Abstract
Ein Instrumentensystem (18) enthält ein endoskopisches Instrument (20), das einen an einem flexiblen Tragarm (22) endseitig angebrachten, in einen Patienten (2) einführbaren Arbeitskopf (24) enthält, einen in der Körperoberfläche (4) des Patienten (2) platzierbaren Trokar (10), der aufweist: einen relativ zum Patienten (2) ortsfixierbaren Grundkörper (12) mit einer Durchtrittsöffnung (28) für das Instrument (20), einen Führungskörper (14), der bei eingesetztem Instrument (20) einen Längsabschnitt (43) des Tragarms (22) mit einem Führungskanal (30) derart führend umschließt, dass zumindest an diesem eine räumliche Führungsrichtung (42) des Tragarms (22) bestimmt ist, ein auf den Führungskörper (14) einwirkendes, eine gezielte Veränderung einer Relativlage (R) der Führungsrichtung (42) zum Grundkörper (12) bewirkendes Stellelement (44a, b).

Die Erfindung betrifft ein Instrumentensystem.
Die Erfindung betrifft ein Instrumentensystem für die mini- malinvasive Chirurgie.
Minimalinvasive Eingriffe nehmen im Bereich der klinischen Chirurgie einen zunehmend größeren Stellenwert ein. Wurden für kleine chirurgische Eingriffe noch vor wenigen Jahren re¬ lativ große Bereiche des Situs eröffnet, um eine Navigation des Chirurgen durch natürliche Landmarken zu ermöglichen, so lässt sich beobachten, dass eine Vielzahl dieser Eingriffe heute mittels Laparoskopie und optischer Unterstützung in Form von Endoskopie durchgeführt wird. Dieser Trend wird sich vermutlich, gestützt durch die unmittelbaren Vorteile minima- linvasiver Eingriffe, weiter fortsetzen. Endoskope, Lapa- roskope etc. werden im folgenden allgemein als endoskopische Instrumente bezeichnet, die in der Regel durch einen im Pati¬ enten platzierten Trokar in dessen Inneres verbracht werden. Instrumente und Trokar bilden zusammen ein Instrumentensys¬ tem.
Bekannt sind weiterhin teleoperierte Roboterassistenzsysteme als Instrumentensysteme, bei denen der Chirurg an einer Kon¬ sole Bewegungseingaben vornimmt. Die Bewegungseingaben werden über eine geeignete Kinematik skaliert auf die im Körper des Patienten befindlichen Instrumente übertragen. Auch hierzu werden in der Regel Trokare benutzt. Mit anderen Worten trägt ein Roboterarm ein Instrument, wobei wiederum der Roboterarm durch den Chirurgen fernbedient wird. Bei derzeit stattfin¬ denden Marktentwicklungen spielen die genannten Roboterassistenzsysteme eine entscheidende Rolle.
Für die Auslegung der Kinematik der genannten Assistenzsysteme gibt es zahlreiche Lösungsmöglichkeiten. Die verwendeten
Instrumente sind in der Regel starr. Deren Bewegung wird durch die Bewegung eines außerhalb des Patienten befindlichen Roboterarms bewirkt. Hierfür ist bisher ein störend großer Platzbedarf nötig.
Es sind auch neuere Konzepte denkbar, welche diesem Nachteil entgegenwirken. Gemäß diesen werden die Instrumente selbst mit immer mehr Bewegungsfreiheitsgraden ausgestattet. Viele Bewegungen der Instrumente finden dann im Körper des Patien- ten statt. Derartige Bewegungen müssen nicht mehr durch das
Roboterassistenzsystem außerhalb des Körpers getätigt werden. Der benötigte Arbeitsraum für den Roboter bleibt außerhalb des Körpers daher begrenzt. Der Hauptteil der Operationsbewe¬ gungen wird also durch eine Mehrfreiheitsgradkinematik inner- halb des Situs umgesetzt.
