Abstract
Die Erfindung betrifft flüssigkeitsgefüllte Mikropartikel, die ihre Wirkung nach einem neuen Prinzip entwickeln, Verfahren zu deren Herstellung und die Verwendung als Therapeutika und/oder Diagnostika, wobei die Freisetzung von verkapseltem Arzneistoff und/oder Ultraschallkontrast erzeugenden Gasblasen aus den erfindungsgemäßen Mikropartikeln durch Anwendung von diagnostischem Ultraschall erfolgt.
Description
Flüssigkeitsgefüllte Mikropartikel mit neuem Wirkungsprinzip und deren Verwendung als Diagnostika und Therapeutika
Die Erfindung betrifft flüssigkeitsgefüllte Mikropartikel entsprechend den Patentansprüchen, die ihre Wirkung nach einem neuen Prinzip entwickeln, Verfahren zu deren Herstellung und die Verwendung als Therapeutika und/oder Diagnostika. Dabei erfolgt die Freisetzung von verkapseltem Arzneistoff und/oder Ultraschallkontrast erzeugenden Gasblasen aus den erfindungsgemäßen Mikropartikeln durch Anwendung von diagnostischem Ultraschall.
Es ist seit längerem bekannt, daß durch Injektion von Zubereitungen, die feine Gasblasen in freier [Echovist®, US 5 393 524, WO 94/16739 (Quay)] oder stabilisierter Form [(Levovist®, EP 89901933.5 und EP 93912897.1 (Stein et al.), WO 94/28780, WO 94/28873 (Unger), EP 0 458 745 (Schneider et al.)] enthalten, Ultraschallkontrasteffekte im Herz-Kreislauf-system, Leber, Niere, Milz u.a. Organen erzielt und diagnostisch genutzt werden können. Gemeinsam ist den dazu verwendeten Zubereitungen die Eigenschaft, daß die Echokontrast bewirkenden Gasblasen bereits als Blasen appliziert werden.
PCT WO/9428939 beschreibt die Stabilisierung niedrigsiedender, wasserunlöslicher und gasbildender Substanzen unter Verwendung von Tensiden in Form von Emulsionstropfen. Überraschenderweise wurde nun gefunden.daß die Freisetzung eines in der verkapselten Flüssigkeit gelöst, emulgiert oder suspendiert vorliegenden Arzneistoffes oder die Gasblasenentstehung infolge einer über der Siedepunkt liegenden Temperatur durch Verkapseiung mit einer stabilen, bioabbaubaren Polymerhülle effektiv verhindert werden kann. Die Polymerhülle bewirkt dabei eine Siedepunktserhöhung, so daß eine ungesteuerte Gasfreisetzung bei Applikation infolge der Körpertemperatur oder offener Lagerung oberhalb der Siedetemperatur wie bei den bisher bekannten Zubereitungen dieser Art [(US 5 393 524, WO 94/16739 (Quay), WO 94/28780 (Unger)], vermieden wird.
Die Freisetzung der Ultraschallkontrast erzeugenden Gasblasen und somit die Nutzung als Diagnostikum sowie zur zielgerichteten Arzneistofffreisetzung ist an die Anwendung von diagnostischem Ultraschall gebunden. Durch Beschallung wird dem verkapselten Flüssigkeitstropfen zusätzlich Energie zugeführt, so daß der Dampfdruck des
verkapselten Flüssigkeit steigt. Je nach Stabilität der die Flüssigkeitstropfen einschließenden Polymerhülle sowie der Leistung und Frequenz des diagnostischen Ultraschalls wird die Polymerhülle infolge des steigenden Dampfdruckes zerstört und die verkapselte Flüssigkeit beginnt zu sieden. Dabei werden der verkapselte Arzneistoff und/oder Ultraschallkontrast bewirkende Gasblasen in der gewünschten Körperregion freigesetzt.
