- With reference to FIGS.1 to 7B, for example, a cleaning device for a surgical treatment instrument according to a first embodiment of the present invention will be described as a medical device.
- The surgical treatment instrument which is a cleaning object in the present embodiment is an
ultrasonic treatment instrument 1. Theultrasonic treatment instrument 1 includes ahandle unit 2 shown in FIG. 2, aprobe unit 3 shown in FIG. 3A, and avibrator unit 4 shown in FIG. 3B. When theultrasonic treatment instrument 1 is used, therespective units - As shown in FIG. 3B, the
vibrator unit 4 includes a cylindricalexterior cover 5, and an ultrasonic vibrator and horn (not shown) are disposed in theexterior cover 5. Ahand piece cord 7 to be electrically connected to a power supply source (not shown) is connected to a proximate-end of thevibrator unit 4. A power for driving is supplied to the ultrasonic vibrator from the power supply source via thehand piece cord 7 to drive the ultrasonic vibrator. A connection portion 9 to be connected to thehandle unit 2 is formed in a front peripheral edge of thevibrator unit 4. - As shown in FIG. 3A, the
probe unit 3 includes a rod-shapedvibration transmission member 10, and ultrasonic vibration generated in the ultrasonic vibrator of thevibrator unit 4 is transmitted to a distal-end treatment portion by this rod-shapedvibration transmission member 10. Amale screw 11 to be screwed in a female screw formed in the distal-end of the horn is formed in a proximate-end position of thevibration transmission member 10. Thevibration transmission member 10 includes: a conical proximate-end horn 12 for enlarging an amplitude of the ultrasonic vibration amplified by the horn on the side of thevibrator unit 4 further into a second stage; amiddle portion 13 positioned on the side of a distal-end of the proximate-end horn 12; a conical distal-end horn 14, positioned on the side of the distal-end of themiddle portion 13, for enlarging the amplitude of the ultrasonic vibration amplified by the proximate-end horn 12 into a final stage; and a cylindrical distal-end 15 positioned on the side of the distal-end of the distal-end horn 14. The ultrasonic vibration generated in the ultrasonic vibrator is gradually amplified by the horn of thevibrator unit 4, the proximate-end horn 12 in thevibration transmission member 10, and the distal-end horn 14, and is transmitted to the distal-end 15 of thevibration transmission member 10. In themiddle portion 13 of thevibration transmission member 10, a plurality of flange-shapedsupport members 16 are positioned in second and subsequent nodes on the side of the distal-end of the transmitted ultrasonic vibration. - As shown in FIG. 2, the
handle unit 2 includes anoperation portion 19, aninsertion sheath portion 17 including alongitudinal overcoat tube 20 rotatably attached to theoperation portion 19, and a distal-end function portion (treatment portion) 21 disposed in the distal-end of theinsertion sheath portion 17. Theoperation portion 19 of thehandle unit 2 includes an operation portionmain body 22, a fixedhandle 23 formed integrally with the operation portionmain body 22, and amovable handle 25 rotatably attached to the operation portionmain body 22 via ahandle support shaft 24. - As shown in FIG. 5, a cylindrical
joint member 32 disposed in the operation portionmain body 22 via apipe fixing member 31 is fixed/connected to the proximate-end portion of theinsertion sheath portion 17. Thejoint member 32 is attached to the operation portionmain body 22 by a fixingring 33 so as to rotatable around an insertion portion axis. Arotary operation knob 34 is fixed to thejoint member 32. When therotary operation knob 34 is rotated/operated, theinsertion sheath portion 17 can be rotated/operated around a sheath axis. - A cylindrical driving force transmission
intermediate member 38 to be connected to anoperation rod 60 described later is fitted in thejoint member 32 so as to be movable in an insertion portion axial direction. Apin 39 is attached to a rear-end portion of the driving force transmissionintermediate member 38. Thispin 39 is fitted/engaged in aguide groove 41 which is cut/formed in thejoint member 32 so as to extend in the insertion portion axial direction. This regulates the rotation of the driving force transmissionintermediate member 38 and allows only the movement into the insertion portion axial direction. AnO ring 42 is fitted around the outer periphery of the distal-end portion of the driving force transmissionintermediate member 38, and theO ring 42 is disposed between the driving force transmissionintermediate member 38 andjoint member 32. - As shown in FIG. 5, a distal-end outer peripheral portion of a
