Abstract
Ein Implantat (1) zum Einsetzen zwischen Wirbelkörper der Wirbelsäule als Platzhalter für aus der Wirbelsäule entfernte Bandscheiben, Wirbel oder Wirbelteile, weist ein erstes Implantatteil (2), ein zweites Implantatteil (3), die in Richtung ihrer koaxialen Längsachsen zur Längenänderung des Implantats (1) gegeneinander verstellbar sind, und eine Ansatzplatte (5a, 5b) auf, die an dem freien Ende mindestens eines der beiden Implantatteile (2, 3) in im Wesentlichen zur Längsachse senkrechter Ausrichtung durch Mittel zur lösbaren Befestigung anschließbar ist. Die Ansatzplatte (5a, 5b) umfasst eine kreisförmige Grundform (8), wobei die kreisförmige Grundform (8) am Umfang durch Superpositionen von im Wesentlichen kreisförmigen Plattenextensionen (9, 10) mit unterschiedlichen Durchmessern und / oder unterschiedlichen Überlappungsgrad mit der kreisförmigen Grundform (8) modifiziert ist.
Description
Implantat zum Einsetzen zwischen Wirbelkörper der Wirbelsäule
Die Erfindung betrifft ein Implantat zum Einsetzen zwischen Wirbelkörper der Wirbelsäule als Platzhalter für aus der Wirbelsäule entfernte Bandscheiben, Wirbel oder Wirbelteile, mit einem ersten Implantatteil und einem zweiten Implantatteil, die in Richtung ihrer koaxialen Längsachsen zur Längenänderung des Implantats gegeneinander verstellbar sind.
Ein derartiges Implantat ist beispielsweise aus der DE 103 11 477 A1 bekannt, das sich in der Praxis gut bewährt hat und durch die ihm eigene hervorragende Förderung der Ausbildung einer knöchernen Verbindung zwischen den Wirbelkörpern mit einer individuellen Anpassbarkeit des Implantats an die in situ vorliegenden Gegebenheiten auszeichnet, die erreicht wird, indem ein erstes und ein zweites Implantatteil in Richtung ihrer koaxialen Längsachsen verstellbar sind und eine Ansatzplatte an mindestens einem freien Ende durch Mittel zur lösbaren Befestigung anbringbar ist.
In ihrem Bestreben dieses bewährte Implantat für den Einsatz unter
Extrembelastungsbedingungen der Wirbelsäule weiterzuentwickeln, haben
die Anmelder die Interaktionen zwischen dieser Ansatzplatte des Implantats und einer Grundplatte eines Wirbelkörpers bzw. einer Deckplatte eines Wirbelkörpers eingehenden Untersuchungen unterzogen, wobei besondere Abhängigkeiten der Belastbarkeit der Grundplatte eines Wirbelkörpers bzw. der Deckplatte eines Wirbelkörpers festgestellt werden konnten.
Der Erfindung liegt daher die Aufgabe zugrunde, ein Implantat der eingangs genannten Art so auszubilden, dass bei Einsatz des Implantates für Extrembelastungsbedingungen eine verbesserte Schonung der Wirbelkörper erreicht werden kann.
Diese Aufgabe wird bei dem eingangs genannten Implantat dadurch gelöst, dass die Ansatzplatte eine kreisförmige Grundform aufweist und dass die kreisförmige Grundform am Umfang durch von im Wesentlichen kreisförmigen Plattenextensionen mit unterschiedlichen Durchmessern und / oder unterschiedlichen Überlappungsgrad mit der kreisförmigen Grundform modifiziert ist. Mit diesem Implantat ist der Vorteil verbunden, dass mit der Superposition der kreisförmigen Plattenextensionen durch die kreisförmige Ausgestaltung eine bessere Lastverteilung in situ auf besonders belastbare Bereiche der Grundplatten bzw. der Deckplatten der Wirbelkörper erreicht wird, ohne den Abdeckungsgrad über die Ansatzplatte unnötig zu steigern und dadurch das Einwachsen des Knochenmaterials in das Implantat zu behindern. Insbesondere die Randbereiche der Grundplatten bzw. der Deckplatten der Wirbelkörper weisen gegenüber den Innenbereichen der Grundplatten bzw. der Deckplatten der Wirbelkörper eine erhöhte Festigkeit auf, so dass eine stärkere Lastaufnahme an den Randbereichen für die Wirbelkörper schonender ist.
Aus diesen Gründen ist es besonders vorteilhaft, wenn die kreisförmigen Plattenextensionen eine Teilmenge an kreisförmigen Plattenextensionen umfassen, die einen verbreiterten Halsabschnitt zu der kreisförmigen Grundform aufweisen. Dadurch wird eine bessere Lastaufnahme an den Randbereichen ermöglicht.
Um eine weitergehende Anpassung des Implantats an die Gestalt der Grundplatte bzw. der Deckplatte des Wirbelkörpers und eine schonendere Druckbelastung der Grundplatte bzw. der Deckplatte des Wirbelkörpers zu ermöglichen, können die kreisförmigen Plattenextensionen mit den verbreiterten Halsabschnitt sich von dem verbreiterten Halsabschnitt zu der Umrandungsfläche in der Höhe verjüngen, so dass ein keilförmiger Querschnitt erreicht werden kann.
