Abstract
아크릴-고무 하이브리드를 약물·점착층으로 포함하는 펜타닐 경피 패치제로, 펜타닐의 피부투과도가 일정하게 3일간 유지되면서 피부와의 접촉을 긴밀하게 하여 수분이나 땀의 방출로 인한 피부탈착이 개선되고 피부 자극이 개선된 펜타닐 경피 패치제가 제공된다.
Description
본 발명은 펜타닐 경피 패치제에 관한 것이다. 더 상세하게는 아크릴-고무 하이브리드 약물·점착층을 포함하여서 펜타닐의 피부투과도는 일정하게 유지하면서 피부와의 접촉을 긴밀하게 하여 과도한 수분이나 땀의 방출로 인한 제제의 피부탈착이 개선된 펜타닐 경피 패치제에 관한 것이다.
본 발명에서 경피 패치제, 경피흡수 패치제, 경피투여 패치제 등의 용어는 모두 피부를 통해 약물을 전달하는 경피 약물 전달계 (Transdermal drug delivery system) 기술의 경고형 제제(고형화된 제제)를 나타낸다.
통증은 통증 유발경로와 관련된 유해수용기 및 기타 신경세포들이 위해 자극에 반응하여 척수를 통하여 뇌로 메시지를 보냄으로 인지된다. 펜타닐 및 그 유도체들은 아편양 제제(Opioid)로서 척수의 아편성 물질 수용체에 작용하여 신경전달물질의 방출을 억제해서 결국 통증 메시지가 척수 뉴런으로 전달되는 것을 막아 진통작용을 나타낸다. 아편양 제제는 진통작용에 있어서 천장 효과(ceiling effect)가 있는 NSAIDs와는 달리 그 효능과 간편한 적정(titration), 위험-효과 비율(risk-to-benefit ratio)의 이점 때문에 중증도부터 극심한 통증에 이르기까지 모든 통증의 치료에 유용하게 사용되고 있다.
펜타닐은 피부를 통해서 투여하기에 적합하고 피부를 통해 투여할 경우 부착하는 간단한 방법으로 투여할 수 있고, 간(liver) 초회통과를 거치지 않을 뿐만 아니라 환자의 편의성, 복약 순응도 상승, 지속적인 진통작용, 간호노력의 감소 그리고 수면 방해의 감소 등의 장점이 생긴다. 피부를 통해서 펜타닐을 투여하는 제제는 특히 암성 통증 환자에 많이 사용되는데 암에 의한 체온 상승 시에는 피하조직으로부터의 흡수량이 증가하여 작용시간이 단축될 수 있다.
이미 상용화된 펜타닐의 경피 투여 제제로는 듀로제식(Durogesic, Janssen), 듀로제식 디트랜스 패치(Durogesic D-trans, Janssen), 펜타닐 경피 시스템 (Fentanyl transdermal system) CII (mylan, Teva, Watson), 펜스터드 (Fenstud) (Rusan)등이 있으며 듀로제식, 듀로제식 디트랜스 패치를 제외한 나머지는 복제의약품이다. 듀로제식은 저장기 형 (reservoir type)으로서 에탄올과 물로 구성된 액상의 약물저장층에 펜타닐이 혼합 용해되어 있는 시스템이다. 이 저장기형 경피 투여 제제는 지지층, 약물 및 경피흡수 촉진제로서, 에탄올을 함유한 겔의 저장기 층, 방출속도 조절막층, 점착층 및 박리층으로 이루어져 있어, 약물의 방출속도를 손쉽게 제어하고 피부투과를 촉진할 수 있는 장점이 있다. 그러나 피부투과촉진제로 사용되는 에탄올로 인한 자극성 때문에 3일 정도의 장기간 사용 시에는 피부자극 등 부작용을 나타낼 수 있다. 또한 상기 제제는 여러 층으로 구성되어 있으므로 상당히 복잡한 제조 공정을 필요로 하며, 피부 부착감이 좋지 않으며, 약효 발현이 12시간 이상 경과되어야 (지연 시간) 나타나는 등의 단점을 지니고 있다. 또한 저장기 형의 펜타닐의 경피 투여 제제는 저장기가 새는 경우 활성 화합물-함유 저장기 충전물이 넓은 영역에 걸쳐 피부와 접촉하게 되고 활성 화합물이 지나치게 높은 투여량으로 흡수되는 단점을 갖는다. 특히 펜타닐 및 이의 유도체가 과량으로 인체에 투여되는 경우 매우 급속한 호흡 저하가 발생하여 치명적인 사고가 야기되므로 특히 위험하다 (Clinical Pharmacokinetics. 2000, 38(1), 59-89).
이러한 단점을 극복하기 위해 아크릴 점착제를 사용하여 매트릭스(matrix)형태의 모노리틱(monolithic) 패치가 개발되었으며 이 제제는 약물의 안정적 전달, 에탄올에 의한 피부자극 감소 등의 성능의 개선을 이루었으나 땀이나 수분에 의한 패치의 탈락 및 점착력으로 인한 피부자극 등을 완전히 해결하지는 못하였다.
