Abstract
Es wird ein Kugelkopfgelenk für ein künstliches Hüftgelenk beschrieben, welches eine in das Acetabulum einsetzbare metallische Halbschale (1) aufweist, in die ein Inlay (2) als Gleitpartner für eine auf einem Hüftstiel sitzende Gelenkkugel (3) in einen konischen Klemmsitz setzbar ist, wobei das Inlay (2) eine komplementär zur Gelenkkugel (3) ausgebildete Ausnehmung (4) aufweist, in welcher die Gelenkkugel (3) bei Ausführung ihrer Rotationsbewegung abrollt. Das Inlay (2) besteht aus einem elastischen spreizbaren Material und ist so dimensionert, daß es nach Einlegen der Gelenkkugel (3) in seine Ausnehmung (4) über deren Äquator hinübergreift. Zwischen dem Inneren der metallischen Halbschale (1) und dem Außeren des Inlays (2) ist eine zusätzliche Rastverbindung (5/6) vorgesehen.
Description
Kugelkopfgelenk für ein künstliches Hüftgelenk
Beschreibung
Die vorliegende Erfindung betrifft ein Kugelkopfgelenk für ein künstliches Hüftgelenk, wie es heute üblicherweise zum Einsatz kommt.
Es weist eine in das Acetabulum einsetzbare metallische Halbschale auf, wie sie beispielsweise bekannt ist aus der DE-C-40 21 677. In diese metallische Halbschale ist ein Inlay als Gleitpartner für eine auf einem Hüftstiel sitzende Gelenkkugel in einen konischen Klemmsitz setzbar, wobei das Inlay eine komplementär zur Gelenkkugel ausgebildete Ausnehmung aufweist, in welcher die Gelenkkugel bei Ausführung ihrer Rotationsbewegung abrollt.
Das Inlay kann beispielsweise aus hochverdichtetem Polyethylen oder aber aus einer Keramik oder einem Kompositwerkstoff bestehen.
Die metallische Halbschale, die aus der besagten Druckschrift bekannt geworden ist, weist eine dreidimensionale offenmaschige Raumnetzstruktur auf, in welche Knochenmaterial einwachsen kann, um so für eine absolut sichere Sekundärfixation der Halbschale im Acetabulum zu sorgen.
Allerdings kommt es in 2 bis 8 % der Fälle gleichwohl zu einer Luxation des Gelenkes, allerdings nicht aufgrund einer Lockerung der Halbschale im Acetabulum, sondern vor allem, weil die Gelenkkugel aus der Ausnehmung im Inlay gewissermaßen herausrutschen kann.
Vor diesem Hintergrund ist es die Aufgabe der vorliegenden Erfindung, ein gattungsgemäßes Kugelkopfgelenk so weiterzubilden, daß der Luxation der Gelenkkugel aus dem Inlay heraus wirksam entgegengewirkt wird.
Gelöst wird diese Aufgabe durch das Kugelkopfgelenk mit den Merkmalen des Anspruchs 1. Vorteilhafte Weiterbildungen ergeben sich aus den Unteransprüchen.
Demnach wird vorgeschlagen, daß das Inlay aus einem elastischen spreizbaren Material besteht und so dimensioniert ist, daß es nach Einlegen der Gelenkkugel in seine Ausnehmung über deren Äquator hinübergreift. Die Gelenkkugel sitzt also quasi in einem Käfig, den das Inlay ausbildet. Die so gebildete Funktionseinheit aus Inlay und Gelenkkugel sitzt sodann in dem schon angesprochenen konischen Klemmsitz in der metallischen Halbschale. An sich wäre ein derartiger Sitz in der Halbschale bei entsprechend flachem Konuswinkel auch sicher. Da aber aufgrund der Käfiglagerung der Gelenkkugel im Inlay Querkräfte vom Inlay auf die Halbschale übertragen werden können, die bei herkömmlichen Konstruktionen für eine Luxation der Gelenkkugel aus dem Inlay sorgen würden, ist darüberhinaus eine Rastverbindung zwischen dem Inneren der metallischen Halbschale und dem Äußeren des Inlays vorgesehen. Bei Herstellung der Verbindung zwischen dem Inlay und der metallischen Halbschale wird das Inlay in das Innere der Halbschale gedrückt, so daß das Inlay zwar in bekannter Weise in einen konischen Klemmsitz mit dem Inneren der Halbschale kommt, aber zusätzlich noch durch die erwähnte Rast- oder snap-in- Verbindung in seiner Lage gesichert ist. Ein Verrutschen oder Verkeilen des Inlays nach der Implantation des künstlichen Hüftgelenkes ist mit dieser Maßnahme sehr unwahrscheinlich. Die Rastverbindung stellt - wie schon erwähnt - also eine zusätzliche Sicherung des Sitzes des Inlays in der metallischen Halbschale dar, da aufgrund der neuartigen Käfighalterung der Gelenkkugel in der Ausnehmung im Inlay die Übertragung oder Weiterleitung von Querkräften ermöglicht. Die Sicherheit
gegenüber einer Luxation der Gelenkkugel aus der Ausnehmung im Inlay wird also quasi erkauft durch die zusätzliche Sicherung des Inlays in der metallischen Halbschale durch die Rastverbindung.
