当个人发生疾病且求助于正规医疗体系时, 一项医疗程序随即开始。 狭义的 医疗大致包括门诊医疗以及住院医疗。 门诊医疗主要包括病人的个人基本资料建 立,安排病人至相关门诊单位,专科医师问诊及 /或安排检査 (X光、血液、尿液…-, 医师建立医嘱, 给药。 而疾病若是需要住院治疗, 则住院医疗将接续在门诊医疗 之后发生, 主要包括入院, 护士收集及记录病人生命征象、 住院医师收集病人过 去病史, 主治医师建立医嘱、 依医嘱进行给药及各项治疗程序, 且在整个住院阶 段持续的观察病人的健康状况。
上述的医疗目的旨在使病人痊愈, 或者是使病情获得控制, 其在疾病发生之 后开始进行, 属于被动式的治疗疾病。 而诸如高血压、 高血脂、 糖尿病…等慢性 疾病, 以上述医疗程序使病情稳定之后, 更重要的是出院之后病人的自我健康管 理, 包括饮食控制、 运动量、 血压情形、 血糖情形、 服药情形…等, 若能管理得 当, 将可使此类疾病复发或恶化的机率大幅下降。 但是, 在现阶段, 医疗院所及 医师却对病人离院之后的情形往往一无所知, 而病人也在缺乏专业支持的情况下 忽略自我照顾, 以致于病情难以获得有效控制与改善。
除此之外, 若病人未达住院程度, 而是因慢性病于门诊追踪治疗, 或健康检 査时发现需要处理的问题时, 治疗者或管理者若不清楚病人的居家情况, 即无法 作出适当的建议。 发明内容
本发明的目的在于,提供一种整合互动式健康管理系统,将被照护者自我健康 管理的相关资讯, 例如饮食、 运动量、 血压值、 血糖值、 体温、 呼吸、 脉搏、 体 重…等资料与医疗院所或健康管理中心等单位的医疗服务平台连结, 使该等单位 中参与治疗的相关人员可以充份掌握被照护者的健康状况。 为便于说明, 以下内文中提及 「医疗院所或健康管理中心」 时以 「医疗保健 单位」 一词简称之。
本发明的目的在于,提供一种整合互动式健康管理系统,该系统以一整合式资 讯显示单元以图像及数据化的方式表现被照护者连续一段时间的健康相关资讯, 例如以曲线图、 条棒图表现被照护者每次的血压值, 以便被照护者及医疗保健单 位的相关人员可较容易地读取该等资料并判断健康管理概况。
本发明的目的在于,提供一种整合互动式健康管理系统,该系统所提供的整合 式资讯显示单元系依被照护者的疾病而分门别类的显示相关的健康管理资讯以及 医疗保健单位的医疗记录。 例如, 被照护端若有高血压及肝硬化等问题, 则该整 合式资讯显示单元可将有关高血压及肝硬化的健康项目及医疗记录分开显示。

本发明的目的在于,提供一种整合互动式健康管理系统,被照护者可连结至一 商业交易单元而选购健康产品或了解其他健康资讯, 被照护者于需要时亦可通过 该整合互动式健康管理系统而与医疗保健单位连结以谘询意见。

上述医疗服务平台更包括一决策判断知识库 16及一警示系统 17。 该决策判断 知识库 16依据该被照护端生理资讯单元 13的资料判断被照护端目前的生理状况, 发现被照护端目前所面临的健康问题, 并订定生理资讯警示机制, 于必要时利用 该警示系统 17发送一警示讯息 (e- mail或简讯)至预设的手机或电子信箱, 以即时 通知被照护者的家属或医疗保健单位的相关人员。
兹以一糖尿病患者为例, 说明本发明健康管理系统大致的运作情形。 首先, 该糖尿病患者至一医疗院所就医, 该医疗院所的电子化病历单元 1 1详细的记录该 病患的个人资料、 病程纪录, SOAP (主、 客观陈诉、 诊断评估、 治疗计划), 药物 使用, 生命征象, 过去病史, 相关的医疗实验室检验资料及影像诊断报告…等, 该位病患的主治医师可以随时透过该医疗院所的电脑设备读取该位病患的所有资 料。 该位病患于康复离院后参与该医疗院所所提供的互动式健康管理方案, 因此 于居所建立上述的居家端平台 20, 于该被照护端身份单元 12, 建立被照护端的身 份认证资料, 并与上述电子化病历资料单元中相对应的被照护端病历连结。 配给 该病患一整合了携移式生理讯号监测装置、 身份提供单元及电脑系统、 传输界面 合并建置的模组化血糖检测设备, 病患每次检测的血糖值即可直接上传至该被照 护端生理资讯单元 13及该被照护端身份单元 12, 该被照护端生理资讯单元 13处理 并储存该血糖值资料, 主治医师可随时透过该医疗院所的电脑设备读取该居家端 平台 20所回传的资料, 以了解该病患的血糖的控制情形, 并依据该电子化病历单 元 1 1中所记载的病史、 医疗记录充份了解该病患的健康状况。而该位病患的饮食、 运动项目、运动时间、服药时间等藉由该他项资讯提供单元 23(例如电脑或行动通 讯装置及专属软体)传输予该被照护端他项资讯单元 14,让医师了解该病患的日常 生活大致状况。 而当医师或被照护端任何一方有问题需要面对面直接沟通时, 可 利用上述通讯平台 30进行彼此的视讯连线的方式进行沟通。
上述的决策判断知识库 16及警示系统 17应用需要即时处理的紧急情况, 例如 被照护端的生命征象 (心跳、 血压、 呼吸)不稳定, 上述医疗服务平台的决策判断 知识库可主动判断此一问题, 并进一步利用该警示系统 17发出 e- mail或简讯, 即 时通知被照护者的家属或主治医师, 即时对病患提供医疗协助。
虽然本发明是以一个最佳实施例做说明, 但精于此技艺者能在不脱离本发明 精神与范畴下做各种不同形式的改变。 以上所举实施例仅用以说明本发明而已, 非用以限制本发明的范围。 举凡不违本发明精神所从事的种种修改或变化, 倶属 本发明申请专利范围。

