Спосіб формування анастомозу на шлунково-кишковому тракті полягає у наступному.The method of forming an anastomosis in the gastrointestinal tract is as follows.
Оперативне втручання проводять при чотирикратному збільшенні за допомогою лобового нейрохірургічного освітлювача. При цьому використовують мікрохірургічний інструментарій та атравматичні голки з синтетичними нитками з вікрилу та пролену розміром 4-0. Формують однорядний вузловий прецизійний шов, зшиваючи підслизовий і м'язово-підслизовий шари, при цьому прокольні канали виконують такими, які не сполучаються ні з просвітом кишки, ні з черевною порожниною на рівні ранової щілини. При створенні анастомозу вузловими швами першим швом зшивають підслизові шари з обох боків, наступним - м'язово-підслизові, потім знов лише підслизові і так далі циркулярно. При цьому слизову та серозну оболонки не прошивають і в шов не захоплюють. Таким чином, проводять точне зіставлення шарів відділів шлунково-кишкового тракту, що зшиваються, та забезпечують біологічну герметичність анастомозу.Operative intervention is carried out at a four-fold increase with the help of a frontal neurosurgical illuminator. Запропонований спосіб ілюструється наступним клінічним прикладом. Хвора П., 55 років (медична карта Мо 0526/10) госпіталізована у хірургічне відділення Кримського республіканського клінічного онкологічного диспансеру 28.01.2010 р. у плановому порядку з діагнозом: рак прямої кишки ТЗ МО МО, І сет., часткова кишкова непрохідність. Після дообстеження та проведення передопераційної підготовки 02.02.2010 р. хворій проведено оперативне втручання - черевно-анальну резекцію прямої кишки. Сигмовидна кишка низведена до кукси прямої кишки та сформовано анастомоз за запропонованим способом, використовуючи однорядний вузловий прецизійний шов. Тривалість операції - 2 години 10 хвилин.The proposed method is illustrated by the following clinical example. The duration of the operation is 2 hours and 10 minutes.
Післяопераційний період протікав гладко, без ускладнень. Шкірні шви видалено на 8-9 добу - рана загоюється первинним натягом, та 12.02.2010 р. хвора у задовільному стані виписана на амбулаторне лікування. Оглянута через 1 рік після операції, скарг не виказує, диспепсичних явищ не має, пролонгації захворювання не діагностовано.The postoperative period was smooth, without complications.