지연시간(lag time)을 단축시키기 위한 시도로서, 약물의 빠른 방출을 유도하기 위해 펜타닐의 확산속도가 빠르고 용해도가 낮은 실리콘계열이나 고무계열의 점착제들을 이용하여 패치들을 개발하였다. 실리콘 계열 점착제로는 내-아민성 폴리디메틸실록산 (amine-resistant polydimethylsiloxane)을 사용하는 매트릭스형 펜타닐 패치제가 개발되었으며, 고무계열 점착제로는 고분자와 저분자의 폴리이소부틸렌(Poly isobutylene, PIB)을 혼합하여 점착기제를 구성하거나 스티렌 블록코폴리머 계통의 스티렌-이소프렌-스티렌(Styrene-Isoprene-Styrene, SIS)과 폴리이소부틸렌(PIB)의 매트릭스형 펜타닐 패치제가 고안되었다. 상기의 패치제들은 약물의 피부투과 속도를 증가시켜 패치의 면적을 감소시키는 등의 효과가 있었으나, 제제의 매트릭스를 구성하는 고분자가 지나치게 소수성이어서 땀이나 수분에 의한 패치의 탈락 등의 문제가 여전히 남아있다.
펜타닐의 용해도가 각각 다른 점착성 중합체로 이루어진 매트릭스층을 인접하여 제조하는 제제는 투여 초기에는 펜타닐에 대한 용해도가 낮은 점착성 중합체로부터 일정량의 펜타닐이 빨리 방출되어 체내 유효 혈중농도 도달시간을 줄임으로써 진통효과의 발현시간을 앞당길 수 있으며, 일정시간 후에는 펜타닐의 용해도가 높은 점착성 중합체로부터 유효 혈중농도를 유지할 수 있을 만큼 약물이 방출되어 장기간 진통효과를 유지할 수 있다. 그러나 상기 제제를 제조하기 위해서는 두 종류의 약물 함유 점착용액을 별도로 제조하여 한 종류의 약물 함유 점착용액은 지지층 위에, 나머지 한 종류의 약물 함유 점착용액은 박리층 위에 도포 건조한 후, 이 두 약물 함유 점착층이 서로 마주보도록 하여 서로 겹쳐지지 않고 인접하게끔 정밀히 합지해야 한다. 이어 인접한 각각의 약물 함유 점착층이 일정한 면적 비율이 되도록 정확히 절단하여야 제조가 가능하므로, 제조공정도 복잡할 뿐만 아니라 공정에서 발생되는 펜타닐의 손실율도 매우 크다.
따라서 제조가 간단하면서도, 펜타닐을 장기간(3일) 일정하게 전달할 수 있고, 장기간 피부 부착성을 유지하면서도 피부를 자극하는 부작용이 없는 펜타닐 경피 패치제의 개발이 당업계에서 요망되어 왔다.
펜타닐을 장기간 일정하게 전달할 수 있고 장기간 피부부착성을 유지할 수 있는 경피흡수 패치제를 제조하기 위해 본 발명자들이 지속적인 연구를 수행한 결과, 아크릴-고무 하이브리드(Acrylic-Rubber hybrid)를 약물·점착층으로 사용하는 경우, 제조방법도 간단하면서 펜타닐을 일정속도로 장기간 피부를 통해 전달할 수 있으며 부착력이 개선된 본 발명의 패치제를 개발하게 되었다.
따라서 본 발명의 목적은 아크릴-고무 하이브리드를 약물·점착층으로 함유하는 펜타닐 경피 패치제로, 펜타닐의 피부투과도가 일정하게 3일간 유지되면서 피부와의 접촉을 긴밀하게 하여 수분이나 땀의 방출로 인한 피부 탈착이 개선된 펜타닐 경피 패치제를 제공하는 것이다.
상기 목적을 달성하고자, 본 발명은 지지체 및 상기 지지체의 일 측면에 약물·점착층을 갖는 펜타닐 경피 패치제로, 상기 약물·점착층은 아크릴-고무 하이브리드 중합체 및 약물로서 펜타닐, 펜타닐 유도체 또는 이들의 약제학적 염을 포함하는 펜타닐 경피 패치제를 제공한다.
필요에 따라서, 본 발명에 따른 펜타닐 경피 패치제는 일면에 주변부점착필름 및/또는 박리필름층을 추가로 포함할 수 있다.
본 발명에서, 약물·점착층은 아크릴-고무 하이브리드 중합체 및 약물을 포함한다.
아크릴계 중합체는 사용되는 모노머에 따라 작용기를 갖게 할 수도 작용기가 없게 할 수도 있으며 약물용해도가 좋고 수분에 대한 저항성이 높으나, 일반적으로 높은 약물 용해도 때문에 고무계 및 실리콘 중합체에 비해서 약물의 피부흡수가 떨어진다. 따라서 일반적으로 아크릴계 중합체를 점착 기제로 사용하는 경우 약물을 고농도로 함유시키거나 경피흡수촉진제를 사용한다. 한편 고무계 및 실리콘 중합체는 약물의 확산 및 피부투과도가 높으나 물질 자체가 소수성이 높아 수분 및 땀에 의해 점착력이 약해지는 문제점이 있다.
