Description
The useful model belongs to medicine, namely to surgical stomatology and maxillofacial surgery and can be used for the treatment of superficial purulent-inflammatory diseases of the soft tissues of the maxillofacial area.
Purulent-inflammatory diseases of the maxillofacial area occupy a significant percentage of patients (from 40-60 95) who are receiving inpatient treatment. Difficulties in the treatment of patients with purulent surgical infection are due to the growing polyresistance of modern microflora to various antibacterial drugs, the increase in the frequency of seeding of microbial associations, including anaerobic pathogens from wounds, and the weakening of the general and local protective reactions of the macroorganism to infection. Which in turn leads to deterioration of the course of the disease and leads to the development of local and general complications. Features of the clinical course of purulent-inflammatory diseases directly depend on the adequate surgical intervention and the simultaneous observance of aesthetic aspects, which determines the need for the development of new technologies of complex, medicinal and physiotherapeutic effects on the inflammatory focus of the soft tissues of the face.
25The method of treatment of abscesses of the maxillofacial region and neck that is closest to the useful model claimed in technical terms is the method of treatment of abscesses of the maxillofacial area and neck, which includes opening the abscess, draining and subsequent washing of the cavity, according to which the opening of the abscess is performed by piercing it with a needle in the form of a modified Deschamps needle with a sharp point end and the round working part, and for drainage by reversing the needle, a perforated drainage tube is inserted into the abscess cavity (Method of treatment of abscesses of the maxillofacial area and neck. 25

However, this method is not always effective and often after dissection of the skin and subcutaneous tissue often causes bleeding, painful irritation of the patient, in addition, in the postoperative period after such an dissection, the formation of a scar and causing a corresponding cosmetic defect of the facial area at the dissection site is not excluded, which reduces the effectiveness of the treatment .
The useful model is based on the task of finding an effective method of surgical treatment of superficial purulent-inflammatory processes of the maxillofacial region by performing surgical intervention with the scalpel of the EMEO 5RESTASHM electrosurgical device to ensure
C0 access to the focus of inflammation and optimal conditions for wound reparative processes in the postoperative period with simultaneous minimization of cosmetic defects and shortening of treatment times.
The task is solved by the method of treatment of superficial purulent-inflammatory diseases of the maxillofacial area, which includes opening of an abscess and/or phlegmon, drainage and subsequent washing of the purulent cavity with an antiseptic solution, according to a useful model, dissection of the skin and subcutaneous tissue at the site of surgical intervention performed with the scalpel of the EMEO 5RESTKOM electrosurgical apparatus in monopolar mode, causing an explosion of intracellular molecular energy and evaporation of liquid in direct contact between cells and the electrode and preventing injury to adjacent tissues.
It is thanks to making incisions with the scalpel of the electrosurgical apparatus of the EMEO model
ZRESTCOM provide in monopolar mode atraumatic soft tissue incision and coagulation of blood vessels due to the concentration on a small surface area of the active electrode of a high-density HF current, which causes the evaporation of liquid from the tissues when the electrode is in direct contact with the cells, stopping bleeding during surgery without injuring adjacent tissues, which reduces pain, accelerates wound healing in the postoperative period and minimizes cosmetic defects.
In this way, a set of known and proposed essential features created a new technical solution, which provides a qualitatively new technical result, sufficient to fulfill the task of a useful model.
The method is carried out as follows.
After infiltrative analgesia with an anesthetic solution, with the help of an EMEO 5BRESTAKOM electrosurgical device in monopolar mode, a scalpel incision is made in the skin and subcutaneous tissue at the site of the surgical intervention, causing an explosion of intracellular molecular energy and evaporation of fluids in direct contact between the cells and the electrode and preventing injury to adjacent tissues. Later, the cavity is drained and washed with an antiseptic solution.
Example. Patient V. 34 years old.
She was hospitalized in the inpatient department of the maxillofacial area 3 with a diagnosis of adenophlegmon of the submaxillary area on the left.
After premedication and infiltrative analgesia, the patient underwent an incision of the skin and subcutaneous tissue in the submaxillary area on the left using the proposed method using the EMEO 5RESTKOM electrosurgical device in monopolar mode, 3 cm long.
The high-frequency current passing through the tissue caused the effect of cutting and coagulation, which greatly facilitated further surgical intervention. Stupidly, with the help of a mosquito, the abscess was penetrated, about 5 ml of pus was obtained, after which the wound was washed with an antiseptic solution and drained.
In the postoperative period, a decrease in pain, acceleration of wound healing and wound epithelization was observed. A barely visible scar remained. There were no complaints from the patient.
Preliminary studies allow us to draw conclusions that the use of an electrosurgical scalpel in the surgical treatment of superficial abscesses and phlegmons facilitates surgical intervention due to simultaneous dissection and coagulation of soft tissues, significantly reduces the volume of injected anesthetic and accelerates the wound healing process while ensuring high aesthetic results in the postoperative period.