Bevorzugt weist bei dem Implantat die kreisförmige Grundform einen inneren Ring auf und bilden die Superpositionen der im Wesentlichen kreisförmigen Plattenextensionen einen äußeren, entsprechend modifizierten Ring. Ein
Vorteil dieser Ausgestaltung liegt darin, dass insbesondere die Öffnung in der Mitte der Ansatzplatte die Ausbildung einer knöchernen Verbindung zwischen den Wirbelkörpern fördert.
Insbesondere hat es sich als vorteilhaft erwiesen, wenn die kreisförmigen Plattenextensionen mindestens teilweise nach aufsteigender Größe oder abfallender Größe entlang der Umrandungsfläche der Ansatzplatte angeordnet sind. Diese Modifikation der Ansatzplatte ermöglicht eine geeignetere Verteilung der Druckfläche, ohne dass die Fläche der Ansatzplatte vergrößert werden müsste, so dass die gewünschte Förderung der Ausbildung einer knöchernen Verbindung für dieses Implantat erhalten bleibt.
Bei einer vorteilhaften Ausgestaltung des Implantats weist die Ansatzplatte an ihrer Umrandung eine konvexe Basisfläche über einen Winkel von 80° bis 120° auf. Alternativ und / oder zusätzlich kann die Ansatzplatte an ihrer Umrandung eine konkave Stirnfläche über einen Winkel von 5° bis 40° aufweisen. Zweckmäßig ist es bei dem Implantat weiterhin, wenn die
Ansatzplatte spiegelsymmetrisch mit einer Symmetrieachse von der Mitte der konkaven Stirnfläche zu der Mitte der konvexen Basisfläche ausgestaltet ist.
Dem Lastverhalten wird besonders gut entsprochen, wenn die kreisförmigen Plattenextensionen von der konvexen Basisfläche zu der konkaven Stirnfläche mit aufsteigender Größe angeordnet sind.
Weiterhin betrifft die vorliegende Erfindung eine Ansatzplatte für ein Implantat nach einem der vorhergehenden Gesichtspunkte.
Im Folgenden wird die Erfindung an einem in der Zeichnung dargestellten Ausführungsbeispiel näher erläutert; es zeigen:
Fig. 1 eine perspektivische Darstellung eines aus einem ersten
Implantatteil und einem zweiten Implantatteil bestehenden Implantats mit einer Ansatzplatte nach der vorliegenden Erfindung,
Fig. 2 das Implantat aus der Figur 1 in einer Seitenansicht,
Fig. 3 eine isolierte, perspektivische Darstellung einer Oberseite einer oberen Ansatzplatte nach der vorliegenden Erfindung, und
Fig. 4 eine isolierte, perspektivische Darstellung einer Unterseite einer oberen Ansatzplatte nach der vorliegenden Erfindung.
In den Figuren 1 und 2 ist ein Implantat 1 dargestellt, das zum Einsetzen zwischen in der Zeichnung selber nicht dargestellten Wirbelkörpern der Wirbelsäule als Platzhalter für aus der Wirbelsäule entfernte Bandscheiben, Wirbel oder Wirbelteile dient. Das Implantat 1 umfasst ein erstes Implantatteil 2 und ein zweites Implantatteil 3, die in Richtung ihrer koaxialen Längsachsen zur Längenänderung des Implantates 1 gegeneinander verstellbar sind. Die Längenänderung kann z. B. über einen dem zweiten Implantatteil 3 zugeordneten drehbaren Gewindering 4 durchgeführt werden.
Das Implantat 1 weist weiterhin mindestens eine, in dem gezeigten Ausführungsbeispiel eine obere Ansatzplatte 5a und eine untere Ansatzplatte 5b auf, die an den freien Enden der beiden Implantatteile 2, 3 in im Wesentlichen zur Längsachse des Implantates 1 senkrechter Ausrichtung durch Mittel zur lösbaren Befestigung auswechselbar angeschlossen sind, wobei die obere Ansatzplatte 5a und die untere Ansatzplatte 5b zur Anpassung an die Form der Grundplatte des Wirbelkörpers bzw. an die Form der Deckplatte des Wirbelkörpers unterschiedlich ausgestaltet sein können.
Die Figuren 3 und 4 zeigen eine isolierte, perspektivische Darstellung eine Oberseite bzw. einer Unterseite einer oberen Ansatzplatte 5a nach der vorliegenden Erfindung. Die obere Ansatzplatte 5a weist eine kreisförmige Grundform 8 auf, deren Umrandungsfläche durch Superpositionen von im Wesentlichen kreisförmigen ersten Plattenextensionen 9 und zweiten Plattenextensionen 10 modifiziert ist. Die zweite Plattenextension 10 weist einen größeren Durchmesser als die erste Plattenextension 9 auf und die erste Plattenextension 9 und die zweite Plattenextension 10 können sich im
Grad der Überlappung mit der kreisförmigen Grundform 8 der oberen Ansatzplatte 5a unterscheiden.