Eine Herausforderung besteht nun bei diesen Ansätzen darin, eine Vielzahl von Instrumentenbewegungsfreiheitsgraden bei kleinem Instrumentendurchmesser zu realisieren, um das In- strument mit einem geeigneten Trokardurchmesser platzieren zu können. Ein derartiger Durchmesser liegt z.B. im Bereich unter 10 mm. Denkbar ist es z.B., die Achsen der Instrumenten- freiheitsgrade hauptsächlich durch Drahtseilkonstruktionen anstelle von elektrisch betätigten Aktuatoren anzutreiben, damit das Instrument kostengünstig bleibt. So ist ein Dispo- sable-Konzept zu bewerkstelligen.
In nachteiliger Weise müssen für jeden Bewegungsfreiheitsgrad mindestens zwei Drahtseile durch die Instrumentenstruktur ge- führt werden, um zwei Bewegungsrichtungen bei dosierter Kraft beziehungsweise dosiertem Drehmoment realisieren zu können. Dies resultiert für ein Instrument, dessen Bewegung die sechs Freiheitsgrade des Raumes abdeckt, an der ungünstigsten Stel¬ le bereits in der Verwendung von zwölf Drahtseilen. Selbst wenn zwar die erste der sechs Achsen direkt außerhalb des Pa¬ tienten angetrieben wird, ist in der Regel auch für einen Arbeitskopf des Instruments, also einen Endeffektor, eine Funk¬ tionalität gewünscht. Z.B. soll an der Instrumentenspitze ei-
ne Greifzange betätigbar sein. Hierfür werden dann zusätzlich zwei Drahtseile benötigt. Auch diese müssen durch die Instru¬ mentenstruktur geführt werden. Fig. 3 zeigt schematisch einen Auszug aus einem denkbaren Instrument 20, das einen Tragarm 22 und einen an diesem endsei- tig angebrachten Arbeitskopf 24 enthält. Ein Aktuator 38 am Arbeitskopf 24 in Form einer Schere wird über Steuerleitungen 32 in Form von Drahtseilen betrieben. Die Betätigung stellt einen ersten Freiheitsgrad dar. Der Arbeitskopf 24 ist außerdem gegenüber dem Tragarm 22 rotierbar, was einen zweiten Freiheitsgrad bzw. einen zweiten Aktuator 38 darstellt. Auch dieser wird über zwei Steuerleitungen 32 in Form von
Drahtseilen betrieben. Der Tragarm 22 weist starre Armsegmen- te 100 auf, die über ein Gelenk 102 miteinander verbunden sind. Auch das Gelenk 102 stellt damit einen Aktuator 38 dar, welcher ebenfalls über einen zwei Steuerleitungen 32 bzw. Drahtseile bewegt wird. Fig. 3 zeigt die Realisierung eines Instruments von 8mm
Durchmesser, anhand der eine Vorstellung von der Komplexität eines derartigen Aufbaus gegeben wird. Fig. 3 zeigt die Komplexität des Aufbaus bei drei Freiheitsgraden. Die Erweite¬ rung auf mehr als fünf bis sechs Freiheitsgrade bei gleichem oder sogar geringerem Durchmesser ist sehr wahrscheinlich nicht mehr umsetzbar.
Aufgabe der Erfindung ist es, ein verbessertes Instrumentensystem für die minimalinvasive Chirurgie anzugeben.