slider receiver member 45 is screwed in the driving force transmissionintermediate member 38, and both the portion and the member are coaxially assembled/fixed. Aslider member 46 is slidably fitted in the outer periphery of the driving force transmissionintermediate member 38 on the side of the rear end. Acoiled spring 47 is wound around the outer periphery of theslider receiver member 45 between the driving force transmissionintermediate member 38 and theslider member 46. - A first packing48 in the form of the O ring is attached to the inner surface of the driving force transmission
intermediate member 38. When theprobe unit 3 is attached to thehandle unit 2, thefirst packing 48 is bonded to the outer periphery of thevibration transmission member 10 of theprobe unit 3 to hold thevibration transmission member 10 and to close a gap between the units. Asecond packing 49 having the O-ring shape is attached to a hand-side end of theslider receiver member 45. When theprobe unit 3 is attached to thehandle unit 2, thesecond packing 49 is bonded to the outer periphery of thevibration transmission member 10 of theprobe unit 3 to hold thevibration transmission member 10 and to close the gap between the units. - Since the
packings packings vibration transmission member 10 to ultrasonically vibrate is steadily supported without any difficulty. - An engaging
pin 51 to engage with theslider member 46 in the operation portionmain body 22 is disposed in themovable handle 25 of thehandle unit 2. When themovable handle 25 is rotated centering on thehandle support shaft 24, the movement of themovable handle 25 is transmitted to theslider member 46 via the engagingpin 51. Since thehelical compression spring 47 is constantly attached in a compressed/urged state, a certain or more operation force applied to themovable handle 25 in a closing direction is absorbed by deformation of thehelical compression spring 47. Accordingly, theslider receiver member 45 is slid/operated in a state in which an overload applied to theslider receiver member 45 is prevented. - As shown in FIG. 5, a
vibrator connection portion 52 for detachably attaching thevibrator unit 4 is disposed in a rear-end portion in the operation portionmain body 22 of thehandle unit 2. Thevibrator connection portion 52 includes a cylindrically formedframe member 53 and acylindrical connection member 54. Theframe member 53 andconnection member 54 are screwed/fixed into the inner surface of the operation portionmain body 22. A screwed/fixed portion of theframe member 53 is positioned before theconnection member 54. The rear-end portion of theframe member 53 is concentrically disposed inside theconnection member 54, When thevibrator unit 4 is attached to thehandle unit 2, a front-end portion of the cylindricalexterior cover 5 of thevibrator unit 4 is fitted between theframe member 53 andconnection member 54. The connection portion 9 of thevibrator unit 4 is fitted into engaged with a distal-end engagement portion 55 of theframe member 53, so that thevibrator unit 4 is fitted to thehandle unit 2. The connection portion 9 is fitted, for example, by a C ring so as to swell outwards for plurality of through-holes 56 are appropriately scattered and disposed in a wall portion of theframe member 53. - Next, the insertion
sheath port ion 17 of thehandle unit 2 will be described. As shown in FIG. 4A, theovercoat tube 20 of theinsertion sheath portion 17 includes an outer pipe (overcoat tube) 61 and aninner pipe 62, and the outer periphery of theouter pipe 61 is subjected to an electric insulating treatment by an insulatingtube 63. In the electric insulation, theinsertion sheath portion 17 may also be subjected to treatment such as coating. As shown in FIG. 4B, theouter pipe 61 includes amain channel 65 for passing thevibration transmission member 10 of theprobe unit 3, and a sub-channel 66 for passing theoperation rod 60. Similarly as shown in FIG. 4B, for theinner pipe 62 forming themain channel 65, a part of a circular sectional portion is crushed to be flat to form a gas a the sub-channel 66 between the section and the inner surface of theouter pipe 61. A through-hole 67 is formed in one or more appropriate portions of theinner pipe 62 disposed opposite to the sub-channel 66, so that themain channel 65 can be connected to the sub-channel 66. - As shown in FIG. 4A, the distal-