Der besondere Vorteil besteht in der gezielten Freisetzung ausschließlich in der gewünschten Körperregion. Dadurch werden Gasverluste bei der Lungenpassage wie bei der üblichen i.v. Applikation und die Gefahr einer möglichen Lungenembolie infolge unkontrollierter Blasengrößen vermieden.
Überraschenderweise wurde weiterhin gefunden, daß zusammen mit niedrigsiedenden Flüssigkeiten bzw. deren Gemischen weitere Substanzen, bevorzugt Arzneistoffe, verkapselt werden können. Diese werden, wie für die Ultraschalldiagnostik beschrieben, an der gewünschten Körperregion durch Einwirkung von diagnostischem Ultraschall freigesetzt, was eine lokal hohe Arzneistoffkonzentration bewirkt und die Toxizität für den Gesamtorganismus herabsetzt.
Die Herstellung der erfindungsgemäßen Partikel ist nur über ein neues Verfahren zur in situ-Polymerisation möglich, wobei das zu polymerisierende Monomer vollständig oder partiell mit einem Polymerisationinhibitor (wie Hydrochinon oder Schwefeldioxid) oder einem Polymerisationinhibitorengemisch gesättigt wird. Der Polymerisations-inhibitor steuert die Polymerisationsgeschwindigkeit und ist somit essentielle Voraussetzung für eine erfolgreiche Verkapseiung.
Zur Herstellung der erfindungsgemäßen Mikropartikel wird eine Mischung aus der leichtsiedenden Flüssigkeit oder eines Flüssigkeitsgemisches (0,001% bis 50%) mit Wasser oder einer wäßrigen Tensidlösung (0,001 bis 20%) mit einem pH-Wert von 0 bis 10 hergestellt und mechanisch (Rührwerk, Homogenisator) oder mittels Ultraschall (US- Bad) emulgiert. Anschließend wird das mit dem Polymerisationinhibitor modifizierte Monomer zugegeben.wobei dieses polymerisiert und die erfindungs-gemäßen Partikel bildet. Zur Verminderung eines möglichen Monomerrestgehaltes kann die Polymerisation durch Zugabe eines Polymerisationsinitiators (z.B. Wasserstoffperoxid, tert.-Butylper-oxid) oder durch Bestrahlung mit UV-Licht oder radioaktiver Strahlung angeregt werden.
Zur Verkapseiung von Arzneistoffen werden diese in der leichtsiedenden Flüssigkeit bzw. einem Flüssigkeitsgemisch gelöst oder suspendiert und anschließend, wie oben beschrieben, verkapselt.

Beispiel 1
100 ml einer 1 % Triton X-100 und 1% Zonyl FSO100 enthaltenden Lösung (pH 7) werden mit 6,0 g Perfluorpentan versetzt (250 ml Schott-Flasche), auf 10°C temperiert und durch Schütteln emulgiert. Beispiel 2

Beispiel 3
Einer narkotisierten Ratte (Wistar Han, männlich, 150 g) werden 0,5 ml der Suspension aus Beispiel 1 gegeben. Anschließend wird das Tier sonografisch unter Verwendung eines ATL Ultramark 9, Schallkopf LS10-5 untersucht. Dabei ist eine deutliche B-Bild Kontrastanhebung in Leber, Niere, Vena hepatica und Vena cava inferior zu beobachten.
Beispiel 4
15 ml Aqua pur. werden mit 0,1 N NaOH auf pH 7,0 eingestellt, anschließend 1 ,0 g Perfluorpentan versetzt (20 ml Glasvial, verschlossen), 5°C temperiert und zwecks Emulgierung 30 sec im Ultraschallbad beschallt. Dann werden unter Rühren (Magnetrührer) 1 ,5 ml 0,3% SO enthaltendes Butylcyanacrylat zugetropft. Nach 2 h Rühren wird durch Zugabe von NaCI isotonisiert.
