Abstract
Die Erfindung betrifft ein dentales Implantatset (1), aufweisend: ein Implantat (2), ein granuläres Knochenersatzmaterial (3), eine Membran (4). Um ein Implantatset (1) der eingangs genannten Art zur Verfügung zu stellen, durch das der Prozess der Augmentation verbessert wird, schlägt die Erfindung vor, dass das Knochenersatzmaterial (3) und die Membran (4) von harter und fester Beschaffenheit ist.
Description
Dentales Implantatset
Die Erfindung betrifft ein dentales Implantatset gemäß dem Oberbegriff des Anspruches 1.
Implantate, Knochenersatzmaterialien, Membrane sowie chirurgische Instrumente, insbesondere Implantatbohrer, werden als essentielle Bestandteile und Instrumentarien im Rahmen einer Implantatbehandlung bei nicht optimalen Knochenverhältnissen eingesetzt.
Ursache dieser nicht optimalen Knochenverhältnisse ist die oftmals große zeitliche Distanz zwischen Zahnverlust und Implantation. Hierdurch kommt es teilweise zu einer unregelmäßigen Atrophie, d .h. zu einer Rückbildung bzw. Abbau der Kieferkämme sowohl in vertikaler als auch in transversaler Richtung.
Präimplantativ wird hierzu zunächst nach Öffnen des Zahnfleisches an der gewählten Implantatstelle mittels eines Schnittes oder einer Stanze mit Implantatbohrern der Knochen für die geplante Position und Implantatgröße aufbereitet. Aus dem Stand der Technik ist eine Vielzahl von Implantatbohrern bekannt.
Durch Einsatz von synthetisch hergestellten granulären Knochenersatzmaterialien werden dann die abgebauten Anteile der zahntragenden Knochen rekonstruiert. Ein Beispiel für ein granuläres Knochenersatzmaterial geht aus der DE 10 2006 037 362 B3 hervor, die ein Knochenersatzmaterial offenbart, das Calciumcarbonat enthält.
Durch Einsatz von zusätzlichen Membranen wird dabei eine Abschirmung gegen die Invasion von Bindegewebe erreicht, was die knöcherne Regeneration des Defektes verhindern würde. Als Materialien für Membrane werden im Stand der Technik bevorzugt synthetische Polymere, wie Polylactide oder Polyglycolide sowie deren chemische Modifikation verwendet. Bei den Membranen handelt es sich um sehr dünne elastische Folien. Man unterscheidet resorbierbare Membranen, die vom Körper abgebaut werden und nicht entfernt werden müssen, sowie nicht resorbierbare Membranen.
Alternativ wird intraimplantativ nach Einbringen des dentalen Implantats der verbliebene Knochendefekt mit synthetischem Knochenersatzmaterial aufgefüllt und wiederum mit einer Membran versehen.
Die aus dem Stand der Technik bekannten Knochenersatzmaterialien besitzen eine körnige Struktur und haben, je nach Korngröße, nach dem Einbringen die Eigenschaft, unter äußerem Druck verpresst werden zu können, was zu einer nicht gewünschten Unregelmäßigkeit in der sich final bildenden Knochenstruktur führt. Derartige Veränderungen der Augmentationsstruktur, d .h. der Vergrößerung der Knochenmasse, können bereits durch den Vorgang des Nahtverschlusses der Implantationswunde intraoperativ induziert werden.
Zudem bietet die Elastizität und Plastizität der derzeitig verwendeten Membrane bei der Augmentation keinen ausreichenden Schutz vor einer externen Kompression. Die herkömmlich eingesetzten Membrane liefern momentan auch keine optimale Abschirmung vor invasierenden Bindegewebszellen. Die zirkulär erforderliche, subperiostale, d.h. unter dem Knochenoberhäutchen befindliche Knochenstärke um das Implantat von 1,5 bis 2 mm wird darüber hinaus im Rahmen der Verwendung der herkömmlichen Knochenersatzmaterialien in Verbindung mit der Membran oftmals unterschritten, was die Prognose der Dauerhaftigkeit des Implantats erheblich verschlechtert.