Die Aufgabe wird gelöst durch ein Instrumentensystem gemäß Patentanspruch 1. Das Instrumentensystem umfasst ein endoskopisches Instrument, das wiederum einen flexiblen Tragarm und einen endseitig am Tragarm angebrachten, in einen Patienten einführbaren Arbeitskopf umfasst. Das Instrumentensystem umfasst weiterhin einen in der Körperoberfläche eines Patienten platzierbaren Trokar. Dieser umfasst einen Grundkörper mit einer Durchtrittsöffnung für das Instrument. Der Grundkörper
ist relativ zum Patienten beziehungsweise an diesem ortsfi- xierbar. Der Trokar umfasst außerdem einen Führungskörper. Ist das Instrument in den Trokar eingesetzt, umschließt der Führungskörper einen Längsabschnitt des Tragarms mit einem Führungskanal derart führend, dass zumindest im Bereich des Führungskanals eine räumliche Führungsrichtung des Tragarms bzw. dessen Längsabschnittes bestimmt ist. Der Trokar weist außerdem ein Stellelement auf, welches auf den Führungskörper derart einwirkt, dass dieses eine gezielte Veränderung der Relativlage der Führungsrichtung zum Grundkörper bewirkt.
Zur Benutzung des Instrumentensystems wird zunächst der Tro¬ kar wie gewohnt am Patienten platziert, d.h. dessen Grundkörper wird in der Regel an der Hautoberfläche des Patienten an¬ gelegt und dort ortsfixiert, z.B. durch Klebeband. Das In¬ strument, beziehungsweise dessen Tragarm wird mit dem Ar¬ beitskopf voran durch die Durchtrittsöffnung in den Trokar und durch diesen hindurch in den Patienten eingeführt, bis der Arbeitskopf im Patienten zu liegen kommt.
Das Instrument wird hierbei an dem, dem Arbeitskopf gegenüber liegenden Ende des Tragarms gehalten, ist also z.B. in einem oben genannten Roboterarm fixiert. Der Tragarm ist flexibel genug, dass er den Bewegungen des Trokars folgen kann. Im Bereich der Durchtrittsöffnung z.B. bleibt der Tragarm unveränderter Lage. Im Bereich des Führungskörpers wird der Tragarm bewegt. Dies führt zu einer In-Sich-Verformung des Tragarms. Zumindest der vom Führungskörper beeinflusste Längsabschnitt des Tragarms kann also entsprechend in eine gewünschte Füh¬ rungsrichtung ausgerichtet werden. Die Flexibilität des Trag¬ arms ist hierbei in der Regel jedoch auch so gering gewählt, dass sich durch Bewegung des Längsabschnittes des Tragarms auch der Arbeitskopf in eine gewünschte Richtung bewegt. Mit anderen Worten hat das vom Führungskörper bis zum Arbeitskopf ragende Teil des Tragarms genügend Eigenstabilität, um eine entsprechende Ausrichtung des Arbeitskopfes durch den Füh¬ rungskörper gezielt zu ermöglichen. Die Führungsrichtung beschreibt also eine Ausrichtung des Tragarms in einer ge-
wünschten Richtung, die dann in der Regel etwa der Ausrichtung des Arbeitskopfes entspricht. Der Führungskörper bildet z.B. das dem Grundkörper gegenüberliegende Ende des Trokars und stellt so die maximale Beweglichkeit und die beste Füh¬ rungsmöglichkeit des Arbeitskopfes bereit.
Gemäß der Erfindung wird also vorgeschlagen, das Antriebskonzept eines Instruments - zumindest zum Teil - auf den Trokar zu verlegen. Der Trokar führt mit anderen Worten eine zumindest grobe Positionierung des Arbeitskopfes im Arbeitsraum durch. So wird auch eine Orientierungseinstellung einer Instrumentenspitze, also des Arbeitskopfes erreicht. Mit ande¬ ren Worten wird also statt eines starren Trokars ein mit ei¬ ner steuerbaren Bewegungsmöglichkeit ausgerüsteter Trokar vorgeschlagen, die zumindest zur groben Positionierung beziehungsweise Lenkung des Instruments dient. Der Trokar verformt damit das Instrument beziehungsweise dessen Tragarm in eine gewünschte Arbeitsraumposition, ohne dass hierbei ein Übergang bzw. eine Grenze von elastischer zu plastischer Verformung des Instrumentenschaftes, also des Tragarms überschrit¬ ten wird. Der Arbeitskopf, also die Instrumentenspitze kann damit innerhalb eines durch die Bewegungsmöglichkeiten des Trokars aufgespannten Arbeitsraums gezielt bewegt werden. Für diese Bewegung ist keinerlei aktive Eigenbeweglichkeit im In¬ strument selbst notwendig, welches daher besonders einfach konstruiert werden kann.
