また、例えば特開2012-210445号公報で開示された処置具において、プローブと支持部とが接触した状態でプローブに超音波振動を伝達し続けると、プローブにキズが付いたりするなどプローブが破損するおそれがある。
図1に示すように、この実施形態に係る処置システム10は、処置具12と、振動子ユニット14と、電源として用いられるコントローラ16と、フットスイッチ18とを有する。
なお、作用部44及びプローブ24の処置部74によって、生体組織を把持する把持部が形成される。
図2に示すように、振動伝達部72の外周には、軸方向に離間して複数の保持部材76a,76b,…が配設されている。例えば、振動伝達部72の超音波振動の振動の節の位置であって最先端の位置に、保持部材76aを配置する。超音波振動子62の共振周波数が47kHzである場合には、保持部材76aから略50mm後端側に保持部材76bを配設し、以下、略50mm間隔で保持部材76c,76d,…を順次配設する。これら保持部材76a,76b,…は、非導電性(電気絶縁性)を有するリング状のゴム材により形成されている。保持部材76a,76b,…が配設された振動伝達部72はシース42内に挿通されている。保持部材76a,76b,…は、振動伝達部72が金属材製のシース42の内周面に接触するのを防止している。
図3A及び図3Bに示すように、作用部44は、可動ハンドル54の操作により動作されるジョー82と、ジョー82に配置されプローブ24の処置部74に対向し生体組織を押圧して把持する押圧部84とを有する。押圧部84は、PTFE等の耐熱性、耐摩耗性及び非導電性(電気絶縁性)を有する樹脂材製のパッド(緩衝部)92を有する。そして、パッド92は、生体組織を把持する把持面96を形成する。パッド92のうち処置部74に対向する表面(把持面96)は、例えばギザギザや梨地等、生体組織に対する滑り留めとして形成されていることが好適である。
離隔位置から支持位置に切り替えられた後のコントローラ16による支持位置の制御を第2ステージの制御とする。第2ステージではコントローラ16の後述するステージ切替部112により設定部114によりユーザの好みや処置対象に合わせて適宜に設定された制御が行われ、生体組織の処置が進められたり、処置を一旦停止したりする。すなわち、コントローラ16は、第2ステージでは、定電流制御を行っても良いし、電流を適宜に可変させる制御を行っても良い。なお、第1ステージの制御から第2ステージの制御に切り替えられる際、コントローラ16による制御は連続している。
電力出力部106及びインピーダンス検出部108は振動子ユニット14の振動子62に接続されている。異常検出部110は電力出力部106及びインピーダンス検出部108に接続されている。異常検出部110は、電力出力部106の出力量の異常、インピーダンス検出部108の検出値の異常等を検出可能である。
ユーザは処置ユニット22に対して、プローブ24を装着して処置具12を形成する。このとき、シース42の先端部42aの内周面又はその近傍の内周面の支持部100は、プローブ24に対して離隔した離隔位置にある。ユーザはさらに処置ユニット22及びプローブ24に対して振動子ユニット14を装着する。そして、振動子ユニット14をコントローラ16に接続する。ユーザは、コントローラ16の設定部114で処置内容や処置のための制御内容を適宜に設定する。
このように、振動のエネルギーはプローブ24から制振合金材製の支持部100に伝達されて吸収されるので、プローブ24の超音波振動に対してブレーキング作用を働かせる。
なお、プローブ24が撓まされたときには、プローブ24に追従して動き振動を吸収する支持部100に当接して支持位置で支持される。このため、プローブ24が撓まされたときに、プローブ24に伝達された超音波振動が支持部100によってシース42に伝達されるのを防止できる。すなわち、プローブ24が撓まされたとき、プローブ24に伝達された超音波振動がシース42に影響を与えるのを支持部100によって防止することができる。したがって、超音波振動が伝達されたプローブ24が支持部100に当接されたとき、プローブ24、支持部100の破損を防止できるだけでなく、シース42を破損させるのを防止することができる。
図8Aに示す例では、コントローラ16は、第1ステージにおいてプローブ24が支持部100に当接して支持位置(第2ステージ)に切り替えられたとき、第2ステージの初期では、インピーダンス検出部108が切り替えを検知する直前に比べて振幅を小さくするように電力を超音波振動子62に供給する。その後、コントローラ16は、プローブ24が支持部100に支持された状態を維持しながら超音波振動を間欠的に出力させている。このように、超音波振動を間欠的に出力させることにより、生体組織の切開を続けながら、プローブ24の発熱及び、支持部100の発熱及び伝熱を抑制することができる。図8A中、第2ステージの超音波振動の最大振幅は第1ステージと同じ状態として描いたが、振幅を大きくしても、小さくしても良いことはもちろんである。そして、コントローラ16は、離隔位置から支持位置に切り替えられた後、適宜の時間の経過後、超音波振動を完全に停止させる。
図8Bに示す例では、コントローラ16は、第1ステージにおいてプローブ24が支持部100に当接して支持位置(第2ステージ)に切り替えられたとき、第2ステージの初期では、インピーダンス検出部108が切り替えを検知する直前に比べて振幅を徐々に小さくするように電力を超音波振動子62に供給する。その後、コントローラ16は、プローブ24が支持部100に支持された状態を維持しながら超音波振動を連続的に出力するが、第1ステージに比べて振幅を小さくするとともに、一定に維持した状態で出力を続けている。このように、超音波振動の振幅を減少させながら振動させているので、生体組織の切開を続けることができる。そして、コントローラ16は、離隔位置から支持位置に切り替えられた後、適宜の時間の経過後、超音波振動を完全に停止させる。
