Abstract
본 발명은 청미래덩굴의 뿌리줄기인 토복령 추출물을 유효성분으로 함유한 알러지성 질환에 효과적인 약학조성물, 화장료 조성물 및 기능성 식품조성물에 관한 것으로서, 보다 구체적으로는 극성용매에 가용한 토복령 추출물을 함유함으로써 전 세계적으로 발병율이 증가하고 있는 아토피성 피부염을 포함한 알러지성 질환에 효과적인 약학조성물, 화장료 조성물 및 기능성 식품조성물에 관한 것이다. 상기와 같은 본 발명에 따르면, 토복령 단일 추출물을 유효성분으로 함유한 약학조성물, 화장료 조성물 및 기능성 식품조성물을 제공함으로써, 장기간 투여에도 부작용 및 내성과 독성이 없으며, 면역질환의 발생에 중요한 역할을 하는 혈청 중의 IgE, Th2 및 Th1의 사이토카인 농도 감소, 비만세포(mast cell)의 히스타민 및 염증 유발성 활성물질의 분비 억제, 이들을 매개로 한 면역성 과민반응의 억제, 피부염 지수 감소, 피부조직의 비만세포(mast cell) 침윤 및 활성화 감소 등을 통하여 알러지성 염증반응의 발생 및 만성적 염증상태를 감소시키는 효과가 있다.

본 발명은 청미래덩굴의 뿌리줄기인 토복령 추출물을 유효성분으로 함유한 알러지성 질환에 효과적인 약학조성물, 화장료 조성물 및 기능성 식품조성물에 관한 것으로서, 보다 구체적으로는 극성용매에 가용한 토복령 추출물을 함유함으로써 전 세계적으로 발병율이 증가하고 있는 아토피성 피부염을 포함한 알러지성 질환에 효과적인 약학조성물, 화장료 조성물 및 기능성 식품조성물에 관한 것이다.
알러지성 질환은 아토피성 질환이라고도 하는데, 전신성 과민증(anaphylaxis), 알러지성 비염 또는 고초열(allergic rhinitis, hay fever), 아토피성 피부염(atopic dermatitis, eczema), 알러지성 천식(allergic 또는 atopic asthma) 및 식품 알러지(food allergy) 및 두드러기(urticaria) 등이 있으며, 개발국가 국민의 약 25%가 고통당할 정도로 전 세계적인 질병이며 산업화로 인한 환경 악화로 그 유병율이 매년 증가하고 있다(SJ Galli 등, Nature. 454: 445-454, 2008). 최근 알러지성 질환이 급증하고 있으며 이에 대한 많은 연구가 수행되고 있다.
아토피성 염증질환 중에서도 아토피성 피부염은 전 세계적으로 소아의 10~20%, 성인의 1~3%의 유병율을 갖고 있으며 점차 발병율이 증가하고 있는 추세이며, 심한 소양증과 흉한 피부병변으로 인해 불쾌하고 괴로움을 야기하는 만성 염증성 피부질환이다. 이 질환은 유전적 질환으로서 경미한 피부 자극이 가려움으로 인지되고 긁거나 비비게 되면 증가된 면역 반응을 유발하게 되며 증가된 면역 반응은 가려움증, 피부색조, 소포진, 피부박리, 피부비늘화, 피부삼출, 피부껍질생성, 피부주름현상, 피부박리, 피부건조증, 건피증, 모공각피증, 백색피부묘화증 등의 다양한 특징을 갖는 염증성 피부 상태인 피부염을 발생시켜 정신적 문제까지 야기하는 질병이다(Abramovits W. J. Am. Acad. Dermatol. 53: S86-93, 2005).
아토피성 피부염을 비롯한 다양한 알러지성 염증질환은 최초 노출 알러젠에 대한 감작(sensitization)으로부터 시작되어 동일 알러젠에 대한 2차 노출 후 발생하는 3개의 연속된 경시적 단계, 즉 초기 반응(early-phase reactions), 지연 반응(late-phase reactions) 및 만성적 알러지성 염증상태로 진행한다. 감작기에는 알러젠에 의해 Th2-세포 반응이 유도되고, Th2-세포가 생성한 IL-4 및 IL-13과 같은 사이토카인에 의해 매개되는 체액성 면역이 우세한 조건에서 B 임파구로부터 면역글로불린 유형전환 재조합기작에 의해 많은 면역글로불린 이형(isotype) 중 하나인(Coffman RL. et al., J. Immunol . 136, 949-64, 1986) IgE가 생성된다(Maggi E., Immunotechnology , 3 , 233-244, 1998). 환자의 유전자형, 알러젠 유형 및 농도, 알러젠과 면역계의 접촉 유형(양, 빈도, 경로) 등의 요인들이 Th2-세포 반응의 강도와 다양성(생성 항체 유형, 면역학적 내성 등)에 영향을 준다(Galli Stephen J., Nature 454:445-454, 2008).
초기 반응은 1형 즉시형 과민반응이라고도 하는데, 알러젠에 대한 2차 노출 후 수분내 발생하며 주로 비만세포(mast cell)에 의해 분비된 다양한 염증매개물질에 의해 발생한다. 즉, 감작이 발생한 환자는 감작에 의해 이미 생성된 알러젠-특이적 IgE가 비만세포의 세포막에 있는 고친화성 IgE receptor(수용체)인 FcεRI에 결합되어 있으며, 이들 IgE가 재침입 알러젠에 결합해 교차연결되고 수용체의 응집현상은 복잡한 세포내 신호전달 과정을 활성화하여 히스타민, 사이토카인, 단백질분해효소 등의 세포내 과립 저장 물질, prostaglandins, leukotrines, PAF 등의 지질유래 물질, 사이토카인, chemokines, 성장인자 등과 같은 유전자발현 생성물 3군의 다양한 생물학적 활성물질을 분비한다. 초기반응과 관련된 급성 증상들로 반응 발생 부위에 따라 알러지성 비염에서 급성 비결막염, 급성천식발작, 음식 알러지에서 두드러기, 위장관 반응들을 유발하고, 혈관확장, 피부 또는 결막의 홍반, 혈관투과성 증대, 조직 부종, 기관지평활근 수축, 통증감각신경자극에 의한 재치기, 가려움증, 기침, 급성적 기능변화(기도 점액분비, 두드러기, 구토, 설사 등) 등 다양한 염증반응을 유발한다. 또한, 3군의 다양한 생물학적 활성물질의 전신성 분비는 anaphylaxyis(전신성 과민증)를 유발한다.
지연기 반응은 알러젠에 대한 2차 노출 후 2~6시간 후 발생하며 6~9시간에 정점에 도달한다. 초기반응이 반드시 수반되고 1~2일 후 소실된다. 지연기 반응은 IgE 및 알러젠에 의해 활성화된 주거성 비만세포(resident mast cell)나 알러젠으로부터 유도된 peptides를 인식하는 T cell들로부터 분비된 다양한 생리활성 물질들이 Th2 세포, 호산구(eosinophils), 호염구(basophils), 호중구, 단핵구 및 기타 백혈구들을 직간접적으로 유인(TNF-α, LTB4, IL-8, CCL2 등)하고 활성화(TNF-α, IL-5)하며, 비만세포와 같은 주거성 세포들이 지속적으로 생리활성 물질들을 생성시켜 발생한다. 지연기 반응의 임상적 특징은 주거성 세포들과 알러젠 노출 부위로 유인된 이러한 순환성 백혈구들의 활성화 결과이며, 아토피성 피부 및 천식의 하부 기도에 발생한 지연기 반응에 유인된 백혈구는 T 세포(지연반응 초기단계에서는 Th2 세포, 후기단계에서는 Th1 세포들이 사이토카인 환경의 변화에 기여), 과립구인 호산구, 호중구 및 호염구, 또한 단핵구 등으로서 알러지성 염증 발생에 기여한다. 지연기 반응은 피부에 부종, 통증, 작열감, 홍반을 유발하고 폐에서는 기도수축과 점액과다분비 등을 유발한다. 이러한 지연기 반응은 감지할 만한 초기 반응 없이 발생하는 환자에서는 IgE 보다는 항원-특이적 T 세포와 항원을 포함하는 면역 복합제에 의해 활성화된 골수 세포들에 의해 발생될 수 있다.
