Description

用于接骨板的组合式锁定螺钉及包括其的接骨板系统 技术领域
本发明涉及医疗器械植入物, 具体涉及骨科创伤治疗手术中使用 的锁定螺钉和接骨板系统。 背景技术
骨折外科手术治疗的目的是为了重建解剖和恢复功能。 按照 AO
(ASIF ) 的原则, 骨折内固定是一种建立于精确的骨折解剖复位、 稳 定的固定、 尽可能保留骨的血运 (血液供应) 、 早期功能性活动的基 础上的综合治疗方法。 骨折内固定早期釆用的是普通接骨板技术。 但是, 普通接骨板系 统没有可用于锁定的结构, 使得普通接骨板系统的螺钉和接骨板的连 接不能形成固定。 所以, 普通接骨板系统存在生物力学缺陷: 压迫骨 膜、 影响骨折端血液供应, 这导致传统的牢固固定的钢板接骨术具有 相当高的并发症发生率, 包括感染、 钢板断裂、 延迟愈合及不愈合。 近年来发展了锁定接骨板技术, 其依靠接骨板孔上的锥螺紋和锁 定螺钉头部锥螺紋的连接来产生锁定效果的。 锁定接骨板系统不依赖 骨 -钢板界面之间的摩擦力, 从而降低了骨膜的损伤, 尽可能多地保留 了骨的血运。 锁定接骨板系统的稳定性靠具有角度稳定的螺钉与钢板 之间的界面维持。 由于这种锁定内固定器具有稳定的整体性, 其锁定 头的螺钉的抗拔出力较普通螺钉高出很多, 因此目前锁定接骨板技术 在市场上的运用越来越多。 中国发明专利申请 CN2011 10003221.0 公开一种具有万向锁定螺 钉的钢板螺钉系统, 该系统包括钢板和至少两根具有锥形钉帽的锁定 螺钉。 中国实用新型专利 CN201020132646.2 公开一种固定骨骼的锁定 钢板装置, 其包括锁定螺钉、 开设有锁定螺纹孔的锁定钢板、 外径与 锁定螺钉的头部相一致的第一锁定套筒和内径略大于锁定螺钉的头部 的第二锁定套筒。 中国实用新型专利 CN200920026695.5 公开一种股骨近端锁定板 内固定系统, 其结构包括锁定板、 锁定螺钉, 锁定板上端边缘开有缝 合孔, 粗隆钩通过固定环固定在缝合孔上, 锁定板上部开有股骨颈锁 定钉孔, 钉孔通过螺纹固定有股骨颈锁定钉。 不过, 锁定接骨板系统的锁定螺钉和接骨板的连接虽然可以形成 固定, 但锁定螺钉轴向的抗拔出能力和径向抗旋转能力不理想。 而且, 锁定接骨板较普通钢板远为昂贵, 有些骨折也不必用锁定接骨板进行 治疗。 另外, 手术中很难利用锁定接骨板来对骨折进行充分复位, 尤 其是只有锁定孔的特殊钢板。 在此情形中, 在钢板固定以前, 必须先 将骨折端复位。 一旦锁定螺钉透过钢板打入骨端, 骨端就不能通过打 入另外螺钉或使用加压装置等方式来调整位置。 发明内容
本发明的目的是解决普通接骨板系统的螺钉和接骨板的连接不能 形成固定的问题以及锁定接骨板系统的锁定螺钉和接骨板的连接之后 的锁定螺钉轴向的抗拔出能力和径向抗旋转能力不足的问题。 根据本发明的第一方面,提供一种用于接骨板的组合式锁定螺钉, 特别适合于普通接骨板系统。 所述锁定螺钉包括用于植入人体的接骨 螺钉和锁定部件, 接骨螺钉在其头部上设置有相对于其纵向轴线倾斜 一角度的贯穿孔, 以供锁定部件延伸穿过, 且所述贯穿孔在孔内壁上 设有接合装置, 用于接合锁定部件上的相应的接合装置。 在本发明的 实施例中, 接骨螺钉的接合装置和锁定部件的接合装置包括螺紋, 锁 定部件为锁定钉。 根据本发明, 贯穿孔相对于接骨螺钉的纵向轴线倾斜的角度在

如图 5所示, 组合式锁定螺钉 Γ包括接骨螺钉 3'和锁定针 4。 接 骨螺钉 3 '与实施例 1 的接骨螺钉 3基本相同。 在本例中, 斜螺孔直径 2.5〜10mm, 夹角与实施例 1中的相同。 实施例 2与实施例 1的区别在于锁定针 4。 如图 6所示, 锁定针 4 包括前端的锁定针杆 12, 用于植入人体内, 和可折断的尾翼 10。此外, 锁定针杆 12在与尾翼 10相连接的一端上设有夹持槽 1 1。 手术时, 将锁定针 4沿着接骨螺钉 3上的斜螺孔 6植入人体。 植 入到位后, 折断尾翼 10, 仅留下设有夹持槽 1 1的锁定针杆 12, 如图 7 所示。 植入时, 通过夹持工具 (未示出)夹住尾翼 10来植入锁定针 4; 取出时, 在尾翼 10折断的情况下通过夹持工具夹住夹持槽 1 1 来取出 锁定针杆 12。 本发明的组合式锁定螺钉既能单独地运用 AO标准的接骨板和螺 丝钉技术、 也能单独地运用内固定支架的原则、 还能将两种方法的结 合使用。 因此, 本发明对任何骨折的治疗都能取得最好的临床效果。 本领域技术人员可以理解, 以上描述只是示例性的。 在不背离本 发明的思想和范围的情况下, 本领域技术人员可以对本发明作出多种 修改和变化。

Claims

权 利 要 求 书
1. 一种用于接骨板的组合式锁定螺钉, 其特征在于, 所述锁定螺 钉包括用于植入人体的接骨螺钉和锁定部件, 接骨螺钉在其头部上设 置有相对于其纵向轴线倾斜一角度的贯穿孔, 以供锁定部件延伸穿过, 且所述贯穿孔在孔内壁上设有第一接合装置, 用于接合锁定部件上的 相应的第二接合装置。
2. 权利要求 1 所述的组合式锁定螺钉, 其中所述角度在 10°-30° 之间。
3. 权利要求 1或 2所述的组合式锁定螺钉, 其中所述接骨螺钉的 第一接合装置和所述锁定部件的第二接合装置包括螺紋。
4. 权利要求 1或 2所述的组合式锁定螺钉, 其中所述锁定部件包 括设有所述第二接合装置的前端锁定部分和连接于前端锁定部分且可 折断的尾端部分。
5. 权利要求 4所述的组合式锁定螺钉, 其中所述前端锁定部分在 与尾端部分相连接的一端上设有夹持槽。
6. 权利要求 1或 2所述的组合式锁定螺钉, 其中接骨板为普通接 骨板。
7. 一种接骨板系统, 其包括接骨板和一个或多个权利要求 1-6中 任一项的组合式锁定螺钉。