Claims

权 利 要 求 书
1 .一种整合互动式健康管理系统, 包括:
一医疗服务平台, 至少包括一电子化病历单元、 一被照护端身份单元、 一被 照护端生理资讯单元; 该被照护端身份单元, 储存及处理每一被照护端的身份资 料, 并与上述电子化病历单元中相对应的被照护端病历连结; 该被照护端生理资 讯单元, 与该被照护端身份单元连结, 储存及处理该被照护端所提供的每一笔生 理资讯;
至少一居家端平台, 建置于一被照护端的居所, 至少包含一身份提供单元及 一生理资讯提供单元; 该身份提供单元向该被照护端身份单元传输被照护端的身 份; 该生理资讯提供单元向该被照护端生理资讯单元传输被照护端的生理资讯; 一通讯平台, 提供上述居家端平台及该医疗服务平台视讯连结及资料传输连 结。
2.如权利要求 1所述的整合互动式健康管理系统, 其特征在于, 该生理资讯以 自我检测的方式而测得。
3.如权利要求 2所述的整合互动式健康管理系统, 其特征在于, 该生理资讯至 少包括血压、 心跳、 呼吸频率、 体温、 血糖或体重参数。
4.如权利要求 2所述的整合互动式健康管理系统, 其特征在于, 该生理资讯提 供单元系一种携移式生理讯号监测装置。
5.如权利要求 1所述的整合互动式健康管理系统, 其特征在于, 该医疗服务平 台更包括一被照护端他项资讯单元, 该居家端平台更包括一他项资讯提供单元; 该被照护端他项资讯单元, 与该被照护端身份单元连结, 储存该被照护端所提供 的非生理资讯, 该他项资讯提供单元向该被照护端他项资讯单元传输被照护端的 非生理资讯。
6.如权利要求 5所述的整合互动式健康管理系统, 其特征在于, 该非生理资讯 至少包括服药时间、 运动项目、 运动时间或饮食项目。
7.如权利要求 1所述的整合互动式健康管理系统, 其特征在于, 该医疗服务平 台还包括一决策判断知识库及一警示系统, 该决策判断知识库依据该被照护端生 理资讯单元的资料判断被照护端目前的生理状况, 于必要时利用该警示系统对特 定装置发送一警示讯息。
8.如权利要求 7所述的整合互动式健康管理系统, 其特征在于, 该特定装置为 指定的电子信箱或行动通讯装置。
9.如权利要求 7所述的整合互动式健康管理系统, 其特征在于, 该警示讯息为 电子邮件或简讯。
1 0.如权利要求 1所述的整合互动式健康管理系统, 其特征在于, 该医疗服务 平台还包括一整合式资讯显示单元, 与上述电子化病历单元、 被照护端生理资讯 单元及被照护端他项资讯单元连结, 以图像及数据化的方式表现上述各单元的资 料。
1 1 .如权利要求 10所述的整合互动式健康管理系统, 其特征在于, 该整合式资 讯显示单元依被照护者的疾病而分门别类的显示上述各单元的资料内容。
12.如权利要求 1所述的整合互动式健康管理系统,其特征在于,更包括一医疗 资讯系统界接模组, 使该医疗服务平台与其他医疗资讯系统整合。
