' To all whom t may concern:
Be it known that I, WILLIAM LINVEL HENRY, a citizen of the United States 'of America, residing at Louisville, in the county of Jeli'erson and State of Kentucky, have invented certain new and useful Improvements in Manifold Receipt, Way-Bill,-
persons for whose use it is intended, con-` venient in manipulation and use and particularly well adapted forV filing, record and accounting purposes. Another object of this invention is to provide a combined form of the character hereinbefore referred to which will give auditors, route agents and likeofHcials an absolute and positive check upon the local agents within the territory under their charge and will also afford' a local* agent an absolute and positive check upon the drivers, way-bill clerks and receiving clerks connected with his oiice, and which will assist the agent (or his cashier) in making settlement with the driver and will afford a check on the statement used in balancing his account. A third object of this invention is to reduce the amount of clerical work connected with the receiving, transportation and delivery of goods by common carriers, such as express companies. Referring particularly to Fig. 2, the topmost sheet or blank form A consists of the stub A", the receipt proper A and the stlcker A', the receipt proper'being separated from the stub and sticker by lines of perforations, as shown. This topmost form A is to be filled out by the driver or receiving clerk, who takes the package from the consignor. The driver or receiving clerk, in filling out the receipt proper A,
inserts the date in the space at a, the value l (if given) at b, the name of the consignor at c, the name of the consignee at d, the address of the consignee at e, the number of packages and their alleged contents at f"= and signs the receipt for the company at g. If the goods aresentcollect ondelivery, the amount to be collected is filled in at It. The driver (or receiving clerk) at the same time fills out also the stub Ax inserting the date at the name of the consignor at c* and the name and address of the consignee at d", and he also at this same time of receiving the goods fills out the sticker A', inserting the date at'a, the name of the consignor at c', the value-(if given) at 6^ and, if the goods are sent collect on delivery, he fills in the amount to be collected at h. Hethen detaches lthe sticker A from the'receipt proper A by tearing` it olf along the line of perforations i and pastes this sticker A to the package.l He then detaches the receipt proper A from the stub Ax by tearing v it off along the line ofperforations i and gives this receipt to the consignor. The
stub AX is left in the drivers book as a record. On arriving at the express companys office, the driver turns in the package, to-
cal in form with the upper half of the receipt proper A. Between the way-bill form B and the receipt proper A` there 1s interposed a piece of carbon paper (or manifold paper) which is so laid-or arranged that, when the driver fills out the receipt proper he also fills out on the way-bill the date, the value of the goods, the name of the consignor, the name and address of the consignee, the 'number of pieces and their alleged contents and, if the goods are sent collect on delivery, the amount to be collected.
The way-bill form contains also spaces as follows: The space j for the insertion of the weight; thespace la for the amount paid; the spaces ma and n for the correction ofthe amount charged, the amount to be added being inserted in the space m and the amount to be deducted being inserted in the space n. Formed in one with the way-bill B is the delivery sheet C which is folded along the line of perforations z' and which is the lowermost of the three sheets A, B, C. The delivery sheet C may be said to consist of three parts, namely, the delivery sheet proper C the sending-office stub CX and the consignees receipt C. Interposed between the way-bill sheet B and the delivery sheet C is a piece of carbon (or manifold) paper which overlies the delivery-sheet proper C and the sending-office stub C". It is is to be observed that no part of the way-bill sheet overlies the sending-office stub C", and, by looking at Fig. 1, it will be seen that the drivers stub Ax of the receipt sheet A must overliey the sending-office stub CX of the delivery sheet C. Again, the sticker A overlies the first messeriger space t at the outer end of the way bill; and this first messenger space t lie: directly above the consignees receipt C at the outer end of the delivery-sheet C; but no part of either sheet of carbon-paper extends below7 the sticker A or below the first messenger space t. The carbon-paper between the receipt sheet A and the Way-bill sheet B does, however, extend below the drivers stub AX and over the sending-office stub Cx, with the result that when the driver fills out the date, the name of the consigner and the name and address of the consignee on the drivers stub AX, in his book, he fills out the corresponding spaces ax, c and d* on the office stub CX. Furthermore, the delivery-sheet proper C is a duplicate of the way-bill sheet B, except for the first messenger space 15 at the outer end of the latter; and, when the driver makes out the receipt proper A, he fills in not only the upper half of the body of the way-bill sheet B but also the upper half of the delivery sheet proper C; and, when the way-bill clerk fills in the lower portion of the way-bill sheet and thereby completes the same, he simultaneously fills in the lower portionof the delivery-sheet proper C and completes it. -Again, on the way-bill form B and the delivery sheet C, the words, Paid beyond, are omitted and the space containing the word, Paid, and lying between the spaces containing the words, Weight and Add, respectively, is divided into two spaces 011e of which containsthe Word, Advance, and the other of which contains the word, Collect, as is illustrated in Fig. 5, at lo* and 0x, respectively.

0r, to recapitulate more briefly yet with reference to the structure, that the invention, as illustrated, comprises two sheets, each sheet `being composed of divisions with printed indicia on each sheet which registers with corresponding indicia on the other sheet; that the -lower sheet has two leaves, the upper one of which is detachable from and shorter than the lower one, so that it does not cover the inner end or inner stu end of the lower sheet. f
I claim:
l. A combined manifold receipt and record form, comprising a top sheet with stubs at each end, the right end stub being detachable from the stop sheet and the top sheet detachable from the left hand stub, a folded leaf lying. beneath the top sheet, one of the folds being detachable from the other folds, the upper part of the folded leaf being shorter than the lower part of the folded leaf, and indicia printed on the several sheets vand stubs and registering when the sheets and stubs are super-posed.
2. A combined manifold receipt and record form, comprising two sheets, each sheet composed of similar divisions with printed indicia on each division registering with printed indicia on the opposing divisions,l the lower sheet having two leaves, the upper one of which is detachable from and shorter than the lower one so that it does not cover the left hand end division of the lower sheet.
3. A combined manifold receipt and record form, comprising a top sheet, with a detachable stub at the right hand end and a stub at the left hand end. fromi which the top sheet can be detached, the stub at the right end being gummed to form a sticker to be applied to a package, and a folded leaf provided with a left-hand stub and detachable fromthe left hand stub and lying beneath the top sheet, one of the leaves of the folded sheet being shorter than the other leaf so that it does not cover the left-hand stub.
