- the portable terminal device 11 instructs the data center 14 to take medication schedule information for each prescription drug. Request to send.
- the data center 14 performs a medication schedule generation process in response to a request from the portable terminal device 11 to generate medication schedule information, and uses information indicating the medication period (medication schedule) of each prescription drug as medication schedule information. It transmits to the portable terminal device 11.
- the medication schedule generation process is performed for each dispensing receipt information recorded in the recording unit 44, for example.
- step S11 the extraction unit 51 reads the dispensing receipt information from the recording unit 44 and performs an analysis process on the read dispensing receipt information, so that the prescription date of each prescription for each medicine (prescription drug) from the dispensing receipt information, Extract dispensing date, prescription days, and dosage form code.
- the dispensing quantity shown in FIG. 4 is extracted as the number of prescription days.
- step S12 the identifying unit 52 sets internal medicines, infusions, and hot water medicines to be processed among the medicines (prescription medicines) for which information is extracted from the dispensing receipt information. That is, a pharmaceutical product whose dosage form code is “1”, “6”, or “7”, that is, a pharmaceutical product whose dosage form is any of internal use, soaking, or hot water, is a pharmaceutical product to be processed.
- step S13 the medication schedule generation unit 53 obtains the medication period of each pharmaceutical product to be processed based on the dispensing date and the number of prescription days for each pharmaceutical product to be processed.
- the medication schedule generation unit 53 sets the medication start date as Da and the medication end date as Da + Ma-1, from Da to Da + Ma-1.
- This period is defined as the period of medication (medication schedule).
- the dispensing date is the medication start date
- the prescription date may be the medication start date. This is because the prescription date and the dispensing date are often the same day.
- the number of medicines for internal medicine, decoction, and hot water medicine is taken as the number of days taken, taking the number of days taken as a unit, so the number of medicines obtained from the dispensing receipt information, that is, the number of days prescribed and the actual number of days taken In most cases. Therefore, for medicines such as internal medicines, decoction, and hot water medicines, an almost accurate medication schedule can be obtained from the dispensing date and the number of prescription days of those pharmaceuticals obtained from the dispensing receipt information.
- the effects of taking medicines last for several days.
- a drug is referred to as a long-acting drug, and the drug effect of this long-lasting drug is sustained for, for example, 7 days.
- the effect-maintaining drug is prescribed for two weeks, the number of prescription days will be two days, and there will be a difference between the medication period obtained from the prescription days and the actual medication period.
- the medication schedule generation unit 53 takes the drug as a prescription day of the drug, that is, the number of days to be taken is the same as the prescription days of the other drug to be processed. Ask for a period. Whether or not a drug is a sustained-effect drug can be identified by division based on the number of days for prescription of the drug and the number of days for prescription of another target drug.
- drug A is a drug taken every day
- drug B is a sustained effect drug taken once a week
- these drug A and drug B are prescribed for two weeks.
- the prescription days Ma 14
- the drug B is considered to be a sustained effect drug
- the number of days of drug B taken is set to Ma.
- the pharmaceutical B is a long-acting drug even by determining whether other conditions are satisfied. It is possible to specify whether it exists.
- some medicines gradually decrease or gradually increase the prescription amount, and such medicines are prescribed in multiple prescriptions and dispensed on the same dispensing day.
- the prescription amount of the pharmaceutical gradually decreases because the dispensing amount is not added by the gradual reduction therapy. Therefore, in such a case, what is necessary is just to make the sum of the prescription days of each prescription of a pharmaceutical into an actual medication day. Also, if the medication timings indicated by the usage codes overlap for different prescriptions, the drugs for those prescriptions should be taken on different days, so the sum of the prescription days for each prescription is actually The number of days of medication
- the medication schedule generation unit 53 specifies that the same medicine divided into a plurality of prescriptions on the same dispensing day is to be taken on different days, the sum of the prescription days of those prescriptions is calculated as the actual number of prescriptions. You may make it be a medication day.
- the medication schedule generation unit 53 obtains the medication period of each pharmaceutical product to be processed based on the dispensing date and the number of prescription days of each pharmaceutical product to be treated.
- the medication period obtained in step S13 is also referred to as a medication period Ta.
- the medicines to be processed in the process of step S12 that is, internal medicines, decoction, and hot water, are also referred to as pre-processing medicines.
- the identification unit 52 treats medicines other than the internal medicine, the decoction, and the hot water medicine among the medicines (prescription medicines) for which information is extracted from the dispensing receipt information.
- a pharmaceutical product whose dosage form code is “2”, “3”, “4”, “5”, or “9”, that is, the dosage form is an internal drop, take-over (medication), injection Drugs that are either topical, or material, are treated as pharmaceuticals.
- step S ⁇ b> 15 the medication schedule generation unit 53 uses the medication period Ta of the pre-processing target pharmaceutical that is the processing target in the processing of step S ⁇ b> 12 for each processing target pharmaceutical to determine the medication period of each processing target pharmaceutical Ask.