아크릴-고무 하이브리드 중합체는 아크릴 주쇄(acrylic backbone)에 저분자량의 고무 거대단량체(rubber marcromer)이 그래프트된 중합체이다.
일반적으로 아크릴 주쇄를 형성하기 위한 단량체는 2-에틸헥실 아크릴레이트, 부틸 아크릴레이트, 이소-옥틸 아크릴레이트과 같은 유리전이 온도가 낮은 단량체 (초기 점착력을 부여), 메틸 아크릴레이트, 메틸 메타아크릴레이트과 같은 유리전이 온도가 높은 단량체(응집력을 부여), 아크릴산, 2-하이드록시에틸 아크릴레이트과 같은 작용기를 갖는 단량체(작용기와 점착력을 부여) 등이 사용되고 추가적으로 비닐아세테이트(vinyl acetate)와 같은 단량체가 사용되기도 한다. 고무 거대단량체는 예를 들어 메타크릴레이트를 말단기(end group)로 가지는 폴리 에틸렌-부틸렌 등이 있으며, 아크릴 주쇄에 고무 거대단량체가 그래프트되면 소수성을 갖는 도메인이 추가된다.
아크릴-고무 하이브리드 중합체는 용액, 분산 또는 에멀젼 중합법으로 제조되며, 라디칼 반응을 통해 중합되는데 알킬 에스터 (alkyl ester)계인 아크릴계 단량체를 주 단량체로 사용하고 이중결합을 말단기에 가지는 고무 거대단량체 (rubber macromer)를 공단량체(comonomer)로 하여 AIBN (azobis(isobutylronitrile)과 같은 라디칼 개시제를 사용하여 라디칼 중합시킨다. 이와 같은 중합은 아크릴 단량체의 반응성이 고무 거대단량체에 비해 반응성이 높기 때문에 아크릴 주쇄에 저분자량의 고무계 중합체를 곁가지로 갖는 다음과 같은 형태를 갖는 아크릴-고무 하이브리드 중합체가 형성된다.
이 소수성을 갖는 도메인 때문에, 작용기를 갖는 아크릴계 중합체의 특성에 소수성의 장점이 추가되어 소수성 약물의 용해도가 증가하거나 소수성 첨가제들(예를 들어 탄화수소수지 점착부여 수지 또는 테르펜 점착부여 수지, 미네랄 오일, 폴리부텐 등)을 점착층에 쉽게 혼합시킬 수 있으며 고무가 가지는 높은 약물 방출력이 아크릴 주쇄에 더하여져 두 중합체의 장점을 갖게 된다.
상기와 같이 중합하여 사용될 수도 있으며, 또한 아크릴-고무 하이브리드 중합체는 시판되고 있는 듀로택 (Duro-tak, Henkel corporation) 즉, Duro-tak 87-502A, Duro-tak 87-503A, 또는 Duro-tak 87-504A 등을 사용할 수도 있으며 기타 제조사의 아크릴-고무 하이브리드 중합체를 사용할 수 있다.
본 발명에서 아크릴-고무 하이브리드 중합체는 약물·점착층의 60~95 중량부로 포함된다. 아크릴-고무 하이브리드 중합체가 60 중량부 이하인 경우에는 이 중합체가 가지는 점착 물성이나 응집력등이 많이 훼손되어 적합한 물성을 나타내지 못한다. 또한 아크릴-고무 하이브리드 중합체가 95 중량부 이상인 경우에는 약물의 농도가 너무 낮아 패치를 통해 약물을 체내로 필요량만큼 전달하는 것이 어렵다.
본 발명에서 사용되는 약물인 펜타닐은 자유염기(free base) 형태인 펜타닐 또는 이의 유도체인 알펜타닐, 카르펜타닐, 로펜타닐, 레미펜타닐, 서펜타닐 및 트레펜타닐, 또는 이들의 약제학적으로 허용가능한 염 등이 있다. 약제학적 염은 염산, 질산, 인산, 황산, 브롬화수소산, 요드화수소산, 아질산 또는 아인산과 같은 무기산류, 지방족 모노 및 디카르복실레이트, 페닐-치환된 알카노에이트, 하이드록시 알카노에이트 및 알칸디오에이트, 방향족 산류, 지방족 및 방향족 설폰산류와 같은 무독성 유기산의 염일 수 있다.
본 발명에서 약물·점착층 내에 펜타닐은 5~20 중량부로 포함되는 것이 바람직하다. 5 중량부 미만이면 제형 내에서 약물의 농도가 지나치게 낮으므로 필요한 양의 약물을 체내로 전달할 수 없고 20 중량부 이상이면 약물의 피부흡수는 증가하나 제제의 두께가 지나치게 낮아 패치의 제조가 어렵고 약물의 피부흡수도 너무 높아 패치의 사용 중 환자가 지나치게 약물에 노출되어 위험한 상황이 생길 수 있다.