Beispiel 5
100 ml einer 1 % Triton X-100 enthaltenden wäßrigen Lösung (pH 7) werden mit 6 g Perfluorhexan versetzt (250 ml Schott-Flasche), auf 5°C temperiert und anschließend durch kräftiges Schütteln emulgiert. Dann werden unter Rühren (Magnetrührer) 10 ml 0,3% SO2 enthaltendes Butylcyanacrylat zugetropft. Nach 2 h Rühren wird durch Zugabe von NaCI isotonisiert.

Dann werden unter Rühren (Magnetrührer) 1 ml 0,3% SO2 enthaltendes Butylcyanacrylat zugetropft. Nach 2 h Rühren wird durch Zugabe von NaCI isotonisiert.
Beispiel 7
10 ml einer 1% Triton X-100 enthaltenden wäßrigen Lösung (pH 7) werden mit 7,0 g Perfluorpentan versetzt (20 ml Vial, verschlossen), auf 10°C temperiert und mittels Magnetrührer emulgiert. Dann werden unter Rühren (Magnetrührer) 1 ml 0,5% SO enthaltendes Ethylcyanacrylat zugetropft. Nach 2 h Rühren wird durch Zugabe von 0,1N NaOH der pH-Wert auf 7,0 eigestellt und durch NaCI-Zugabe isotonisiert.
Beispiel 8
10 ml einer 1 % Triton X-100 und 1 % g Zonyl FSO100 enthaltenden Lösung (pH 7) werden mit 0,35 g Perfluorpentan und 0,35 g i-Pentan versetzt (20 ml Vial, verschlossen), auf 10°C temperiert und durch Schütteln emulgiert. Dann werden unter Rühren (Magnetrührer) 1 ml 0,4% SO2 enthaltendes i-Butylcyanacrylat zugetropft.
Anschließend wird 2 h gerührt und durch Zugabe von NaCI isotonisiert.

Beispiel 9
1 ,0 g 0,3% SO2-enthaltendes Butylcyanacrylat, 0,7g i-Pentan, 0,1 g Span 80 und 0,01g Ölrot werden zusammen durch kräftiges Schütteln emulgiert und unter Rühren (Magnetrührer) zu 10 ml einer 1% Triton X-100 und 1% g Zonyl FSO100 enthaltenden Lösung (pH 7) gegeben. Anschließend wird 2 h gerührt. Durch Beschallung mit diagnostischem Ultraschall (ATL, UM9, LS 10-5) wird der verkapselte Farbstoff freigesetzt.

Dann werden unter Rühren (Magnetrührer) 1 ,5 ml 0,4% SO2 enthaltendes Acrolein zugetropft. Nach 2 h Rühren wird durch Zugabe von NaCI isotonisiert.
Beispiel 11
0,35 g Perfluorpentan und 0,35 g Sudan III enthaltendes n-Hexan werden gemischt und zu 10 ml Aqua pur. (mit 0,1 N NaOH auf pH 7eingestellt) gegeben (20 ml Vial, verschlossen), auf 10°C temperiert und dann mittels Ultraschallbad 30 sec emulgiert. Dann werden unter Rühren (Magnetrührer) 1 ml 0,3% SO2-enthaltendes Butylcyanacrylat zugetropft. Anschließend wird 2 h gerührt und durch Zugabe von NaCI isotonisiert.
Zur Freisetzung des verkapselten Farbstoffs wird die Suspension 2 min mit diagnostischem Ultraschall beschallt, wobei sie sich rot färbt
Beispiel 12
10 ml einer 1% Triton X-100 enthaltenden wäßrigen Lösung (pH 7) werden mit 7,0 g Perfluorpentan versetzt (20 ml Vial, verschlossen), auf 10°C temperiert und mittels

Beispiel 13
in vitro-Kontrastprüfung (ATL Ultramark 9, Schallkopf L10-5, Focus 2 cm) Zu 600 ml Wasser (Becherglas, auf 37°C temperiert) wird 1 ml der Zubereitung aus Beispiel 1 gegeben. Der Schallkopf taucht ca. 1 mm in die Flüssigkeit ein. Die Schalleistung (Mechanical Index) beträgt 0,2 (Höchststufe 1 ,2). Es ist über einen
Zeitraum von 2 h kein in vitro-Kontrast zu sehen. Nach 2 h wird die Schalleistung auf MI 0,7 erhöht. Ca. 1 sec. später tritt starker, langanhaltender Ultraschallkontrast auf, der min. 1 h anhält.