- the medication schedule generation unit 53 takes the longest period of the medication period Ta of the pre-processing target pharmaceutical as the medication period of the processing target pharmaceutical as it is.
- the medication medication preparation date is set as the medication start date of the medicine to be treated
- the medication end date of the medication period Ta is You may make it be a medication end date.
- the dispensing date of the medicine to be processed is set as the start date of taking
- the number of days of the taking period Ta is set as the number of taking days
- the period determined from the start date and the number of taking days is set as the taking period of the medicine to be processed. Good.
- a medicine such as a topical medicine to be processed in the process of step S14 is often set to “1” because the medicine is handled in a certain amount. That is, the prescription days indicated by the dispensing quantity often do not match the actual days taken.
- these drugs are often prescribed almost at the same time as other drugs such as internal medicine.
- internal medicines and external medicines may be prescribed in one examination.
- an internal medicine was prescribed in a single examination, but when a patient took the internal medicine, symptoms such as itchiness occurred, so that the patient was examined again and an external medicine was prescribed.
- a medicine such as an external medicine to be processed in the process of step S14 may be prescribed almost at the same time as a medicine such as an internal medicine to be processed in the process of step S12.
- the actual period for taking medicines such as external medicines is almost the same as the period for taking other medicines such as internal medicines prescribed at the same time.
- the medication schedule generation unit 53 assumes that the medication period of the pharmaceutical product such as the external medicine that was processed in step S14 is the same as the medication period Ta of the pre-processed pharmaceutical product that was prescribed almost simultaneously. Eliminates the discrepancy between prescription days and medication days that occur for drugs such as
- each prescription date (dispensing date) is within a specific period such as one month or one week. I just need it. That is, the medication period of the pharmaceutical product to be processed in step S15 may be the medication period of the pre-processed pharmaceutical product dispensed within a specific period including the dispensing date of the pharmaceutical product.
- the medication start date is before the medication date of the drug to be treated and from the medication date of the medication to be treated.
- the medication period Ta of the medicinal product starting date closest may be the medication period of the medicinal product to be processed.
- the above-mentioned specific period may be any period, but in addition to the period within the same receipt, for example, the specific period is effective for one month or one week.
- step S15 When the medication period (medication schedule) of the medicine to be processed is obtained as described above, the process proceeds from step S15 to step S16.
- step S16 the medication schedule generation unit 53 uses the medication schedule information as information indicating the medication period of each pharmaceutical obtained in the processing of steps S13 and S15, and the medication schedule generation processing ends.
- the medication schedule information generated by the medication schedule generation unit 53 is transmitted to the portable terminal device 11 by the communication unit 41. Thereby, the portable terminal device 11 can use the medication schedule information received from the data center 14 for various application programs.
- the medication schedule information obtained at the data center 14 is used for detection of contraindications for medication, detection accuracy can be improved as compared with the conventional case.
- a prescribed internal medicine and isoprenaline sulfate which is a topical medicine, are prescribed for 30 days, and the dispensing quantity of isoprenaline sulfate in the receipt is “1”, that is, the number of prescription days is Suppose that it is taken as one day. Also, assume that dl-methylephedrine hydrochloride, which is contraindicated with isoprenaline sulfate, is prescribed to patients on December 13.
- the prescription period of isoprenaline sulfate was conventionally only December 1, it was not possible to detect a contraindication with dl-methylephedrine hydrochloride.
- the period of taking isoprenaline sulfate is the same period as that of the internal medicine prescribed on the same day, that is, the period from December 1 to 30. Therefore, with the dl-methylephedrine hydrochloride It will be possible to detect a drink and issue a warning.
- the data center 14 obtains the medication period Ta of a pre-processed pharmaceutical product that is a predetermined dosage form, and calculates the medication period of a pharmaceutical product of another dosage form using the medication period Ta. .
- the user's (patient) medication behavior itself can be predicted.
- medication behavior can be visualized, for example, in addition to precautions for contraindication of medication intended by the medication notebook, detection of duplicate or overdose medication, health indications due to changes in medication status, etc. It is also possible to perform.
- the obtained medication schedule as a parameter when performing adaptive processing such as information selection, archiving, compression, etc. in cooperation and home nursing between medical institutions, which will be important in the future. it can.
- the information to be extracted may be included in a format different from the case of the dispensing receipt information, so that a proper replacement process is required.
- a medical identification code is acquired as information about the dosage form of the medicine.
- the medical treatment identification code also includes a code indicating the dosage form of the drug dispensed as shown in FIG. For example, when the administered drug is an internal medicine, the medical treatment identification code is “21”, and when the medicine is a non-medication (medicated medicine), the medical treatment identification code is “22”. In addition, when the administered medicine is an external medicine, the medical identification code is “23”.