만성적 알러지성 염증상태는 특정 알러젠에 장기간 또는 반복적으로 노출될 때 항원-유도된 초기 반응 및 지연기 반응을 많이 겪으면서 지속적인 염증이 유발된다. 많은 수의 선천성 및 적응성 면역세포들(백혈구 형태)이 알러젠 노출부위에 존재하고 해당 부위 조직의 세포외 기질의 현저한 변화와 조직을 구성하는 세포들의 수, 표현형 및 기능들이 변화되는 특성을 보인다. 만성적 알러지성 염증상태는 아토피성 피부염, 알러지성 비염, 천식 등에서 영향을 받은 부위의 조직구성 요소의 장기적 변화(혈관생성 증가 등), 상피세포 장벽의 심한 변화와 같은 병변 조직의 재구성(tissue remodelling), 이로 인한 기능적 변화를 유발한다(Leung D.Y. 등, J. Clin. Immunol., 113: 651-657, 2004; Pawankar, R. 등, Immunol. Allergy Clin. North Am. 24: 75-85, 2004, Stephen J Galli & Mindy Tsai, Nature Medicine, 18:693-704, 2012). 즉, 알러지성 비염에서의 비강 폴립 생성 및 상기도 장벽 손상에 의한 만성 감염 위험성 증대, 아토피성 피부염에서의 피부장벽의 기능손상으로 인한 피부의 세균감염 위험 증가 등이 발생한다(Galli Stephen J., Nature 454:445-454, 2008). 대부분의 알러지성 질환자들이 알러젠에 만성적으로 노출되기 때문에 이러한 만성적 알러지성 염증상태 발생 및 유지 기작에 대한 보다 상세한 연구가 필요하다.
상기와 같이, 알러지성 질환은 type 2 T 헬퍼(Th2) 세포의 활성화와 이로부터 분비된 IL-4, 5, 13 등의 사이토카인에 의해 생성된 IgE에 의해 매개되는데, IgE에 의해 비만세포로부터의 분비된 3 유형의 다양한 생리활성 물질들의 작용에 의해 초기 반응, 지연기 반응, 만성 염증성 상태로 진행되어 아토피성 피부염을 비롯한 다양한 알러지성 질환을 발생시키며, 이 과정에서 호산구, 호중구, 호염구 등의 백혈구가 유인되고 이들로부터 분비된 다양한 물질들이 중요한 역할을 한다(Maggi E., Immunotechnology, 3: 233-244, 1998; Vercelli D., Clin. Allergy Immunol ., 16: 179-196, 2002; Pawankar R., Curr. Opin. Allergy Clin. Immunol., 1 : 3-6, 2001).
알러지성 염증질환의 발생 및 악화에는 상기 언급한 것 이상으로 IgE와 비만세포가 상호의존적으로 또는 상호비의존적으로 급성 및 만성 알러지성 염증의 발생에 기여하고 있다.
아토피성 피부염과 같은 단일 알러지성 질환만을 갖고 있는 많은 환자들은 궁극적으로는 알러지성 비염과 알러지성 천식 등 다른 알러지성 질환도 복합적으로 발생하는데 이러한 현상을 알러지 행진 또는 아토피 행진(allergic 또는 atopic march)이라 한다. 이러한 알러지 행진 과정은 알러지성 염증이 상피 장벽기능을 감소시켜 최초 알러젠과 추가적 알러젠들에 대한 면역계의 노출을 증가시키고, 최초의 감작반응에 의해 생성되어 이미 존재하고 있는 알러젠-특이적 IgE가 새로운 알러젠들에 대한 감작에 기여하기 때문이다. IgE 고친화성 세포막 수용체인 FcεRI와 저친화성 수용체인 CD23를 발현한 항원-제시 세포들(APCs, antigen-presenting cells: Langerhans 세포, 수지상세포, B 세포 등)이 이들 수용체와 결합된 알러젠-특이적 IgE를 통해 최초 감작시의 알러젠을 세포내로 도입 처리하거나 새로운 알러젠을 동시에 처리하여 다양한 알러젠 에피토프들(epitope) 및 새로운 알러젠에 특이적인 Th2 세포 생성, 증식 및 활성화를 통해 다양한 구조의 IgE들을 생성시키는 에피토프 확산(epitope spreading)을 유발한다. 이렇게 증가된 국소적 또는 순환성 IgE는 비만세포와 호염구 세포막에 FcεRI 수용체의 발현을 증가시켜 IgE-의존적 알러지성 질환 발생 및 악화를 유발하는 효과기 기능(effector function)을 증강시키고, 궁극적으로 즉시형 반응, 지연기 반응, 만성 알러지성 염증이란 악화성 회로(vicious cycle)가 반복되어 알러지성 염증질환이 더욱 심해지게 된다(Stephen J. Galli 등, Nature 454: 445-454; 2008, Stephen J Galli & Mindy Tsai, Nature Medicine, 18:693-704, 2012).
또한, 어떤 IgE 항체 분자들은 고친화성 수용체에 결합한 후 항원-비의존적인 응집을 통해 항원 부재시도 비만세포로부터 생리적 활성물질을 분비시켜 알러지 질환의 염증상태를 악화시킬 수 있다. IgE-비의존적이고 항원-비의존적인 호염구의 활성화는 비만세포와 무관하게 천식환자의 기도에서 발생하여 알러지성 염증에 기여한다.
한편, 비만세포(mast cells)는 전통적인 IgE-FcεRI 경로를 통한 비만세포 신호전달과정과 상관없이 IgE 비의존적으로 만성 알러지 염증 및 조직 재구성을 유발하는 기작에도 관련되어 있어 알러지 염증질환 발생 및 악화에 크게 기여한다. 예를 들면, IFN-γ, S1P, adenosine, IL-33 등과 같은 용해성 인자들과 Th2 세포와 조절성 T 세포(Treg cells: 비만세포에 있는 OX40L과 상호작용)와 같은 질환 부위에 있는 세포들은 IgE-의존적 비만세포 활성화를 조절하고, 어떤 병원체-관련 분자들(PAMPs)과 사이토카인들(TSLP, IL-33)은 IgE-비의존적 비만세포 활성화를 조절하여 다양한 군의 사이토카인들과 케모카인들을 생성시켜 만성적 알러지 염증질환과 조직의 재구성 및 기능변화를 초래할 수 있다(Stephen J Galli & Mindy Tsai, Nature Medicine, 18:693-704, 2012).
만성 알러지성 염증 질환들은 다양한 세포들(T cells, 호산구, 호염구, 호중구, 단세포, 대식세포, 혈소판, natural killer T cell)과 다양한 사이토카인들(IL-4, IL-5, IL-12, IL-13, IL-15, IL-25, IL-3 등)이 effector cell 또는 effector 인자로서 작용하여 발생하며, 이들의 복잡한 상호작용이 만성 알러지성 염증 조절에 기여하고 각각의 상대적 중요성은 질환의 종류와 환자 개인에 따라 다르다.
알러지 행진 또는 아토피 행진 현상에 의해 아토피 질환을 가진 환자들은 시기적으로 여러 종류의 아토피성 질환이 복합적으로 발생할 수 있는데, 대개 아토피성 천식, 알러지성 비염, 비화농성 중이염, 유스타키오관 질환, 결막염 등에 앞서 아토피성 피부염이 임상적으로 가장 먼저 발병하고, 아토피성 피부염을 가진 어린이의 적어도 50%는 자라면서 아토피성 피부염이 점차 개선되거나 유지된 상태에서 다른 형태의 아토피 질환을 경험한다(Sehra S 등, Crit. Rev. Immunol., 28:15-43, 2008). 이러한 현상은 아토피성 질환들의 병태생리가 공통적인 기전을 가지고 있음을 나타내고, 따라서 아토피성 피부염의 효과적 예방 및 치료를 통해 동반성 아토피성 질환의 발생을 차단하는 것이 바람직하다.
아토피성 피부염의 발생과정은 완전히 규명되지는 않았지만 일반적인 알러지성 염증질환의 발생과정과 대부분 공통적인 기전을 갖고 있다. 즉, T 임파구, Langerhans 세포, 호산구, 피부의 각질세포(keratinocyte) 등의 세포들과 사이토카인, 면역글로불린 등의 인자들이 관련되어 있다.
피부 등 체내에 침투한 알러지 유발 원인물질인 알러젠(allergen)을 섭취 및 처리한 수지상세포 등 항원제시세포(APC)는 CD4+ T 세포를 Th1 또는 Th2 세포로 분화시킨다. CD4+ T-helper 임파구(Th0 세포)와 항원제시세포(APC) 간의 상호작용 시점에 존재하는 사이토카인 환경과 공통자극성 신호들의 종류 등에 따라 Th0 세포가 Th1 임파구 보다는 Th2 임파구로 분화가 되어 증가된 Th2 알러지 반응들을 유발한다. Th1 임파구는 IL-2, IFN-γ, TNF 등을 생성하고 대식세포의 활성화와 지연성 과민반응을 증진시키나, Th2 임파구는 IL-4, IL-5, Il-6, IL-10, IL-13 등을 생성하여 급성 면역반응을 유발하고 이들 사이토카인은 B 임파구로부터 IgE를 생성, 비만세포 및 호산구의 활성화, 1형 과민반응을 증진, B 임파구의 증식 및 분화를 자극, Th1 세포 활성을 억제시킨다. Th1/Th2 균형모델에 의하면, 알러지성 염증질환처럼 Th2 반응 증가가 아토피성 피부염 발생에 기여하지만 실질적 발생기전은 더 복잡하여 biphasic helper T-cell 유형을 나타내는데 아토피성 피부염 환자는 초기 급성기에는 Th2 면역반응이 나타나고 만성 병변이 나타날 때에는 Th1형 면역반응으로 전환한다(Leung DYM, J. Allergy Clin. Immunol 105:860-76, 2000). 만성적 염증유지에는 급성병변과 비교해 볼 때 IL-5, IL-12 발현과 호산구 침윤이 더 관련되어 있는데, IL-12는 Th2 세포 면역반응과 이로부터 생성된 사이토카인들에 의해 유인 및 활성화된 호산구로부터 생성되어 Th1 및 Tho 세포를 활성화시키고 IFN-γ 생성을 증가시켜 Th2 반응을 억제하고 Th1 반응을 강화시켜 아토피성 피부염의 병변이 오래 지속되게 한다.
