold and silver. Can be integrated.
- the superstructure of the present invention is superior in rigidity compared to the conventional superstructure using a material such as a gold alloy, platinum alloy, pure titanium, or titanium alloy for the framework because the framework is made of a cobalt chromium alloy. Can be obtained. As a result, even when the diameter of the superstructure itself is narrowed or shortened, excellent rigidity can be ensured, so the front teeth for which the artificial teeth are to be formed thin and the back teeth for which the height of the artificial teeth is to be formed low It is particularly effective for such as.
- the gold alloy constituting the connecting member has a hardness lower than that of pure titanium or titanium alloy constituting the fixture. For this reason, when an excessive force is accidentally applied between the upper structure and the fixture when the upper structure is connected to the fixture, the connecting member can be deformed, but deformation of the fixture can be prevented. The burden on the patient can be reduced without requiring fixture replacement.
- the framework is perforated with two or more lateral holes.
- the lateral hole includes a large-diameter hole having a large diameter and a small-diameter hole.
- the gap between the framework and the connecting member is very narrow. Therefore, a high technique is required to reliably introduce the brazing material into the gap. By confirming that the brazing material overflows from the other side hole when the brazing material is introduced from one side hole due to the presence of multiple lateral holes, it is visually confirmed that the gap is filled with the brazing material. Can be confirmed.
- the horizontal hole is preferably provided with a large-diameter hole mainly used when the brazing material is introduced and a small-diameter hole with a small diameter.
- the small diameter hole is preferably provided on the side facing the large diameter hole in order to confirm the brazing filler filled from the large diameter hole. The strength of the implant can be maintained by reliable brazing.
- the brazing material contains gold in the range of 80 to 85% by mass and silver in the range of 14 to 17% by mass, whereby a framework made of a cobalt chromium alloy is used. And a connecting member made of a gold alloy can be reliably bonded.
- the connecting member has an engaging portion formed to be engageable with the fixture at an end portion on the side where the connecting member is joined in the axial direction of the connecting member, and an outer periphery.
- a flange portion protruding radially outward from the surface, and the framework is a small-diameter cylindrical member provided on the opposite side of the connecting member to the fixture, and the flange of the connecting member
- a large-diameter cylindrical member provided on an outer peripheral surface side of a portion excluding the portion and an outer peripheral surface side of the small-diameter cylindrical member, and the small-diameter cylindrical member, the large-diameter cylindrical member, and the connecting member of the framework are preferably integrated by brazing with the brazing material.
- the small-diameter cylindrical member and the large-diameter cylindrical member of the framework are joined, and the small-diameter cylindrical member is joined to the opposite side of the connecting member to the fixture, Since the large-diameter cylindrical member is joined to the outer peripheral surface of the portion excluding the flange portion of the connecting member, the framework and the connecting member are joined more firmly. Further, when brazing with the brazing material, the gap between the small diameter cylindrical member and the large diameter cylindrical member and the gap between the large diameter cylindrical member and the connecting member communicate with each other. It is possible to spread all the way to the corners, and it can be surely joined.
- the two or more horizontal holes are formed in the large-diameter cylindrical member of the framework, and at least one of the horizontal holes is formed in a position where the connecting member can be visually observed before the brazing material is introduced. It is preferable that at least one of the lateral holes is formed at a position where the small-diameter cylindrical member can be seen before introduction of the brazing material.
- the brazing material can be introduced into the gap between the large-diameter cylindrical member and the connecting member and between the large-diameter cylindrical member and the small-diameter cylindrical member of the framework, the brazing material is surely distributed in the gap. Can be made.
- the superstructure may have an abutment made of pure titanium or a titanium alloy interposed between the connecting member and the fixture. It can be applied to any case.

- the small-diameter cylindrical member 8 is joined to the side opposite to the side connected to the fixture 2 of the gold abutment 6.
- the small diameter cylindrical member 8 is configured such that the inner peripheral surface thereof is flush with the inner peripheral surface of the gold abutment 6 when joined to the gold abutment 6.
