Specification of Letters Patent.

To all whom it may concern.
Be it known that I, LEROY METZGAR, a citizen of the United States, residing at Fremont, in the county of Sandusky and State of Ohio, have invented certain new and useful Improvements in Physicians Account-Books; and I do declare the following to be a full, clear, and exact description of the invention, such as will enable others skilled in the art to which it appertains to make and use the same, reference being had to the accompanying drawings, and to the letters and figures of reference marked thereon, which form a part of this specifi' cation.
My invention relates to a pocket account book for physicians, and is designed to furnish means for keeping the accounts of a physician with his patients, the accounts including not only the necessary memoranda of names, dates, items and accounts but also blanks for furnishing prescriptions or directions and for keeping a fac-simile duplicate of such prescriptions or directions in connection with the record of the visits of each dav.
My invention is designed also to furnish,
in the same device, blanks for the visiting lists of the physicians as well as blanks for keeping full accounts of accidents. To this end, my invention consists in the devices, construction and arrangement of parts hereinafter described, and shown and illustrated in the accompanying drawings, in which- Figure l is a perspective view of my account b0ok--open, and Fig. 2, a rear view of one of the pads hereinafter referred tto, detached.
Like numerals of reference indicate like parts throughout the drawings.
In the drawings, 1 is one of the covers, backs or lids of the book, 2 the other cover, the two covers being flexibly connected by the part 3. If desired, this part may have a pencil holder, as illustrated in the drawings.
4 is a pad of account-slips nearly as long as the book-covers, 5 is a pad of prescription-blanks considerably shorter than the pad 4:; pad 4 is blocked and bound at one end to one end of a stout back 6 of straw-board or other suitable material. The pad 5 may be secured to the other end of the back 6, but is preferably bound separately, as illustrated in Fig. 1. The free ends of the sheets of pad 5 are adapted to overlap the free ends of the sheets of the pad 4. Bound with and covering the pad 5 is a sheet of carbon paper or manifold paper 7. The sheets of both pads are perforated or weakened on transverse lines near their confined ends. The upper part of the sheet-s of'the pad 4 is printed and provided with such blanks as may be necessary to properly record the account of the patient and for such memoranda as to diagnosis and treatment as may be desired. The lower part of the sheets of pad 4 is intended to receive a record of the prescription or directions given at the time of the visit referred to in the accompanying items of account, and may be employed for retaining copies of receipts given or directions left for the patient. T/Vhen the prescription or directions or other matter are to be written, the lower end of the upper sheet of pad 4 is drawn from under the pad 5 and is placed over the back of the sheet of carbon paper 7 and upon the pad 5. Now, when the prescription or other matter is written it is copied at the same time on the outer leaf of the pad 5.
At the top of the piece 6 is secured a flap 8 which fits snugly into a pocket 9 secured to or made part of the book-back 2, thus providing means for holding the pad in place and for readily removing the pad and for supplying the book with new pads or leaves whenever required. The pad 5 is in like manner provided with a flap 5 fitting into a pocket 5 secured to or made part of book-back 2.
The book-back or cover 1 is provided on its inner side with one or more pockets 10 adapted to receive and retain the leaves of the pad 4 when they are detached from their pad until the physician finds time to transfer the slips to a suitably indexed receptacle.
Upon the inner side of the cover 1, near its top, is formed a pocket 11 arranged to receive and hold a flap 12like the flaps 8 and 5 secured to the back of the upper end of pad 13. This pad consists of a series of sheets or leaves blocked and secured together at their upper ends, and having perforated or weakened lines near their confined ends to permit them to be readily detached. The leaves of the pad 13 have upon one side blanks for visiting lists and upon the other side blanks which may be used,
as occasion requires,for the record of accidents. I It will be seen that by the device here illustrated and described, the account of each patient, together with diagnosis, treatment and prescription or directions, are kept upon detachable leaves which are temporarily deposited in the pockets of the book, which leaves may be later transferred to a permanent filing case; that a fac-simile of the original of the prescription or directions or receipt left by the physician with the patient is preserved without necessitating the trouble of writing a separate copy, thus avoiding mistakes and disputes on this score; that the labor of a physician in keeping a full and accurate account with his patients and a history of his cases is reduced to a minimum, and that the account and history of the case is rendered readily accessible. It will also be seen that the visiting list-either in blank or when filled 0ut-is readily accessible and not easily lost and that an accident-blank is always obtainable in the physicians book. WVhen it is necessary to write upon either pad, the two backs or covers are folded together, thus furnishing a comparatively firm writing surface.
Having described my invention, what I claim and desire to secure by Letters Patent, is
1. Aphysicians account book comprising two flexibly connected book-covers, a pad of leaves having thereon suitable blanks for a physicians memoranda and for prescriptions and directions, said pad being of substantially the length of the book, a second pad shorter than the pad first mentioned and having blanks thereon corresponding to the prescription blanks on the pad first mentioned, means for detachably securing said two pads to one of said book-covers with one pad overlapping the other, a pocket in the other book-cover adapted for the reception of the leaves of the longer pad when detached, and means for manifolding the inscriptions of the prescription blanks of the longer pad upon the corresponding prescription blanks of the shorter pad.
2. A physicians account book comprising two flexibly connected book-covers, a pad of leaves having thereon suitable blanks for a physicians memoranda and for prescriptions and directions, a second pad shorter than the pad first mentioned and having blanks thereon corresponding to the prescription blanks on the pad first mentioned, means for detachably securing said two pads to one of said book-covers with one pad overlapping the other, and means for manifolding the inscriptions of the prescription blanks of one pad upon the corresponding prescription blanks of the other pad.