알러지성 염증질환에서처럼 아토피성 피부염 발생에도 활성화된 Th2 세포반응에 의한 감작현상에 의해 B 임파구로부터 IgE 생성이 증가되고, IgE에 의한 초기 반응인 1형 즉시형 과민반응과 지연기 반응, 만성 지속적 염증상태가 진행된다(Abramovits W., J. Am. Acad. Dermatol. 53:S86-93, 2005). 초기 반응에 의해 Th2 세포와 비만세포로부터 생성된 IL-5, IL-3, GM-CSF와 Langerhans 세포로부터 생성된 TNF-α 등에 의해 호산구(eosinophil) 및 대식세포 등이 피부에 침윤되고, 이로부터 생성된 IL-12, ECP(eosinophil cationic protein) 등이 지연기 반응 및 만성 염증 반응 발생 및 진행에 중요한 역할을 한다.
아토피성 피부염에서는 투과장벽인 피부 각질층(stratum corneum)을 형성하는 각질형성세포(keratinocyte)와 이 세포가 분화된 각질세포(corneocyte)의 역할이 중요하다. 각질층의 손상, 이들 세포의 증식, keratinocyte에 의한 GM-CSF의 과다 생성 등이 Langerhans 세포의 생성 및 성숙 유도, 항원제시 능력 증강을 통해 만성 염증 과정을 개시하고 유지한다. 또한, 다양한 사이토카인들을 분비시켜 호산구 등 염증세포를 피부로 유인하고 Th2 세포반응을 유도하며 keratinocyte 손상시 피부 장벽 결손은 각종 세균의 피부 침투를 허용하여 아토피성 피부염을 악화시키고, keratinocyte 자체가 다양한 사이토카인들을 분비하거나 세포막 공통자극성 분자들을 발현시켜 기억 T 세포를 재활성화하거나 T 세포 조절 기능을 통해 항원제시세포(APC)로 작용하여 지속성 피부 염증 반응을 유지시킨다(Abramovits W., J. Am. Acad. Dermatol. 53:S86-93, 2005; Werfel T. J. Invest Dermatol., 129: 1878-1891, 2009; Kamsteeg M. 등, Am. J. Pathol., 178:2091-2099, 2011).
일반적으로 알러지성 질환의 치료를 위하여 H1 히스타민 수용체를 차단하는 항히스타민제, 스테로이드성 또는 비스테로이드성 항염증제, 류코트리엔 길항제, 베타-교감신경효능약 등과 같은 의약품이 사용된다. 이러한 의약품들은 주로 증후성 개선효과에 유용하고 알러지성 질환의 종류에 따라 다양한 정도의 약효를 나타내지만, 상기 기술한 것과 같이 IgE 생산 억제, 비만세포의 작용을 억제, effector-cell 활성화 억제, FcεRI-의존적 신호전달 억제 등과 같은 알러지성 질환의 근본적인 치료를 제공하지는 못한다.
아토피성 피부염 세계 약물 시장은 2017년까지 지속적으로 성장할 것으로 전망되고 있다. 2009년 7억 2800만 달러 시장규모에서 연평균 성장률 3.3%를 보이며, 2017년 9억 4200만 달러까지 커질 것으로 예측되고 있어 본 발명에 따른 토복령 추출물이 아토피성 피부염 예방 및 치료제로 세계시장에 진출하게 되면 수출 및 기술이전 등을 통한 경제산업적 효과가 클 것으로 예상된다(자료제공: 글로벌인포메이션; 자료출처: GlobalData).
대한민국 등록특허 제10-1156636호(청미래덩굴 추출물을 함유하는 혈관질환의 예방 또는 치료용 약학 조성물), 대한민국 등록특허 제10-0521845호(디오신을 함유한 성장호르몬 분비촉진제), 대한민국 등록특허 제10-1086669호(청미래덩굴 추출물의 제조방법 및 청미래덩굴 추출물 함유 피부 미백용 화장료 조성물) 및 대한민국 등록특허 제10-0593511호(천연물을 이용한 항여드름 화장료)에서 청미래덩굴을 개시하고 있지만, 토복령 단일 추출물이 알러지성 질환의 예방 및 치료에 뛰어난 효능이 있음은 개시되어 있지 않다.
이에 본 발명자들은 아토피성 피부염을 비롯한 알러지 질환에 뛰어난 효능을 갖는 청미래덩굴의 뿌리줄기인 토복령을 이용하여 근본적인 해결책을 제시하고자 노력하였다.
본 발명의 목적은 청미래덩굴의 뿌리줄기인 토복령에 대한 새로운 사용법을 제시하고 토복령의 열수 또는 저급 알코올 추출물을 유효성분으로 하는 약학조성물, 화장료 조성물 및 기능성 식품조성물을 제공함으로써, IgE 생산 억제, 비만세포의 작용 억제, effector-cell 활성화 억제, FcεRI-의존적 신호전달 억제 등 면역 억제를 통하여 아토피성 피부염을 비롯한 알러지성 질환 및 염증성 질환을 근본적으로 해결하고자 한다. 또한, 단백질 및 당 대사에 대한 영향으로 심각한 부작용을 나타내고 있는 dexamethasone과 같은 부신피질호르몬 계열의 염증 치료제를 대체하고, 반복 투여시 약효가 감소하는 약리학적 반응급강하 현상이나 내성발현 등과 같은 단점을 보완하고자 한다.
상기 목적을 달성하기 위하여, 본 발명은 청미래덩굴(Smilax China L.)의 뿌리줄기(Smilacis Chinae Rhizoma)인 토복령 추출물을 유효성분으로 함유하는 알러지성 질환 예방 또는 치료용 약학조성물을 제공한다.
상기 토복령 추출물은 물, 탄소수 1 내지 4개의 저급 알코올 또는 이들의 혼합용매에 가용한 추출물인 것을 특징으로 한다.
상기 알러지성 질환은 아토피성 피부염, 알러지성 피부염, 과민증, 전신성 과민증, 알러지성 비염, 천식, 알러지성 결막염, 두드러기, 접촉성 피부염, 피부소양증, 비화농성 중이염, 유스타키오관 질환, 식품 알러지, 곤충 알러지 또는 약품 알러지인 것을 특징으로 한다.
상기 약학조성물은 정제, 캡슐제, 환제, 과립제, 산제, 엑기스제, 현탁액, 에멀젼, 시럽, 에어로졸의 경구형, 제형, 연고, 도포제, 수액제의 외용제형 또는 정맥, 근육, 피하, 뇌혈관내 투여를 위한 멸균 주사용액 또는 좌제 형태로 제조되는 것을 특징으로 한다.
또한, 본 발명은 청미래덩굴(Smilax China L.)의 뿌리줄기(Smilacis Chinae Rhizoma)인 토복령 추출물을 유효성분으로 함유하는 알러지성 증상 예방 또는 개선용 화장료 조성물을 제공한다.
상기 토복령 추출물은 물, 탄소수 1 내지 4개의 저급 알코올 또는 이들의 혼합용매에 가용한 추출물인 것을 특징으로 한다.
상기 알러지성 증상은 아토피성 피부염, 알러지성 피부염, 두드러기, 접촉성 피부염, 피부소양증, 식품 알러지, 곤충 알러지 또는 약품 알러지인 것을 특징으로 한다.
상기 화장료 조성물은 젤, 화장수, 지용성 파우더, 수용성 파우더, 수용성 리퀴드, 로션, 샴푸, 린스, 클렌저, 크림, 에센스 또는 헤어컨디셔너로 제조되는 것을 특징으로 한다.
또한, 본 발명은 청미래덩굴(Smilax China L.)의 뿌리줄기(Smilacis Chinae Rhizoma)인 토복령 추출물을 유효성분으로 함유하는 알러지성 증상 예방 또는 개선용 기능성 식품조성물을 제공한다.
상기 토복령 추출물은 물, 탄소수 1 내지 4개의 저급 알코올 또는 이들의 혼합용매에 가용한 추출물인 것을 특징으로 한다.
상기 알러지성 증상은 아토피성 피부염, 알러지성 피부염, 과민증, 전신성 과민증, 알러지성 비염, 천식, 알러지성 결막염, 두드러기, 접촉성 피부염, 피부소양증, 비화농성 중이염, 유스타키오관 질환, 식품 알러지, 곤충 알러지 또는 약품 알러지인 것을 특징으로 한다.