특히 펜타닐은 유효 혈중농도 이상이면 호흡 저하 및 근육 경직 등의 부작용이 나타나므로 혈중농도를 일정하게 유지시켜줄 필요가 있는데, 이때 가장 중요한 인자는 약물·점착층 내의 약물의 농도와 점착 기제(중합체)에 대한 약물의 용해도이다. 약물의 경피흡수도와 약물의 농도는 높을수록 동일한 면적과 함량을 갖는 패치에서 비례관계이며 동일한 조건에서 점착 기제에 대한 약물의 용해도는 약물의 경피흡수도와 반비례관계이다. 아크릴-고무 하이브리드 중합체는 대략 아크릴계 점착제와 고무계점착제 사이의 약물용해도를 갖는 것으로 생각할 수 있다.
본 발명에서 약물·점착층에는 필요에 따라 여러가지 첨가제를 사용할 수 있으며 그 첨가제의 종류로는 경피흡수촉진제, 점착부여제, 가소제, 산화방지제, 충진제 등이 있다.
본 발명에서 약물·점착층에는 피부로의 약물 투과속도를 증진시키기 위해 경피흡수 촉진제를 함유할 수 있으며, 경피흡수 촉진제로는 피롤리돈 유도체, C8-18의 탄소사슬을 가지는 지방산 유도체, 또는 트리아세틴 등이 사용될 수 있다. 피롤리돈 유도체로는 N-메틸-2-피롤리돈(N-methyl-2-pyrrolidone), N-카프릴릴-2-피롤리돈(N-carprylyl-2-pyrrolidone), N-도데실-2-피롤리돈(N-dodecyl-2-pyrrolidone) 등이 바람직하고, N-도데실-2-피롤리돈(N-dodecyl-2-pyrrolidone, Lauryl pyrrolidone)인 것이 더욱 바람직하다. C8-18의 지방족 유도체는 글리세롤 라우릴알코올(glycerol lauryl alcohol), 올레일알코올(oleyl alcohol), 이소프로필 미리스테이트(isopropyl myristate), 소르비탄 모노올레이트(sorbitan mono-oleate), 프로필렌 모노라우레이트(propylene monolaurate), 프로필렌 모노올레이트(propylene mono-oleate), 올레일 마크로고 글리세라이드(oleyl macrogoglyceride), 올레인산(oleic acid), 라우로일 마크로고 글리세라이드(lauroyl macrogoglyceride), 리놀레오일 마크로고 글리세라이드(linoleoyl macrogoglyceride), 프로필렌글리콜 카프릴레이트/카프레이트(Propylene glycol caprylate/caprate), 소르비탄 모노스테아레이트 모노올레이트(Sorbitan monostearate mono-oleate), 글리세롤 모노라우레이트(glycerol monolaurate), 프로필렌글리콜 모노라우레이트(propylene glycol monolaurate), 프로필렌글리콜 모노카프릴레이트(propylene glycol monocaprylate), 소르비탄 모노라우레이트(sorbitan monolaurate), 라우릴 락테이트(lauryl lactate), 카프릴릭/카프릭 트리글리세라이드(caprylic/capric triglyceride), 옥수수유 PEG-8 에스터, 옥수수유 PEG-6 에스터 등이 바람직하고 글리세롤 모노올레이트, 글리세롤 모노라우레이트, 프로필렌글리콜 모노라우레이트, 프로필렌글리콜 모노카프릴레이트, 소르비탄 모노라우레이트 등이 더욱 바람직하다.
본 발명에서 상술한 경피흡수 촉진제는 1종 단독 또는 2종 이상을 혼합하여 전체 약물·점착층에 대해 5 ~ 20중량부로 포함하는 것이 바람직하며 더욱 바람직하게는 5~15 중량부 이내로 포함하여, 피부 부작용을 최소화하는 것이 바람직하다.
본 발명에서 약물·점착층에는 점착특성을 개선하기 위하여 점착부여제를 함유할 수 있으며, 점착부여제는 기제에 점착특성(초기 점착력, 점착유지력, 응집력)등의 물성을 개선시키는 역할을 한다. 종류로는 천연 또는 석유화학 수지계열, 친수성 고분자계열, 저분자량의 폴리부텐등이 있으며 그 중 천연 또는 석유화학 수지 계열로는 로진에스터계열 수지나 테르펜계열 수지, 지방족 탄화수소수지, 지환족 탄화수소수지, DCPD(dicyclo pentadiene), 쿠마론-인덴 등이 있으며 이들 수지류는 그 종류에 따라 초기 점착력, 점착유지력, 응집력등을 향상시킨는 역학을 하고 친수성 고분자 계열로는 폴리피롤리돈과 같은 피롤리돈계열의 중합체, 폴리아크릴레이트와 같은 아크릴계열의 중합체, 하이드록시프로필셀룰로오스와 같은 셀룰로오스 개질체 등이 있으며 이들 친수성 고분자들은 응집력을 향상시키며 피부에서 배출되는 수분과 접촉시 점착력을 갖게 되며 폴리부텐은 이소부틸렌을 주체로 하는 저온, 촉매중합반응에 의한 다양한 저분자량의 액상폴리머로서 점착제에 초기 점착력 및 점착유지력을 부여하며 가소성을 부여한다. 그 사용량은 수지의 경우 1~10 중량부로 사용하는 것이 좋으며 더욱 좋게는 5~10 중량부로 사용하는 것이 좋으며 친수성 고분자는 1~10 중량부로 사용하는 것이 좋으며 더욱 좋게는 5중량부 이내로 사용하는 것이 좋고 폴리부텐은 5~20 중량부로 사용하는 것이 좋으며 더욱 좋게는 5~15 중량부로 사용하는 것이 좋다.
