Abstract
Prótesis de cadera perfeccionada consistente en la inserción en el fémur de un vástago de la prótesis con un cóndilo. Así, el hueso pélvico recibe el acetábulo que sirve de cavidad de sujeción para el cóndilo mediante implantación de prótesis perfeccionada en base a su movimiento de rotación mediante rodamientos y sistema de fijación al hueso. El cóndilo dispuesto según una configuración esférica, comporta, junto con el casco esférico en contacto con el hueso pélvico y la tapa roscada de cierre, el emplazamiento en el cual se ubican los rodamientos que garantizan el movimiento de rotación entre los citados huesos. Por último el citado casco esférico en contacto con el hueso pélvico se complementa con unos filamentos de fijación al citado hueso.

DESCRIPCIÓN
"Prótesis de cadera perfeccionada" OBJETO DE LA INVENCIÓN
La presente invención se refiere a una endoprótesis de cadera completas consistentes en la inserción en el fémur de un vástago de la prótesis con un cóndilo. Así, el hueso pélvico recibe el acetábulo que sirve de cavidad de sujeción para el cóndilo mediante implantación de prótesis perfeccionada en base a su movimiento de rotación mediante rodamientos y sistema de fijación al hueso. No obstante, la misma también es aplicable a la articulación de los hombros u otras partes del cuerpo donde es necesario mantener un movimiento de rotación.
ANTECEDENTES EN EL ESTADO DE LA TÉCNICA
La articulación de la cadera es una de las más grandes del cuerpo y soporta el peso del mismo. Es una articulación que consta de una cavidad (acetábulo) y una bola (cabeza del fémur) envueltas por una cápsula que sella y estabiliza la articulación. Esto ayuda a mantener la cadera estable al girar y al hacer otros movimientos, incluso en rangos extremos. Una cadera sana le permite caminar, agacharse y girarse sin dolor.
La articulación de la cadera se forma donde el extremo redondeado del hueso del muslo (cabeza del fémur) se junta con el hueso de la pelvis (acetábulo). La articulación está cubierta por tejidos y su movimiento impulsado por unos músculos potentes. Cuando todas estas partes están sanas, la cadera deberá moverse con facilidad.
• El cartílago es una capa de tejido suave que cubre la cabeza del hueso del muslo (fémur) y también la cavidad del hueso de la pelvis (acetábulo). Un cartílago sano absorbe la tensión y permite que la cabeza del fémur se mueva fácilmente en la cavidad de la pelvis.
• Los músculos impulsan el movimiento de la cadera y las piernas.
• Los tendones sujetan los músculos a los huesos.
La cadera es una de las articulaciones de carga de peso más grandes del cuerpo. Como se trata de una articulación esférica, la cadera puede permanecer estable cuando el cuerpo se inclina, gira o efectúa movimientos extremos. Una cadera sana le permite caminar, agacharse y girar sin dolor; pero si se lesiona, es probable que le cause dolor al moverse. Para reemplazar una cadera natural se emplea una prótesis.
En una cadera sana, los extremos del fémur y la pelvis (zona de unión entre la cadera y el fémur) están recubiertos de cartílago liso. Esto permite que la esfera se deslice sin problemas dentro de la cavidad. Cuando los músculos cercanos soportan el peso y las articulaciones se mueven libremente, es posible caminar sin dolor
Por el contrario, en una cadera dañada, el cartílago desgastado ya no puede servir de amortiguador. A causa de la fricción constante, los huesos ásperos adoptan una forma que se vuelve irregular y su superficie tiene el aspecto de papel de lija. La cabeza del fémur roza contra el acetábulo al mover la pierna, causando dolor y rigidez.
Atendiendo al estado de la técnica en la materia, las prótesis consisten en una esfera artificial que sustituye a la cabeza del fémur, y una copa artificial ocupa el lugar del acetábulo. Se inserta un vástago dentro del hueso, para darle estabilidad. Estas piezas se conectan entre sí para crear su nueva cadera artificial. Las piezas tienen superficies lisas en las zonas que articulan entre si, facilitando el movimiento y reduciendo el desgaste. En las zonas que se unen al hueso pueden tener una superficie rugosa, para que el hueso agarre la prótesis.