Es ist deshalb Aufgabe der vorliegenden Erfindung, ein dentales Implantatset der eingangs genannten Art zur Verfügung zu stellen, dessen Komponenten die Nachteile der herkömmlichen Praxis der Augmentation beseitigen.
Diese Aufgabe wird mit den Merkmalen des Anspruches 1 gelöst. Vorteilhafte Ausgestaltungen der Erfindung ergeben sich aus den abhängigen Ansprüchen.
Die Aufgabe wird erfindungsgemäß dadurch gelöst, dass das Knochenersatzmaterial und die Membran von harter und fester Beschaffenheit sind .
Kernidee der Erfindung ist es, eine elastische Verformung mit einer einhergehenden Volumenveränderung und Dichteerhöhung des Knochenersatzmateriales und der Membran durch eine inkompressible Ausbildung des Knochenersatzmateriales und der Membran zu vermeiden. Ein fester und harter Zustand des granulären Knochenersatzmateriales sowie der Membran, beispielsweise aus Polylactid, ist nahezu in- kompressibel . Die Überführung eines körnigen (granulären) Stoffes für das Knochenersatzmaterial in einen festen und harten Zustand kann durch herkömmliche Verfahren, beispielsweise durch Sintern oder Verklebung mittels Fibrinkleber erfolgen. Die Überführung der Membran in einen harten und festen Zustand kann ebenfalls durch herkömmliche Methoden realisiert werden.
Nach den üblichen Einheil- und Osteoregenerationszeiten wird vorteilhafterweise eine definierte Alveolarknochensituation mit einer optimalen Knochenstärke erreicht. Hierdurch entfallen präimplantative, operative Zusatzeingriffe, was Zeit und Kosten minimiert.
Eine vorteilhafte Weiterbildung der Erfindung sieht vor, dass das Set als chirurgisches Instrument einen Planfräser aufweist. Planfräser haben schon Verwendung im Rahmen von medizinischen Behandlungen gefunden, wie die DE 20 2007 003 035 U l für die Knieendoprothesenversorgung zeigt.
Für die dentale Implantologie kommt zweckmäßigerweise ein Stirnplanfräser in Betracht, der über eine unterseitige Schneidefläche verfügt, womit ein Hohlraum oberhalb der für das Implantat geschaffenen Kavität erzeugt werden kann, der einen größeren Durchmesser als die Kavität aufweist. Das eingesetzte und in die Kavität eingeschraubte Implantat ragt dabei in den Hohlraum hinein, so dass das erfindungsgemäße Knochenersatzmaterial um den oberen Bereich des Implantats anordbar ist. Das vorzugsweise ringförmig um das Implantat bündig angeordnete Knochenersatzmaterial bildet so eine kraft- und formschlüssige Verbindung mit dem Implantat, wie auch nach der Inkorporation mit dem umgebenden Knochen. Anspruch 6 sieht daher vor, dass das Knochenersatzmaterial ringförmig ausgebildet ist.
Eine praktikable Variante der Erfindung sieht vor, dass das Set als chirurgisches Instrument einen Bohrfräser aufweist. Die Verwendung von Bohrfräsern in der Zahnheilkunde ist bekannt. Die DE 35 43 737 AI offenbart einen Bohrfräser für die Wurzelbehandlung . Die Verwendung eines Bohrfräsers hat den Vorteil, dass die Erzeugung der Kavität für das Implantat und die Erzeugung des Hohlraumes für das Knochenersatzmaterial in einem Arbeitsgang vollzogen werden kann.
Zudem sieht die Erfindung ein dentales Implantat vor, das mit einem Knochenersatzmaterial aus Granulat versehen ist, wobei das Knochenersatzmaterial von fester und harter Beschaffenheit ist. Vorgesehen ist also ein einteiliges Gebilde, das sich aus dem Implantat und dem Knochenersatzmaterial zusammensetzt. Zusätzlich kann das Implantat vorzugsweise mit einer festen und harten Membran versehen sein. Sämtliche Bestandteile, d .h. Implantat, Knochenersatzmaterial und Membran, können dabei zweckmäßigerweise lösbar miteinander verbunden sein.
Schließlich sieht die Erfindung auch die Verwendung des erfindungsgemäßen Kno- chenersatzmateriales, der erfindungsgemäßen Membran sowie die Verwendung des Planfräsers für die dentale Implantologie vor.