Eine axiale Verschiebung des Instruments im Trokar bringt hier zumindest einen von außerhalb des Trokars steuerbaren Freiheitsgrad für die Instrumentenbewegung bzw. Bewegung des Arbeitskopfes. Gleiches gilt für eine Rotation des Tragarms und damit des Arbeitskopfes um die Mittellängsachse des In¬ struments. Beide Bewegungen können von außerhalb des Patien¬ ten in einfacher Weise durch Vorschub und Rotation des Instruments bedient werden.
Vorgeschlagen wird damit dann eine Verteilung eines Gesamtantriebskonzeptes auf eine grob Positionierung des Arbeitskop-
fes im Arbeitsraum mit Hilfe des Trokars und eine Feinpositi- onierung beziehungsweise Orientierungseinstellung der Instru- mentenspitze durch die Bewegung des Instruments.
In einer bevorzugten Aus führungs form ist ein Abstand zwischen dem Grundkörper und einem Austrittsende des Führungskörpers mit Hilfe eines Stellelements veränderbar. Mit anderen Worten ist also die axiale Länge des Trokars, die sich in das Innere des Patienten erstreckt, also entlang eines eingesetzten Instruments, variabel gestaltet. Die Führungseigenschaft des Trokars kann somit besonders flexibel im Inneren des Patien¬ ten genutzt werden. Zum Beispiel kann der Führungskörper durch Verlängerung oder Verkürzung des Trokars bei einer gewünschten Führungsrichtung besonders nah an ein zu behandelndes Organ geführt werden, um das Instrument bzw. den Arbeits¬ kopf nur minimal aus dem Führungskörper ragen zu lassen, und um diesen somit besonders exakt führen zu können.
In einer weiteren bevorzugten Aus führungs form enthält der Trokar eine Rücksteileinrichtung, die derart auf den Führungskörper einwirkt, dass sie diesen in eine Grundposition bringt beziehungsweise rückstellt. Die Rücksteileinrichtung sorgt mit anderen Worten dafür, dass der Trokar beim Zurückstellen stets eine bestimmte Grundposition z.B. mit einem bekannten Abstand und einer bekannten Führungsrichtung für das Instrument einnimmt. Hierbei ist dann der Abstand zwischen Grundkörper und Führungskörper und dessen Führungsrichtung bzw. Relativlage zum Grundkörper bekannt. Mit anderen Worten kann so z.B. der patienteninnenliegende Trokarausgang in der Grundposition in eine Ruhestellung beziehungsweise einen Ausgangszustand zurück bewegt werden, nachdem dieser vorher in gewünschte Richtungen bewegt wurde. Auch ein eingesetztes In¬ strument kann somit ebenfalls wieder in eine Ausgangslage ge¬ bracht werden. Insbesondere erfolgt die Rückstellung in die Grundposition automatisch, z.B. immer, wenn keine besondere Relativlage gewünscht ist.
In einer bevorzugten Variante der o.g. Aus führungs form enthält die Rücksteileinrichtung ein die Rückstellung bewirkendes Federelement. Zum Beispiel ist der Grundkörper am einen Ende des Trokars über eine Schraubenfeder mit dem bewegbaren Führungskörper am anderen Ende des Trokars verbunden. Mit anderen Worten muss der Trokar stets durch Aufwendung einer Kraft gegen das Federelement aus der Grundposition gebracht und gehalten werden. Wirkt keine verstellende Kraft auf den Führungskörper ein, richtet sich dieser in einer Grundposition bezüglich seiner Ausrichtung und seines Abstandes zum Grundkörper aus. Die Grundposition ist durch die entspannte Position des Federelements vorgegeben.