Se corta la cabeza del fémur (hueso del muslo) y se prepara la cavidad de la pelvis (la copa o acetábulo) con fresas de hueso semiesféricas, dejando una cavidad donde se va a colocar la copa de la prótesis (cotilo). Generalmente, el cotilo se encaja a presión en la pelvis pero a veces se mantiene en su sitio con tornillos o cemento. Las prótesis de encaje a presión contienen diminutos poros en la superficie, que permiten al hueso crecer en su interior. Se conocen como prótesis con recubrimiento poroso o no cementadas, por contraposición a las prótesis cementadas, que se fijan al hueso con cemento óseo.
En la parte superior del fémur se introduce el nuevo vástago de la articulación de cadera. Una vez que el vástago esté bien fijo, se le acopla la "bola o cabeza de la prótesis" que articula con el cotilo de la prótesis (la nueva copa o cavidad). También el vástago de la prótesis puede fijarse con cemento, o bien ser de encaje a presión recubierto por un poro en el que va a crecer el hueso.
Sin embargo, las técnicas descritas presentan los siguientes inconvenientes;
• Al llevar a cabo un vaciado del hueso y posterior relleno con cemento éste queda debilitado. Si, posteriormente el paciente padece una necrosis en la zona, la única solución es cortar toda la parte afectada, dificultando, así mismo, las posibilidades de recibir una nueva prótesis. De hecho, si esto ocurre más de dos veces, ya no es posible llevar a cabo un nuevo implante.
• La esfera de la prótesis se montan en contacto directo con el hueso receptor de forma que el rozamiento o fricción es muy grande generando habitualmente molestias y dolores al paciente y el correspondiente desgaste del citado hueso.
• En ocasiones, el implante se sale de su emplazamiento provocando una luxación muy dolorosa y perjudicial para el paciente.
A partir de la problemática identificada la "Prótesis de cadera perfeccionada" presenta respecto al estado de la técnica las siguientes ventajas;
• El vástago de la prótesis que se introduce en el hueso se dispone según un tornillo tipo "sacacorchos" evitando la operación previa de vaciado del hueso y los consiguientes inconvenientes anteriormente comentados.
• Así mismo, al fijarse enroscado dentro del hueso, en caso de necrosis u otra eventualidad, es posible retirar la prótesis sin necesidad de dañar el hueso o tener que cortarlo.
• En la parte superior del vástago de la prótesis, antes de unirse a la esfera existe una estructura rectangular o casquillo de sujeción de longitud variable en función del tamaño del hueso,
que por su parte inferior es hueca, permitiendo la entrada del hueso, pero agarrándola y fijándola por el exterior de manera que la fijación quede mucho más sólida, evitando muchas roturas del hueso en esta zona y unos movimientos más cómodos por parte del paciente.
• La parte superior esférico o cóndilo se conforma según una esfera que se mueve en el interior de un casquete esférico a través de unos rodamientos que evitan los desgastes por fricción del hueso anteriormente mencionados. Mientras que la parte exterior del casquete esférico dispone de unos filamentos, los cuales se introducen en el hueso de la cadera quedando fijada a ésta y evitando el rozamiento y los riesgos de luxación.
• La articulación mediante rodamientos, posee un sistema de ajuste entre los elementos que conforman el recinto que garantizan un apriete adecuado.
EXPLICACIÓN DE LA INVENCIÓN
La "Prótesis de cadera perfeccionada" que la que la invención propone resuelve de forma plenamente satisfactoria la problemática anteriormente expuesta, de manera que se constituye en una configuración sobre un eje según un sistema de fijación al fémur a modo de tornillo con terminación en clavo que se introduce en el fémur y concluye su fijación con el asiento del casquillo rectangular sobre la superficie exterior del referido hueso y al que se une solidariamente el cóndilo.
Mientras que el cóndilo dispuesto según una configuración esférica comporta junto con el casco esférico en contacto con el hueso pélvico y la tapa roscada de cierre, el emplazamiento en el cual se ubican los
rodamientos que garantizan el movimiento de rotación entre los citados huesos.
Por último el citado casco esférico en contacto con el hueso pélvico se complementa con unos filamentos de fijación al citado hueso.