Es wird gemäß einer bevorzugten Ausführungsform vorgesehen, daß die Rastverbindung zwischen dem Inlay und der Halbschale mittels einer Nut/Federverbindung herstellbar ist.
Diese kann entweder dadurch realisiert sein, daß die Feder zumindest teilweise zirkulär umlaufend auf den Außenkonturen des Inlays und die Nut auf entsprechender Höhe und in entsprechenden Abschnitten im Inneren der Halbschale zumindest zirkulär umlaufend ausgebildet ist.
Alternativ kann die Nut in den Außenkonturen des Inlays und die Feder im Inneren der Halbschale in entsprechender Höhe und in entsprechenden Abschnitten zirkulär umlaufend ausgebildet sein.
Eine Mischlösung kann ebenfalls vorgesehen sein, wo sich Nutabschnitte in den Außenkonturen mit Federabschnitten auf den Außenkonturen des Inlays abwechseln und sich in entsprechender Weise die Nutabschnitte und die Federabschnitte im Inneren der Halbschale abwechseln.
Die einfachste Ausführung jedoch ist jene, bei der eine Feder auf den Außenkonturen des Inlays vollständig um die Konturen des Inlays herumläuft bzw. eine Nut eingelassen ist. Dann kann es vorteilhaft sein, die Feder als Federring auszubilden. Alternativ kann die Feder als eine Materialanhäufung (Wulst) ausgebildet sein.
Das Zusammensetzen des Kugelkopfgelenks geschieht folgendermaßen:
Zunächst wird das Inlay zum Einsetzen der Gelenkkugel etwas gespreizt, die Kugel in die Ausnehmung im Inlay gesetzt und die Spreizung des Inlays wieder aufgehoben. Sodann umklammert das Inlay die Gelenkkugel und verhindert so wirksam ein Herausrutschen oder eine Luxation der Gelenkkugel aus der Ausnehmung im Inlay. Die Einheit Inlay/Gelenkkugel wird sodann an die metallische Halbschale herangeführt und in dessen Inneres hineingedrückt bis die Rastverbindung hergestellt ist und das Inlay in einem konischen Klemmsitz im Inneren der Metallschale sitzt.
Die Erfindung wird nachfolgend anhand eines Ausführungsbeispiels gemäß der Zeichnungsfiguren näher erläutert. Hierbei zeigt:
Figur 1 in teilweiser Schnittansicht eine metallische Halbschale, in welche die Einheit aus Gelenkkugel und Inlay hineingesetzt wird, und
Figur 2 das Kugelkopfgelenk im montierten Zustand.
Nachfolgend sind gleiche Teile mit denselben Bezugszeichen versehen.
Mit 1 ist bezeichnet eine metallische Halbschale, wie Sie beispielsweise aus der DE-C-40 21 607 bekannt ist. Vorliegend ist die Außenfläche der Halbschale 1 mit einer dreidimensionalen offenmaschigen Raumnetzstruktur 7 belegt, in welche und durch welche hindurch Knochenmaterial vom Acetabulum her wachsen kann, um so für eine sichere Sekundärfixation zu sorgen.
Im Inneren der Halbschale 1 ist eine konische Mantelfläche 8 vorgesehen, an welche sich eine konischen Außenfläche 9 des in das Innere der Halbschale zu setzenden Inlays 2 anlegt. Das Inlay 2 liegt dann in einem konischen Klemmsitz in der Halbschale 1.