- the large-diameter cylindrical member 9 is joined to the outer peripheral side of the small-diameter cylindrical member 8 and the outer peripheral side of the gold abutment 6 excluding the flange portion 6a by a brazing material.
- the large-diameter cylindrical member 9 gradually increases in diameter from the fixture 2 side toward the opposite side in the axial direction, and its outer peripheral surface is the outer periphery of the flange portion 6a when joined to the gold abutment 6 It is comprised so that it may become a smooth curved surface which continues to a surface.
- the large-diameter cylindrical member 9, the small-diameter cylindrical member 8, and the gold abutment 6 are brazed and joined by a brazing material 10 made of a gold alloy containing gold and silver.
- a brazing material 10 for example, gold brazing containing 80% by mass of gold, 17% by mass of silver, and 3% by mass of platinum (trade name S.G1080, Cendres + Metaux, solid phase point 1060 ° C., liquid phase point 1080 ° C), gold brazing containing 85% by weight of gold, 14% by weight of silver and 1% by weight of zinc, platinum and iridium (trade name S.G1030, Cendres + Metaux, solid phase point 990 ° C, liquid phase point 1030 ° C.).
- the small diameter cylindrical member 8 and the gold abutment 6 are joined by laser welding.
- the upper structure 3 is formed as follows. After the fixture 2 is embedded in the patient's jawbone and the fixture 2 is joined to the jawbone, the mold is made (impression) on the area including the end face opposite to the jawbone of the fixture 2 to create a model. To do.
- a cobalt chromium alloy (trade name: Wirobond280) having the above composition is cast and cut to form a large-diameter cylindrical member 9 and a small-diameter cylindrical member 8.
- the large-diameter cylindrical member 9 is provided with a lateral hole 9a penetrating the peripheral wall surface in the thickness direction so as to be filled with a later-described flux and brazing material.
- the horizontal hole 9a functions as an introduction hole for introducing the flux and the brazing material.
- the large-diameter tubular member 9 and the small-diameter tubular member 8 may be subjected to CAD / CAM processing instead of the casting and cutting.
- the gold abutment 6 and the framework 5 are joined by gold brazing as follows, covered with artificial teeth, and then connected to a fixture in the oral cavity by a screw 7.
- Fig. 1 (b) shows a simple method for joining the gold abutment 6 and the framework 5 with a gold braze.
- a large-diameter cylindrical member 9 is erected on the flange portion 6a of the gold abutment 6, and the upper end surface of the flange portion 6a and the lower end surface of the large-diameter cylindrical member 9 are joined by laser welding.
- a small diameter cylindrical member 8 is erected on the end surface opposite to the fitting portion 6b of the gold abutment 6, and the upper end surface of the gold abutment 6 and the lower end surface of the small diameter cylindrical member 8 are laser welded. Join by.
- the flux is poured into the gap between the large-diameter cylindrical member 9 and the small-diameter cylindrical member 8 and the gap between the large-diameter cylindrical member 9 and the gold abutment 6 by pouring flux from the lateral hole 9a.

- the above-mentioned gold solder (trade name S.G1080 or S.G1030) is heated to a temperature in the range of 1100 to 1130 ° C. and melted. Then, when the molten gold brazing is poured from the lateral hole 9a in a state where the large diameter cylindrical member 9, the small diameter cylindrical member 8 and the gold abutment 6 are heated, the fluidity of the flux filled in the gap is increased. In addition, the flux flows out of the gap, and molten gold solder is filled in the gap. At this time, the gap between the large-diameter cylindrical member 9 and the small-diameter cylindrical member 8 and the gap between the large-diameter cylindrical member 9 and the gold abutment 6 communicate with each other.
- the large-diameter tubular member 9, the small-diameter tubular member 8, and the gold abutment 6 are reliably joined by the brazing material 10 made of gold braze, and the framework 5 and the gold abutment 6 are integrated. Can do.
