Description
The useful model belongs to medicine and can be used in the surgical treatment of disorders of the arterial blood supply of the lower extremities.
As a rule, reconstructive operations on affected main arteries with incomplete (stenosis) or complete (occlusion) anatomical overlap (with obliterating atherosclerosis or thromboangiitis) are performed on the basis of the use of shunts, the proximal end of which is connected (anastomosed) to the donor, and the distal - with the receiving artery bypassing the affected area of the trunk artery. The greatest difficulties and complications in such a surgical intervention arise when creating a distal anastomosis due to the small size of the vessels.
- 1996. A known method of creating a distal anastomosis of a shunt and an artery is based on blocking blood circulation in the artery, performing a longitudinal arteriotomy on the latter, cutting the distal end of the shunt in the form of a tongue-shaped piece and fixing the latter to the arteriotomy by applying a seam along the perimeter of the IV.A piece. Dolinin, L.V. Lebedev, I.G. Peregudov Technique of surgical operations on blood vessels. - 1996. - P. 31-38, St. Petersburg.
Blockage of blood circulation in the artery is carried out with the help of a clamp placed on the artery distal to the arteriotomy. However, the use of a clamp does not exclude the danger of injury to the artery, especially in the case of atherosclerotic damage, which can be the cause of intimal tearing, delamination of the artery wall, and subsequent thrombosis. In addition, applying the clamp to the receiving artery requires a more extended separation of it from soft tissues (up to 20-22 mm), which increases the trauma of the surgical intervention. Due to the small diameter of the distal artery, when a vascular suture is placed on it, it is not excluded that the posterior wall of this artery is captured by the needle, which leads to deformation of the anastomosis or even complete overlap of the artery in this area, which can also be the cause of postoperative thrombosis in it.
Since the formation of the distal anastomosis of the shunt and the artery lasts for a certain time (15-30 min.), due to the lack of blood circulation, spasm of the latter occurs, which is also the cause of early thrombosis in it. This completely nullifies the results of surgical intervention and reduces the efficiency of anastomosis formation.
The basis of a useful model is the task of creating a method of forming the distal
From the anastomosis of the shunt and the artery, which excludes the use of clamps and thus reduces the length of the separation of the artery from soft tissues, preserves its integrity, prevents spasm of the artery during the formation of the specified anastomosis, as well as deformation of the latter, and, therefore, prevents thrombus formation in arteries, reduces the trauma of surgical intervention and increases the effectiveness of treatment.
The task is solved by the fact that in the method of forming the distal anastomosis of the shunt and the artery, based on blocking blood circulation in the artery, performing on the last longitudinal arteriotomy, cutting the distal end of the shunt in the form of a tongue-shaped piece and fixing the latter to the arteriotomy by applying a seam along the perimeter of the piece, according to a useful model, blood circulation in the artery is blocked with the help of a two-channel balloon catheter, which is inserted through the arteriotomy into the artery, and through the through channel of the catheter, a physiological solution with vasoactive and anticoagulant medicinal substances is injected into the artery, while applying a suture along the perimeter of the piece is performed in two stages, at the first of which, the rear edge of the anastomosis and partially the front edge are formed in length equal to 3/4 of the circle of the anastomosis, and at the second stage, after removing the catheter from the artery and clamping the shunt in the anastomosis zone, the last sealing stitches are applied.
Blocking blood circulation in the artery being reconstructed, with the help of a balloon two-channel catheter inserted through the arteriotomy inside, does not injure the intima, helps preserve the integrity of the artery wall.
In addition, such an overlap of the blood flow does not require prolonged separation of the artery, which reduces, thus, the traumatic nature of the surgical intervention.
The introduction of saline solution with vasoactive and anticoagulant medicinal substances into the artery through the through channel of the catheter prevents spasm and thrombus formation in the arteries of the distal channel during the formation of the distal anastomosis.
Performing a seam around the perimeter of the piece in two stages, the first of which forms the back edge of the anastomosis and partially the front, equal to approximately 3/4 of the circle of the anastomosis, and applying in the second stage after removing the catheter from the artery and clamping the shunt in the area of the anastomosis of the last sealing sutures excludes capture of the back wall of the artery by the needle due to the location of the catheter in the artery. This factor prevents the deformation of the anastomosis and the overlap of the artery in this area.
Analogous technical solutions with similar features were not found during the patent information search. This indicates that the proposed technical solution is novel and clinically applicable.
The proposed method of creating the distal anastomosis of the shunt and the artery is explained by the drawings, where in fig. 1 shows the stage of performing a longitudinal arteriotomy and blocking the blood flow in the artery using a balloon catheter; in fig. 2 - the stage of applying a continuous wrapping seam at the distance between the back walls of the artery and the shunt; in fig. C - the stage of partial application of the seam on the front wall of the anastomosis of the artery and the shunt; in fig. 4 - the final stage of the formation of the anastomosis of the shunt and the artery.