Zur Arretierung des Inlays 2 in der Halbschale 1 ist vorliegend an den Außenkonturen des Inlays 2 eine zirkulär umlaufende Feder 5 vorgesehen, vorliegend ausgebildet als Materialhäufung. An entsprechender Stelle, d.h. Höhe ist im Inneren der metallischen Halbschale 1 eine zirkulär umlaufende Ringnut 6 eingelassen. Beim Einsetzen des Inlays 2 in die Halbschale 1 schwenkt die die Außenkonturen des Inlays 2 überragende Feder 5 leicht aus, um dann bei Erreichen der Ringnut 6 in der Halbschale 1 auszufedem und sich in die Nut 6 zu erstrecken. Die Dimensionen zwischen Inlay 2 und Halbschale sind so abgestimmt, daß die Feder 5 in die Nut 6 einschwenkt, wenn die konische Klemmverbindung zwischen den Wandflächen 8 und 9 hergestellt ist.
Das Inlay 2 weist eine Ausnehmung 4 auf, die komplementär zu einer Gelenkkugel 3 ausgebildet ist. Die Gelenkkugel 3 rollt in der Ausnehmung 4 ab. In Inneren der Gelenkkugel 3 ist eine konischen Klemmhülse 10 vorgesehen zur Verbindung mit einem konischen Zapfen an dem entsprechenden Hüftgelenksstiel (nicht dargestellt).
Das Inlay 2 ist so überdimensioniert, daß es nach einem gewissen Aufspreizen die Gelenkkugel 3 aufnimmt und diese über deren Äquator hinaus umfaßt, so daß die Gelenkkugel 3 quasi wie in einem Käfig sitzt und nicht ohne weiteres luxieren kann. Hierdurch wird die Sicherheit gegenüber einer Luxation geschaffen.
Claims
1. Kugelkopfgelenk für ein künstliches Hüftgelenk, aufweisend eine in das Acetabulum einsetzbare metallische Halbschale (1), in die ein Inlay (2) als Gleitpartner für eine auf einem Hüftstiel sitzende Gelenkkugel (3) in einen konischen Klemmsitz setzbar ist, wobei das Inlay (2) eine komplementär zur Gelenkkugel (3) ausgebildete Ausnehmung (4) aufweist, in welcher die Gelenkkugel (3) bei Ausführung ihrer Rotationsbewegung abrollt,
dadurch gekennzeichnet,
daß das Inlay (2) aus einem elastischen spreizbaren Material besteht und so dimensioniert ist, daß es nach Einlegen der Gelenkkugel (3) in seine Ausnehmung (4) über deren Äquator hinübergreift, und
daß eine Rastverbindung zwischen dem Inneren der metallischen Halbschale (1) und dem Äußeren des Inlays (2) vorgesehen ist.
2. Kugelkopfgelenk nach Anspruch 1, dadurch gekennzeichnet, daß die Rastverbindung zwischen Inlay (2) und Halbschale (1) mittels einer Nut/Federverbindung (5/6) herstellbar ist.
3. Kugelkopfgelenk nach Anspruch 2, dadurch gekennzeichnet, daß die Feder (5) zumindest teilweise zirkulär umlaufend auf den Außenkonturen des Inlays (2) und die Nut (6) auf entsprechender Höhe und in entsprechenden Abschnitten im Inneren der Halbschale (1) zumindest zirkulär umlaufend ausgebildet ist.
4. Kugelkopfgelenk nach Anspruch 2, dadurch gekennzeichnet, daß die Nut (6) zumindest teilweise zirkulär umlaufend in den Außenkonturen
des Inlays (2) und die Feder (5) auf entsprechender Höhe und in entsprechenden Abschnitten im Inneren der Halbschale (1) zumindest teilweise zirkulär umlaufend ausgebildet ist.
5. Kugelkopfgelenk nach einem der Ansprüche 2 bis 4, dadurch gekennzeichnet, daß die Feder (5) als Federring ausgebildet ist.
6. Kugelkopfgelenk nach einem der Ansprüche 2 bis 4, dadurch gekennzeichnet, daß die Feder (5) als Materialanhäufung ausgebildet ist.
