Abstract
The utility model relates to medicine, in particular to surgery, and can be used by emergency, ambulance and military field services to provide emergency care for head and cervical spine injuries. Surgical drainage containing a tube made of a polyurethane polymer is known to the wall of the distal end of which holes are made for drainage. The proximal end of the tube is equipped with bulges for fixing the tube. Patent No. JP4314454 dated November 5, 1992. The disadvantage of this method is: 1) the tubular canal for outflow is often obstructed by wound and prevents drainage; 2) there is no reliable fixation of the drainage, in which the tube can migrate; 3) the need for manual cutting of the necessary holes in the drainage tube depending on the size of the wound. The expected result is a reliable fixation of drainage in the wound and hygroscopic material directed into the external environment unhindered outflow of wound discharge, local antibacterial the effect, mechanical cleaning of the wound and stimulation of regeneration when removing or changing the drainage, the absence of a local inflammatory or allergic reaction to the drainage. Thus, surgical drainage solves several problems at once: it provides effective drainage, prevents the development of local reactions of the body, prevents possible complications and provides an antibacterial effect in the treatment of soft tissue wounds. An additional advantage is versatility in the form of a simple application and cutting the right size of surgical drainage.
Description
Surgical drainage for the treatment of soft tissue wounds of various etiologies.
The utility model relates to medical equipment, namely, devices for draining soft tissue wounds, and can be used to treat punctured, cut, scalped, lacerated, torn-bruised, bitten, gunshot and fragmentation wounds, as well as for the prevention of complications of postoperative wounds with purulent-inflammatory, infiltrative and other pathological processes.
A large number of surgical drains are known: a rubber strip, a half-tube, tampon drains, flow-washing systems, Redon drainage, single and multi-lumen tubular, capillary, etc. There are different materials from which they are made: polyvinyl chloride, latex rubber, silicone, polymer films from acetate, silicone, cellulose, polyethylene, plexiglass, glass and metal drains. The authors proposed a wide variety of options for providing self-flow, passive and active aspiration of the wound, washing drainage, fixation, etc.
A variety of localization, types, forms of wound cavities and canals requires a differentiated approach to the choice of drainage method. Currently, for the passive drainage of soft tissue wounds, gloves, rubber tubes, tubes and half tubes, turundas are used, however, there are many drawbacks that prevent the effective drainage of wound discharge: wound canal obstruction, retrograde wound discharge dislocation, drainage dislocation, secondary infection attaches, biological incompatibility , trauma and irritation of surrounding tissues, and other complications.
05.11.1992.Known surgical drainage containing a tube of polyurethane polymer, in the wall of the distal end of which holes are made for drainage. The proximal end of the tube is equipped with bulges for fixing the tube. Patent No. JP4314454 dated November 5, 1992.
The disadvantage of this method is:
1) трубчатый канал для оттока часто обтурируется раневым отделяемым и препятствует дренированию раны;1) the tubular channel for the outflow is often obstructed by wound and prevents wound drainage;
2) there is no reliable fixation of the drainage, in which tube migration is possible.
3) the need for manual cutting of the necessary holes in the drainage tube, depending on the size of the wound.
The claimed utility model aims to eliminate the above disadvantages, as well as to ensure adequate drainage of soft tissue wounds of various origins and to provide a local antimicrobial effect.
The task is achieved in that surgical drainage in the form of a tape of the desired size is placed in the wound cavity so that the vertices of the transverse triangular tooth protrusions are facing the outflow of the discharge, after which hygroscopic dressing is placed around the free end of the drainage on the skin surface and a drainage retainer is installed , then the entire dressing is fixed with a band-aid or bandage.
At the same time, the anti-adhesive and antibacterial coating will prevent the development of a secondary infection, ensure a directed unhindered outflow of the wound undergrowth, and the transverse triangular serrated protrusions of the surgical drainage will perform a mechanical cleaning function, preventing the discharge from remaining in the wound when changing the drainage.
The technical result achieved by the claimed combination of essential features is to ensure effective drainage of soft tissue wounds of different genesis, reliable fixation in the drainage wound, local antimicrobial effect, ease of use, reduction in the number of possible complications and reduction of treatment time.

The essence of the proposed utility model is as follows.
The prototype of the proposed surgical drainage is made in the form of an elastic tape (2), characterized in that the surface of the tape is equipped with frequent transverse triangular serrated protrusions on both sides (1), with vertices facing one way to ensure a directed outflow of the wound discharge (6), elastic tapes (2) with transverse teeth 1 made of silicone, latex, polyurethane, polyvinyl chloride and rubber, coated with anti-adhesive and antibacterial coating. The drainage is equipped with an external (outside the wound cavity) fixative of the dressing and the drainage tape (4). Surgical drainage is made in the form of wide tapes (7), for cutting the desired size of the drainage intraoperatively or at the dressing.
The device is manufactured as follows: by extrusion of the precursor components, a homogeneous molten material is obtained, which is introduced under pressure into the mold and then removed. The next stage of manufacture is the application of an anti-adhesive antibacterial coating, packaging together with the clips (4) of the drainage tape, and then sterilization.
The device is used as follows. After all surgical procedures, washing the wound, the proposed drainage is removed from the sterile packaging (7), a strip of the required size is cut out and immersed in the wound to the level of the bottom of the wound cavity, so that the outer end of the drainage is 1 centimeter above the skin surface. A solution with an antiseptic is injected into the wound, ointment or left clean. Then, dressing hygroscopic material is placed around the drainage in the form of napkins with a cut according to the type of “pants” (3), and a latch of drainage and napkins is installed over the napkins. The latch is made in the form of a hard oval plate (4), having a thin slot (5) on one side, extending to the opposite third of the diameter, where it blindly ends. Through the slot (5), a latch (4) is installed between the teeth of the drainage tape (7), which presses the wipes (3) in the area of the wound edges, and also prevents the dislocation of the drainage. Wipes are placed on top of the latch in the required volume and closed with a bandage or adhesive. The directed current of the wound discharge (6) is provided by the teeth (1), which, when in contact with the walls of the wound channel, form a valve mechanism and prevents the reverse current of the discharge. Reliable fixation in the drainage wound, pressure gradient in the wound cavity and medicinal substances contribute to the evacuation of the wound discharge, including from wounds with a deep canal. The drainage is removed together with napkins in one motion, with the aim of mechanically cleaning the wound due to unidirectional teeth on the surface of the tape.
The proposed drainage is convenient for use as a prophylaxis of postoperative complications, as well as for surgical treatment of purulent-inflammatory pathology. Fields of application: postoperative drainage of wounds of soft tissues, drainage of gunshot, lacerated, contaminated and purulent wounds.
