Abstract
A method of correction of joint syndrome in reactive arthropathy in children involves the use of anti-inflammatory gel applications, which include diclofenac sodium and thiosol, on the affected joints. In addition prescribe oral use of the complex homeopathic drug "Incena": children under 12 years - 7 drops of the drug in pure form or diluted in 1 tablespoon of water, children after 12 years - 10 drops of the drug diluted in 1 tablespoon of water, three times a day, for a time sufficient to produce a positive effect.
Description
The proposed useful model belongs to the field of medicine, namely to pediatrics, cardiorheumatology, and can be used to correct the treatment of joint syndrome in reactive arthropathies in children.
Joint syndrome is one of the most frequent pathological conditions observed both in the group of rheumatic diseases and outside of it. In a number of diseases (juvenile rheumatoid arthritis, reactive arthritis, etc.), it plays a leading role in the clinical picture, in others it is one of the clinical manifestations, which complicates the verification of the diagnosis. There are no pathognomonic signs and generally accepted diagnostic criteria for this disease |bitopipi S, Tadai A,
А, 2008). Sita; K, 2009; Nasonov E.L., Nasonova V.A, 2008). The clinical picture is characterized by damage to the joints, ligaments, spine and the development of extra-articular manifestations.
The most typical is the acute onset of the disease with pain and/or asymmetric swelling of the knee and/or ankle joints, involvement of other joints is less common.
В, 20131).A particularly serious analysis is required in the debut of the disease, which is often represented exclusively by arthralgias (Prokhorov E.V., Berezhnoi V.V., Marushko Yu.V. et al. Joint syndrome in the practice of a pediatrician. Donetsk, 2006-145 p.|; |Povorozniuk V. V. Diseases of the musculoskeletal system in people of different ages (selected lectures, reviews, articles). - K., 2009. - P. 480). Reactive arthropathies are the most common pathology among joint lesions in children (45-49 95). This is a group of joint diseases (code МКХ10О МО2), which develop in the background or 1-2 weeks after transferred infections, without an infectious pathogen entering the joint cavity, as a result of impaired immunity in genetically predisposed individuals (Berezhny V.V., Marushko T. V., Marushko Yu.V, 20131).
The frequency of reactive arthropathies in the structure of rheumatic diseases in different countries is from 8 to 41 95. This pathology has a mostly benign course, but in about 20 95 children it can transform into inflammatory chronic arthropathies (juvenile rheumatoid arthritis (MO8.0), juvenile chronic arthritis (MO8.3). According to the WHO, about 4 95 of the world's population have pathological changes in the musculoskeletal system. In recent years, there has been a tendency to increase the number of patients with reactive arthritis (ReA), especially in childhood, which indicates the need to develop effective diagnostic and treatment measures for this pathology

There is no specific therapy for this disease, as its pathogenesis has not been sufficiently studied. Treatment of ReA begins with the elimination of the causative microorganism, as well as the elimination of joint syndrome and extra-articular manifestations. Pathogenetic therapy is aimed at reducing the activity of the inflammatory process in the joints.
Л., НасоноваKnown methods of treatment of joint syndrome, which include parenteral and oral administration of non-steroidal anti-inflammatory drugs (NSAIDs), mainly selective inhibitors of cyclooxygenase-2 - nimesulide, meloxicam (movalis), as well as cyclooxygenase-i inhibitor - diclofenac sodium, with an additional appointment in the early stages of the disease applications of dimethylsulfoxide, gel forms of NSAIDs (E.L. However, the oral use of non-steroidal anti-inflammatory drugs (NSAIDs) in doses sufficient to produce a positive effect often negatively affects the function of the gastrointestinal tract, causing nausea and abdominal pain, diarrhea, damage to the gastric mucosa (ulcers, erosions, gastrointestinal bleeding ), allergic reactions, etc.
The closest to the proposed useful model is the method of treatment of joint syndrome, intended for the local elimination of inflammatory symptoms, which includes the local use of an anti-inflammatory gel, which includes diclofenac sodium and thiozol

Objectively: the right knee joint is enlarged (about 3 cm larger than the left joint), the range of motion is limited, local hyperthermia, tonsils are hypertrophied, loose, palpable submandibular lymph nodes. Vesicular breathing in the lungs. Heart sounds are rhythmic, weakened, weak systolic murmur at the apex and in the M point, BP 130/80. The abdomen is soft, painless, the liver is not enlarged. Comprehensive clinical and diagnostic examinations were performed (general blood test, kidney tests (creatinine, urea, residual nitrogen), liver tests, proteinogram, total protein, seromucoids, rheumatoid factor, coagulogram, general urinalysis, enterobiosis analysis; electrocardiography (ECG) , rheocephalography (REH), ultrasound diagnosis (USD) of the heart, consultation of an ophthalmologist, physiotherapist, otorhinolaryngologist.Ultrasound examination of the joint - signs of chronic arthritis and synovitis.
Diagnosis after clinical and laboratory examination: Reactive arthropathy, joint syndrome, chronic compensated tonsillitis.
The patient was prescribed treatment with the proposed method of correction of joint syndrome in reactive arthropathy in children: applications of anti-inflammatory gel, which includes diclofenac sodium and thiozol, were applied to the affected joint 3 times a week, for 30-40 minutes. In addition, oral use of the complex homeopathic drug "Incena" was prescribed, 10 drops of the drug in its pure form or diluted in 1 tablespoon of water, for 3-4 weeks, that is, enough time to obtain a positive effect.
After the end of the course of treatment, complaints of pain in the knee joint and signs of synovitis disappeared, and three weeks after the start of treatment - a decrease in the clinical symptoms of arthritis in the form of the absence of swelling of soft tissues, normalization of movements in the joint, normalization of laboratory parameters.
38 children aged 7-14 (21 girls and 7 boys) were treated with the proposed method of correction of joint syndrome in reactive arthropathy in children. The drug was prescribed for children under 12 years of age - 7 drops of the drug in its pure form or diluted in 1 tablespoon of water, for children after 12 years of age - 10 drops of the drug diluted in 1 tablespoon of water three times a day for 3-4 weeks. The obtained treatment results indicate a positive effect of the proposed method. Thus, already at the end of the first week of treatment, a reduction or disappearance of pain syndrome, morning stiffness and reduction of swelling around the joint was noted. In the second week, laboratory indicators normalized.
The phenomenon of the pronounced pain-relieving effect of the proposed method is explained by the presence of a double analgesic effect of the anti-inflammatory gel, which includes diclofenac sodium and thiozol, on the affected joint and the oral use of the complex homeopathic drug "Incena" based on plants.
The leading therapeutic effect when using herbal preparations was the analgesic effect, while the elimination of the pain syndrome occurred quite quickly. The development of antiexudative effect was reliably expressed at the end of treatment.
Thus, based on the above, it can be concluded that the proposed method of correction of joint syndrome in children allows to achieve a decrease in local activity and the degree of damage to the joints at an earlier time, a decrease in pain sensations, removal of swelling of soft tissues, to ensure the prevention of the development of an aggressive course diseases and increase the degree of effectiveness of treatment.