end function portion 21 of thehandle unit 2 includes a distal-end chip member 71 attached to the distal-end portion of theouter pipe 61 and formed, for example, of a metal, and a singleswing hold member 74 rotatably attached to the distal-end chip member 71 via asupport shaft 72. Thishold member 74 holds agrasp member 75. The distal end of theoperation rod 60 is connected to the proximate-end portion of thehold member 74 via a support shaft 76. When themovable handle 25 is rotated to move theoperation rod 60 forwards/backwards, thehold member 74 rotates, and an operation of opening/closing the distal-end function portion 21 is carried out. When the distal-end function portion 21 is disposed opposite to the distal-end portion 15 of theprobe unit 3 in thevibration transmission member 10, and a living tissue of a treatment object is grasped by both the portions, a vibration energy is transmitted to the living tissue from thevibration transmission member 10. - Next, a
cleaning device 80 for cleaning theultrasonic treatment instrument 1 will be described. As shown in FIG. 6, thecleaning device 80 includes a nozzlemain body 81 detachably attached to theoperation portion 19 of thehandle unit 2. As shown in FIG. 7A, the nozzlemain body 81 is inserted into thehandle unit 2 from which theprobe unit 3 of theultrasonic treatment instrument 1 has been removed, and is accordingly attached to thehandle unit 2, and theultrasonic treatment instrument 1 is used in this state. The nozzlemain body 81 is not limited to a metal material. - As shown in FIG. 7A, the nozzle
main body 81 of thecleaning device 80 is constituted of a double pipe structure including anouter pipe 82 andinner pipe 83. To attach the nozzlemain body 81 to theoperation portion 19 of thehandle unit 2 from which theprobe unit 3 has been removed, the distal-end portion of the nozzle main body is inserted until reaching themain channel 65 of theinsertion sheath portion 17. A packing for sealing 87 in the form of the O ring is attached to the outer periphery of a distal-end portion 82 a of theouter pipe 82 of the nozzlemain body 81. The packing 87 constitutes first seal means for sealing a gap in themain channel 65, when the distal-end portion 82 a of the nozzlemain body 81 is inserted in themain channel 65 of thehandle unit 2. This packing 87 constitutes an engaging portion for attaching the distal-end portion 82 a inserted in an inlet of themain channel 65 to themedical instrument 1. - A hand-
side portion 82 b of theouter pipe 82 positioned in theoperation portion 19 of thehandle unit 2 is formed to be thicker than the distal-end portion 82 a. The outer periphery of the hand-side portion 82 b having a large diameter constitutes second seal means for sealing a gap, when the nozzlemain body 81 is inserted into thehandle unit 2 to closely contact thesecond packing 49 in thehandle unit 2 with pressure. The outer peripheral portion of the hand-side portion 82 b having the large diameter constitutes an engagingportion 82 d which engages with the second packing 49 of themedical instrument 1. As shown in FIG. 7, a portion in a region between the distal-end portion 82 a and hand-side portion 82 b of theouter pipe 82 of the nozzlemain body 81 is formed as anintermediate portion 82 c which is thicker than the distal-end portion 82 a and is thinner than the hand-side portion 82 b. - The
inner pipe 83 is linearly disposed extending over the hand-side portion 82 b from the distal-end portion 82 a in theouter pipe 82 of the nozzlemain body 81. Moreover, theinner pipe 83 forms a firstwater supply channel 85 leading to themain channel 65. The distal-end of theinner pipe 83 forms a first watersupply nozzle port 135 leading to themain channel 65. An outside space of theinner pipe 83 in the hand-side portion 82 b having the large diameter forms a secondwater supply channel 86 leading to the sub-channel 66. One or more water discharge ports (holes) 90 are formed in a portion positioned behind the packing 87 of the first seal means and positioned before the portion of thesecond packing 49 constituting the second seal means, that is, in a portion of theintermediate portion 82 c herein. Thewater discharge port 90 forms a second water supply nozzle port for supplying water to the sub-channel 66 from the secondwater supply channel 86 of the nozzlemain body 81 via a gap between the outside of theouter pipe 82 and the inside of theslider receiver member 45. The firstwater supply channel 85 leading to themain channel 65 and the secondwater supply channel 86 leading to the sub-channel 66 are formed independently of each other. The water supply ports are independently and individually connected to themain channel 65 andsub-channel 66. - As shown in FIG. 7A, a
