Корисна модель належить до медицини, зокрема до педіатрії, дитячих інфекційних хвороб, і може бути використана для проведення диференційної діагностики вірусних та вірусно- бактерійних гострих кишкових інфекцій, що дозволить призначити адекватне лікування чи провести його корекцію.The useful model belongs to medicine, in particular to pediatrics, children's infectious diseases, and can be used to carry out differential diagnosis of viral and viral-bacterial acute intestinal infections, which will allow to prescribe adequate treatment or carry out its correction.
Серед причин захворюваності і летальності, пов'язаної з інфекційною патологією у дітей, гострі кишкові інфекції (ГКІ) стійко посідають друге місце після гострих респіраторних інфекцій.Among the causes of morbidity and mortality associated with infectious pathology in children, acute intestinal infections (AKI) consistently rank second after acute respiratory infections.
ГКІ у дітей на сучасному етапі характеризуються поліетіологічністю, переважанням ГКІ вірусної етіології (до 50-80 95), відсутністю сезонності захворюваності, мінливістю клінічної картини, та, водночас, недостатньою інформативністю рутинних лабораторних обстежень, особливо на етапі проведення диференційної діагностики.GKI in children at the current stage is characterized by polyetiological factors, the predominance of GKI of viral etiology (up to 50-80 95), the absence of seasonality of the incidence, the variability of the clinical picture, and, at the same time, the insufficient informativeness of routine laboratory examinations, especially at the stage of differential diagnosis.
Основними діагностичними і диференційно-діагностичними критеріями гострих вірусних та вірусно-бактерійних кишкових інфекцій на сьогодні вважають результати вірусологічних обстежень (зокрема визначення антигенів вірусів), бактеріологічних обстежень, проте ці дослідження є досить тривалими; окрім того, виявлення патогенних вірусів чи бактерій кінцево не підтверджує участь цих збудників у патологічному процесі, тому постійно проводиться пошук нових діагностичних підходів (1Ї. Найближчим аналогом запропонованої корисної моделі є спосіб діагностики ротавірусної моно- та мікст-інфекції у дітей, який за результатами визначення рівнів інтерлейкіну-4, субпопуляції СОЗТ-лімфоцитів дозволяє провести диференційну діагностику гострих кишкових інфекцій у дітей, спричинених ротавірусами та ротавірусною мікст-інфекцією (2). Недоліком цього способу є визначення згаданих субстанцій у сироватці крові пацієнтів, що є травмуючою процедурою для дітей раннього віку.Today, the main diagnostic and differential diagnostic criteria for acute viral and viral-bacterial intestinal infections are the results of virological examinations (in particular, the determination of virus antigens), bacteriological examinations, but these studies are quite lengthy; in addition, the detection of pathogenic viruses or bacteria does not conclusively confirm the participation of these pathogens in the pathological process, therefore, new diagnostic approaches are constantly being sought (1Й. The closest analogue of the proposed useful model is the method of diagnosing rotavirus mono- and mixed infection in children, which, based on the results of determining levels of interleukin-4, a subpopulation of TSH lymphocytes, allows differential diagnosis of acute intestinal infections in children caused by rotaviruses and rotavirus mixed infection (2). The disadvantage of this method is the determination of the mentioned substances in the blood serum of patients, which is a traumatic procedure for young children .
В основу корисної моделі поставлена задача шляхом проведення додаткових обстежень удосконалити диференційну діагностику гострих кишкових інфекцій у дітей, які спричинені вірусами чи вірусно-бактерійними асоціаціями.The useful model is based on the task of improving the differential diagnosis of acute intestinal infections in children caused by viruses or viral-bacterial associations by conducting additional examinations.