본 발명에서 약물·점착층에는 가소성을 부여하기 위하여 가소제를 함유할 수 있으며, 가소제는 점착 기제의 유연성을 증가시키는 역할을 하고 그 종류로는 미네랄오일이나 폴리부텐, C8~C18까지의 탄소사슬을 가지는 각종 고급지방산 또는 이 지방산들의 에스터 혹은 유도체 등이 있으며 1~10 중량부로 사용하는 것이 좋으며 더욱 좋게는 5~10 중량부로 사용하는 것이 좋다.
본 발명에서 약물·점착층에는 약물 및 고분자의 산화를 방지하기 위하여 산화방지제를 함유할 수 있으며, 그 종류로는 토코페롤(Tocopherol)류, 프로필갈산 (propylgallic acid), 부틸하이드록시톨루엔 (Butylhydroxytoluene, BHT), 부틸하이드록시아니졸(Butylhydroxyanisole, BHA) 등이 있으며 1 중량부 이하로 사용하는 것이 좋다.
본 발명에서 약물·점착층에는 기계적 강도를 향상시키기 위하여 충진제를 함유할 수 있으며 그 종류로는 산화아연이나 산화티타늄 다공성 실리카 등이 있으며 1~20 중량부로 사용하는 것이 좋으며 더욱 좋게는 5~15 중량부로 사용하는 것이 좋다.
본 발명에서 사용되는 지지체는 펜타닐이 역확산되지 않고 피부 방향으로만 투여되도록 하는 것으로, 폴리에스테르필름, 알루미늄이 증착된 폴리에스테르필름, 다중 증착필름 또는 부직포가 합지된 폴리에스테르필름 중의 1종을 선택하여 사용할 수 있으며, 지지체의 두께는 피부에 붙였을 때 불편을 느끼지 않는 수준으로 10 ~ 200㎛가 바람직하며, 더욱 바람직하게는 10 ~ 50㎛정도가 좋다.
본 발명에서 사용되는 주변부 점착지지층은 점착제를 도포한 필름 또는 직물로서 패치의 약물·점착층 반대면인 지지체에 붙어 있으면서 패치와 함께 피부에 부착되어 패치의 부착력을 향상시킨다. 본 발명에서 사용될 수 있는 주변부점착 지지층은 필름에 점착성 물질을 도포한 것으로서, 이때 필름으로는 투습 통기성을 가지는 기재는 모두 사용될 수 있는데 그 종류로는 폴리우레탄 필름, 에틸렌 비닐아세테이트 공중합체 필름, 폴리에틸렌필름 등의 유연성을 가지는 필름, 다공성 필름, 천공필름, 직조 및 방사를 통해 얻어진 각종 천연 및 합성 직물과 부직포 형태의 가공물 및 고분자에 발포제를 넣어 형성시킨 얇은 폼 등이 있다. 폴리우레탄 필름, 다공성 필름, 천공필름, 직조 및 방사를 통해 얻어진 각종 천연 및 합성 직물과 부직포 형태의 가공물 및 고분자에 발포제를 넣어 형성시킨 얇은 폼 등의 통기성 지지체가 바람직하며, 더욱 바람직하게는 폴리우레탄 및 다공성 필름 등이 좋다.
상기 주변부점착 지지층에 사용되는 점착성 물질은 아크릴계 점착제에서부터 고무계, 실리콘계, EVA계 점착제까지 다양하게 사용될 수 있다.
본 발명에서의 박리필름은 제품이 포장 보관되는 동안에 약물 함유 점착층을 보호하고 제품 사용시 제거할 수 있도록 편리성을 제공하는 기능을 한다. 특별히 제한되지 않지만, 폴리에스테르, 폴리비닐클로라이드, 폴리비닐리덴클로라이드, 폴리에틸렌테레프탈레이트 등으로부터 제조되는 필름, 또는 종이류로서 상질지나 글라신지와 폴리올레핀의 적층 필름을 사용할 수 있다. 박리필름은 약물·점착층에 닿는 표면을 실리콘수지, 지질수지 또는 불소수지를 도포한 것으로 약물의 장기안정성이 우수한 폴리에틸렌테레프탈레이트 필름을 사용하는 것이 바람직하다.
