Procédé pour la contention et la réduction d'effondrement des planchers orbitaires, caractérisé en ce qu'il consiste à fixer sur le rebord orbitaire du patient un plancher orbitaire réalisé en matériau synthétique prédécoupé et prégalbé ne se déformant pas à la température de stérilisation par l'oxyde d'éthylène. Ce procédé facilitant l'adaptation ou l'auto-adaptation à la cavité orbitaire en utilisant deux polymères; l'un thermostable à 37°; l'autre thermomalléable à 37° ou, en utilisant un système de lames rigides réunies par des ponts intercalaires de moindre résistance.
Description
La présente invention concerne une technique nouvelle pour la contention et la réduction d'effondrement des planchers orbitaires.
On sait que l'on désigne par plancher orbitaire un ensemble d'os très minces constituant une surface horizontale galbée sur laquelle repose le globe oculaire, sa partie antérieure constitue le rebord orbitaire antéro inférieur.
En cas de traumatisme facial, cet os se rompt en créant des lésions fonctionnelles (diplopie) et esthétiques (enophtalmie). La qualité de reconstruction de cet os est alors importante car les séquelles de cette opération sont difficilement opérables.
Jusqu'à présent, le procédé le plus usuel consistait à pratiquer une homogreffe à partir d'un fragment de côte ou d'os iliaque du patient. L'opération était longue et douloureuse, car il est difficile de travailler l'os. On a bien proposé de remplacer l'os par un silicone médical.
Toutefois ce matériau est trop souple, ce qui exige l'emploi de lames épaisses, qui se travaillent également difficilement, ce qui interdisait la récréation du galbe originel. Au surplus, ce matériau provoque fréquemment des réactions inflammatoires ou exsudatives. Enfin, ce matériau n'est pas ostéosynthésant, c'est-à-dire, n'amorce pas la re-formation de l'os naturel.
L'invention repose sur la découverte que l'emploi de certains polymères biodégradables ou implants biostables ou mixtes parce que bien tolérés, permet de créer des pièces possédant exactement les mêmes dimensions, la même structure et les mêmes qualités biomécaniques que l'os originel. De tels matériaux sont notamment ceux décrits dans le brevet français
78.29878, et qui consistent en une matrice de poly-acide lactique contenant des éléments de renfort à base d'acide poly-glycolique, avec éventuellement une addition de phosphate tricalcique ou d'autres substances phamacodynamiques influençant la cicatrisation, telles que antibiotiques, anti-inflammatoires, opacifia ou témoins de dégradation, accélérant la cicatrisation osseuse.
Il est en effet possible de mouler directement avec ce matériau un plancher orbitaire prédécoupé et prégalbé, qui ne se déforme pas à la température de stérilisation par l'oxyde d'éthylène (60º), et qui peut être vissé au rebord orbitaire en soutenant efficacement l'oeil en porte à faux (réalisant la technique chirurgicale dite du Bαlcan).
Pour améliorer encore les modalités d'application de ces implants, il est prévu, selon l'invention :
- de réaliser la partie centrale plus dure que les côtés, pour en faciliter l'adaptation, - de créer des zones de renfort au moyen de parties saillantes, par exemple selon un schéma ayant la forme d'une toile d'araignée ; ce principe pouvant permettre la découpe du matériau en limitant les possibilités de micro fractures ; ou au moyen de fils noyés dans la masse ou en surface
- et de moduler sa durée résorption en pratiquant des perforations dans le produit de manière à augmenter sa surface de contact avec les tissus naturels.
Enfin, pour éviter que l'implant en place ne se déplace accidentellementj on peut y pratiquer des rugosités standard ou orientées vers l'avant, qui peuvent elles-mêmes être en phosphate, les surfaces des perforations étant marquées sur l'implant.
Une telle pièce peut même servir de console par dessus une fracture, si on la fixe au rebord oculaire au moyen de vis resorbables, auquel cas l'implant participe à l'ostéosynthèse est à 1 'aggregation squelettique elle-même.
L'invention sera encore mieux comprise en se référant à la description détaillée suivante de son application à la réalisation de planchers orbitaires, cette description se référant au dessin annexé sur lequel :
- les figures 1a et 1b représentent en vue de dessus et en coupe la forme prédécoupée d'origine.
