Description of the invention
М., еї аі. 2003).The proposed useful model belongs to medicine, namely to surgery. 2 In recent decades, there has been a constant increase in the number of patients with acute small intestinal obstruction. Late seeking help, diagnostic errors, insufficient effectiveness of treatment measures in this pathology causes a high percentage of complications that occur in the postoperative period and lead to death. The mortality rate for this disease reaches 2595. The main cause of mortality remains the leakiness of the seams of interintestinal anastomoses | Melnikov B.O., et al., 1989; 70 Sumyn V.V., et al., 1992; Ligonenko O.V. et al., 2005) and is accompanied by a mortality rate of 60-9090. In the structure of postoperative mortality, failure of anastomosis sutures accounts for 27.5-66,790 (O.E. Bobrov, 2004; KI haz: MM, ei ai. 2003).
Currently, there are no generally accepted methods for determining the tightness of the seams of interintestinal anastomoses.
The basis of a useful model is the task of developing a method of determining the tightness of the seams of interintestinal 79 anastomoses, which would allow surgeons to visually detect places of leaky seams during surgery and eliminate this technical error.
вThe task is solved by creating a method for determining the tightness of the seams of interintestinal anastomoses, which includes performing surgical intervention in patients under general anesthesia. After resection of the non-viable part of the small intestine and imposition of an interintestinal anastomosis, nasogastric intubation of the small intestine is performed with a probe, which is carried directly to the site of the anastomosis and 50 ml of 195 aqueous solution of methylene blue is introduced into the probe. Above and below the anastomosis, the assistant compresses the intestine, creating increased pressure in this area. Observe the appearance of seepage of blue-colored liquid at the place of application of sutures - this indicates the leakiness of the sutures and the place in the anastomosis where it is necessary to strengthen the wall of the intestine. The absence of seepage of blue-colored liquid indicates the tightness of the applied seams. in
Example: Patient K., 48 years old, was admitted to the surgical department with complaints of cramp-like pain in the abdomen, nausea, vomiting, general weakness, non-emission of gas and stool. Sick for 18 hours.
Objectively: the general condition is difficult. Pulse 118 in iv., A/T 90/40 mmHg. General blood analysis: Er. 4.5X10 12, Leukots. 18.7x10 9, stickworm. 2895. ESR 34 mm/h. On the X-ray examination of the abdominal organs, there is a large number of Kloiber cups. The patient was diagnosed with intestinal obstruction. Surgery was performed under general anesthesia. After resection of a non-viable part of the small intestine and imposition of an interintestinal anastomosis, nasogastric intubation of the small intestine was performed with a probe, which was carried out directly to the site of the anastomosis, and 5 Oml of 195 aqueous solution of methylene blue was injected into the probe. Above and below the anastomosis, the intestine was compressed, creating increased pressure in this area. The appearance of blue-colored fluid seepage was observed at the 3rd place of suturing - this indicated the leakiness of the seams and the place in the anastomosis, where the intestinal wall was strengthened. When the intestine is repeatedly compressed, it is noted that there is no seepage of blue-colored liquid, which indicates the tightness of the sutures.
17 patients were operated on by the indicated method of determining the tightness of the seams of interintestinal anastomoses, in 3 of them leakage of blue-colored liquid was detected. Additional sutures were applied to these places, which made it possible to prevent further development of postoperative peritonitis. All patients recovered. c Thus, the obtained results allow the use of this method when performing resection
