Abstract
A method of treating dystrophic pathologies of the retina and optic nerve by administering a stem cell suspension comprises making a suspension using nerve stem cells. Said suspension is administered endovitreally, in a volume of 0.1 ml with a number of cells greater than 5 × 107 in 1 ml, and additionally injected under the tenon shell extract of fetal eye tissues in a volume of 0.3-0.5 ml and also additionally parabulbarly administered fetal placenta extract in a volume of 0.1-0.2 ml. Fetal material is obtained after medical abortion in the period of 8-12 weeks of gestation, while part of the biological materials of fetal origin are removed for analysis for the absence of aerobic and anaerobic bacterial infections, mycoses, viral infections such as HIV-1/2, HCV, HBV, HSV-1/2, CMV and also in the absence of Treponema pallidum, Mycoplasma hominis, Ureaplasma, Chlamydia and Toxoplasma gondii. Karyotyping of fetal stem cells is performed to exclude abnormalities in the number of chromosomes and to determine the sex of the fetus, while for the treatment of men use fetal stem cell preparations made from male fetal tissues, and for the treatment of women - from female fetal tissues. Before administration, the suspension of nerve stem cells is thoroughly washed from the cryoprotectant. Endovitrial administration is performed after a complete ophthalmological examination, under local drip-gel anesthesia with blepharostat and sterility, using an insulin syringe through the pars plana - 4 mm from the limbus, in a volume of 0.1 ml, displacing the conjunctival fold from the place of projection of the needle prick with sterile cotton swab or tweezers, without cutting the conjunctiva. The introduction is carried out in the lower nasal quadrant. The needle is inserted 1/2 of its length, pointing to the center of the eyeball and slowly injecting the drug, and after the needle is removed, the conjunctival fold is returned to its original position to cover the injection site, after injection for 5 days use antibacterial eye drops.
Description
The utility model belongs to the field of medicine, namely ophthalmology, and can be used to treat dystrophic pathologies of the retina and optic nerve, in addition to the exudative form of age-related macular dystrophy. Sight is extremely important in human life.
A decrease in visual acuity leads to a significant decrease in the quality of life, and loss of vision leads to disability and makes a person dependent on the help of others. Modern ophthalmology has made significant progress in the treatment of diseases associated with pathologies of the optical system of the eye. However, methods of treatment of pathologies of the sensory system of the eye (retina, optic nerve) have limited effectiveness. At the same time, dystrophic pathologies of the retina and optic nerve are one of the leading causes of progressive deterioration of people's vision. This necessitates the use of new, more effective methods of treatment of these pathologies.
(5,61.For a long time, it was believed that the neurons that make up the retina and form the optic nerve have a very limited potential for regeneration (1). But modern research has shown that the regeneration of damaged cells of the retina and optic nerve can be significantly increased thanks to biological stimulation with substances produced by stem cells 1(2,3,4). Also, stem cells can be integrated into the structure of the retina and differentiate into neurons, thus replacing the cells that died (5.61.
Currently, there are several methods of using fetal stem cells to improve the condition of the retina. The method of treatment of dystrophic and vascular diseases of the retina and optic nerve of the eye (ША 134459 )) involves the treatment of dystrophic and vascular diseases of the retina and optic nerve by using autologous biomaterials: cryolizate of platelets in the form of parabulbar injections and instillations of eye drops and stem cells in the form of retrobulbar and 'ections. The disadvantage of this method is that the specified routes of administration and types of stem cells have a limited effect on the state of the retina and optic nerve. The retina is a system of nerve cells, so it is most appropriate to use nerve stem cells. However, the specified method involves the use of autologous stem cells, which may include autologous hematopoietic or autologous mesenchymal stem cells.
Another well-known method proposed to improve the condition of the retina is a method
From the complex treatment of retinal dystrophy (ША 23129 )), which involves the treatment of retinal dystrophy by the administration of the drug fetoplacental complex, according to which the drug "Cryocel" (nerve cells) is administered by orthotopic administration parabulbar or under Tenon's membrane in the amount of 0, 5 ml. Once or repeatedly depending on the clinical situation. The disadvantage of this method is that these methods of introduction of stem cells have a limited effect on the condition of the retina. The retina is a system of nerve cells located on the inner surface of the back of the eye, so the most appropriate is the endovitreal route of introducing neural stem cells, which provides these cells with access to the inner surface of the eye. Another disadvantage of this method is that it does not involve preliminary washing of cells from the cryoprotectant.
