Description
The useful model belongs to medicine, namely to surgery, and can be used in the treatment of difficult-to-heal wounds by the method of vacuum therapy, as well as in the suction of fluid from organ cavities.
Wound drainage with subatmospheric negative pressure is widely used both for acute wounds and chronic wounds. There are known methods that allow for the drainage of deep, narrow wounds, the extraction of exudate from the wound channel.
The closest in essence is the method of vacuum therapy in the treatment of tunnel wounds (US patent No. 58030534, IPC AbEI1TM 1/00, publ. 04.10.2011), which includes hermetic fixation with an adhesive film in the wound cavity of a porous elastic sponge and the proximal end of the drainage tube, which contacting each other, connecting the distal end of the tube with a source of negative pressure, and performing drainage. A cylindrical, elastic, liquid-permeable applicator with a closed distal end and an open proximal end is pre-introduced into the wound. Due to the relative rigidity, the applicator can be inserted into the wound, overcoming resistance.
The known method is ineffective in the case of tunnel through wounds. The complexity of treating penetrating wounds is due to their "architecture". Through wounds have a long, narrow course of an irregular shape and affect several anatomical structures, which makes it much more difficult to prevent the accumulation of exudate in the closed space of certain areas of the wound cavity, which can cause complications, primarily in the form of suppuration and, as a result, delayed wound healing.
The disadvantage of the known method is also the use of a cylindrical elastic applicator, which is a semi-solid draining element. The created negative pressure is unevenly distributed throughout the draining element: higher in the proximal part, lower in the distal part. This may not allow sufficient evacuation of exudate from the distal part of the wound.
The basis of this useful model is the task of creating a method of vacuum therapy in the treatment of penetrating wounds by creating uniformity of negative pressure applied to the wound along the draining element, which will ensure reliable evacuation of exudate and uniform distribution of the therapeutic effects of negative pressure in the wound.
The problem is solved by the fact that in the method of vacuum therapy in the treatment of through wounds, which includes hermetic fixation with an adhesive film in the wound cavity of a porous elastic sponge and the proximal end of the drainage tube, which are in contact with each other, the connection of the distal end of the tube with a source of negative pressure , and performing drainage, according to the useful model, the porous elastic sponge is given a shape close to the shape of the wound channel, around the circumference of the drainage tube along its middle part equal to the length of the wound channel, holes with a diameter of 1-3 mm are made at a distance of 10-20 mm from each other and the tube is inserted inside the sponge so that the perforated part is completely hidden, after which the sponge is placed in the through channel of the wound so that the sponge fills it, and both exits of the wound channel are hermetically closed with an adhesive film, and the ends of the tube are connected to the source of negative through a U-shaped adapter pressure
Giving the sponge the shape of the wound channel and perforating the middle part of the drainage tube with the placement of the perforated part inside the sponge ensures reliable drainage of the entire length of the wound channel with the provision of therapeutic effects of negative pressure along its entire length, and also reduces the likelihood of the formation of "blind", non-draining spaces in the wound channel , which in turn reduces the likelihood of complications due to the formation of non-draining spaces.
Connecting the ends of the drainage tube through a U-shaped adapter with a source of negative pressure allows you to create a loop that allows you to apply negative pressure on both sides of the wound channel and maintain uniform pressure throughout the entire drainage space of the wound.
Thus, reliable evacuation of exudate from the wound is carried out, the frequency of purulent complications decreases.
The proposed method allows to reduce the frequency of complications in the treatment of penetrating wounds in the form of the formation of blind spaces and suppurations, and to ensure the fastest possible healing of this type of wounds.
The essence of the method is explained by graphic material.

Неперфоровані кінці дренажної трубки виводять з обох кінців рани і під'єднують через У-подібний перехідник 5 до джерела негативного тиску 6 (дивись фіг.2).The method of vacuum therapy in the treatment of penetrating wounds is carried out as follows. 60 The preparation of the drainage bandage is performed in sterile conditions using sterile materials and tools. Porous elastic sponge 1 is given the shape of a wound channel (see Fig. 1). Then holes with a diameter of 1-3 mm at a distance of 10-20 mm from each other are made along the circumference of the drainage PVC tube 2 along its middle part equal to the length of the wound channel. With the help of a drainer or a clamp, the tube is inserted into the sponge along its long axis so that the perforated part is completely hidden. After that, the sponge is placed in the through wound channel so that the sponge fills it, and both exits of the wound channel are hermetically closed with an adhesive film 4. The non-perforated ends of the drainage tube are brought out from both ends of the wound and connected through the U-shaped adapter 5 to the source of negative pressure 6 (see fig. 2).
Drainage is carried out in standard modes
Example. Patient K. is 27 years old. Diagnosis: blind gunshot wound of the soft tissues of the left thigh. Day-old injury. The wound is tamponed with antiseptic wipes. A wound channel about 20 cm long extends from the outside to the inside of the left thigh, from top to bottom, from back to front. The bottom of the wound of the muscle of the medial surface of the middle third of the thigh. The wound channel does not reach the skin of the medial surface by about 2-3 cm. The primary surgical treatment of the wound of the soft tissues of the left thigh was performed in a traditional way. At the final stage, the method of vacuum therapy is applied in the treatment of penetrating wounds. An incision is made along the medial surface of the thigh so that the wound becomes through. Drainage was provided using a drainage tube in the form of a loop. The middle part of the tube is perforated with holes of 1-3 mm at a distance of 10-20 mm from each other and passed through a sponge pre-formed according to the shape of the wound channel. The resulting product was immersed in the wound through and through. Both ends of the wound were sealed with an adhesive film. The ends of the drainage tube were connected to a source of negative pressure through a MU-shaped adapter. Drainage was carried out with rarefaction - 125 mm Hg. Art. within 4 days. On the 4th day, when revising the wound, no blind spaces were found in the wound channel. Soft tissues were viable, small island areas of necrosis were found. The formation of granulation tissue was visible in places. It was still necessary to use this method once in the form of a 4-day session, after which the amount of wound secretion decreased. Then the treatment was carried out in a traditional way. Complete primary healing occurred on the 19th day.