본 발명에 따른 펜타닐 경피 패치제는 제조과정이 단순하고, 펜타닐의 피부투과도는 3일간 일정하게 유지하면서 약물·점착층과 피부의 접촉을 긴밀하게 하고 수분이나 땀의 방출로 인한 패치제의 피부탈착이 방지되고, 피부 자극이 개선되었다.

이하, 본 발명의 이해를 돕기 위하여 구체적인 실시예를 통하여 본 발명의 구성 및 효과를 보다 상세히 설명하기로 한다. 그러나 하기 실시예는 본 발명을 보다 명확하게 이해시키기 위하여 예시한 것일 뿐이며, 본 발명의 권리범위가 하기 실시예에 의해 한정되는 것은 아니다. 또한 제조 과정에서 각 조성의 첨가 순서는 하나의 예를 서술한 것이므로 이에 의해 한정되는 것은 아니다.
실시예
실시예 1
아크릴-고무 하이브리드 점착제 (Duro-tak 87-504A, Henkel Corporation) 100g(고형분 49g)과 톨루엔 30g, 에틸아세테이트 33.5g을 넣고 250mL 용량의 유리용기에 넣어 롤믹서를 사용하여 균일하게 섞어 고형분 30%로 희석시킨다. 희석된 Duro-Tak 87-504A 점착제 18g(고형분 5.4g)과 펜타닐 0.6g을 30mL 유리 바이알(vial)에 넣고 펜타닐을 점착액에 완전히 녹인다. 펜타닐을 녹인 점착액을 실리콘이 코팅된 두께 75㎛ 폴리에틸렌 테레프탈레이트(PET) 필름 위에 건조시 두께가 40㎛가 되도록 코팅하여 건조한 후에, 한쪽 면을 PET 필름 12㎛에 폴리에틸렌(PE) 필름 40㎛가 적층된 필름의 PET면을 덮고 라미네이트시키고 이후 21㎠의 크기로 절단하여 패치제를 제조했다.
실시예 2
아크릴-고무 하이브리드 중합체를 Duro-tak 87-504A 대신에 Duro-tak 87-503A (Henkel Corporation) 100g(고형분 43.5g)과 톨루엔 20g, 에틸아세테이트 25g을 250mL 용량의 유리용기에 넣어 롤믹서를 사용하여 균일하게 섞어 고형분 30%로 희석시킨다. 희석된 Duro-Tak 87-503A점착제를 사용하여 실시예 1과 동일한 과정으로 패치제를 제조했다.
실시예 3
희석된 Duro-Tak 87-504A 점착제 16g(고형분 4.8g)과 펜타닐 1.2g을 30mL 유리 바이알(vial)에 넣고 펜타닐을 점착액에 완전히 녹인다. 펜타닐을 녹인 점착액을 실리콘이 코팅된 두께 75㎛ 폴리에틸렌 테레프탈레이트(PET) 필름 위에 건조시 두께가 20㎛가 되도록 코팅하여 건조한 후에, 한쪽 면을 PET 필름 12㎛에 폴리에틸렌(PE) 필름 40㎛가 적층된 필름의 PET면을 덮고 라미네이트시키고 이후 21㎠의 크기로 절단하여 패치제를 제조했다.
실시예 4
희석된 Duro-Tak 87-504A 점착제 16g(고형분 4.8g), 펜타닐 0.6g과 글리세롤 모노올레이트 (Glycerol monooleate, GMO, PECEOL? Gatte Fosse) 0.6g을 30mL 유리 바이알에 넣고 점착액에 펜타닐과 GMO를 완전히 녹인다. 이후 실시예 1과 동일한 과정으로 패치제를 제조했다.
실시예 5
Duro-Tak 87-202A(Henkel Corporation) 점착제를 실리콘이 코팅된 두께 75㎛ PET 필름 위에 건조시 두께가 30㎛가 되도록 코팅하여 건조한 후, 종이에 폴리에틸렌이 적층되어 있고 실리콘 처리가 되어 있는 이형지를 케리어로 사용하여 도포된 폴리우레탄필름 (정우 PU, 폴리우레탄 두께 20㎛)의 우레탄면으로 건조된 점착층 위에 덮고 라미네이트 시킨다. 그 후에 PET 이형 필름을 벗겨내고 실시예 1과 같이 제작된 패치의 PET/PE적층 필름의 PE면을 폴리우레탄 위에 코팅된 점착제 위에 부착하고 패치의 PET 이형 필름을 제거한 다음 50㎛ 두께의 실리콘 처리된 PET 이형필름을 덮고 패치가 가운데 위치하도록 하고 35㎠의 면적으로 절단하여 주변부 점착필름을 가지는 패치제를 제조했다.
실시예 6
상기 실시예 3과 동일하게 제조하되, 글리세롤 모노올레이트 대신에 프로필렌글리콜 모노라우레이트(Propylene glycol monolaurate, Lauroglycol? 90, Gatte Fosse)를 사용하여 패치제를 제조했다.
실시예 7
상기 실시예 3과 동일하게 제조하되, 글리세롤 모노올레이트 대신에 소비탄 모노올레이트 (Sorbitan monooleate, Span 80, 일본준약)를 사용하여 패치제를 제조했다.