- the horizontal hole 9 a of the large-diameter cylindrical member 9 is filled with the brazing material 10.
- the upper structure 3 itself is also a very small structure, and the space between the large-diameter cylindrical member 9, the gold abutment 6, and the small-diameter cylindrical member 8 is as very narrow as 10 to 50 ⁇ m as described above. For this reason, it is very difficult and skillful to fill the brazing filler metal with a single horizontal hole.
- the large-diameter hole which is a large-diameter horizontal hole
- the small-diameter hole which is a small-diameter horizontal hole, with a diameter of about 0.8 mm to 1.0 mm.
- These horizontal holes may be provided anywhere, but the large-diameter hole is preferably at a position where the connecting member can be visually observed before the introduction of the brazing material. This is because it is important that the gold abutment 6 and the large-diameter cylindrical member 9 at the base of the implant superstructure are brazed reliably.
- the small-diameter hole which is a small-diameter lateral hole, is provided in the upper part (position of FIG. 1 (b) 9b) facing the large-diameter hole. If the gold brazing filled from the large-diameter hole is confirmed from other lateral holes provided at the upper part of the position facing the large-diameter hole, it is considered that the gap existing in the upper structure 3 is almost surely filled. .
- the gold abutment 6 and the large-diameter cylindrical member 9 can be brazed reliably by replenishing the gold brazing from the lateral hole provided in the lower part of the upper structure 3.
- the horizontal hole shown in FIG. 1 (b) is merely an example, and it goes without saying that the diameter and the drilling position should be appropriately selected according to the shape and size of the teeth.
- each lateral hole (9a, 9b, 9c) with a brazing filler metal is easy by introducing a cylindrical introduction part (11a, 11b, 11c) called chimney to facilitate the introduction of the brazing filler metal.
- a cylindrical introduction part 11a, 11b, 11c
- Chimneys may be provided in all the lateral holes as shown in FIG. 1B, or may be provided only in the large-diameter holes.
- the chimney is excised together with the gold wax remaining in the chimney after filling the gap with the gold wax.
- the outer surface of the large-diameter cylindrical member 9 and the outer surface of the flange portion 6a of the gold abutment 6 are smooth based on the model. Cutting and polishing so that the curved surface is continuous.
- the artificial tooth 4 when porcelain is used as the artificial tooth 4, a paste containing raw material powder and a solvent is applied to the outer surface of the framework 5 (excluding the inner wall surface 8 a of the small-diameter cylindrical member 8). After being applied to the shape, the raw material powder is fired by heating at a temperature of about 780 to 1000 ° C. to volatilize the solvent. Thereby, the artificial tooth 4 provided with the through-hole 4a joined to the framework 5 and used as an access hole can be formed.
- the raw material powder for example, trade name Reflex® Porcelain (Wieland) can be used, and as the solvent, for example, Reflex® Mixing® Liquid (Wieland) can be used.
- the through hole 4a is filled by filling with a hard resin or the like. Thus, the upper structure 3 is formed.
- the framework 5 is made of a cobalt chrome alloy, and the cobalt chrome alloy has a 0.2% proof stress (physical strength) of 720 MPa and an elastic modulus of 210 GPa.
- pure titanium used in a conventional superstructure framework has a 0.2% proof stress of 340 MPa, and gold alloys and platinum alloys have an elastic modulus of 100 GPa.
- the superstructure 3 of the present embodiment has excellent rigidity and can secure strength as compared with the conventional superstructure whose framework is made of a gold alloy, platinum alloy, pure titanium, or titanium alloy.
- the upper structure 3 of the present embodiment is firmly joined to the framework 5 and the gold abutment 6 by the brazing material 10, the upper structure 3 is formed by fitting the fitting portion 6b to the hexagonal portion 2a. When connecting to the fixture 2, it is possible to prevent the joint from being detached.