DESCRIPCIÓN DE LOS DIBUJOS
Para complementar la descripción que se está realizando y con objeto de ayudar a una mejor comprensión de las características de la invención, de acuerdo con un ejemplo preferente de realización práctica de la misma, se acompaña como parte integrante de dicha descripción, un juego de dibujos en donde con carácter ilustrativo y no limitativo, se ha representado lo siguiente:

En las citadas figuras se pueden destacar los siguientes elementos constituyentes;

2. Hueso pélvico
3. Cavidad acetabular.
4. Tornillo que se introduce en el fémur.
5. Casquillo de asiento final del tornillo sobre el fémur.
6. Cóndilo.
7. Casco esférico en contacto con el hueso pélvico
8. Tapa de cierre emplazamiento de rodamientos.
9. Junta tórica.
10. Rodamientos.
1 1 . Filamentos de fijación al hueso pélvico.
12. Refuerzo de unión entre tornillo y cóndilo.
13. Taladros para uso de llave de apriete.
14. Útil para uso de martillo en enclavamiento del conjunto.
15. Pieza de titanio en sustitución de cabeza de fémur.
EJEMPLO DE REALIZACIÓN PREFERENTE
A modo de ejemplo de realización preferente de "Prótesis de cadera perfeccionada", este se puede fabricar en titanio y ser utilizada para llevar a llevar a cabo una sustitución de la cabeza del fémur (1 ) en su articulación con el hueso pélvico (2) tal y como se aprecia en la Figura 1 y Figura 2.
Así la prótesis objeto de invención, se aloja sobre la cavidad acetabular (3) y se constituye por su elemento de fijación al fémur a modo de tornillo (4) con terminación en clavo que se introduce en el fémur (1 ) y concluye su fijación con el asiento del casquillo (5) sobre la superficie exterior del referido hueso unido solidariamente al cóndilo (6).
Mientras que el cóndilo (6) dispuesto según una configuración esférica comporta junto con el casco esférico (7) en contacto con el hueso pélvico y la tapa roscada de cierre (8) provista de su correspondiente junta tórica (9), el emplazamiento en el cual se ubican los rodamientos (10) en acero inoxidable que garantizan el movimiento de rotación entre los citados huesos.
Por último el citado casco esférico (7) en contacto con el hueso pélvico (2) se complementa con unos filamentos (1 1 ) de dos milímetros de diámetro y longitud adecuada para garantizar la fijación al citado hueso.
Por último, la figura 3, figura 4 y figura 5 se muestran las correspondientes llaves, útiles o accesorios que facilitan los procedimientos de unión o fijación de los diferentes elementos que intervienen.
A modo de realización alternativa tal y como se aprecia en la figura 6, en ocasiones existe un deterioro oxeo tal en la zona de acceso al fémur, que es necesario que el tornillo (4) se conforme según dos ejes a través de una pieza intermedia en sustitución de la cabeza del fémur (15) para garantizar el acceso al hueso en una zona sana o que proporcione la suficiente resistencia.
No se considera necesario hacer más extensa esta descripción para que cualquier experto en la materia comprenda el alcance de la invención y las ventajas que de la misma se derivan. Los materiales empleados, dimensiones, soluciones técnicas propuestas para garantizar la fijación del casquillo (5), el ajuste de la tapa de cierre (8)
o su aplicación serán susceptibles de variación siempre y cuando ello no suponga una alteración en la esencialidad del invento.
Claims
1. - "Prótesis de cadera perfeccionada" caracterizado por los elementos que conforman su sistema de fijación al fémur según un tornillo con terminación en clavo y provisto de terminación en casquillo concebido para su fijación sobre la superficie exterior del referido hueso y al que se une solidariamente el cóndilo.
2. - "Prótesis de cadera perfeccionada" según reivindicación 1 , caracterizada por los elementos que conforman la articulación haciendo uso de rodamientos según un cóndilo dispuesto en una configuración esférica, una envolvente exterior o casco esférico en contacto con el hueso pélvico y la tapa roscada de cierre hasta conformar un emplazamiento en el cual se ubican los rodamientos que garantizan el movimiento de rotación entre los citados huesos.
3. - "Prótesis de cadera perfeccionada" según reivindicación 1 y 2, caracterizado porque su casco esférico o envolvente en contacto con el hueso pélvico se complementa con unos filamentos de fijación al citado hueso.
4. - "Prótesis de cadera perfeccionada" según reivindicación 1 , 2 y 3 caracterizada porque alternativamente el conjunto puede conformarse según dos ejes principales a través de una pieza intermedia en sustitución de la cabeza del fémur (15).