- les figures 2a et 2b sont des vues analogues, la même forme étant galbée.
- la figure 3 est semblable à la figure 2a, les bords en étant amincis.
- la figure 4 illustre schématiquement un motif de renfort de la forme de la figure 3. - les figures 5a, 5b, 5c et 5d sont des coupes partielles agrandies illustrant divers modes de renfort.
- les figures 6, 7 et 8 illustrent diverses variantes de la pièce de la figure 3.
- la figure 9 illustre une autre variante de réalisation d'une pièce de ce genre. - les figures 10a et 10b illustrent une adaptation nouvelle d'une telle pièce, - les figures 11a, 11b et 11c illustrent une variante d'une pièce de ce type permettant de moduler son temps de résorption chez le patient.
- et les figures 12a et 12b illustrent une variante de réalisation dans laquelle la pièce est solidaire d'un élément facilitant sa mise en place.
Les figures 1 à 5 illustrent le principe d'obtention d'une forme prédécoupée selon l'invention.
Le schéma prédécoupé représente le point de départ ; celui-ci étant un triangle curviligne reproduisant la forme du plancher orbitaire. Ce triangle curviligne 1 plat et d'épaisseur constante (figures 1a et 1b), ce triangle est tout d'abord galbé pour s'adapter au profil concave de la cavité oculaire (figures 2à/2b), puis les bords en sont amincis pour qu'il subsiste un épaississement central (figure 3).
La forme ainsi obtenue doit comporter des moyens de renfort et/ou d'adaptation volumétrique.
Ainsi, on a représenté en 2 sur la figure 4 un motif préféré de renfort d'une forme 1 conforme elle-même à la figure 3.
Ce motif est en forme d'étoile d'araignée, comportant des zones de renfort radiaux et concentriques. Ces moyens de renfort peuvent être des fils en matériau résistant, tels que 2a, noyés dans le matériau de la forme 1 (figure 5a), ou consister en nervures plus épaisses, sur une face seulement (2b) ou sur les deux faces (2c, 2c') de la forme 1 (figures 5b et 5c), ou consister en une combinaison des deux , à savoir des nervures 2d et des fils 2d', associés à la même forme 1 (figure 5d).
En ce qui concerne l'adaptation volumétrique du produit au patient, divers moyens sont possibles dans le cacre de l'invention.
Un premier moyen est l'amincissement périphérique de la forme 1, comme il est spécifié avec référence à la figure 3, ci-dessus.
Selon un second moyen, on peut associer dans la même pièce, un produit stable à 37º et un produit thermomalléable à 37º, ce qui permet une auto-adaptation du produit en place chez le patient.
Un tel produit est illustré à la figure 6, où la forme 1 est la réunion d'une zone centrale 1A, en produit stable à 37º, plus rigide et assurant une contention efficace, avec une-zone périphérique 1B, en produit malléable à la température du corps, déterminant l'auto-adaptation de la forme 1.
Selon un troisième moyen, illustré aux figures 7 et 8, on peut prévoir sur la forme 1 une découpe particulière, selon une incision unique
3 (figure 7) ou multiple 3a, 3b, 3c, créant un plancher multifolié (figure 8).
Selon un quatrième moyen, illustré à la figure 9, on peut utiliser pour réaliser la forme 1 un produit composite interrompu, comportant des lamelles individuelles 4a, 4b, 4c, réunies par des ponts intercalaires 5, qui peuvent être soit le matériau de renfort prévu, soit le même matériau que les lamelles, mais amenés, de manière à se prêter à l'adaptation volumétrique chez le patient, en se déformant ou même en"craquant".
Il doit être bien entendu que les diverses possibilités ainsi of fertes par l'invention sont subordonnées à la sélection d'un matériau approprié pour la forme 1.
Parmi les matériaux déjà connus dans ce domaine, on rappellera notamment ceux faisant l'objet des brevets français 76.28163 et 78.29878, utilises seuls ou en asso iation ensemble ou avec d'autres matériaux, ainsi que sous forme de produits composites. Au surplus, ces matériaux peuvent être utilisés conjointement avec des produits contribuant à l'accélération de la cicatrisation osseuse, notamment le phosphate tricalcique.
