Abstract

L'invention concerne une fiche d'enregistrement de données pour la lecture opto-électronique, du type de ceux utilisés dans des systèmes de documentation de type test, examens, rapports cliniques, recherches épidémiologiques et analogue, qui comprennent au moins une partie destinée à être remplie par l'intéressé avec ses données personnelles, contenant plusieurs prémarques (1) destinées à effectuer des marquages manuels de la part de l'utilisateur, caractérisée de manière que chaque prémarque (1) correspondant à la lettre proprement dite, contiennent une seconde prémarque (2) matérialisée en trois petits (3) signes, répartis selon les sommets d'un triangle imaginaire, de manière que ces signes puissent être reliés par l'utilisateur par l'intermédiaire de traits (4) ou de marques rectilignes qui correspondent d'un point de vue formel et positionnel aux différents types d'accents existants.