supply nozzle member 91 is attached to the proximate-end of the nozzlemain body 81. The supplynozzle inlet member 91 includes afirst supply inlet 92 leading to the firstwater supply channel 85, and asecond supply inlet 93 leading to the secondwater supply channel 86. Thefirst supply inlet 92 is coaxially disposed toward a rear part with respect to the axial direction of the nozzlemain body 81. Thesecond supply inlet 93 is disposed in an intermediate portion of thesupply nozzle member 91 so as to protrude sideward with respect to the axial direction of the nozzlemain body 81. Furthermore, a front-end frame portion 91 a of thesupply nozzle member 91 is extended in a thick cylindrical shape, and aconnection portion 94 including a structure similar to that of the connection portion 9 of theprobe unit 3 is constituted in the front-end frame portion. For example, aC ring 95 is attached to theconnection portion 94 to form a shape swelling outwards. Theconnection portion 94 is fitted into and engaged with the distal-end engagement portion 55 of theframe member 53 of thehandle unit 2 to attachably/detachably fix the nozzlemain body 81 to thehandle unit 2. - Next, a function in a case in which the
cleaning device 80 of the present embodiment is used to clean theultrasonic treatment instrument 1 will be described. - To use the
ultrasonic treatment instrument 1, thehandle unit 2,probe unit 3, andvibrator unit 4 are assembled in a state shown in FIG. 1. Moreover, when theultrasonic treatment instrument 1 is used, theultrasonic treatment instrument 1 becomes dirty. Therefore, the instrument needs to be cleaned every use. - Here, apparent dirt on the surface of the
ultrasonic treatment instrument 1 can quickly be removed to a certain degree even by shower cleaning with a treatment instrument cleaner or by exposure to running water, but the inside of themain channel 65 passed through thevibration transmission member 10 of theprobe unit 3 or the inside of the sub-channel 66 passed through theoperation rod 60 for operating the distal-end function portion 21 also becomes dirty. The dirt inside the channel cannot quickly or easily be removed even by the shower cleaning or the exposure to the running water. - To solve the problem, the
cleaning device 80 of the present embodiment is used to remove the dirt inside themain channel 65 or the sub-channel 66. That is, as shown in FIG. 7A, the nozzlemain body 81 is inserted into theoperation portion 19 of thehandle unit 2 from which theprobe unit 3 has been removed, and theconnection portion 94 of the nozzlemain body 81 is fitted into/engaged with the distal-end engagement portion 55 in theframe member 53 of thehandle unit 2 to fix the nozzlemain body 81 to thehandle unit 2. At this time, the water supply port of the firstwater supply channel 85 leads only to themain channel 65, and the water supply port of the secondwater supply channel 86 leads only to the sub-channel 66. - Subsequently, supply tube line means such as a water supply tube connected to a water supply is individually connected to the
first supply inlet 92 andsecond supply 93 of thesupply nozzle member 91. As the water supply, it is possible to use a water supply tank or pump in which the treatment instruments such as the ultrasonic treatment instrument are disposed and which is incorporated in the cleaner for the treatment instrument, to carry out the shower or running water cleaning. Two water supply tubes (not shower connected to the water supply are separately connected to thefirst supply inlet 92 andsecond supply inlet 93. - Pressurized cleaning water from the water supply through the respective water supply tubes separately connected to the
first supply inlet 92 andsecond supply inlet 93 is supplied to themain channel 65 via the firstwater supply channel 85, and also supplied to the sub-channel 66 via the secondwater supply channel 86, and high-pressure cleaning water pressurized from the water supply is forcibly supplied to themain channel 65 andsub-channel 66. Therefore, the dirt inside themain channel 65 andsub-channel 66 is passed together with the cleaning water on the distal-end side of each channel, so that the dirt inside the channels can strongly be removed. The pressurized cleaning water from the water supply is independently and separately supplied to themain channel 65 andsub-channel 66. This can prevent the cleaning water from running only in one direction by a difference in channel resistance between the channels. Especially, the supply of the cleaning water into thethin sub-channel 66 can be prevented from weakening, and both thechannels - In the cleaning treatment, the
ultrasonic treatment instrument 1 to which thecleaning device 80 has been attached is installed in a cleaning tank or tray in the cleaner, and the apparent surface part of theultrasonic treatment instrument 1 is preferably simultaneously cleaned by the shower cleaning or by the exposure into the running water. - The cleaning device according to a second embodiment of the present invention will be described with reference to FIGS. 8A and 8B. In the present embodiment, the same components as those of the first embodiment are denoted with the same reference numerals, and detailed description thereof is omitted.
