The useful model refers to the field of medicine, namely surgery and oncology, and can be used 2 in the surgical treatment of stomach cancer - gastroectomy.
The most difficult and responsible stage of gastroectomy is the formation of the esophageal-intestinal anastomosis, or in other words, the reconstruction of the gastrointestinal tract. But it should be noted that operations of this kind are often complicated by the need to cross the common bile duct, which may be due to the spread of the tumor process. In addition, taking into account the great technical complexity of performing a 70-gastroectomy, such crossing may be accidental, and thus there is a need not only to impose an esophageal-intestinal anastomosis, but also to perform a pancreatoduodenal resection.
These complications, which are due to the crossing of the common bile duct, make the operation technically more difficult, long and ultimately significantly increase the risk for the patient's life.
In addition, the elimination of these complications requires a very high qualification of the surgeon. 19 The specified stage of the operation must satisfy the conditions of maximum reliability, namely, the anastomosis must provide good functional properties, for example, arereflux, etc.
To date, various methods of reconstruction of the gastrointestinal tract after gastrectomy are known and used - these are vertical gastrointestinal anastomoses according to Kazansky, Orru-Hanta-Nakaime, intussusception anastomosis. In recent years, "reservoir anastomoses" and 20 "coupling arereflux sputum-small-intestinal anastomosis" according to Bondar's method (see

However, all of these methods do not provide sufficiently reliable actions aimed at eliminating defects that arise during gastrectomy.
зIn modern restorative surgery, the use of the 29 small intestine during gastrectomy is very relevant. with
Of all known methods of reconstruction of the gastrointestinal tract after removal of the stomach, the method of reconstruction of the gastrointestinal tract by using a loop of the small intestine taken 30-50 cm from the duodenal-jejunal junction (ligament of Treitz) is the closest. A T-shaped loop is formed from it, unfolded isoperistally. The esophagus is fixed to the abductor knee of the loop, an opening is created in the abductor knee, C 30 through which the esophagus is brought into the lumen of the abductor knee by the type of intussusception. Cross loop "I
A clutch is created around the esophageal-intestinal anastomosis of the T-loop.
The task of a useful model is to improve the method of reconstruction of the gastrointestinal tract by using such a combination of surgical actions and techniques that make the operation less traumatic, reliable and thereby reduce the risk to the patient's life. 3o The task is solved by the fact that the method of reconstruction of the gastrointestinal tract after removal of the stomach, accompanied by simultaneous damage to the common bile duct, according to a useful model, includes stretching the loop of the small intestine through the window in the mesentery of the transverse colon, while stretching the loop of the small intestine is antiperistaltically unfolded and a coupling is formed from it, then the distal end of the esophagus is immersed in the lumen of the circumferential part of the loop and the coupling formed in this way is wrapped with 50 esophageal-intestinal anastomosis, after which a pancreaticojejunal anastomosis is formed on the adduction part of the loop according to type "c" end, in side" on the internal "lost" drainage and "end-to-side" choledochojejunostomy on the "z" external frame drainage, after which a Brown anastomosis is applied between the adductor and abductor parts of the small intestine, which is fixed in the mesenteric window of the transverse colon.
The authors of this useful model have defined surgical actions and methods, which in their totality allow to apply 45 choledochocheal and pancreatointestinal anastomoses without removing the head of the pancreas and 12 o's of the jejunum. "" Brown's anastomosis is applied between the abductor and adductor parts of the loop of the small intestine, as a result of which pancreatic juice and bile flow freely through the applied anastomosis into the lumen of the small intestine and take part in the digestion process. "" 20 Although the proposed method requires greater skill of the surgeon, it is the least traumatic than those used before, is more reliable and ultimately reduces the risk to the patient's life.
T» The useful model is explained by the method implementation scheme and a specific example.
The scheme of the implementation of the method is presented in Fig., in which: 1 - esophageal-intestinal anastomosis, surrounded by a sleeve formed by a loop of the small intestine; 52 2 - Brown's anastomosis between the adductor and abductor parts of the small intestine; c Z - the stump of the duodenum; 4 - pancreatojejunoanastomosis; - choledochojejunostomy; 6 - external frame drainage; because 7 is an internal "lost" frame drain.
Example
Patient K., born in 1951, was hospitalized in the surgical department of the Transcarpathian Regional Clinical Oncology Clinic with a diagnosis of gastric antral cancer. In the anamnesis of life, it was established that in 2001 the patient underwent a pyloroplasty operation due to a perforated ulcer of the duodenum. because the tumor was recognized as operable, a decision was made to perform a gastroectomy.
During the operation, after mobilization and removal of the stomach, a complete transection of the common bile duct was performed in connection with the spread of the tumor to the duodenum.
Surgical operations aimed at the reconstruction of the gastrointestinal tract were performed as described above and shown in Fig.
Pancreatojejunoanastomosis was applied with the help of a single-row knotted suture with an atraumatic method
RKOGEM-4,/0 on "lost" internal drainage, which is a multi-perforated polyvinyl chloride tube. Hepatojejunoanastomosis was also applied with a single-row suture on the external frame drainage. 70 After the operative period, leakage of bile was observed through the insurance drain placed around the hepaticojejunal anastomosis. It was stopped without surgical intervention on the 6th day after surgery against the background of "ucreatide" injections. The passage through the gastrointestinal tract was restored, after 4 months the external drain was removed, the bile flow through it before removal was 80-100 ml.
Thus, the claimed method expanded the possibilities of surgical restoration of the gastrointestinal tract after removal of the stomach, complicated by the crossing of the main bile duct. The method is reliable and less traumatic.