실시예 8
상기 실시예 3과 동일하게 제조하되, 글리세롤 모노올레이트 대신에 라우릴 피롤리돈 (Lauryl pyrrolidone, Surfadone? LP-300, ISP)를 사용하여 패치제를 제조했다.
실시예 9
먼저 30mL 유리 바이알에 폴리비닐피롤리돈(Poly vinylpyrrolidone, PVP) K-25 (Plasdone? K-25, BASF) 0.1g과 이소프로판올(Isopropanol) 2g을 넣고 녹인다. PVP K-25가 완전히 녹으면 희석된 Duro-Tak 87-504A 점착제 15.65g(고형분 4.7g), 펜타닐 0.6g과 글리세롤 모노올레이트 (Glycerol monooleate, GMO, PECEOL? Gatte Fosse) 0.6g을 30mL 유리 바이알에 넣고 점착액에 펜타닐과 GMO를 완전히 녹인다. 이후 실시예 1과 동일하게 패치제를 제조했다.
비교예 1
아크릴 점착제 (Duro-tak 87-4287, Henkel Corporation) 100g(고형분 39g)과 톨루엔 30g을 250mL 용량의 유리용기에 넣어 롤믹서를 사용하여 균일하게 섞어 고형분 30%로 희석시킨다. 희석된 Duro-Tak 87-4287점착제 18.0g(고형분 5.4g)과 펜타닐 0.6g을 30mL 유리 바이알(vial)에 넣고 펜타닐을 점착액에 완전히 녹인다. 이후 실시예 1과 동일하게 패치제를 제조했다.
비교예 2
아크릴 점착제 (Duro-tak 87-2852, Henkel Corporation) 100g(고형분 33.5g)과 톨루엔 11.65g을 250mL 용량의 유리용기에 넣어 롤믹서를 사용하여 균일하게 섞어 고형분 30%로 희석시킨다. 희석된 Duro-Tak 87-2852점착제 18.0g(고형분 5.4g)과 펜타닐 0.6g을 30mL 유리 바이알(vial)에 넣고 펜타닐을 점착액에 완전히 녹인다. 이후 실시예 1과 동일하게 패치제를 제조했다.
비교예 3
아크릴 점착제 (Duro-tak 87-4098, Henkel Corporation) 100g(고형분 38.5g)과 톨루엔 28.35g을 250mL 용량의 유리용기에 넣어 롤믹서를 사용하여 균일하게 섞어 고형분 30%로 희석시킨다. 희석된 Duro-Tak 87-4287점착제 18.0g(고형분 5.4g)과 펜타닐 0.6g을 30mL 유리 바이알(vial)에 넣고 펜타닐을 점착액에 완전히 녹인다. 이후 실시예 1과 동일하게 패치제를 제조했다.
비교예 4
고분자량 폴리이소부틸렌 고무(Oppanol B-100, BASF)를 톨루엔에 10% 농도로 녹인 용액10g(고형분 1g), 저분자량 폴리이소부틸렌 고무(HIMOL 5H, NIPPON PETROCHEMICALS CO.LTD.)를 40%농도로 녹인 용액 5g(고형분 2g), 테르펜 레진(Sylvares TR7115, Arizonachemical) 2.1g, 미네랄 오일(Kaydol, Sonneborn, INC.) 0.6g을 30mL 유리 바이알(vial)에 넣고 펜타닐을 점착액에 완전히 녹인다. 펜타닐을 녹인 점착액을 실리콘이 코팅된 두께 75㎛폴리에틸렌 테레프탈레이트(PET) 필름 위에 건조시 두께가 80㎛가 되도록 코팅하여 건조한 후에 한 쪽 면을 PET 필름 12㎛에 폴리에틸렌(PE) 필름 40㎛가 적층된 필름의 PET면을 덮고 라미네이트시키고 이후 21㎠의 크기로 절단하여 패치제를 제조했다.
시험예
상기 실시예들에서 제조된 패치제의 품질수준을 평가하기 위하여, 경피투과 시험, 점착력 시험 및 부착성 시험을 실시하였다. 참고예로 현재 시판되고 있는 펜타닐 패치제인 듀로제식 디트랜스 제품(3일 제형, 한국얀센)에도 동일한 시험을 실시하였다.
피부투과시험
실시예 1~9, 비교예 1~4의 패치제와 듀로제식 디트랜스 패치제(참고예)에 대하여 피부투과시험을 실시하였다.