Beispiel 14
Versuchsaufbau wie bei Beispiel 12, wobei das Wasser 55°C temperiert wird. Nach Zugabe von 0,5 ml Probe hergestellt entsprechend Beispiel 5 ist bei Schalleistung (Mechanical Index) 0,3 (Höchststufe 1 ,2) über einen Zeitraum von 2 h kein in vitro- Kontrast zu sehen. Nach 2 h wird die Schalleistung auf Stufe 0,8 erhöht. Ca. 1 sec. später tritt starker, ianganhaltender (>30 min) Ultraschallkontrast auf.
Beispiel 15
20 ml einer 0,25 M HCI werden mit 1 % Phosphorsäure und 40 mg Triton X-100 versetzt. Dieser Mischung werden 1 ,4 g Perfluorpentan zugesetzt. Das Vial wird verschlossen, auf 10 °C temperiert und 30 sec im Ultraschallbad emulgiert. Dann werden unter Rühren 0,4 ml Etylcyanacrylat zugetropft. Nach 24 h Rühren wird durch Zugabe von 1 N NaOH ein pH-Wert von 4 eingestellt und mit 0,9% NaCI isotonisiert.
Beispiel 16
20 ml einer 0,25 M H2SO4 werden mit 1 % Citronensäure und 40 mg Triton X-100 versetzt. Dieser Mischung werden 0,7 g Perfluorpentan zugesetzt. Das Vial wird verschlossen, auf 10 °C temperiert und 30 sec im Ultraschallbad emulgiert. Dann

Zu 20 ml Schwefliger Säure (pH 2,0) werden 0,7 ml Perfluorpentan zugesetzt. Das Vial wird verschlossen, auf 10 °C temperiert und 30 sec im Ultraschallbad emulgiert. Dann werden unter Rühren 0,4 ml Etylcyanacrylat zugetropft. Nach 24 h Rühren wird durch Zugabe von 1 N NaOH ein pH-Wert von 4 eingestellt und mit 0,9% NaCI isotonisiert.
Beispiel 18
Zu 20 ml Schwefliger Säure (pH 2,0) werden 0,7 ml Perfluorpentan zugesetzt. Das Vial wird verschlossen, auf 10 °C temperiert und 30 sec im Ultraschallbad emulgiert. Dann werden unter Rühren 0,4 ml eines 1 :1-Gemisches aus Etylcyanacrylat und Butylcyanacrylat-zugetropft. Nach 24 h Rühren wird durch Zugabe von 1 N NaOH ein pH-Wert von 4 eingestellt und mit 0,9% NaCI isotonisiert.
Beispiel 19
Zu 20 ml Schwefliger Säure (pH 2,5) werden 0,5 ml Perfluorhexan zugesetzt. Das Vial wird verschlossen, auf 10 °C temperiert und 30 sec im Ultraschallbad emulgiert. Dann werden unter Rühren 0,4 ml Butylcyanacrylat zugetropft. Nach 24 h Rühren wird durch Zugabe von 1 N NaOH ein pH-Wert von 4 eingestellt und mit 0,9% NaCI isotonisiert.
Claims
1. Flüssigkeitsgefüllte Mikropartikel, dadurch gekennzeichnet, daß die Mikropartikel mit einer niedrigsiedenden, organischen Flüssigkeit oder einem Flüssigkeitsgemisch gefüllt sind, die oberhalb der Siedetemperatur der verkapselten Flüssigkeit oder des Flüssigkeitengemisches erst unter Beschallung mit diagnostischen Ultraschall aufsiedet, den Kapselinhalt freisetzt und Ultraschallkontrast erzeugt.