Im Folgenden wird die Erfindung anhand der Zeichnungen näher erläutert. Es zeigt in schematischer Darstellung :
Fig . 1 in einer Schnittansicht das dentale Implantatset gemäß der Erfindung, wobei die Komponenten des Sets nach der Implantation gezeigt sind,
Fig . 2 in einer Schnittansicht eine für das Implantat erzeugte Kavität und ein für das Knochenersatzmaterial erzeugter Hohlraum in einem Kieferknochen,
Fig . 3 in einer Schnittansicht einen Planfräser als weitere Komponente des erfindungsgemäßen dentalen Implantationssets, wobei der Planfräser während des Einsatzes bei einer dentalen Implantologie gezeigt ist und
Fig . 4 die Schritte einer dentalen Implantologie in einer Schnittansicht, bei denen Komponenten des erfindungsgemäßen dentalen Implantatsets eingesetzt werden.
Fig . 1 zeigt das erfindungsgemäße dentale Implantatset, das mit dem Bezugszeichen 1 versehen ist und dessen Komponenten im Zustand des in den zahnlosen Bereich des Kieferknochens 5 eingesetzten Implantats 2 und des über den Implantatbereich genähten Zahnfleisches 6 gezeigt sind .
Zu den Komponenten des Implantatsets 1 gehören neben dem Implantat 2 das Knochenersatzmaterial 3 und die Membran 4. Vor Einsetzung des Implantats 2 wurde mittels eines herkömmlichen Implantatbohrers eine Kavität 9 mit einer fest definierten und präzisen Breite und Tiefe, wie auch Fig . 2 veranschaulicht, erzeugt, in der in der in Fig . 1 dargestellten Ausführungsform der Erfindung ein hälftiger Anteil des Implantats 2 eingesetzt und eingeschraubt ist.
Um das Implantat 2 herum ist ringförmig das Knochenersatzmaterial 3 angeordnet. Entgegen dem Stand der Technik befindet sich das granuläre Knochenersatzmaterial 3 nicht in einem losen körnigen, sondern in einem festen und harten Zustand, so dass das Knochenersatzmaterial 3 als ringförmiger harter Block einen zuvor für die Knochenaugmentation erzeugten und in Fig . 2 gezeigten Hohlraum 15 passgenau ausfüllt. Wie aus Fig. 2 zudem hervorgeht, ist der Hohlraum 15 bei durchbohrtem und geöffnetem Zahnfleisch 6 von einer Längsseite 16 und einer kürzeren Seite 8 begrenzt, die am oberen Ende der Kavität 9 ausgeformt ist, so dass die Kavität 9 im Gegensatz zu der herkömmlichen Praxis gewissermaßen durch den einen größeren Durchmesser als die Kavität 9 aufweisenden Hohlraum 15 eine räumliche Ausdehnung erfährt.
Das erfindungsgemäße dentale Implantatset 1 weist zudem, wie Fig. 1 weiter zeigt, eine Membran 4 auf, die, wie das Knochenersatzmaterial 3, von fester und harter Beschaffenheit ist. Die einteilig ausgebildete Membran 4 umgibt sowohl das obere
stirnseitige Ende des Implantats 2 und des Knochenersatzmateriales 3 als auch einen Teil der Wandung des als harten ringförmigen Blockes ausgebildeten Knochenersatzmateriales 3 und passt sich somit der oberen und seitlichen Außenkontur des Knochenersatzmateriales 3 bzw. des Implantats 2 an. Am unteren Ende des Teiles der Membran 4, der der seitlichen Außenkontur des Knochenersatzmateriales 3 zugewandt ist, weist die Membran ein plastisch verformbares Endstück 18 auf, um im Übergang der Membran 4 zum Knochen 5 eine optimale Adaptation zu erreichen.
Das Implantat 2 ist lösbar mit der Membran 4 verbunden. Hierzu verfügen die Membran 4 über ein Loch 7a und das Implantat 2 über einen zum Loch 7a korrespondierenden Druckknopf 7, so dass die Membran 4 auf das Implantat 2 aufsetzbar bzw. aufsteckbar ist. Der Druckknopf 7 ist Teil einer Verschlussschraube, die integraler Bestandteil des Implantats 2 ist und mit der das obere Ende des Implantats 2 geschlossen wird .