상기 기능성 식품조성물은 분말, 과립, 정제, 캡슐, 시럽제, 음료, 츄잉 껌, 차, 캔디, 비타민/미네랄 복합제, 캐러밀 제품, 식품, 음료, 빙과류 또는 과자류로 제조되는 것을 특징으로 한다.
상기와 같은 본 발명에 따르면, 토복령 단일 추출물을 유효성분으로 함유한 약학조성물, 화장료 조성물 및 기능성 식품조성물을 제공함으로써, 장기간 투여에도 부작용 및 내성과 독성이 없으며, 면역질환의 발생에 중요한 역할을 하는 혈청 중의 IgE, Th2 및 Th1의 사이토카인 농도 감소, 비만세포(mast cell)의 히스타민 및 염증 유발성 활성물질의 분비 억제, 이들을 매개로 한 면역성 과민반응의 억제, 피부염 지수 감소, 피부조직의 비만세포(mast cell) 침윤 및 활성화 감소 등을 통하여 알러지성 염증반응의 발생 및 만성적 염증상태를 감소시키는 효과가 있다.
도 1은 본 발명에 따른 토복령 추출물(10, 30, 100μg/ml)이 RBL-2H3 세포주로부터 항원-항체 반응에 의해 유도된 히스타민 함유 과립의 탈과립 현상을 억제하는 실험 결과를 나타낸 것이다. 본 발명의 실험 결과에서 NS는 항원을 처리하지 않은 native군을 의미하며 PP2는 비만세포의 알레르기 유발물질 분비를 강하게 억제하는 것으로 알려진 Src family kinase inhibitor 계열의 표준대조물질로서 10μM 첨가하였다.
도 2는 본 발명에 따른 토복령 추출물(100, 300mg/kg)을 생쥐에 단회 투여 후 관찰한 수동 피부 아나필락시 실험 결과를 나타낸 것이다. DPH는 대표적인 항히스타민제로서 50mg/kg 첨가하였다.

이하, 본 발명을 상세히 설명한다.
본 발명에서 사용한 토복령은 한국을 비롯하여 중국, 일본 등에 분포하는 청미래덩굴(Smilax China L.)의 뿌리줄기(근경, Smilacis Chinae Rhizoma)로서, 전통적인 한의약재 또는 민간요법에서 널리 사용되어 왔다. 해독작용 및 제습작용, 관절 동통, 근육마비, 설사, 이질, 수종, 임병, 종독, 나력, 치질, 위암, 식도암 등에 치료 작용이 있는 것으로 알려져 있다.
청미래덩굴은 백합목 백합과의 덩굴성 낙엽 관목으로 근경은 옆으로 뻗으며 불규칙적으로 만곡되고 비후하고 단단하며 수염뿌리가 드물게 나 있다. 줄기는 단단하고 높이는 2m까지 되며 가시가 성글게 나 있다. 잎은 원형 내지 넓은 타원형이며 끝 부분은 약간 돌출되었거나 무딘 원형이다. 건조한 뿌리줄기는 약간 만곡하고 결절상으로 불규칙적으로 함몰되어 있다.
본 발명은 청미래덩굴(Smilax China L.)의 뿌리줄기(Smilacis Chinae Rhizoma)인 토복령 추출물을 유효성분으로 함유하는 알러지성 질환 예방 또는 치료용 약학조성물을 제공한다. 이는 혈청내 IgE 감소를 표적으로 하고 있는 현재 알러지 면역요법의 효율을 증가시켜 주는 효과가 있다.
상기 토복령 추출물은 물, 탄소수 1 내지 4개의 저급 알코올 또는 이들을 혼합한 극성용매에 가용한 추출물인 것이 바람직하다. 토복령 추출물은 토복령을 건조시켜 분쇄한 후, 토복령 건조 중량의 약 5 내지 20배, 바람직하게는 약 5 내지 15배에 달하는 부피의 물, 메탄올, 에탄올, 프로판올 및 부탄올과 같은 저급 알코올 또는 이들의 약 1:0.1 내지 1:10의 혼합비를 갖는 혼합용매로, 바람직하게는 물 또는 70 내지 100v/v% 에탄올로 30 내지 100℃, 바람직하게는 50 내지 100℃의 추출 온도에서 30분 내지 3일, 바람직하게는 30분 내지 1일 동안 열수 추출, 환류 냉각 추출 또는 초음파 추출 등의 추출방법으로 1회 내지 10회, 바람직하게는 2회 내지 3회 연속 추출하여 얻은 후, 여과지로 여과하고 여과액을 회전진공농축기로 30 내지 100℃, 바람직하게는 50 내지 70℃에서 감압농축한 후, 잔사를 진공 동결건조법을 이용하여 물, 저급 알코올 또는 이들의 혼합용매에 용해 가능한 토복령 추출물을 수득할 수 있다. 열수 추출물의 경우는 상기와 같이 추출하고 여과하여 용해상태로 추출할 수도 있다.
상기 알러지성 질환에는 아토피성 피부염(atopic dermatitis), 알러지성 피부염(allergic dermatitis), 과민증, 전신성 과민증(anaphylaxis), 알러지성 비염(allergic rhinitis), 천식(asthma), 알러지성 결막염(allergic conjunctivitis), 두드러기(urticaria), 접촉성 피부염(contact dermatitis), 피부소양증(skin itching), 비화농성 중이염, 유스타키오관 질환, 식품 알러지, 곤충 알러지 또는 약품 알러지 등이 있다.
상기 약학조성물은 정제, 캡슐제, 환제, 과립제, 산제, 엑기스제, 현탁액, 에멀젼, 시럽, 에어로졸의 경구형, 제형, 연고, 도포제, 수액제의 외용제형 또는 정맥, 근육, 피하, 뇌혈관내 투여를 위한 멸균 주사용액 또는 좌제 형태로 제조될 수 있다. 토복령 추출물의 약제학적 투여 형태는 단독으로 또는 기타 치료학적으로 유용한 약학적 활성물질을 함유할 수 있으며, 약제학적 조성물의 제조에 통상적으로 허용되는 적절한 담체와 함께 배합하여 통상적인 방법에 의해 다양한 형태로 제형화할 수 있다.
토복령 추출물의 약제학적 조성물에 포함될 수 있는 담체, 부형제 및 희석제로는 덱스트로즈, 락토즈, 수크로스, 전분, 솔비톨, 만니톨, 자일리톨, 아카시아 고무, 칼슘 실리케이트, 칼슘 포스페이트, 알기네이트, 젤라틴, 셀룰로즈, 메틸 셀룰로즈, 미정질 셀룰로스, 물, 프로필히드록시벤조에이트, 광물유, 메틸히드록시벤조에이트, 탈크, 마그네슘 스테아레이트 등이 있다. 제제화할 경우에는 보통 사용하는 결합제, 붕해제, 충진제, 증량제, 계면활성제, 습윤제 등의 희석제 또는 부형제를 사용하여 조제될 수 있다.
경구투여를 위한 고형제제에는 정제, 캡슐제, 환제, 과립제, 산제 등이 포함되며 이러한 고형제제는 상기 토복령 추출물에 적어도 하나 이상의 부형제, 예를 들면 전분, 수크로스 또는 락토오스, 젤라틴 등을 섞어서 조제된다. 또한, 단순한 부형제 이외에 탈크, 마그네슘 스테아레이트 등의 윤활제도 사용될 수 있다. 경구투여를 위한 액상제제로는 현탁제, 내용액제, 유제, 시럽제 등이 있는데 빈번히 사용되는 단순 희석제인 물, 액체파라핀 외에 여러 가지 부형제, 예를 들면 방향제, 보존제, 습윤제, 감미제 등이 포함될 수 있다.
비경구 투여를 위한 용액 또는 현탁액 형태의 주사제는 피하, 정맥내, 근육내, 또는 복강내로 투여될 수 있다. 일반적으로 주사가 가능한 용액 또는 현탁액은 물, 염수, 수성 텍스트로스 및 관련 당 용액제, 비휘발성 오일, 에탄올, 글리세린, 폴리에틸렌글리콜, 프로필렌글리콜과 같은 글리콜류 등의 약제학적으로 허용되는 액상 담체 중에 유효량이 토복령 추출물을 균질하게 혼합시켜 제조할 수 있다. 이외에도 필요에 따라 가용화제, 항세균제, 킬레이트제, 완충제, 보존제와 같은 보조제를 포함시킬 수 있다.
도포제는 통상적인 제조방법에 따라 크림제, 연고제, 현탁제, 에멀젼 등 다양한 형태로 용이하게 제조할 수 있다. 연고 또는 크림형으로 제조할 경우, 일반적인 수중유형 또는 유중수형의 베이스에 본 발명의 토복령 추출물을 함유시키고, 향료, 킬레이트제, 색소, 산화방지제, 방부제 등을 필요에 따라 사용할 수 있고 물성 개선을 목적으로 비타민, 단백질, 미네랄 등을 병용할 수 있다.