Another well-known method proposed to improve the state of the retina is the method of treating dystrophic and vascular diseases of the retina and optic nerve (ША 27832 И), which involves the treatment of dystrophic and vascular diseases of the retina and optic nerve by using the cryopreserved embryofetal drug Hemocord, which is administered parabulbarly, once at 0 .5 ml, in combination with exposure to a pulsed electromagnetic field with a frequency of 50 Hz, an induction of 6 mT on the orbital area, which is used after administration and for 8-10 days, with an exposure of 8-10 minutes. Hemocord is a suspension of cryopreserved cells from cord blood in autologous plasma. The disadvantage of this method is that these routes of administration and types of stem cells have a limited effect on the condition of the retina and optic nerve. Umbilical cord blood contains hematopoietic and a limited number of mesenchymal stem cells. The retina is a system of nerve cells located on the inner surface of the back of the eye, so it is most appropriate to use nerve stem cells and the endovitreal route of administration, which provides these cells with access to the inner surface of the eye.
Also, a common shortcoming of all these known methods is that they do not specify a limitation regarding the possibility of their use in the case of the exudative form of age-related macular dystrophy of the retina. The pathogenesis of this pathology is associated with the excessive appearance of newly formed vessels under the pigment epithelium and neurosensory retina, which is accompanied by swelling of the retina, accumulation of fluid in the subretinal space, and hemorrhages. Different types of stem cells are able to produce masciag epaoipeiiai! agoUlLy Tasioi, which stimulates the formation of new blood vessels, which makes it inappropriate to use stem cells for the treatment of this pathology.
The useful model is based on the task of improving the known method of treating dystrophic pathologies of the retina and optic nerve by introducing a suspension of stem cells.
The task is solved by the fact that neural stem cells are used to make the suspension, the mentioned suspension is injected endovitreally, in a volume of 0.1 ml with the number of cells greater than 5x10" in 1 ml, and also an extract of fetal tissue is additionally injected under Tenon's sheath of the eye in a volume of 0.3-0.5 ml, and also an extract of the fetal placenta in a volume of 0.1-0.2 ml is additionally injected parabulbarly.
Fetal material is obtained after performing a medical abortion in the period of 8-12 weeks of gestation, while a part of biological materials of fetal origin is removed for conducting analyzes for the absence of aerobic and anaerobic bacterial infections, mycoses, such viral infections as NIM-1/2, NSU, NVU, NBU-1/2, SMMU and also for the absence of Tgeropeta raiidit, Musoriazyeta Potipiz, Ogeariazta, Spiatuaiia apa Tohoriazta dopo.
Karyotyping of fetal stem cells is carried out to rule out abnormalities in the number of chromosomes and to determine the sex of the fetus, while for the treatment of men, preparations of fetal stem cells are used, made from the tissues of a male fetus, and for the treatment of women - from the tissues of a female fetus.
Before administration, the suspension of neural stem cells is thoroughly washed from the cryoprotectant.
Endovitreal injection is carried out after a complete ophthalmological examination, under local drop-gel anesthesia with the application of a blepharostat and ensuring sterility, with the help of an insulin syringe through the eye: riapa - 4 mm from the limbus, in a volume of 0.1 ml, by shifting the conjunctival folds from the point of projection of the needle injection with a sterile cotton turund or tweezers, without cutting the conjunctiva.
The injection is carried out in the lower nasal quadrant, while the needle is inserted 7/2" of its length, directed to the center of the eyeball and slowly injecting the drug, and after the needle is removed, the conjunctival fold is returned to its original position to cover
Antibacterial eye drops are used at the injection site for 5 days after the injection.
Since the retina is a system of nerve cells located on the inner surface of the back of the eye, it is most appropriate to use nerve stem cells and the endovitreal route of administration, which provides these cells with access to the inner surface of the eye. Additional administration under Tenon's membrane of fetal eye tissue extract and parabulbar administration of fetal placenta extract provide synergy of this process, significantly increasing the effectiveness of treatment.
It is expedient when the fetal material is obtained after a medical abortion (with the consent of the woman) in the period of 8-12 weeks of gestation, while part of the biological materials of fetal origin is removed to conduct analyzes for the absence of aerobic and anaerobic bacterial infections, mycosis, such viral infections as NIM- 1/2, NSU, NVU, NBU-1/2, SMMU and also for the absence of Tteropeta raiidit, Musoriazta Ppotipiv, Ogeariazta, Spiatuaia apa Tohoriazta dopo.
At the same time, fetal stem cell karyotyping is additionally carried out in order to rule out abnormalities in the number of chromosomes and to determine the sex of the fetus, and for the treatment of men, fetal stem cell preparations made from the tissues of a male fetus are used, and for the treatment of women - from the tissues of a female fetus.

Thanks to the action of stem cells and biologically active substances contained in the extracts of fetal eye and placenta tissues, this method counteracts the development of destructive processes that underlie the pathogenesis of age-related retinal dystrophy and optic nerve pathology, and promotes the regeneration of affected tissues. Thus, the specified method can be used to improve the condition of patients suffering from dystrophic pathologies of the retina and optic nerve.