In einer weiteren Aus führungs form enthält das Instrument eine im Tragarm verlaufende Steuerleitung, welche zur Ansteuerung eines Aktuators des Tragarms und oder des Arbeitskopfes dient. So können durch die Aktuatoren weitere Bewegungsfrei¬ heitsgrade am Tragarm oder Arbeitskopf realisiert werden. Ein Aktuator ist z.B. ein Gelenk im Tragarm oder ein betätigbares Werkzeug am Arbeitskopf wie ein Greifer oder eine Schere.
Für entsprechende Steuerleitungen zu Aktuatoren ist also wesentlich mehr Platz als bei solchen Instrumenten vorhanden, die alleine durch interne Steuerleitungen bewegt werden, da die Grundbewegungsfunktionalität erfindungsgemäß durch den Trokar realisiert ist und somit der gesamte innere Raum des Instruments z.B. für Steuerleitungen zur Verfügung steht. Ist im Tragarm Platz für mehrere Steuerleitungen, können auch mehrere Aktuatoren am Instrument selbst vorgesehen werden.
Durch das erfindungsgemäße verteilte Antriebskonzept verrin¬ gert sich also die Anzahl der durch das Instrument zu führen¬ den Seilzüge bei gleicher Funktionalität. Das Instrument kann daher einen sehr kleinen Durchmesser annehmen. Zudem bleibt das Instrument auf Grund seiner geringen Komplexität kosten¬ günstig, so dass es vorteilhaft als Disposable-Element konzi¬ piert werden kann.
In einer bevorzugten Variante dieser Aus führungs form ist die Steuerleitung mechanisch arbeitend ausgebildet. Zum Beispiel ist die Steuerleitung ein oben bereits erläuterter Seilzug, welcher durch das Instrument beziehungsweise dessen Tragarm geführt ist. Zwei oder vier Seilzüge sind wegen des verteil¬ ten Antriebskonzepts hierbei jedoch wesentlich einfacher und problemloser in ein Instrument zu integrieren, als die o.g. zwölf Seilzüge. Eine Funktionalität an der Instrumentenspitze wird also z.B. auf bekannte Art und Weise durch Seilzugmecha- nismen realisiert.
In einer bevorzugten Aus führungs form der Erfindung ist das Stellelement des Trokars mechanisch arbeitend ausgebildet. Auch hier sind z.B. Seilzüge denkbar, mit deren Hilfe der Führungskörper im Trokar bedient beziehungsweise manipuliert wird .
In einer bevorzugten Variante dieser Aus führungs form enthält das Instrumentensystem einen Antrieb, welcher eines der
Stellelemente, insbesondere derjenigen des Trokars betätigt. Der Antrieb wiederum kann z.B. durch eine Computersteuerung bedient werden, so dass letzten Endes eine systemgesteuerte Veränderung der Führungsrichtung oder eines Abstandes zwischen Grundkörper und Führungskörper, d.h. eine computerge- steuerte Bewegung des Instruments im Patienten durch den Trokar erfolgen kann.
Für eine weitere Beschreibung der Erfindung wird auf die Ausführungsbeispiele der Zeichnungen verwiesen. Es zeigen, je- weils in einer schematischen Prinzipskizze:
Fig. 1 ein erfindungsgemäßes Instrumentensystem beim Einsatz in einem Patienten in einer Grundposition,
Fig. 2 das Instrumentensystem aus Fig. 1 in einer Arbeitspo- sition,
Fig. 3 ein Instrument mit komplexem Seilzugmechanismus.
Fig. 1 zeigt einen Ausschnitt eines Patienten 2, nämlich dessen Körperoberfläche 4 in Form der Bauchdecke und dessen In¬ neres 6 in Form des Bauchraumes. In die Körperoberfläche 4 ist operativ ein Einschnitt 8 eingebracht, durch welchen ein Trokar 10 in den Patienten 2 eingeführt ist. Der Trokar 10 weist einen Grundkörper 12 auf, welcher am Patienten 2 ortsfixiert ist, nämlich an dessen Bauchdecke 4 z.B. mit Hilfe nicht dargestellter Klebebänder fixiert ist. Der Trokar 10 weist außerdem einen Führungskörper 14 auf, der durch einen flexiblen Mantel 16 mit dem Grundkörper 12 verbunden ist.