본 발명의 토복령 추출물의 투여량은 환자의 체중, 질병의 중증도, 약물제형, 건강상태, 투여경로 및 투여기간에 따라 변화될 수 있다. 바람직한 효과를 위해서는 1일 0.001 내지 10g/kg, 바람직하게는 0.01 내지 0.1g/kg으로 투여하는 것이 좋고, 이에 제한하는 것은 아니다. 투여는 1일 1회 또는 수회에 나누어 투여할 수 있다.
또한, 본 발명은 청미래덩굴(Smilax China L.)의 뿌리줄기(Smilacis Chinae Rhizoma)인 토복령 추출물을 유효성분으로 함유하는 알러지성 증상 예방 또는 개선용 화장료 조성물을 제공한다.
상기 토복령 추출물은 물, 탄소수 1 내지 4개의 저급 알코올 또는 이들을 혼합한 극성용매에 가용한 추출물인 것이 바람직하다.
상기 알러지성 증상에는 아토피성 피부염, 알러지성 피부염, 두드러기, 접촉성 피부염, 피부소양증, 식품 알러지, 곤충 알러지 또는 약품 알러지 등이 있다.
상기 화장료 조성물은 젤, 화장수, 지용성 파우더, 수용성 파우더, 수용성 리퀴드, 로션, 샴푸, 린스, 클렌저, 크림, 에센스 또는 헤어컨디셔너로 제조될 수 있다. 필요에 따라 유화제, 방부제, pH 조절제, 향료 등을 첨가하여 통상의 화장료 제조방법에 따라 제형화할 수 있다.
또한, 본 발명은 청미래덩굴(Smilax China L.)의 뿌리줄기(Smilacis Chinae Rhizoma)인 토복령 추출물을 유효성분으로 함유하는 알러지성 증상 예방 또는 개선용 기능성 식품조성물을 제공한다.
상기 토복령 추출물은 물, 탄소수 1 내지 4개의 저급 알코올 또는 이들을 혼합한 극성용매에 가용한 추출물인 것이 바람직하다.
상기 알러지성 증상에는 아토피성 피부염, 알러지성 피부염, 과민증, 전신성 과민증, 알러지성 비염, 천식, 알러지성 결막염, 두드러기, 접촉성 피부염, 피부소양증, 비화농성 중이염, 유스타키오관 질환, 식품 알러지, 곤충 알러지 또는 약품 알러지 등이 있다.
상기 기능성 식품조성물은 분말, 과립, 정제, 캡슐, 시럽제, 음료, 츄잉 껌, 차, 캔디, 비타민/미네랄 복합제, 캐러밀 제품, 식품, 음료, 빙과류 또는 과자류로 제조될 수 있다. 토복령 추출물을 함유한 건강기능식품 또는 식품보조제는 기타 성분의 종류 및 함량을 통상의 제조방법에 따라 첨가하여 제조할 수 있다.
이하, 실시예를 통하여 본 발명을 더욱 상세히 설명하고자 한다. 이들 실시예는 오로지 본 발명을 예시하기 위한 것으로서, 본 발명의 범위가 이들 실시예에 의해 제한되는 것으로 해석되지 않는 것은 당업계에서 통상의 지식을 가진 자에게 있어서 자명할 것이다.
실시예 1. 물 용매를 이용한 토복령 추출물의 제조
서울 경동시장의 한약재상에서 건조된 토복령을 구입하여 실험에 사용하였다. 분쇄한 상태의 토복령 100g에 2L의 3차 증류수를 가하여 교반한 다음, 100℃를 유지하는 추출온도에서 150분 동안 열수를 이용하여 추출하고 서서히 냉각하여 여과시켰다. 상기 여과된 여액을 분리하여 55 내지 65℃에서 감압농축하고 동결건조하여 토복령 분말 엑스 약 10g(수율 약 10w/w%)을 얻었다.
확보된 토복령 추출물은 4℃에서 보관하면서 실험에 사용하였으며, 토복령 추출물의 건조 분말은 증류수 또는 적정농도의 dimethylsulfoxide(DMSO)에 일정농도로 용해하거나 적정농도의 Carboxymethyl cellulose(CMC)나 Hydroxypropyl- methylcellulose(HPMC)에 일정농도로 현탁하거나 열수 추출물 형태로 실험에 사용하였다.
실시예 2. 에탄올 용매를 이용한 토복령 추출물의 제조
상기 실시예 1.과 같이 분쇄된 토복령 100g에 1L의 90.0 내지 95.5v/v% 에탄올 수용액을 가하여 교반한 다음, 50℃를 유지하는 추출온도에서 sonication을 병행하여 2주 내지 3주 동안 환류 추출한 후 여액을 분리하여 감압농축하였다. 최종적으로 40℃에서 24시간 동안 동결건조시켜(cold trap, -70℃) 토복령 분말 엑스 약 10g(수율 약 10w/w%)을 얻었다.
실험예 1. 토복령 추출물에 의한 RBL-2H3 세포주(비만세포)의 알레르기 유발물질 분비 억제
체내에서 알레르기를 유발하는 비만세포(RBL-2H3 세포주)로부터 항원-항체반응에 의해 유도된 알레르기 유발물질의 분비(탈과립)에 대한 상기 실시예 1.로부터 제조한 토복령 추출물의 억제효과를 조사하였다.
RBL-2H3 세포는 글루타민, 항생제 및 10% 우혈청(bovine serume albumin)이 보충된 최소배지에서 배양되었다. 분비실험을 하기 위하여 세포를 트립신-EDTA에 의해 수거한 후, 24-well 배양 플레이트에 well당 200,000개의 세포를 완전배지 상에서 25ng/ml DNP-특이성 IgE와 함께 overnight 배양하였다. 상기 배양된 세포를 PIPES 완충액(25mM PIPES, pH 7.2, 159mM NaCl, 5mM KCl, 0.4mM MgCl2, 1mM CaCl2, 5.6mM glucose, 및 0.1% BSA)으로 세척한 다음, 항원을 첨가하기 전에 토복령 추출물 또는 이를 녹인 용매를 배지에 첨가하고 30분 동안 전 배양하였다. 전 배양 후 DNP-BSA 항원을 최종농도 20ng/ml로 첨가하여 10분간 자극을 유도하였다.
알레르기 유도물질의 분비정도는 배지 중에 분비된 탈과립의 표식자인 hexosaminidase의 활성을 p-nitrophenyl-acetyl-β-D-glucosaminide로부터 유리된 p-nitrophenol의 양으로부터 결정하였다. 상기 hexosaminidase의 활성은 p-nitrophenyl-acetyl-β-D-glucosaminide를 기질로 사용하여 측정하였으며, 탈과립도는 상기 p-nitrophenyl-acetyl-β-D-glucosaminide로부터 유리된 p-nitrophenol농도를 ELISA reader를 이용하여 400nm파장에서 측정하였다.
도 1에서, 알레르기를 유발하는 비만세포인 RBL-2H3(Funaba, M. et al., Cell Biol. Int., 27:879-85, 2003; Jeong, H.J. et al., Cytokine, 18:252-9, 2002)에 대하여 본 발명에 따른 토복령 추출물이 히스타민을 비롯한 비만세포의 다양한 알레르기 유발물질의 분비를 토복령 추출물 10, 30, 100μg/ml 농도에서 각각 6.7, 25.7, 35.8%를 억제하여 농도 의존적 억제작용 효과를 나타내었다. 그 최고 분비의 50%를 억제하는 토복령 추출물의 농도(IC50)는 약 273μg/ml로 산출되었다. 비만세포의 알레르기 유발물질 분비를 강하게 억제하는 것으로 알려진 Src family kinase inhibitor 계열의 표준대조물질인 PP2 10μM은 분비를 완전히 억제하는 것으로 나타났다. 이로부터 본 발명의 토복령 추출물은 비만세포의 활성화 및 알레르기성 물질의 분비를 차단함으로써 알레르기 억제효과가 있음을 확인하였다.
실험예 2. 항원-감작 마우스 모델에서 토복령 추출물의 항알러지 효능
마우스를 이용하여 수동 피부 과민증반응(PCA, passive cutaneous anaphylaxis) 실험을 수행하여 토복령 추출물의 항알러지 효능을 확인하였다. 수동 피부 아나필락시스 반응은 IgE 관련 작용기전에 의해 비만세포로부터 누출된 히스타민, 프로스타글란딘류, 류코트리엔류 등과 같은 화학 매개물질의 작용으로 혈관 벽의 투과성이 증가됨에 따라 면역반응이 발생한 마우스의 조직으로 청색 색소인 Evans blue가 누출되고, 상기 누출된 색소량을 측정하여 과민증 발생 정도를 측정하는 유용한 실험이다.