The method of creating the distal anastomosis of the shunt and the proposed artery is carried out as follows.
3.The recipient artery 1 is isolated from the tissues surrounding it, in the area where the anastomosis is planned. The distal end of the shunt 2 is cut out by obliquely cutting it to form a tongue-shaped piece 3.
Longitudinal arteriotomy 4 and insertion through it into the artery 1 of a balloon two-channel catheter 5. Through the blind channel 6 of the catheter, a liquid (saline solution) is supplied under pressure to its balloon 7, under the action of which the balloon is inflated and pressed against the inner surface of the artery wall, blocking the blood flow in it . Such an overlap of blood flow in artery 1 does not injure the walls of this artery and does not require its prolonged release from soft tissues.
After blocking the blood flow in the artery through channel 8 of the catheter, a physiological solution with vasoactive and anticoagulant medicinal substances is injected into this artery, which prevents spasms and thrombus formation in the arteries of the distal channel during the surgical intervention.
At the same time, the anastomosis of the shunt 2 and the artery 1 is performed. First, the step of applying a continuous wrapping seam 9 is performed at the distance between the back walls of the artery and the shunt and partially on the front wall along a length approximately equal to 3/d of the anastomosis circle. Upon completion of suture 9, its free ends are stretched to the sides, which leads to the connection of the edges of the artery and the shunt, which achieves the sealing of this area of the anastomosis.
The presence in the lumen of the artery 17 of the trunk of the catheter 5 prevents capture by the needle at
By placing a seam on the back wall of the artery, it eliminates, thus, its deformation, which eliminates the cause of thrombus formation in this area of the artery. Catheter 5 is removed from the artery, the last sealing sutures 10 are placed on the front wall of the anastomosis, and shunt 2 is clamped with a clamp 11 to prevent retrograde blood flow through the specified shunt. The formation of the proximal anastomosis of the shunt with the donor artery is performed according to a well-known method, after which the clamp 11 is removed and blood circulation is restored.
A clinical example.
Patient A., 62 years old, was hospitalized with complaints of intense ischemic pain at rest, necrosis in the area of the I-II toes of the right foot. He has been suffering from obliterating atherosclerosis of the lower arteries for the past 5 years. The deterioration of the condition of the right lower limb occurred when necrosis appeared in the area of the I-II toes of the right foot.
Transradial aorto-arteriography was performed. Revealed: stenosis of the common iliac, occlusion of the superficial femoral, anterior tibial arteries on the right.
Stenting of the right common iliac artery was performed, and two days later femoral-tibioperoneal prosthesis was performed on the right with a combined shunt. Isolation of the tiibioperoneal trunk was carried out at a length of up to 12 mm, which was sufficient for longitudinal arteriotomy and introduction of a balloon catheter. The artery was not clamped. The shunt was formed by anastomosing a linear synthetic prosthesis made of polytetrafluoroethylene with a diameter of b mm and a segment of the autovein. When applying the distal anastomosis of the autovenous segment of the shunt and the tibioperoneal trunk, the blood flow was blocked with the help of a balloon two-channel catheter that was inserted into the tibioperoneal trunk, which prevented capture of the posterior wall of the artery by the needle.
The formation of the anastomosis was carried out in two stages: in the first stage, the rear edge of the anastomosis of the artery and the shunt was connected and partially the front one in length, equal to approximately 3/4 of the circle of the anastomosis. -3) sealing seams. Visual control did not reveal deformation of the anastomosis. During the operation, a warm physiological solution with heparin and papaverine was injected twice through the catheter. The postoperative period was uneventful, the phenomena of critical ischemia of the right lower limb stopped. Ischemic pains in the foot disappeared, necrosis on the 1st and 1st toes cleared and healed. Postoperative ultrasound 60 dopplerography, which was carried out three days after the operation, did not reveal signs of thrombosis of the operated artery and diagnosed the main blood flow through the posterior tibial and fibular arteries and the absence of blood flow disorders in the anastomosis zone.
The method was developed and implemented in clinical practice in 12 patients.
A comparative analysis of the results of the implementation of the proposed method of forming the distal anastomosis of the shunt and the artery during reconstructive operations on the affected main vessels shows that the length of the separation of the artery from the soft tissues decreases by 1.6-1.7 times and is no more than 12 mm and decreases, thus thus, the traumatic nature of the operation. Not a single case of deformation of the anastomosis due to the capture of the back wall of the artery by the needle, as well as injury to the latter when blood flow was blocked in it, was recorded. There was no thrombosis in the reconstructed arteries in the postoperative period.