2. Flüssigkeitsgefüllte Mikropartikeln, dadurch gekennzeichnet, daß die Größe der Mikropartikel im Bereich von 0,05 bis 7 μm für i.v. Anwendung und < 50 μm für
Nichtgefäßanwendung (orale, intravesikal, ) liegt.
3. Flüssigkeitsgefüllte Mikropartikel nach Anspruch 1 , dadurch gekennzeichnet, daß die Mikropartikel aus in situ poiymerisierten synthetischen Polymeren, insbesondere Polyalkylcyanacrylaten, Methacrylaten, Dimethacrylaten,
Alkylcyanacrylat-Styren-Copolymeren, Urethanmethacrylat, Acrolein und Derivaten, Acrylsäuren, Acrylnitril, Acrylsäurechloriden, Methacryinitrilen, α-substituierten
Acroleinderivaten bestehen.
4. Flüssigkeitsgefüllte Mikropartikel nach Anspruch 1 , dadurch gekennzeichnet, daß die Mikropartikel niedrigsiedende, organische Flüssigkeiten, insbesondere i- Pentan, Pentan, Perfluorpentan, n-Hexan, Cyclopentan, 2-Methylpentan, 2,2- Dimethylbutan, Diethylether, 3-Methyl-1-buten, Perfluorhexan, 1 ,1 Dichlorethylen, 2- Methyl-2-buten, Perfluorheptan, Perfluorcyclohexan, Perfluorcyclopentan, Perfluor-2- buten, Hexafluor-1 ,3-butadien, Hexafluorcyclobuten, 1 ,1 ,1 , 2,3, 3-Hexafluopropan, Perfluor-2-buten oder deren Gemische aus mindestens 2 Substanzen enthalten.

Flüssigkeitsgefüllte Mikropartikel nach Anspruch 1 , dadurch gekennzeichnet, daß die Mikropartikel neben den in Anspruch 3 genannten synhetischen Polymeren und der in Anspruch 4 genannten Kapselfüllung einen Weichmacher, insbesondere Triacetin, Ester höherer Fettsäuren wie Butylstearat, Isopropylmyristat, Isopropyl- palmitat oder Oleyloleat, Phthalate wie Dibutyl-, Diethyl-, Diisooctyl-, Diisobutyl-, Dicapryl-, Dinonyl-, Dimethylcyclohexyl- oder Diethylhexylphthalat, Sebacate wie Dibutyl-, Diethyl-, Diethylhexyl-, Dinonyl-, Diisooctyl-, Polypropylen- oder Dimethoxy- cyclohexylsebacat, Adipate wie Dibutyl-, Diethylhexyl-, Dinonyl-, Polypropylen-, Dimethylcyclohexyl-, Dibutoxyethyladipat, Paraffine, Miglyol®, pflanzliche Öle wie Erdnuß-, Leinen-, Raps-, Rizinus-, Oliven-, Mandel-, Sonnenblumen-, Sesam- oder Baumwoilsamenöl oder deren Gemische aus min. 2 Substanzen enthalten. 6. Flüssigkeitsgefüllte Mikropartikel nach Anspruch 1 , dadurch gekennzeichnet, daß die Herstellung in Wasser oder einem tensidhaltigen Dispergiermedium, das Polyethylenoxid-Polypropylenoxid-Copolymere, Sorbitanfettsäureester, Polyoxyethylenfettalkoholether, Phosphatidylcholin, Polyoxyethylenfettsäureester, Fluortenside, Nonoxynol, Octoxynol, autoklavierte Gelatine oder deren Gemische enthält, erfolgt.
7. Flüssigkeitsgefüllte Mikropartikel nach den Ansprüchen 1 - 6, dadurch gekennzeichnet, daß die Mikropartikel zusätzlich zur der verkapselten Flüssigkeit einen oder mehrere Arzneistoffe, DNA, RNA, Proteine, Farbstoffe, Röntgen- oder NMR-Kontrastmittel enthalten.