(1) 단회 경구 투여시의 항알러지 효능 실험
5주령의 수컷 ICR mice를 오리엔트바이오로부터 공급받아 실험 전 1주 적응기 동안 사료와 물을 마음껏 섭취하도록 하였다. 사육실의 온도는 22±1℃, 습도는 55±10%를 유지하였고, 매일 명암주기가 각 12시간이 되도록 조절하여 사육하였다. 5개 실험군으로 나누어 다음과 같은 방법으로 PCA 실험을 수행하였다(Lim BO 등, Polygoni cuspidati radix inhibits the activation of Syk kinase in mast cells for antiallergic activity. Exp Biol Med 232:1425-1431, 2007; Kabu K 등 Zinc is required for FcεRI-mediated mast cell activation. J Immunol 177:1296-1305, 2006).
Anti-DNP-BSA 특이적 IgE 항체 0.5㎍을 마우스 한 쪽 귀에 피내주사 (intradermal injection)하고 24시간 경과 후 토복령 추출물(100, 300mg/kg)과 표준 대조물질인 diphenylhydramine(DPH, 50mg/kg)을 30% DMSO(Dimethyl sulfoxide)에 각각 녹여 경구투여(Oral administration) 하였다. 한 시간 뒤 250㎍ 항원(DNP-BSA)과 4% Evans blue 색소를 용해한 인산완충식염수(PBS, phosphate-buffered saline) 250㎕를 마우스의 꼬리에 정맥주사(intravenous injection)하여 수동 피부 과민증 반응을 일으켰다. 한 시간 후 마우스를 경추 탈골하여 치사시킨 다음 면역반응에 의해 귀 혈관으로부터 누출된 Evans blue 색소량을 측정하기 위해 귀를 적출하여, 700㎕ formamide 용액에서(63℃) overnight 배양을 통해 색소를 추출한 후 620nm에서 흡광도를 측정하였다.
도 2에서, 토복령 추출물(100, 300mg/kg)을 단회 경구투여시, 마우스 한 쪽 귀에 누출된 Evans blue 양은 복용량의 용량 의존적으로 감소하였다. 특히 토복령 추출물 300mg/kg 경구투여군은 대표적 항히스타민제인 DPH 50mg/kg와 유사한 정도로 과민증반응을 억제하였다.
(2) 1주 반복 경구 투여시의 항알러지 효능 실험
토복령 추출물(3, 10, 30mg/kg), diphenylhydramine(DPH) 50mg/kg을 각각 10% DMSO(dimethyl sulfoxide)에 녹여 6일 동안 동일한 시간에 1일 1회 경구투여(oral administration) 하였다. 6일째 되는 날 경구투여한 후 Anti-DNP-BSA 특이적 IgE 항체(0.5㎍)를 마우스 귀에 피내주사(intradermal injection)하고 24시간 경과 후, 7일째 경구투여(oral administration) 하였다. 한 시간 뒤 250㎍ 항원(DNP-BSA)과 4% Evans blue을 용해한 PBS 250㎕를 마우스의 꼬리에 정맥주사(intravenous injection)하여 수동 피부 과민증 반응을 일으켰다. 한 시간 후 마우스를 경추 탈골하여 치사시킨 다음 귀 혈관으로부터 누출된 Evans blue 색소량을 측정하기 위해 귀를 적출하여, 700㎕ formamide 용액에서(63℃) overnight 배양을 통해 색소를 추출한 후 620nm에서 흡광도를 측정하였다.
도 3에서, 토복령 추출물을 7일 반복 투여시 상기 (1)의 단회 투여 실험과 마찬가지로 마우스 한 쪽 귀에서 누출된 Evans blue 양은 용량 의존적으로 PCA 반응을 억제하였고, 특히 토복령 추출물 30mg/kg 경구 투여군은 대표적 항히스타민제인 DPH 50mg/kg과 유사한 억제 효과를 나타내었다.
상기와 같은 결과로부터 토복령 추출물이 비만세포의 활성화 및 비만세포로부터 히스타민을 비롯한 다양한 알레르기성 물질들의 분비를 억제함으로써 알레르기 질환을 예방 및 치료할 수 있음을 확인하였다.
특히, 토복령 추출물을 1일 1회, 7일 동안 반복투여시 수동 피부 과민성 반응 억제 효과는 1회 투여시보다 약 10배 낮은 농도에서도 동등 이상의 효과를 나타내었다. 대부분의 약들이 반복투여시 약효가 감소하는 약리학적 반응급강하 현상이나 내성발현 등과 같은 단점을 나타내는 것을 고려할 때, 장기적인 약물 복용이 필수적인 아토피성 피부염 등의 면역질환의 예방 및 치료에 있어서 본 발명의 토복령 추출물이 이상적인 약리작용 및 효능을 가질 것으로 기대된다.
실험예 3. 아토피성 피부염 마우스 모델에서 토복령 추출물의 항알러지 효능
(1) 햅텐-감작 Nc/Nga 마우스 아토피성 피부염 동물모델 준비
알러지성 질환을 가진 동물로부터 항알러지 효능을 확인하기 위하여 아토피성 피부염 동물모델로 많이 사용되고 있는 Nc/Nga 마우스를 이용하여 다양한 실험을 실시하였다.
다수의 아토피성 피부염 동물 실험에 사용되어 온 Nc/Nga 마우스는 SPF(specific pathogen-free) 환경에서는 건강하게 유지되나, 비멸균성 일반 환경에서는 6-7주령에 아토피성 피부염이 자연 발생하며 혈장 중의 IgE 또한 주령에 따라 증가하여 16-18주령에서 정점에 도달하고 이 시기에 피부건조, 결절성 병변, 가려움증 등의 발생과 더불어 피부조직의 병리학적 변화를 수반하여, 전형적인 아토피성 피부염을 발생하는 동물로써 인체의 아토피성 피부염 연구에 적합한 모델로 알려져 있다(Vestergaard C. et al., J. Overproduction of Th2-specific chemokines in NC/Nga mice exhibiting atopic dermatitis-like lesions. Clin. Invest., 104, 1097-1105, 1999; Suto H. et al., NC/Nga mice: a mouse model for atopic dermatitis. Int Arch Allergy Immunol 120 (suppl 1):70-75, 1999; Matsumoto M et. al., IgE hyperproduction through enhanced tyrosine phosphorylation of Janus kinase 3 in NC/Nga mice, a model for human atopic dermatitis. J Immunol. 162:1056-63, 1999).
그러나 Nc/Nga 마우스는 일반 환경에서는 피부염의 발생이 상당한 변화를 보이고 SPF 환경에서는 비정상적 피부 변화를 보이지 않기 때문에 아토피성 피부염의 발생을 보다 재현성 있게 발생시키는 모델로서, 햅텐(hapten: picryl chloride)을 반복처리(sensitization and challenge)하여 공기조절 환경에서도(air-controlled barrier system) Nc/Nga 마우스에 100% 피부염을 발생시키는 모델을 이용하여 인체의 아토피성 피부염을 연구하고 있다(Ohmura T et al., role of substance P in an NC/Nga mouse model of atopic dermatitis-like disease, Int Arch Allergy Immuno 133:389-397, 2004).
본 실험에서는 picryl chloride를 햅텐으로 사용한 Nc/Nga 마우스 모델을 이용하여 토복령 추출물의 항알러지 효능 실험을 실시하였다.
실험을 위하여 5주령 Nc/Nga 암컷 마우스 25마리(Charles River, Japan)를 2주일 동안 새로운 SPF 환경에 적응시켰다. 아토피성 피부염 발생군에는 8주령 때부터 마우스에 에탄올과 아세톤(4:1 용적비)의 혼합물에 용해된 5% TNCB(2-Chloro-1,3,5-Trinitro benzene, picryl chloride) 150㎕를 햅텐으로 사용하여 복부 및 양쪽 귀 피부에 한번 도포하여 감작시키고, 5일후에 올리브유에 용해된 1% TNCB 150㎕를 등 피부에 적용하고, 1% TNCB 150㎕를 1주 간격으로 8번 더 적용하여 재감작(challenge)시켜 가려움증, 홍반, 팽윤, 출혈 등을 포함한 피부염 지수를 증가시키고 아토피성 피부염을 발생시켰다.
상기 실시예 1.에서 준비된 토복령 추출물(10, 30mg/kg), 용매(3% HPMC, hydroxypropyl methylcellulose), 양성 대조군으로서 덱사메타손(2.5mg/kg)을 8주령 때부터 17주령 때까지 1일 1회 9주간 경구투여한 4개의 처치군과 TNCB에 의한 감작 및 재감작을 하지 않은 1개의 군(naive)을 포함한 5개 실험군으로 준비하였다. 5개 실험군의 모든 마우스에서 TNCB 감작전(0일), 감작후 21일(3주), 35일(5주), 63일(9주)째에 마우스를 희생시킨 후 혈액을 채취하여 혈청을 분리하였다.