Bei dem Implantat 1 kann die kreisförmige Grundform 8 einen inneren Ring mit einer Öffnung 13 umfassen. Die Superpositionen der ersten Plattenextension 9 und der zweiten Plattenextension 10 bilden einen äußeren, entsprechend modifizierten Ring, bei dem ein äußerer Teil der zweiten Plattenextension 10 über den äußeren Ring hinausragt. Weiterhin kann die zweite Plattenextension 10 einen verbreiterten Halsabschnitt aufweisen und sich von dem verbreiterten Halsabschnitt zu der Umrandungsfläche in der Höhe verjüngen, so dass ein keilförmiger
Querschnitt entsteht, der an seiner Spitze über den äußeren Ring hinausragt.
An der Umrandungsfläche weist die obere Ansatzplatte 5a eine konvexe Basisfläche 11 über einen Winkel von 80° bis 120° auf. Gegenüber der Basisfläche 11 ist eine konkave Stirnfläche 12 über einen Winkel von 5° bis 40° angeordnet. Bevorzugt ist die obere Ansatzplatte 5a spiegelsymmetrisch mit einer Symmetrieachse von der Mitte der konkaven Stirnfläche 12 zu der Mitte der konvexen Basisfläche 11 ausgestaltet. Insbesondere können die ersten und die zweiten Plattenextension 9, 10 an der Umrandung der Ansatzplatte 5a in aufsteigender Größe von der konvexen Basisfläche 11 zu der konkaven Stirnfläche 12 angeordnet sein.

Claims
1. Implantat (1 ) zum Einsetzen zwischen Wirbelkörper der Wirbelsäule als Platzhalter für aus der Wirbelsäule entfernte Bandscheiben, Wirbel oder Wirbelteile, mit einem ersten Implantatteil (2), einem zweiten Implantatteil (3), die in Richtung ihrer koaxialen Längsachsen zur Längenänderung des
Implantats (1 ) gegeneinander verstellbar sind, und einer Ansatzplatte (5a, 5b), die an dem freien Ende mindestens eines der beiden Implantatteile (2, 3) in im Wesentlichen zur Längsachse senkrechter Ausrichtung durch Mittel zur lösbaren Befestigung anschließbar ist, dadurch gekennzeichnet, dass die Ansatzplatte (5a, 5b) eine kreisförmige
Grundform (8) aufweist und dass die kreisförmige Grundform (8) am Umfang durch Superpositionen von im Wesentlichen kreisförmigen Plattenextensionen (9, 10) mit unterschiedlichen Durchmessern und /oder unterschiedlichem Überlappungsgrad mit der kreisförmigen Grundform (8) modifiziert ist.
2. Implantat (1 ) nach Anspruch 1 , dadurch gekennzeichnet, dass die kreisförmigen Plattenextensionen (9, 10) eine Teilmenge an kreisförmigen Plattenextensionen (10) umfassen, die einen verbreiterten Halsabschnitt zu der kreisförmigen Grundform (8) aufweisen.
3. Implantat (1) nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass zumindest ein Teil der kreisförmigen Plattenextensionen (9, 10) sich von dem verbreiterten Halsabschnitt zu der Umrandungsfläche in der Höhe verjüngt.
4. Implantat (1 ) nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass die kreisförmigen Plattenextensionen (9, 10) mindestens teilweise nach aufsteigender Größe oder abfallender Größe entlang der Umrandungsfläche der Ansatzplatte (5a, 5b) angeordnet sind.

Implantat (1 ) nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass die kreisförmige Grundform (8) einen inneren Ring mit einer Öffnung (13) umfasst und dass die Superpositionen der im Wesentlichen kreisförmigen Plattenextensionen (9, 10) einen äußeren, entsprechend modifizierten Ring bilden.
6. Implantat (1 ) nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass die Ansatzplatte (5a, 5b) an ihrer Umrandung eine konvexe Basisfläche (1 1 ) über einen Winkel von 80° bis 120° aufweist.
7. Implantat (1) nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass die Ansatzplatte (5a, 5b) an ihrer Umrandung eine konkave Stirnfläche (12) über einen Winkel von 5° bis 40° aufweist.
8. Implantat (1 ) nach Anspruch 7 in Abhängigkeit von Anspruch 6, dadurch gekennzeichnet, dass die kreisförmigen Plattenextensionen (9, 10) von der konvexen Basisfläche (11 ) zu der konkaven Stirnfläche (12) mit aufsteigender Größe an der Umrandung angeordnet sind.
9. Implantat (1 ) nach Anspruch 8 oder nach Anspruch 7 in Abhängigkeit von Anspruch 6, dadurch gekennzeichnet, dass die Ansatzplatte (5a, 5b) spiegelsymmetrisch mit einer Symmetrieachse von der Mitte der konkaven Stirnfläche (12) zu der Mitte der konvexen Basisfläche (11 ) ausgestaltet ist.
10. Ansatzplatte (5a, 5b) für ein Implantat (1) nach einem der vorhergehenden Ansprüche.