Поставлена задача вирішується тим, що у способі диференційної діагностики вірусних та вірусно-бактерійних гострих кишкових інфекцій у дітей раннього віку, що включає проведення імунологічних досліджень, згідно з корисною моделлю, у пацієнтів протягом перших 24 годин від появи перших симптомів ГКІ та повторно, на 5 добу від початку хвороби, визначають концентрації секреторного імуноглобуліну А у копрофільтратах і за динамікою результатівThe task is solved by the fact that in the method of differential diagnosis of viral and viral-bacterial acute intestinal infections in young children, which includes conducting immunological studies, according to a useful model, in patients within the first 24 hours from the appearance of the first symptoms of GKI and again, at 5 days from the onset of the disease, determine the concentration of secretory immunoglobulin A in coprofiltrates and the dynamics of the results

Секреторний імуноглобулін А (5ІдДА) є одним з головних факторів гуморального імунітету слизових оболонок, який забезпечує пригнічення адгезії збудників, зниження колонізації слизових оболонок кишок патогенними і умовно-патогенними мікроорганізмами. Окрім цього 5ІДА нейтралізує віруси, зв'язує токсини, аглютинує бактерії, зв'язується з різними алергенами, в тому числі харчовими, не викликаючи при цьому ушкодження тканин організму. 5іІдА виробляється постійно: він утворюється з імуноглобуліну А (ІА), що продукується плазматичними клітинами, шляхом приєднання цього імуноглобуліну специфічних білків - т. 3. секреторних компонентів. Водночас, продукція 5ідА значно збільшується при контакті вірусних антигенів, бактерійних антигенів та токсинів з антиген-презентуючими клітинами лімфоїдної тканини шлунково-кишкового тракту. Оскільки інтенсивність реплікації вірусів в ендотеліоцитах при вірусних гастроентеритах значно випереджує інтенсивність розмноження бактерій у просвіті кишок при бактерійних ГКІ, можна спостерігати стрімке зростання рівнів 5 ТДА у копрофільтраті у відповідь на контакт вірусних антигенів зі слизовою кишок |З|.Secretory immunoglobulin A (5IdDA) is one of the main factors of the humoral immunity of the mucous membranes, which ensures the suppression of the adhesion of pathogens, the reduction of the colonization of the mucous membranes of the intestines by pathogenic and opportunistic microorganisms. In addition, 5IDA neutralizes viruses, binds toxins, agglutinates bacteria, binds to various allergens, including food allergens, without causing damage to body tissues. 5iIdA is constantly produced: it is formed from immunoglobulin A (IA), which is produced by plasma cells, by attaching specific proteins to this immunoglobulin - 3. secretory components. At the same time, the production of 5idA significantly increases when viral antigens, bacterial antigens and toxins come into contact with antigen-presenting cells of the lymphoid tissue of the gastrointestinal tract. Since the intensity of viral replication in endotheliocytes in viral gastroenteritis significantly outpaces the intensity of bacterial reproduction in the lumen of the intestines in bacterial GKI, a rapid increase in the levels of 5 TDA in coprofiltrate can be observed in response to the contact of viral antigens with the intestinal mucosa |Z|.
Спосіб диференційної діагностики вірусних та вірусно-бактерійних ГКІ у дітей раннього віку здійснюють таким чином. ГКІ (гіпертермія, діарея, блювання) та на 5 добу хвороби - проводять забір фекалій у стерильну пробірку. Надалі з 1 граму фекалій готують копрофільтрат: додають стерильний фосфатний буфер, центрифугують з подальшим фільтруванням надосадкової рідини через міліпорові фільтри. Методом ІФА з використанням комерційних наборів для визначення секреторних імуноглобулінів, наприклад, набору "ДА секреторньій-ИФА-Бест", в копрофільтратах визначають вміст секреторних імуноглобулінів А... При високому рівні 5ІдДА в копрофільтраті на 5 добу захворювання або значному підвищенні концентрації 5ідА, порівняно з 1 днем від початку госпіталізації, встановлюють вірусну етіологію ГКІ.GKI (hyperthermia, diarrhea, vomiting) and on the 5th day of the disease - feces are collected in a sterile test tube. Для підтвердження ефективності запропонованого способу було обстежено 39 пацієнтів віком від 1 до 4 років, в яких було діагностовано ГКІ вірусної (19 дітей) і вірусно-бактерійної (20 дітей) етіології. Контрольну групу було сформовано з 10 здорових дітей віком від 1,5 до З років.To confirm the effectiveness of the proposed method, 39 patients aged 1 to 4 years were examined, who were diagnosed with GKI of viral (19 children) and viral-bacterial (20 children) etiology. The control group was formed from 10 healthy children aged from 1.5 to 3 years.
При госпіталізації дітям проводили загальноклінічні обстеження, бактеріологічні та вірусологічні дослідження фекалій. Усім пацієнтам основної групи (при госпіталізації і на 5 добу) і контрольної групи (при проведенні профілактичного огляду) було проведено визначення секреторних імуноглобулінів у копрофільтраті методом ІФА з використання комерційних наборів "ІА секреторньій-ИФА-Бест".During hospitalization, the children underwent general clinical examinations, bacteriological and virological examinations of feces. All patients of the main group (during hospitalization and on the 5th day) and the control group (during a preventive examination) were determined to have secretory immunoglobulins in the coprofiltrate by the ELISA method using commercial kits "IA Secretory-IFA-Best".
Вміст секреторного імуноглобуліну А у копрофільтраті на початку захворювання при вірусних ГКІ (18,526,05) відповідав показникам контрольної групи, а при вірусно-бактерійних - рівень 5ідА (35,4-6,6) вдвічі перевищував показники у групі контролю. В динаміці захворювання було виявлено значне вірогідне (р«0,01) наростання концентрації імуноглобулінів у копрофільтраті, особливо стрімко рівень зідА (175,05213,6) підвищувався у дітей з вірусними ГКІ.The content of secretory immunoglobulin A in the coprofiltrate at the beginning of the disease in viral GKIs (18,526.05) corresponded to the indicators of the control group, and in viral-bacterial ones - the level of 5idA (35.4-6.6) twice exceeded the indicators in the control group. За отриманими даними, на 5 добу від початку захворювання при вірусних ГКІ спостерігалося 10- кратне, статистично достовірне наростання рівнів 5ТдА в копрофільтраті. При ГКІ, викликаних вірусно-бактерійними асоціаціями, підвищення рівнів 5ІдА (55,16:28,1) було недостовірним.According to the obtained data, on the 5th day from the onset of the disease, a 10-fold, statistically significant increase in the levels of 5TdA in coprofiltrate was observed in viral GKI.