Au titre de variantes dans la mise en oeuvre de l'invention, on peut mentionner diverses adaptations de la forme de base, en fonction de certaines situations propres aux patients. Selon une première adaptation, illustrée par les figures 10a et 10b, ladite forme peut constituer un dispositif permettant à la fois l'ostcosynthèse et l'ostcoplasti" du plancher oculaire. Dans ce cas, la forme 6 est fixée sur une zone de fracture réduite 7 au moyen de vis d'ostéosynthèse 8.
Selon une seconde adaptation, les nervures ou aspérités telles que représentées en 2 sur les figures 4 et 5 peuvent être utilisées comme moyen de rétention.
Selon une troisième adaptation illustrée par les figures 11a, 11b et 11c, un moyen de moduler le temps de résorption du plancher consiste à percer dans ce dernier un nombre déterminé de trous, la durée de résorption étant d'autant plus courte que le nombre de ces trous sera plus élevé, puisque la présence de chaque trou accroît la surface de contact de l'organisme avec le matériau biorésorbable. Ainsi, si sur la figure 11a le plancher 1 n'est pas percé dans les emplacements prévus 9a, 9b, 9c, 9d, la durée de résorption sera de l'ordre de 18 mois ; si l'un des emplacements, 9a, est percé, cette durée est ramenée à 12 mois (figure 11b) et si deux emplacements 9a et 9b sont percés, cette durée est abaissée à 6 mois (figure 11c). Il devient donc possible de moduler à volonté ce temps de résorption, en fonction des conditions spécifiques du patient, à partir d'un produit unique, ce qui a le grand avantage de faciliter la fabrication du produit de base, dont la résistance intrinsèque demeure constante.
Dans la variante illustrée aux figures 12a et 12b, montrant un plancher orbitaire 1 respectivement de face et de profil, à ce plancher est associé par moulage une tige d'injection 10 faisant un angle, facilitant le positionnement et les manipulations per-opératoires ; elle est de plus autocassable en 11.
Enfin, il convient de préciser que les produits composites décrits ci-dessus peuvent comporter d'autres variantes de modes de réalisations, notamment des réalisations dans lesquelles les zones ou moyens de renfort sont disposés dans le sens transversal.
Claims
1. Procédé pour la contention et la réduction d'effondrement des planchers orbitaires, caractérisé en ce qu'il peut consister à fixer sur le rebord orbitaire du patient, un plancher orbitaire réalisé en matériau synthétique prédécoupé et prégalbé ne se déformant pas a la température de stérilisation par l'oxyde d'éthylène.
2. Procédé selon la revendication 1, caractérisé en ce que ledit schéma prédécoupé représente le point de départ ; celui-ci étant un triangle curviligne.
3. Procédé selon la revendication 1, caractérisé en ce que ledit matériau synthétique est un polymère biodégradable, biostable ou mixte.
4. Procédé selon les revendications 1 et 2, caractérisé en ce que la zone centrale de l'implant est plus dure que les côtés.
5. Procédé selon les revendications 1 à 3, caractérisé en ce que la plaque comporte des zones de renfort crées au moyen de parties saiLlantes.
6. Procédé selon les revendications 1 à 4; 5 caractérisé en ce que le schéma du renfort, parties saillantes ou fils noyés dans la masse, a la forme d'une toile d'araignée ; ce principe pouvant permettre la découpe du matériau en limitant les possibilités de micro-fracture.
7. Procédé selon les revendications 1 à 5, caractérisé en ce que la durée de résorption de la plaque est modulée au moyen de perforations modifiant sa surface de contact avec le corps du patient.
8. Procédé selon les revendications 1 à 7, caractérisé en ce que l'implant est immobilisé contre tout déplacement au moyen de rugosités ou stries orientées vers l'avant.
9. Procédé selon les revendications 1 à S, caractérisé en ce qu'il comporte une tige' d'injection auto cassable, facilitant le positionnement et les manipulations per-opératoires.
10. Procédé selon les revendications 1, 2 et 3 , permettant 1 'autoadaptation du produit en associant un matériau thermostable à.37º et un matériau thermomalléable à 37º.
11. Procédé selon les revendications 1, 2, 3 et 4,, permettant l'auto- adaptation du produit en utilisant un matériau composite interrompu comportant des lamelles réunies par des ponts intercalaires de moindre résistance.
12. Flaque d'ostéosynthèse comportant les caractéristiques selon les revendications 1 à 11.