Fig . 3 zeigt als weitere Komponente des in Fig. 1 gezeigten erfindungsgemäßen dentalen Implantationssets 1, und zwar während eines dentalen Implantationsschrittes einen Planfräser 17 in Gestalt eines Stirnplanfräsers. Der Planfräser 17 weist ein Stirnfräserelement 10 auf, das unterseitig über eine Schneide 12 verfügt. Unterhalb und mittig des Stirnfräserelementes 10 ist ein Zentrierstift 13 angeordnet, dessen Länge und Tiefe der Länge und Tiefe der in Fig. 2 gezeigten Kavität 9 entspricht. Wie aus Fig. 3 weiter hervorgeht, befindet sich oberhalb des Stirnfräserelementes 10 ein Befestigungselement 14, das die Verbindung zu einem in Fig . 3 nicht gezeigten Antrieb herstellt.
Alternativ kann statt des Planfräsers 17 auch ein Bohrfräser vorgesehen sein, der statt des Zentrierstiftes 13 über einen Bohranteil verfügt, so dass die Funktion eines Versenkbohrers und eines Planfräsers in einem Werkzeug vereint ist. Hierdurch wird vorteilhafterweise erreicht, dass die Erzeugung des in Fig . 2 gezeigten Hohlraumes 15 und die Erzeugung der in Fig. 2 gezeigten Kavität 9 in einem Arbeitsgang realisiert werden kann.
Die Schritte einer dentalen Implantation, bei der das in Fig. 1 gezeigte Implantatset 1 sowie der in Fig . 3 dargestellte Planfräser 17 eingesetzt werden, veranschaulicht Fig . 4. Nachdem das Zahnfleisch 6 geöffnet und die Kavität 9 - wie in der dentalen Implantologie üblich - mittels eines herkömmlichen Implantatbohrers erzeugt worden ist, wie Fig. 4a veranschaulicht, kommt in einem nächsten und in Fig. 4b gezeigten Schritt der Planfräser 17 zum Einsatz. Der Durchmesser des Zentrierstiftes 13 des Planfräsers 17 entspricht dabei dem Durchmesser der Kavität 9. Der in Gestalt eines Stirnplanfräsers vorliegende Planfräser 17 fräst mittels der Schneide 12 den in Fig . 2 zeigten genormten Hohlraum 15. Danach wird, wie Fig . 4c verdeutlicht, das Knochenersatzmaterial 3, und zwar in Form eines ringförmigen Blockes, in den Hohlraum 15 eingesetzt. Vor Einsetzung des Implantats 2 ist das Knochenersatzmaterial 3 unverschieblich und einteilig mit dem Implantat 2 verbunden. Alternativ kann das Knochenersatzmaterial 3 auch nach dem Eindrehen des Implantats 2 in den in Fig . 2 dargestellten Hohlraum 15 eingesetzt und von dem Implantat 2 und dem Knochen 5 aufgenommen werden. Hierbei kann auch durch eine abweichende Formgebung des Knochenersatzmateriales 3 eine grundsätzliche Veränderung der finalen Knochensituation herbeigeführt werden, und zwar im Sinne einer horizontalen und/oder vertikalen Knochenaugmentation.
Nachdem das Implantat 2, das mit dem Knochenersatzmaterial 3 versehen ist, eingedreht worden ist, wird das Implantat 2 mit einer Verschlussschraube geschlossen, die oberseitig mit dem Druckknopf 7 abschließt (Fig . 4d).
Wie weiter aus Fig . 4d hervorgeht, wird zur Abdeckung des Implantationsbereiches die feste und harte Membran 4 eingebracht, die mit dem Loch 7a versehen ist und auf dem leicht überkonturierten Rand nach dem "Druckknopfprinzip" aufgeknöpft wird . Im Übergang zwischen durch die Membran 4 abgedecktem Knochenersatzmaterial 3 und Knochen 5 befindet sich das plastisch verformbare Endstück 18 der Membran 4, um eine optimale Adaption, auch zum Knochen 5, zu erreichen.