구체적으로 프란츠 (Franz) 확산 셀(cell) (유효면적: 0.64cm2, 수용상의 부피: 5.2㎖)을 사용하여 싱크 조건(sink condition) 하에서 실시하였다. 수용상은 pH 7.4의 인산염 완충된 식염수 (Phosphate buffered saline; PBS)을 사용하였다. 먼저 수용상을 프란츠 확산 셀에 채우고 32±0.5℃로 유지시킨 다음, 시료를 원형(면적 0.64cm2)으로 잘라 준비된 사체피부(human cadaver skin epidermis) 중앙부에 붙이고, 시료가 부착된 피부를 프란츠 확산 셀의 수용부 상부에 올려놓은 후, 도너부를 덮어 클램프로 고정시킨 후 600rpm의 속도로 마그네틱 바를 회전시키며 실험을 진행하며 3, 6, 9, 12, 24, 36, 48, 60, 72시간에 각각의 셀에서 수용상을 250ul씩 채취하고 동일한 양의 신선한 수용상을 채워 넣었다. 채취한 수용층 내의 펜타닐의 함량은 HPLC로 분석하였다. HPLC 분석조건은 USP 29의 시트르산펜타닐염의 분석조건을 기본으로 유량과 주입량을 보유한 기기에 맞게 변경하여 사용하였다. 실험결과는 표 1 및 도 3 및 4에 표시하였다.

피부투과실험의 결과로서 최대 피부투과 속도(J max), 초기부터 24시간 동안의 평균피부투과 속도(J 0-24), 24시간부터 72시간까지의 평균 피부투과 속도(J 24-72)는 표 1에 나타낸 바와 같으며, 초기부터 24시간 동안의 평균 피부투과 속도와 24시간부터 72시간까지의 평균 피부투과 속도의 비 (J 0-24 / J 24-72)는 실시예 모두 참조예보다 높았으며 이로부터 72 시간 동안 참조예 보다 일정한 속도로 약물을 피부를 통해 전달할 수 있음을 알 수 있다.
도 3 및 도 4에 표시된 바와 같이, 실시예 패치제는 초기에 빠른 속도로 약물을 전달하고 최대 피부투과 속도에 도달한 이후에는 피부투과 속도가 점차 감소하며 24시간 이후에는 감소폭이 줄어들어 거의 일정한 속도로 약물이 피부를 투과되었다. 또한 실시예 모두 참조예, 비교예 보다 높은 피부투과 속도를 가지며 이는 동일한 면적의 패치제에서 보다 많은 양의 약물을 빠르게 피부를 통해 전달할 수 있음을 알 수 있다.
점착력 시험
인장강도 시험기를 사용하여 점착력을 측정하였다. 시험방법은 미국공업규격 중 프로브 트랙 시험 (Probe tack test; ASTM D2979)에 따라 3회 시험 후, 평균값을 산출하여 표 2에 나타내었다.

상기 표 2에서 확인된 바와 같이, 본 발명에 따른 패치제의 점착력은 참조예의 패치제의 점착력과 대등하거나 더 우수한 점착력을 보였다.
부착성 시험
펜타닐이 마약이므로 무모 쥐(Hairless mouse) 10마리씩을 사용하여 등에 72시간 동안 실시예 1~9의 Probe tack 실험의 상위 6개 및 참조예의 패치제를 부착한 후 표 3과 같이 부착성을 평가하였으며, 그 평균값을 표 4에 나타내었다. 참고로 쥐에 대한 부착성 시험 결과는 쥐의 활동성을 감안할 때, 인간에게 적용시에 비하여 약 70%의 값이라고 여겨진다.

표 4에서 알 수 있듯이, 본 발명에 따른 실시예 1~9의 Probe tack 실험의 상위6개 패치제는 3일 제형의 참고예의 패치제와 대등하거나 그 이상의 부착력을 나타냈다. 특히, 주변부 점착 지지층을 포함하는 실시예 5의 패치제는 활동성이 많은 쥐에게 부착했음에도 3일간 부착되어 있었다.

- 지지체 및 상기 지지체의 일면에 약물·점착층을 갖는 펜타닐 경피 패치제로,상기 약물·점착층은 아크릴-고무 하이브리드 중합체 90~60 중량부, 및 약물로서 펜타닐, 펜타닐 유도체 및 이들의 약제학적 염으로 구성되는 군으로부터 선택되는 하나 이상을 5~20 중량부로 포함하는 것인 펜타닐 경피 패치제.
- 제 1항에 있어서, 상기 펜타닐 유도체는 알펜타닐, 카르펜타닐, 로펜타닐, 레미펜타닐, 서펜타닐 또는 트레펜타닐인 것인 펜타닐 경피 패치제.
- 제 1항에 있어서, 약물·점착층은 20~100㎛두께를 가지는 것인 펜타닐 경피 패치제.
- 제 1항에 있어서, 아크릴-고무 하이브리드 중합체는 2-에틸헥실 아크릴레이트, 부틸 아크릴레이트, 이소-옥틸아크릴레이트, 메틸 아크릴레이트, 메틸 메타아크릴레이트, 아크릴산, 2-하이드로시에틸 아크릴레이트, 비닐아세테이트로 구성된 아크릴계 중합체에 폴리에틸렌-부틸렌, 폴리에틸렌-비닐아세테이트, 폴리부틸렌, 폴리스티렌-이소프렌, 또는 폴리스티렌-이소프렌-부타디엔의 고무계 중합체가 그라프트되어 있는 것인 펜타닐 경피 패치제.