- As shown in FIGS. 8A and 8B, in a
cleaning device 100 according to the present embodiment, aframe member 101 and a cylindrical watersupply port member 102 which are separate members are added to thesupply nozzle member 91. As shown in FIG. 8A, in the same manner as in the first embodiment, theframe member 101 includes theconnection portion 94 which is fitted into the distal-end engagement portion 55 of theframe member 53 of thehandle unit 2 and engaged with the distal-end engagement portion 55. The cylindrical watersupply port member 102 is coaxially disposed in theframe member 101, and the distal-end portion extends ahead of theframe member 101 to surround the outer periphery of theouter pipe 82. A space between theouter pipe 82 and the watersupply port member 102 forms a thirdwater supply channel 103. Athird supply inlet 96 is disposed in rear-end portions of theframe member 101 and watersupply port member 102. Thethird supply inlet 96 is connected to the thirdwater supply channel 103. - As shown in FIG. 8A, the water
supply port member 102 is substantially closely fitted into the inner surface of theframe member 53 of thehandle unit 2, and is positioned in this state. A packing 104 constituting third seal means is disposed in the outer periphery of the watersupply port member 102. When thesupply nozzle member 91 is attached to thehandle unit 2, the packing 104 seals between theframe member 53 and watersupply port member 102 of thehandle unit 2. One or more water supply ports (holes) 105 connected to the thirdwater supply channel 103 are positioned before a position where the packing 104 is disposed, and disposed in an outer peripheral wall of the watersupply port member 102. - When the
cleaning device 100 according to the present embodiment is used, in the same manner as in the first embodiment, the nozzlemain body 81 is inserted in theoperation portion 19 of thehandle unit 2 from which theprobe unit 3 has been removed. Theconnection portion 94 of the nozzlemain body 81 is fitted into/engaged with the distal-end engagement portion 55 in theframe member 53 of thehandle unit 2 to fix the nozzlemain body 81 to thehandle unit 2. At this time, the water supply port of the firstwater supply channel 85 is connected to themain channel 65, and the water supply port of the secondwater supply channel 86 is connected to the sub-channel 66. Simultaneously, one or morewater supply ports 105 connected to the thirdwater supply channel 103 are directly connected to an inner space of theoperation portion 19 of thehandle unit 2. - Supply pipe line means such as a water supply tube connected to the water supply is individually connected to the
first supply inlet 92,second supply inlet 93, andthird supply inlet 96. In the same manner as in the first embodiment, as the water supply, it is possible to use a water supply pump incorporated in a treatment instrument cleaning machine in which the treatment instruments such as the ultrasonic treatment instrument are disposed to carry out the shower cleaning or the running water cleaning. - The pressurized cleaning water from the water supply is supplied to the
main channel 65 via the firstwater supply channel 65 through each water supply tube separately connected to thefirst supply inlet 92 andsecond supply inlet 93, and is supplied to the sub-channel 66, via the secondwater supply channel 86. The pressurized cleaning water from the water supply preferably supplied to themain channel 65 andsub-channel 66. Therefore, the dirt in themain channel 65 andsub-channel 66 is pressed together with the cleaning water in the side of the distal-end of each channel, and the dirt in channel can strongly be removed. - Additionally, the pressurized cleaning water from one or the water supply is spouted into the inner space of the
operation portion 19 of thehandle unit 2 from one or morewater supply ports 105 via the third water supply channel 106 through the water supply tube connected to thethird supply inlet 96, and the inside of theoperation portion 19 of thehandle 2 is cleaned with the cleaning water. The cleaning water which has overflowed as the inner space of theoperation portion 19 flows out to the outside of theoperation portion 19 through the through-holes 56 of theframe member 53. Therefore, a peripheral region of theframe member 53 can be cleaned by the cleaning water flowing outwards from the inside. The cleaning water which has overflowed as the inner space of theoperation portion 19 can be discharged to the outside through window holes (not shown) formed in a side wall of theoperation portion 19 of thehandle unit 2, because the engagingpin 51 of themovable handle 25 can movably be passed, and the inside of theoperation portion 19 can be cleaned with the cleaning water flowing outwards from the inside. - A third embodiment of the present invention will be described with reference to FIGS. 9A and 9B. In the present embodiment, the same components as those of the first embodiment are denoted with the same reference numerals, and the detailed description is omitted.