Schließlich wird, wie Fig . 4e zeigt, die Wunde durch Schließung und Vernähung des Zahnfleisches 6 geschlossen.
Die vorliegende Erfindung beschränkt sich in ihrer Ausführung nicht auf das vorstehend angegebene bevorzugte Ausführungsbeispiel. Vielmehr ist eine Anzahl von Varianten möglich, welche von der dargestellten Lösung auch bei grundsätzlich anders gearteten Ausführungen Gebrauch machen. Beispielsweise kann das Knochenersatzmaterial 3 und die Membran 4 auch aus anderen als in der Beschreibung angeführten Materialien bestehen, oder in Verbindung mit einer Direktimplantation nach Extraktion eines Zahnes, bei der der Durchmesser des leeren Zahnfachs größer als der Implantatdurchmesser ist.

Claims
1. Dentales Implantatset (1), aufweisend :
- ein Implantat (2)
- ein granuläres Knochenersatzmaterial (3),
- eine Membran (4),
dadurch gekennzeichnet,
dass das Knochenersatzmaterial (3) sowie die Membran (4) von harter und fester Beschaffenheit ist.
2. Set nach Anspruch 1,
dadurch gekennzeichnet,
dass es als chirurgisches Instrument einen Planfräser (17) aufweist.
3. Set nach Anspruch 1,
dadurch gekennzeichnet,
dass es als chirurgisches Instrument einen Bohrfräser aufweist.
4. Set nach Anspruch 2,
dadurch gekennzeichnet,
dass der Fräser (17) ein Stirnplanfräser ist.
5. Set nach Anspruch 3,
dadurch gekennzeichnet,
dass das Fräserelement unterseitig eine Schneide aufweist.
6. Set nach einem der Ansprüche 1 bis 5,
dadurch gekennzeichnet,
dass das Knochenersatzmaterial (3) ringförmig gebildet ist.
7. Set nach einem der Ansprüche 1 bis 6,
dadurch gekennzeichnet,
dass die Membran (4) aus Polylactid gebildet ist.
8. Set nach einem der Ansprüche 1 bis 4,
dadurch gekennzeichnet,
dass das Implantat (2) mit dem Knochenersatzmaterial (3) versehen ist.
9. Set nach einem der vorhergehenden Ansprüche,
dadurch gekennzeichnet,
dass die Membran (4) mit einem Loch (7a) versehen ist.
10. Set nach einem der vorhergehenden Ansprüche,
dadurch gekennzeichnet,
dass die Membran (4) ein plastisch verformbares Endstück (18) aufweist.
11. Knochenersatzmaterial (3) aus Granulat,
dadurch gekennzeichnet,
dass es von harter und fester Beschaffenheit ist.
12. Material nach Anspruch 11,
dadurch gekennzeichnet,
dass es ringförmig gebildet ist.
13. Membran (4) gekennzeichnet durch eine harte und feste Beschaffenheit.
14. Membran nach Anspruch 13,
dadurch gekennzeichnet,
dass sie mit einem Loch (7a) versehen ist.
15. Membran nach Anspruch 13 oder 14,
dadurch gekennzeichnet,
dass sie aus Polylactid ist.
16. Membran nach einem der Ansprüche 13 bis 15,
dadurch gekennzeichnet,
dass sie ein elastisches Endstück (18) aufweist.
17. Dentales Implantat,
dadurch gekennzeichnet,
dass es mit einem Knochenersatzmaterial (3) aus Granulat versehen ist, wobei das Knochenersatzmaterial (3) von harter und fester Beschaffenheit ist.
18. Implantat nach Anspruch 17,
dadurch gekennzeichnet,
dass es mit einer harten und festen Membran (4) versehen ist.
19. Implantat nach Anspruch 17 oder 18,
dadurch gekennzeichnet,
dass die Membran (4) mit dem Implantat lösbar verbunden ist.
20. Verwendung des Knochenersatzmateriales (3) gemäß einem der Ansprüche 11 oder 12 für die dentale Implantologie.
21. Verwendung der Membran (4) gemäß einem der Ansprüche 13 bis 16 für die dentale Implantologie.
22. Verwendung des Planfräsers (17) gemäß Anspruch 2 für die dentale Implantologie.