図8Cに示す例では、コントローラ16は、第1ステージにおいてプローブ24が支持部100に当接して支持位置(第2ステージ)に切り替えられたとき、第2ステージにおいてインピーダンス検出部108が切り替えを検知する直前に比べて振幅を大きくするように電力を超音波振動子62に供給する。その後、コントローラ16は、プローブ24が支持部100に支持された状態を維持しながら超音波振動を連続的に出力し、かつ、第1ステージに比べて振幅を大きくするとともに、一定に維持した状態で出力する。そして、コントローラ16は、離隔位置から支持位置に切り替えられた後、適宜の時間の経過後、超音波振動を完全に停止させる。このように、超音波振動の振幅を上昇させながら振動させているので、生体組織の切開を図8A及び図8Bに示す場合よりも、短時間で終了させることができる。
この実施形態に係る処置具12のシース42の先端部42aの内周面又はその近傍の内周面に設けられた支持部100は、超音波振動が伝達された状態のプローブ24の振動の節に相当する位置から中心軸Cに沿って前方に離れた位置にあり、プローブ24が処置部74への負荷に応じて撓まされることでプローブ24に対する離隔位置とプローブ24に当接して支持する支持位置とが切り替えられる。支持部100は、超音波振動が伝達された状態のプローブ24が撓まされて離隔位置から支持位置に切り替えられたときに超音波振動が伝達された状態のプローブ24の振動に追従して動き、超音波振動が伝達された状態のプローブ24が離隔位置から支持位置に切り替えられたときにプローブ24に伝達された振動を吸収する、導電性を有する金属材製の素材で形成されている。このため、超音波振動が伝達された状態のプローブ24が支持部100に当接したとき、支持部100はプローブ24の振動に追従(共振)し、プローブ24にキズを付けることを防止できる。したがって、振動が伝達されているプローブ24に撓みが生じてプローブ24がシース42に近接して支持部100に当接しても、プローブ24が破損するのを防止することができる。
支持部100は、プローブ24を削るのを防止するとともに、プローブ24により削られるのを防止する耐摩耗性を有することが好ましい。すなわち、超音波振動が伝達されたプローブ24は支持部100を削ることを極力防止できる。このため、振動が伝達されているプローブ24に撓みが生じてプローブ24がシース42に近接して支持部100に当接しても、支持部100及び支持部100が配置されたシース42を破損させるのを防止することができる。
支持部100がこのように形成されているので、プローブ24がシース42に近接して支持部100に当接したとしても、プローブ24に超音波振動を発振し続けて、生体組織の処置を続ける選択をすることができる。したがって、この実施形態に係る処置具12は、プローブ24に撓みが生じて、プローブ24の撓みを支持部100で支持した状態で、生体組織に対して確実に切開等の処置を行うことができる。
図9に示すように、支持部100に隣接して例えばPTFE材等の電気絶縁性を有するとともに耐熱性、耐摩耗性を有する樹脂材98を配置することも好適である。樹脂材98は適宜の角度の略円弧状(略U字状)、又は、リング状等であることが好適である。樹脂材98は支持部100よりもシース42の先端部42aの内周面のうちの先端側にあり、プローブ24が撓まされたときに、支持部100に当接されるよりも先に当接するように形成されていることが好適である。すなわち、プローブ24は先に樹脂材98に当接し、さらに処置部74等に外力が負荷されてプローブ24の撓み量が大きくなると、樹脂材98が削られたり、弾性変形したりして支持部100に当接される。このため、プローブ24に超音波振動が伝達されている状態で撓まされても、伝達されていない状態で撓まされても、樹脂材98でプローブ24を支持することができる。なお、支持部100は、樹脂材98に対して耐熱性及び耐摩耗性が高く形成されていることが好ましい。
作用部44の押圧部84は、パッド92と、高周波エネルギーで生体組織を処置するための電極(第2電極)94とを有する。具体的には、電極94はパッド92に埋め込まれている。この電極94はコントローラ16の高周波エネルギー出力部132に電気的に接続されているとともに、支持部100に電気的に接続されている。
図12Bに示すように、コントローラ16は短絡検知回路(検知部)142を有する。
プローブ24は、超音波振動が伝達されるとともに、電極(第1電極)としても機能する。プローブ24はコントローラ16の高周波エネルギー出力部132に電気的に接続されている。プローブ24はさらに短絡検知回路142に電気的に接続されている。
作用部44の押圧部84は、パッド92と、電極(第2電極)94とを有する。具体的には、パッド92には電極94が埋め込まれている。この電極94はコントローラ16の高周波エネルギー出力部132に電気的に接続されている。

- 中心軸を有する筒状体と、
その基端から先端に向かって超音波振動が伝達され、前記筒状体に挿通され前記筒状体の先端に対して先端側に突出し、導電性を有するプローブと、
前記超音波振動が伝達された状態の前記プローブの振動の節に相当する位置から前記中心軸に沿って前方又は後方に離れた位置にあり、前記プローブがその負荷に応じて前記中心軸から外れるように撓まされることで前記プローブに対する離隔位置と前記プローブに当接して支持する支持位置とが切り替えられ、前記超音波振動が伝達された状態で前記離隔位置から前記支持位置に切り替えられたときに、前記プローブの振動に追従して動き、前記支持位置で前記プローブに伝達された振動を吸収する、導電性を有する支持部と