(2) 아토피 피부염 발생 중증도(severity of atopic dermatitis) 분석
피부염 증상의 발생정도 및 토복령 추출물의 피부염 발생억제 효능을 비교하기 위하여 상기 (1)에서 준비한 5개 마우스 실험군에 대해 마우스 피부의 피부염 중증 지수(skin severity score)를 1주에 한번씩 9주 동안 관찰하여 분석하였다. 총 임상 피부 염증 중증도 지수(total clinical skin severity score)는 발적(erythema, hemorrhage), 부종(edema, swelling), 피부탈착(excoriation, erosion), 건조 상태(dryness) 등 4종류의 징후(signs) 및 증상들(symptoms)에 대한 개별 점수를 합하여 산출하였다. 상기 4항목에 대해 증상이 없을 때를 0, 경미한 증상의 경우 1, 중등도 증상인 경우 2, 심한 증상인 경우 3의 점수를 부여하였다.
도 4는 5개 실험군의 등쪽 피부에서 관찰된 임상적 피부염증 상태를 나타내는 대표적 사진이며(9주간 처리후), 도 5는 임상적 피부염증 상태를 수치화한 총 임상 피부염증 중증도 지수이다.
TNCB는 9주에 걸쳐 반복 처리시, 아토피성 피부염 같은 피부병변이 경시적으로 증가하였으며, 토복령 추출물(10, 30mg/kg)을 9주간 반복적으로 경구투여 하였을 때 피부염증 상태가 투여 1주일 후부터 감소하기 시작하였다. 또한, 투여 3주차부터는 두 용량 모두에서 유의적으로 감소하였으며, 30mg/kg 투여군에서는 투여 후 6주차부터 9주차까지 7주차를 제외하고 모두 dexamethasone 투여군과 동등한 정도로 총 임상 피부염증 중증도 지수를 감소시켰다.
이는 토복령 추출물이 경구투여 후 약 3주차부터 아토피성 피부염 발생을 유의적으로 억제하여 투여 후 9주차까지 그 효능이 일정하게 유지되고 있음을 보여주고 있으며, 현재까지 개발된 의약품 중 가장 우수한 염증 치료제 중 하나인 dexamethasone과 동등한 정도의 효능을 보이고 있음을 나타내고 있다.
특히, 상기와 같은 특성은 토복령 추출물이 단백질 및 당 대사에 대한 영향으로 심각한 부작용을 나타내고 있는 dexamethasone과 같은 부신피질호르몬 계열의 염증 치료제를 대체하여 아토피성 피부염 치료제로 널리 사용될 수 있음을 시사한다.
(3) 혈청 IgE 농도 분석
상기 (1)에서 채취된 혈청으로부터 TNCB 특이적(TNCB-specific) IgE의 농도를 ELISA 방법(Biolegend 사)으로 측정하였다.
도 6은 5개 실험군의 혈청 중 총 IgE 항체의 농도를 보여주고 있다.
TNCB로 감작 및 재감작시, 혈청 중 총 IgE 항체가 3주, 5주, 9주 시간 경과에 따라 TNCB 처리하지 않은 실험군(naive군)에 비해 점차 급격히 증가하였다. 이는 IgE 증가가 상기 (2)에서 기술한 총 임상 피부염증 중증도 지수의 경시적 증가에서 반영된 TNCB에 의한 아토피성 피부염의 발생에 대한 세포면역학적 기작임을 제시하며, 본 발명과 관련하여 사용된 TNCB를 처리한 Nc/NGa 마우스가 인체의 아토피 피부염 연구에 사용될 수 있는 유용한 질환모델인 것을 제시하고 있다.
토복령 추출물(10, 30mg/kg)을 매일 1회씩 9주 동안 경구투여시 IgE 항체가 유의적으로 용량 의존적으로 감소하였으며, 30mg/kg 투여군에서는 dexamethasone 투여군과 동일한 수준으로 IgE 농도가 감소하였다(3주, 5주). 이러한 결과는 토복령 추출물이 dexamethasone처럼 알러지를 유발하는 IgE를 감소시키는 기작에 의해 아토피성 피부염 발생을 억제 또는 치료할 뿐만 아니라 부신피질 호르몬이 갖는 다양한 부작용을 감소시킬 수 있는 가능성을 제시하고 있다.
(4) 혈청 cytokines 농도 분석
상기 (1)에서 채취된 혈청으로부터 아토피성 피부염 발생과 관련된 것으로 알려져 있는 사이토카인인 IL-5 및 IFN-γ의 경시적 변화를 Bio-Plex Pro Mouse Cytokine assay kit(Biorad 사)를 사용하여 Bio-PlexTM suspension array system (Biorad 사)으로 분석하였다.
도 7 및 8은 각각 5개 실험군의 혈청 중 IL-5 및 IFN-γ의 농도를 나타낸 것이다.
IL-5의 경우, TNCB control이 TNCB 처리하지 않은 실험군(naive군)에 비해 3주, 5주까지 증가하였다가 9주째 감소하는 경향을 나타내었으며, 토복령 추출물(10, 30mg/kg)을 매일 1회씩 9주 동안 경구투여시 3주, 5주째 농도 의존적으로 유의적인 감소를 나타내었다. dexamethasone의 경우, 3주째 약간 감소시켰으나 5주, 9주에서 TNCB에 의한 IL-5의 증가를 감소시키지 않았으며 9주째 오히려 증가시켰다.
IFN-γ의 경우, TNCB control은 3주, 5주, 9주에 걸쳐 증감을 반복하였으며, TNCB 처리하지 않은 실험군(naive군)에 비해 증가하였다. 토복령 추출물 투여시 5주 및 9주째에서 TNCB 처리에 의해 증가한 IFN-γ를 TNCB 비처리군과 같은 수준으로 감소시켰다. 반면에 dexamethasone 투여군의 경우, 3주째는 감소시켰으나 5주째와 9주째는 TNCB control과 유사한 농도를 나타내었다.
상기와 같은 결과는 토복령 추출물이 강한 면역억제제인 스테로이드성 약물과는 달리 TNCB 처리 후 관찰된 대부분의 기간에서 Th1 면역(IFN-γ)과 Th2 면역(IL-5) 모두를 감소시켜 알러지 증상을 개선하고 예방할 수 있음을 나타낸다.
(5) 피부조직에 대한 조직학적 분석
조직학적 검사를 위해, 상기 (1)에서 준비한 5개 마우스 실험군에 대해 9주간(63일) 토복령 추출물 또는 용매를 경구투여한 후 마우스 등쪽 피부 환부의 중앙부에서 피부조직을 1cm x 1cm의 크기로 채취하여 10% 포름알데하이드 용액에 고정한 다음 파라핀으로 포매하고(pH 7.2) 두께 4μm의 절편을 제작하였다. 조직 절편을 헤마톡실린-에오신(hematoxylin-eosin)법으로 염색한 후, 광학현미경을 이용하여 100배로 확대하여 대표적 부위의 표피층의 두께를 비교하였으며, 또한 toluidine blue법으로 염색한 후, 200배 및 400배로 확대하여 진피층에 대한 비만세포의 침윤 및 활성화에 의한 탈과립 현상을 비교 분석하였다.
도 9는 5개 실험군의 등쪽 피부에서 얻은 조직절편을 헤마톡실린-에오신(hematoxylin-eosin)법으로 염색한 후 관찰한 표피/진피 두께를 나타내는 대표적 사진이며, 도 10은 5개 실험군의 등쪽 피부에서 얻은 조직절편을 toluidine blue법으로 염색한 후 200배 및 400배로 확대하여 진피층에 대한 비만세포의 침윤 및 활성화에 의한 탈과립 현상을 비교 분석한 대표적 사진이다.
TNCB에 의한 9주간에 걸친 감작 및 반복적 재감작은, TNCB 처리하지 않은 실험군(naive군)에 비해 표피의 두께 및 각질화를 현저히 증가시켰으며, 토복령 추출물(10, 30mg/kg)을 9주간 반복적으로 경구투여 하였을 때 표피의 두께 및 각질화는 TNCB 비처리군과 비슷한 상태로 감소되었으며, dexamethasone 9주 처리시도 표피의 두께 및 각질화는 TNCB 비처리군과 비슷한 상태로 감소하였다.
또한, TNCB 처리는 진피의 비만세포 침윤 및 비만세포 활성화에 의한 탈과립 현상을 증가시켰으며, 토복령 추출물(10, 30mg/kg)을 9주간 반복적으로 경구투여 하였을 때 이러한 비만세포의 침윤 및 활성화가 현저하게 감소하였으며, dexamethasone 9주 처리시도 토복령 추출물 30mg/kg 투여시와 비슷한 정도로 비만세포의 침윤 및 활성화가 감소하였다.
따라서 토복령 추출물이 상기 총 임상 피부염증 중증도 지수의 감소와 더불어 표피의 비후/비대를 감소시키고, 아토피성 피부염의 발생과 밀접한 관계가 있는 진피내 비만세포의 침윤을 감소시켜 피부의 전반적인 임상 상태를 증진시키는 것을 확인하였다.