8. Flüssigkeitsgefüllte Mikropartikel nach Anspruch 7, dadurch gekennzeichnet, daß die Mikropartikel Zytostatika, Hormone, Enzyme oder Vaccine enthalten.
9. Flüssigkeitsgefüllte Mikropartikel nach den Ansprüchen 1 - 8, dadurch gekennzeichnet, daß die Freisetzung der verkapselten Substanz erst unter Anwendung von diagnostischem Ultraschall in der gewünschten Körperregion erfolgt.

Verfahren zur Herstellung der erfindungsgemäßen Mittel nach einem der Ansprüchen, dadurch gekennzeichnet, daß das zu polymerisierende Monomer vollständig oder partiell mit einem Polymerisationsinhibitor, insbesondere Schwefeldioxid, Schwefelmonoxid, Schwefeltrioxid, Schwefeltetroxid, Dischwefeltrioxid, Distickstoffoxid, Chinon, Hydrochinon, p- Benzochinon, starke organische und anorganische Säuren, Sauerstoff oder Gemischen aus min. 2 der genannten Substanzen, gesättigt wird.
1 1. Verfahren zur Herstellung von Mitteln nach Anspruch 10, dadurch gekennzeichnet, daß die Polymerisationsinhibitorkonzentration 0,00001 bis 20% beträgt.
12. Verfahren zur Herstellung von Ultraschallkontrastmitteln nach den
Ansprüchen 1 bis 10, dadurch gekennzeichnet, daß zur Herstellung des erfindungsgemäßen Präparates eine Emulsion aus der niedrigsiedenden Flüssigkeit oder eines Flüssigkeitsgemisches in Wasser oder einer wäßrigen Tensidlösung oder eines Tensidlösungsgemisches hergestellt und dieses anschließend unter Rühren das Monomer oder Monomerengemisch zugegeben sowie dispergiert wird.
13. Verfahren zur Herstellung von Ultraschallkontrastmitteln nach Anspruch 12, dadurch gekennzeichnet, daß der Anteil der niedrigsiedenden Flüssigkeit oder des Flüssigkeitsgemisches am Volumen der Emulsion 0,001% bis 50% beträgt.
14. Verfahren zur Herstellung von Ultraschallkontrastmitteln nach Anspruch 12, dadurch gekennzeichnet, daß die Tensidkonzentration 0,001 bis 20% beträgt.
15. Verfahren zur Herstellung von Ultraschallkontrastmitteln nach Anspruch 12, dadurch gekennzeichnet, daß das Dispergiermedium (Emulsion) einen pH-Wert von 0 bis 10 aufweist.
16. Verfahren zur Herstellung von Ultraschallkontrastmitteln nach den Anspruch 12, dadurch gekennzeichnet, daß die Temperatur während des Herstellprozesses niedriger als die Siedetemperatur der niedrigsiedenden Flüssigkeit oder des Flüssigkeitsgemisches (unter Normaldruck) ist.
17. Verfahren zur Herstellung von Ultraschallkontrastmitteln nach Anspruch 12, dadurch gekennzeichnet, daß die Dispergierung mittels mechanischen Rührwerk, Homogenisator oder Ultraschall oder durch Kombination aus mindestenz 2 dieser Methoden erfolgt.
18. Ultraschallkontrastmittel nach Anspruch 1 , dadurch gekennzeichnet, daß durch Zugabe osmotisch aktiver Substanzen, insbesondere Natriumchlorid, Mannit, Galaktose, Glucose, Fruktose, Isotonie eingestellt wird.
19. Ultraschallkontrastmittel nach Anspruch 1 , dadurch gekennzeichnet, daß der Ultraschallkontrast erst durch Beschallung mit Ultraschall, insbesondere diagnostischen Ultraschall, hervorgerufen wird.
20. USKM, dadurch gekennzeichnet, daß Effekte mit deutlich erhöhten nichtlinearen Anteilen entstehen.