- the gold alloy constituting the gold abutment 6 connected to the fixture 2 has a smaller hardness than the pure titanium or titanium alloy constituting the fixture 2. Therefore, when an upper force is applied between the upper structure 3 and the fixture 2 when the upper structure 3 is connected to the fixture 2 due to an inappropriate joining position or the like, the gold abutment The deformation of the fixture 2 can be prevented by the deformation of the fitting portion 6b of 6 (fail-safe mechanism). Thereby, it is not necessary to replace the fixture 2 embedded in the jawbone, and the burden on the patient can be reduced.
- the upper structure 3 is joined to the framework 5 and the gold abutment 6 by brazing, unlike the laser welding, almost no bubbles are generated in the gap between the framework 5 and the gold abutment 6. Therefore, air is not released from the gap when the raw material powder is fired, and the artificial tooth 4 can be prevented from opening a hole.
- the shape of the artificial tooth 4 is different depending on the position required in the oral cavity. For example, when the artificial tooth 4 is used for the front teeth, it is desired to make the artificial tooth 4 thin, and when it is used for the back teeth, the artificial tooth 4 is desired to have a low height. There is demand.
- the framework 5 since the framework 5 has excellent rigidity, the length orthogonal to the axial direction of the framework 5 is shortened (diameter is reduced), or the axial length is shortened (height is reduced). ), It is possible to ensure excellent rigidity. Thereby, the artificial tooth 4 of a desired shape can be formed.
- the upper structure 3 of the present embodiment can form the framework 5 in a thin shape, the upper structure 3 can be shaped to rise substantially vertically from the fixture 2. Thereby, the circumference

The chimney is cut off together with the gold braze remaining in the chimney after filling the gap with the gold braze 61 and polished so that the lateral hole filled with the gold braze and the outer surface of the framework are smoothly continuous curved surfaces.
- the upper structure 53 of the present embodiment is superior to the conventional upper structure using a material such as a gold alloy, platinum alloy, pure titanium, or titanium alloy for the framework because the framework 54 is made of a cobalt chromium alloy. Stiffness can be obtained.
- the gold alloy constituting the gold abutment 56 has a smaller hardness than the pure titanium or titanium alloy constituting the fixture 52 or the abutment 56. For this reason, when an excessive force is accidentally applied between the upper structure 53 and the fixture 52 when the upper structure 53 is connected to the fixture 52, the gold cylinder 55 is deformed, but the fixture 52 and the abutment are deformed. 56 deformation can be prevented.

- the brazing operation is a brazing material containing 80% by mass of gold, 17% by mass of silver, and 3% by mass of platinum (trade name S.G1080, Cendres + Metaux, solid phase point 1060 ° C., liquid phase Point 1080 ° C.), brazing temperature 1100 to 1130 ° C., mooring time 1 minute, under reduced pressure.
- Comparative Example 1 a cast body made of a gold alloy and having a diameter of 2 mm and a length of 20 mm was produced as the first adherend and the second adherend.
- the gold alloy contains 58% by mass of gold, 29% by mass of palladium, and 8% by mass of silver, and the remainder contains iridium, tin, and ruthenium, and has a solid phase point of 1215 ° C. and a liquid phase point of 1305 ° C.
- the first adherend and the second adherend were exactly the same as in Example 1, and after performing the brazing operation, the brazing sample was taken out into the atmosphere and allowed to cool, whereby a brazed sample was obtained. Obtained.
- the obtained brazed sample was subjected to a tensile test in exactly the same manner as in Example 1, and the brazing strength was calculated. As a result, it was 460 MPa.
- Example 1 has a strength superior to that of Comparative Example 1. From this, the superstructure of this embodiment in which the framework made of a cobalt chrome alloy and the connecting member made of a gold alloy are joined by a brazing material made of a gold alloy containing gold and silver is the conventional structure of the gold It is apparent that the alloy framework and the connecting member have superior strength as compared with the superstructure in which the brazing material made of the gold alloy is joined.