(6) 말초 혈액중의 백혈구 분석
아토피성 피부염은 해당 피부 병변의 염증상태 증가, 다양한 염증세포의 침윤 및 말초혈액의 염증세포의 증가 등을 특징으로 하고 있다. 토복령 추출물 경구투여가 말초혈액 중의 다양한 염증세포 수에 미치는 영향을 분석하기 위해, 상기 (1)에서 준비한 5개 마우스 실험군에 대하여 9주간(63일) 토복령 추출물 또는 용매를 경구투여한 후 혈액을 채취하여 총백혈구, 호중구, 임파구 및 호산구의 수를 자동혈액분석기로 측정하였다.
도 11은 5개 실험군에서 채취한 혈액 중 각종 염증세포의 수를 나타낸다.
그 결과, TNCB control은 총백혈구, 호중구, 임파구 및 호산구 수를 모두 증가시켜 아토피 증상의 발현과 더불어 말초혈액 중의 염증세포도 증가시키는 것으로 나타났다. 토복령 추출물(10, 30mg/kg)을 9주간 경구투여시 이들 염증세포가 유의적으로 감소하였으며 이러한 결과는 항염작용이 우수한 면역억제제인 덱사메타손 투여시에도 관찰되었다. 이는 토복령 추출물이 항염증 작용을 통해 아토피성 피부 병변을 예방 및 치료할 수 있음을 나타낸다.
실험예 4. 아토피성 피부염 환자의 피부 염증 감소
토복령 추출물이 아토피성 피부염 환자의 피부 병변을 개선하는지 관찰하기 위해 1명의 알러지 환자를 대상으로 상기 실시예 1.에서 제조한 토복령 추출물을 60일간 매일 3회씩 환부에 바르고, 3회씩 경구로 복용시켰다.
그 결과, 도 12에서 보는 바와 같이 아토피성 피부염 환자의 각종 신체 부위에 발생한 피부염 증상을 현저하게 개선시켰다.
본 실험에서 얻은 모든 측정치는 평균±표준편차로 제시하였다. 평균의 차이는 Student's t test를 이용하여 통계적으로 검정하였으며, 유의수준은 5% 이하로 하였다.
하기에 본 발명의 토복령 추출물을 포함하는 여러 가지 제제예를 설명하고 있으나, 이는 본 발명을 제한하고자 함이 아닌 단지 구체적으로 설명하고자 함이다.
제제예 1. 정제의 제조
실시예 1.의 토복령 열수추출물 ..................100㎎

스테아린산 마그네슘..............................5mg
상기의 성분들을 혼합하고 통상적인 정제의 제조방법에 따라 습식과립법 또는 건식과립법을 이용하여 정제를 제조하였다.
제제예 2. 캡슐제의 제조
실시예 1.의 토복령 열수추출물......................100㎎
전분.......................................................50mg
유당.......................................................50mg
탈크.......................................................1mg
스테아린산마그네슘............................. 5mg
상기의 성분들을 혼합하고 통상적인 캡슐제의 제조방법에 따라서 캡슐제를 제조하였다.
제제예 3. 액제의 제조
실시예 1.의 토복령 열수추출물 .....................500㎎
이성화당..................................................10g
자당.........................................................10g
레몬향.....................................................적당량
정제수를 가하여 전체..............................50㎖
상기의 성분들을 통상적인 액제의 제조방법에 따라서 혼합하고 갈색병에 충전하고 멸균시켜서 액제를 제조하였다.
제제예 4. 주사제의 제조
실시예 1.의 토복령 열수추출물.........................50㎎
소디움 메타비설파이트............................2.6㎎
메틸파라벤...............................................0.5㎎
프로필파라벤...........................................0.1㎎
주사용 멸균증류수................................적당량
상기의 성분들을 혼합하고 통상적인 방법으로 2ml로 한 후, 앰플에 충전하고 멸균하여 주사제를 제조하였다.
제제예 5. 음료의 제조
실시예 1.의 토복령 열수추출물.........................100㎎
비타민B2....................................................5㎎
비타민B6....................................................5㎎
비타민C...................................................80㎎
식이섬유..............................................1,000㎎
액상과당............................................10,000㎎
유화제....................................................100㎎
레몬향......................................................50㎎
정제수.....................................................적당량
상기의 성분들을 혼합하여 용량이 50ml가 되도록 하고, 혼합액을 3μm의 필터로 통과시켜 여과한 최종 혼합액을 90~93℃에서 20초간 1차 살균하여 50ml 병에 충진하고 80~85℃에서 20분간 2차 살균하여 음료제품을 제조하였다.
이상, 본 발명내용의 특정한 부분을 상세히 기술하였는바, 당업계의 통상의 지식을 가진 자에게 있어서, 이러한 구체적인 기술은 단지 바람직한 실시태양일 뿐이며, 이에 의해 본 발명의 범위가 제한되는 것이 아닌 점은 명백할 것이다. 따라서 본 발명의 실질적인 범위는 첨부된 청구항들과 그것들의 등가물에 의해 정의된다고 할 것이다.
Claims (12)
- 청미래덩굴(Smilax China L.)의 뿌리줄기(Smilacis Chinae Rhizoma)인 토복령 추출물을 유효성분으로 함유하는 알러지성 질환 예방 또는 치료용 약학조성물.
- 제 1항에 있어서,상기 토복령 추출물은 물, 탄소수 1 내지 4개의 저급 알코올 또는 이들의 혼합용매에 가용한 추출물인 것을 특징으로 하는 알러지성 질환 예방 또는 치료용 약학조성물.
- 제 1항에 있어서,상기 알러지성 질환은 아토피성 피부염, 알러지성 피부염, 과민증, 전신성 과민증, 알러지성 비염, 천식, 알러지성 결막염, 두드러기, 접촉성 피부염, 피부소양증, 비화농성 중이염, 유스타키오관 질환, 식품 알러지, 곤충 알러지 또는 약품 알러지인 것을 특징으로 하는 알러지성 질환 예방 또는 치료용 약학조성물.
- 제 1항에 있어서,상기 약학조성물은 정제, 캡슐제, 환제, 과립제, 산제, 엑기스제, 현탁액, 에멀젼, 시럽, 에어로졸의 경구형, 제형, 연고, 도포제, 수액제의 외용제형 또는 정맥, 근육, 피하, 뇌혈관내 투여를 위한 멸균 주사용액 또는 좌제 형태로 제조되는 것을 특징으로 하는 알러지성 질환 예방 또는 치료용 약학조성물.
- 청미래덩굴(Smilax China L.)의 뿌리줄기(Smilacis Chinae Rhizoma)인 토복령 추출물을 유효성분으로 함유하는 알러지성 증상 예방 또는 개선용 화장료 조성물.
- 제 5항에 있어서,상기 토복령 추출물은 물, 탄소수 1 내지 4개의 저급 알코올 또는 이들의 혼합용매에 가용한 추출물인 것을 특징으로 하는 알러지성 증상 예방 또는 개선용 화장료 조성물.
- 제 5항에 있어서,상기 알러지성 증상은 아토피성 피부염, 알러지성 피부염, 두드러기, 접촉성 피부염, 피부소양증, 식품 알러지, 곤충 알러지 또는 약품 알러지인 것을 특징으로 하는 알러지성 증상 예방 또는 개선용 화장료 조성물.
- 제 5항에 있어서,상기 화장료 조성물은 젤, 화장수, 지용성 파우더, 수용성 파우더, 수용성 리퀴드, 로션, 샴푸, 린스, 클렌저, 크림, 에센스 또는 헤어컨디셔너로 제조되는 것을 특징으로 하는 알러지성 증상 예방 또는 개선용 화장료 조성물.
- 청미래덩굴(Smilax China L.)의 뿌리줄기(Smilacis Chinae Rhizoma)인 토복령 추출물을 유효성분으로 함유하는 알러지성 증상 예방 또는 개선용 기능성 식품조성물.
- 제 9항에 있어서,상기 토복령 추출물은 물, 탄소수 1 내지 4개의 저급 알코올 또는 이들의 혼합용매에 가용한 추출물인 것을 특징으로 하는 알러지성 증상 예방 또는 개선용 기능성 식품조성물.
- 제 9항에 있어서,상기 알러지성 증상은 아토피성 피부염, 알러지성 피부염, 과민증, 전신성 과민증, 알러지성 비염, 천식, 알러지성 결막염, 두드러기, 접촉성 피부염, 피부소양증, 비화농성 중이염, 유스타키오관 질환, 식품 알러지, 곤충 알러지 또는 약품 알러지인 것을 특징으로 하는 알러지성 증상 예방 또는 개선용 기능성 식품조성물.
- 제 9항에 있어서,상기 기능성 식품조성물은 분말, 과립, 정제, 캡슐, 시럽제, 음료, 츄잉 껌, 차, 캔디, 비타민/미네랄 복합제, 캐러밀 제품, 식품, 음료, 빙과류 또는 과자류로 제조되는 것을 특징으로 하는 알러지성 증상 예방 또는 개선용 기능성 식품조성물.