Der Trokar 10 ist Teil eines Instrumentensystems 18, mit Hil¬ fe dessen am Patienten 2 eine medizinische Maßnahme durchge¬ führt werden soll. Das Instrumentensystem 18 umfasst außerdem ein endoskopisches Instrument 20 in Form eines Laparoskops, welches wiederum einen flexiblen Tragarm 22 enthält, an dessen in den Patienten 2 einführbaren Ende ein Arbeitskopf 24 angebracht ist. Zu Beginn der medizinischen Maßnahme wird zunächst der Trokar 10 bzw. der Grundkörper 12, wie in Fig. 1 dargestellt, am Patienten 2 befestigt. Anschließend wird das Instrument 20 in Richtung des Pfeils 26 in den Trokar 10 eingeführt. Hierzu weist der Grundkörper 12 eine Durchtrittsöffnung 28 und der Führungskörper 14 einen Führungskanal 30 auf. Im Inneren des Instruments 20 verlaufen mehrere Steuerleitungen 32 in Form von Drahtseilen, welche zur Bedingung des Arbeitskopfes 24 dienen und mit denen der Arbeitskopf 24 sowohl um eine Achse 34 in Richtung des Doppelpfeils 36 schwenkbar ist als auch ein am Arbeitskopf 24 angebrachter Aktuator 38 in Form einer Schere in Richtung des Doppelpfeils 40 bedienbar ist.
Der Führungskanal 30 ist derart ausgebildet beziehungsweise wirkt derart mit dem Tragarm 22 zusammen, dass er im Bereich des Führungskanals 30 eine Führungsrichtung 42 für den dort einliegenden Längsabschnitt 43 des Tragarms 42 vorgibt. Die Führungsrichtung bestimmt die räumliche Ausrichtung des Trag¬ arms 22 an der Stelle des Längsabschnitts 43. Da der Tragarm
22 nicht unbegrenzt flexibel ist, ist damit auch die Raumlage des Arbeitskopfes 24 bestimmt. Mit anderen Worten tritt der Tragarm 22 an einem Austrittsende 31 aus dem Führungskörper 14 bzw. dem Führungskanal 30 aus. Das Austrittsende 31 weist einen Abstand d zum Grundkörper 12 auf.
Der Trokar 10 weist außerdem Stellelemente 44a, b in Form ei¬ nes Drahtseils auf, welches geeignet ist, eine gezielte Lage¬ änderung des Führungskörpers 14 bezüglich des Grundköpers 12 herbeizuführen. Mit anderen Worten wird also die Relativlage R zwischen Führungskörper 14 und Grundkörper 12 bestimmt. Durch Veränderung der Relativlage R wird die Richtung der Führungsrichtung 42 verändert und somit die Ausrichtung des Tragarms 22 und damit des Arbeitskopfes 24.
Fig. 1 zeigt den Trokar in einer Grundposition G, d.h. mit einer bestimmten, vorgegebenen Relativlage R.
Fig. 2 zeigt eine Situation, in welcher die Stellelemente 44a, b betätigt wurden und somit auch die Lage der Führungs¬ richtung 42 verändert wurde. Der Trokar befindet sich also in einer Arbeitsposition A, in der die Relativlage R verändert wurde . Im Beispiel wurden beide Stellelemente 44a, b bewegt, jedoch das Stellelement 44b mehr verkürzt als das Stellelement 44a. Dies hat den Effekt, dass nicht nur die Raumlage der Füh¬ rungsrichtung 42 verändert ist, sonder sich auch der Abstand d zwischen dem Führungskörper 14 und dem Grundkörper 12 ge- genüber der Grundposition G verkleinert hat. Durch diese Beweglichkeit ist das Instrument 20 insbesondere bezüglich sei¬ nes Arbeitskopfes 24 in einem weiten Bereich im Inneren 6 des Patienten manövrierbar. Zur Bedienung der Stellelemente 44a, b sind jeweilige Antriebe 46a, b vorgesehen, die elektrisch ansteuerbar sind und somit im Rahmen einer Computersteuerung zur Ausrichtung des Instruments 20 mit Hilfe des Führungskörpers 14 benutzt werden können.