probe unit 3 to supply a high-frequency current to the treatment portion. - That is, for the ultrasonic treatment instrument of the present embodiment, as shown in FIG. 9, a high-
frequency connection pin 111 is disposed in the electrically insulating operation portionmain body 22 in thehandle unit 2. The high-frequency connection pin 111 is connected to a power supply cord (not shown), and receives supply of a high-frequency current from an external high-frequency power supply. A cylindrical contact-point member 112 is disposed in theoperation portion 19 as thehandle unit 2. A rear end of the contact-point member 112 fitted in theframe member 53 finished as a cylindrical shape. The contact-point member 112 is connected to the high-frequency connection portion viaconductive frame member 53. - In front-end portion of the contact-
point member 112, ordinarily inserted in the cylindricalslider receiver member 45. Anannular packing 113 is attached to the distal-end of the contact-point member 112, and also serves as an electric contact in contact with the outer periphery of thevibration transmission member 10 can beprobe unit 3 attached to thehandle unit 2. The is connected to the high-frequency connection pin 111 via the contact-point member 112 andframe member 53 and the packing 113 constitutes seal means in contact with the outer periphery of the nozzle main body of acleaning device 130, when thecleaning device 130 of the present embodiment is mounted. - The
cleaning device 130 of the present embodiment includes a watersupply nozzle member 132 formed by thepipe material 131, and thepipe material 131 is constituted in the same manner as in theouter pipe 82 of the first embodiment. For thepipe material 131, an inner hole forms onewater supply channel 133. The distal-end of thepipe material 131 forms awater supply port 135 leading to themain channel 65 of thehandle unit 2, when thecleaning device 130 is attached to thehandle unit 2. - A packing for sealing136 in the form of the O ring is attached to the outer periphery of the distal-end portion in the
pipe material 131 of the watersupply nozzle member 132. The packing 136 constitutes the seal means for blocking connection to themain channel 65 of thehandle unit 2. - By the packing136, one or more discharge paths (holes) 137 for connecting the
water supply channel 133 to the outside can also be formed in the distal-end portion of thepipe material 131 of the watersupply nozzle member 132 in a region extending to the seal means which contacts the packing 113. Thedischarge paths 137 lead to the sub-channel 66 of thehandle unit 2, when thecleaning device 130 is attached to thehandle unit 2. Awater supply nozzle 141 connected to thewater supply channel 133 is disposed in the rear end of thepipe material 131 of the watersupply nozzle member 132. - Then, when the
cleaning device 130 of the present embodiment is used, the device is attached to the ultrasonic treatment instrument in the same manner as in the first embodiment. The cleaning water from a supply tube (not shown) connected to thewater supply inlet 141 is supplied to themain channel 65 of thehandle unit 2 from thewater supply port 135 through only onewater supply channel 133 of the watersupply nozzle member 132. - On the other hand, when one or
more discharge paths 137 are disposed, a part of the cleaning water supplied to thewater supply channel 133 is discharged to the outside of the watersupply nozzle member 132 midway from thewater supply channel 133 through one ormore discharge paths 137, and is also supplied to the sub-channel 66. Themain channel 65 andsub-channel 66 can simultaneously be cleaned. - Even when one or
more discharge paths 137 are not disposed, themain channel 65 is connected to the sub-channel 66 via the through-hole 67 formed in theinner pipe 62 in theovercoat tube 20 of theinsertion sheath portion 17 of the ultrasonic treatment instrument (see FIG. 4A). Then, themain channel 65 is connected to the sub-channel 66, the cleaning water mutually flows, and this promotes the cleaning of themain channel 65 andsub-channel 66. - The same device as the
cleaning device 80 according to the first embodiment can be used as the device for cleaning an ultrasonic treatment instrument 110. - As described above, according to the cleaning device of the present invention, especially the channel of the medical instrument, which is not easily cleaned, can efficiently be cleaned.