- the medication schedule generation processing is performed in the data center 14 in the data center 14 in the above.
- the medication schedule generation processing is performed by the portable terminal device 11 or another device to generate medication schedule information. You may be made to do.
- the medication schedule information is information indicating only the medication schedule of each medicine.
- the medication schedule information not only the medication schedule but also the medication probability on each medication date, that is, the patient (user). Information indicating the probability of taking the medicine may also be included.
- step S41 and step S42 is the same as the process of step S11 and step S12 of FIG. 5, the description is abbreviate
- step S43 the medication schedule generation unit 53 obtains the medication period and the medication probability of each pharmaceutical product to be processed, for each pharmaceutical product to be processed, based on the dispensing date and the prescription days of the pharmaceutical product.
- the medication schedule generation unit 53 performs the same process as step S13 in FIG. 5 to obtain the medication period Ta of each pharmaceutical product. Then, for example, for each medicine, the medication schedule generation unit 53 sets the medication probability of the medication in the medication period Ta to “1” and the medication probability in periods other than the medication period Ta to “0”.
- step S44 the identifying unit 52 sets a topical medicine among the medicines (prescription medicines) for which information is extracted from the dispensing receipt information.
- a medicine whose dosage form code is “5” is a medicine to be processed.
- step S ⁇ b> 45 the medication schedule generation unit 53 obtains the medication period of each pharmaceutical product to be processed using the medication period Ta of the pharmaceutical product to be processed, and sets the medication probability of those pharmaceutical products to a predetermined constant value. .
- the medication schedule generation unit 53 sets the one having the maximum length of the medication period Ta of the pre-processing target pharmaceutical as the medication period of the processing target pharmaceutical as it is.
- the medication schedule generation unit 53 sets the medication probability in the medication period Ta of the drug to be processed to a certain constant K, and sets the medication probability in a period other than the medication period Ta to “0”, thereby obtaining a flat probability distribution.
- the constant K which is taken as the probability of medication, is a value of 0 or more and 1 or less, and is determined, for example, for each drug or each drug efficacy classification.
- step S46 the identifying unit 52 treats a non-compliance medicine among the medicines (prescription medicines) for which information is extracted from the dispensing receipt information.
- a medicine whose dosage form code is “3” is a medicine to be processed.
- step S47 the medication schedule generation unit 53 obtains the medication period of each pharmaceutical product to be processed using the medication period Ta of the pharmaceutical product to be processed, and gradually decreases the medication probability of those pharmaceutical products to a predetermined constant value or gradually. Value.
- the medication schedule generation unit 53 sets the one having the maximum length of the medication period Ta of the pre-processing target pharmaceutical as the medication period of the processing target pharmaceutical as it is.
- the medication schedule generation unit 53 calculates the medication probability in the medication period Ta of the medicine to be processed, for example, by calculating the following formula (1).
- K is a constant K which indicates a standard dose probability, for example, a dose probability value of a topical drug
- N indicates a prescription amount (number of prescription days) of the drug to be processed.
- A represents the number of days of the medication period Ta
- F (x) represents a gradual decreasing function where x is the number of days from the prescription date of the drug to be processed. Note that the gradual decrease function F (x) may be a constant.
- the value of the medication probability for each date of the medication period obtained by the formula (1) gradually decreases from the medication start date to the medication end date according to the prescription amount N and the gradual decrease function F (x). .
- the medication probability of the medicine in a period other than the medication period is “0”.
- the gradual decrease function F (x) is a constant, the medication probability during the medication period of the drug to be processed has a constant value.
- step S48 the identifying unit 52 sets a medicine that has not yet been processed among the medicines (prescription drugs) whose information has been extracted from the dispensing receipt information.
- a medicine whose dosage form code is “2”, “4”, or “9” is a medicine to be processed.
- step S49 the medication schedule generation unit 53 obtains the medication period of each pharmaceutical product to be processed using the medication period Ta of the pharmaceutical product to be processed, and sets the medication probability of those pharmaceuticals to “1”.
- the medication schedule generation unit 53 sets the one having the maximum length of the medication period Ta of the pre-processing target pharmaceutical as the medication period of the processing target pharmaceutical as it is.
- the medication schedule generation unit 53 sets the medication probability in the medication period of the drug to be processed to “1” and the medication probability in other periods to “0”.
- the medication period and the probability of medication of the pre-processed pharmaceutical are used as they are as the medication period and the medication probability of the drug whose dosage form is an internal drop, injection, or material. This is because the number of medicines that are injections or materials is small. According to the applicant's survey, it is known from the number of prescriptions for each dosage form that the number of internal medicines, external medicines, and non-compliance medicines accounts for about 99.5% of the total number.
- step S50 the medication schedule generation unit 53 uses the medication schedule information as information indicating the medication period and medication probability obtained in the processing of step S43, step S45, step S47, and step S49, and the medication schedule generation processing is performed. finish.