Der Trokar 10 enthält weiterhin eine Rücksteileinrichtung 48 in Form eines Grundkörper 12 und Führungskörper 14 verbindenden, und den Mantel 16 umgebenden Federelements 50, nämlich einer Schraubenfeder. In der in Fig. 1 gezeigten Grundpositi- on G befindet sich diese im entspannten beziehungsweise kraftlosen Zustand. Im Arbeitszustand A der Fig. 2 ist die Rücksteileinrichtung 48 verformt. Werden ausgehend von der Arbeitsposition A die auf Zug arbeitenden Stellelemente 44a, b wieder entlastet, so gelangt der Trokar 10 selbstständig, d.h. ohne zusätzliche äußere Krafteinwirkung durch die Rückstellkräfte der Rücksteileinrichtung 48, wieder in den in Fig. 1 gezeigten Ruhezustand, also die Grundposition G zurück .
Claims
1. Instrumentensystem (18),
mit einem endoskopischen Instrument (20), das einen an ei- nem flexiblen Tragarm (22) endseitig angebrachten, in einen Patienten (2) einführbaren Arbeitskopf (24) enthält, und mit einem in der Körperoberfläche (4) des Patienten (2) platzierbaren Trokar (10), der aufweist:
einen relativ zum Patienten (2) ortsfixierbaren Grundkör- per (12) mit einer Durchtrittsöffnung (28) für das Instrument (20),
einen Führungskörper (14), der bei eingesetztem Instrument (20) einen Längsabschnitt (43) des Tragarms (22) mit einem Führungskanal (30) derart führend umschließt, dass zumin- dest an diesem eine räumliche Führungsrichtung (42) des
Tragarms (22) bestimmt ist,
ein auf den Führungskörper (14) einwirkendes, eine geziel¬ te Veränderung einer Relativlage (R) der Führungsrichtung (42) zum Grundkörper (12) bewirkendes Stellelement
(44a, b) .
2. Instrumentensystem (18) nach Anspruch 1, bei dem der Abstand (d) zwischen dem Grundkörper (12) und einem Austrittsende (31) des Führungskörpers (14) mit Hilfe eines Stellele- ments (44a, b) veränderbar ist.
3. Instrumentensystem (18) nach einem der vorhergehenden Ansprüche, bei dem der Trokar (10) eine auf den Führungskörper (14) einwirkende, diesen in eine Grundposition (G) bringende Rücksteileinrichtung (48) enthält.
4. Instrumentensystem (18) nach Anspruch 3, bei dem die Rück- stelleinrichtung (48) ein Federelement (50) enthält.
5. Instrumentensystem (18) nach einem der vorhergehenden Ansprüche, bei dem das Instrument (20) eine im Tragarm (22) verlaufende, einen Aktuator (38) des Tragarms (22) und/oder des Arbeitskopfes (24) ansteuernde Steuerleitung (32) ent¬ hält.
6. Instrumentensystem (18) nach Anspruch 5, bei dem die Steu- erleitung (32) mechanisch arbeitend ausgebildet ist.
7. Instrumentensystem (18) nach einem der vorhergehenden Ansprüche, bei dem das Stellelement (44a, b) mechanisch arbei¬ tend ausgebildet ist.
8. Instrumentensystem (18) nach Anspruch 7, mit einem das Stellelement (44a, b) betätigenden Antrieb (46a, b) .
