Протизапальні лікарські засоби, що використовуються в щелепно-лицевій хірургії, можуть використовуватися як з профілактичною, так і з лікувальною метою. Профілактична мета медикаментозного використання даних препаратів передбачає терапію, спрямовану на профілактику розвитку гнійно-запальних процесів в тканинах щелепно-лицевої ділянки та шиї після проведених оперативних втручань, при травматичних ушкодженнях та інших випадках.Anti-inflammatory drugs used in maxillofacial surgery can be used both prophylactically and curatively. The main goal of step therapy is to reduce the duration of parenteral administration of the drug, which ensures a reduction in the cost of the treatment and shortens the patient's stay in the hospital while maintaining the high clinical effectiveness of the therapy. The optimal option of step therapy was the consecutive use of two dosage forms (for parenteral administration and oral administration) of the same NSAIDs, which ensured continuity of treatment.

Під нашим спостереженням знаходилося 129 хворих з різними захворюваннями щелепно- лицевої ділянки. Всіх обстежених ми розділили на наступні п'ять груп: І група - 28 хворих після проведення щелепно-лицевих оперативних втручань з приводу видалення пухлин щелеп (остеобластом, амелобластом, значних пухлиноподібних утворень); ІЇ група - 25 хворих, яким проведені оперативні втручання - паротидектомії (при доброякісних пухлинах) та екстирпації піднижньощелепних залоз (при доброякісних пухлинах або калькульозних субмаксилітах); група - 26 хворих з переломами нижньої щелепи (щілина перелому проходила в межах зубного ряду, тобто переломи щелепи були відкритими); ІМ група - 26 хворих з одонтогенними запальними інфільтратами білящелепних м'яких тканин; М група - 24 хворих з гострими посттравматичними артритами скронево-нижньощелепних суглобів, ускладненими запальними інфільтратами. Всі обстежені були у віці від 16 до 68 років, без супутніх захворювань.We monitored 129 patients with various diseases of the maxillofacial region. All examined were aged from 16 to 68 years, without concomitant diseases.
Для вирішення поставленої задачі всім хворим проводилися клінічні методи обстеження, які включали з'ясування скарг, огляд (визначався ступінь асиметрії обличчя, вираженість набряку, бо гіперемії та інфільтрації слизової оболонки та/або шкірних покривів в області патологічного вогнища), пальпацію. Крім раніше зазначених методів обстеження ми проводили хворим контактну термометрію та загальну термометрію, визначали сатурацію кисню, частоту пульсу та дихання у хвилину, вимірювали артеріальний тиск, проводили загальний аналіз крові (визначали кількість еритроцитів, лейкоцитарну формулу, гемоглобін, ШОЕ), загальний аналіз сечі, визначали біохімічні показники крові (загальний білок, глюкоза, білірубін, сечовина, електролітний склад). To detect the inflammatory process of the mucous membrane of the alveolar process, the Schiller-Pysarev test was determined with the calculation of Svrakov's iodine number.

Зо паротидектомії (при доброякісних пухлинах) та 13 - екстирпації піднижньощелепних залоз (при доброякісних пухлинах або калькульозних субмаксилітах), ми використовували метод ступінчастої терапії: протягом 3-х днів застосували "Диклоберл Мо 75" (внутрішньом'язово), а в наступні 4-6 днів - "Диклоберл ретард" (перорально). Через 5 днів від початку лікування препаратом "Диклоберл": набряк білящелепних м'яких тканин в ділянці післяопераційної рани визначався як помірний у З хворих (12,0 95) та був відсутній у 22 хворих (88,0 95); інфільтрація шкірних покривів в ділянці післяопераційної рани визначалася як помірна у 1 хворого (4,0 95) та була відсутня у 24 хворих (96,0 95); показник термоасиметрії шкірних покривів в ділянці проведеної операції та протилежній стороні нормалізувався та складав 0,6520,2 С; загальна температурна реакція від 37,0 до 37,5 "С спостерігалася у 1 хворого (4,0 95) та нормалізувалася у 24 хворих (96,0 95). На 6-й день загальна температурна реакція нормалізувалася у всіх обстежуваних хворих. У ІІ групі хворих гнійно-запальних ускладнень ми не спостерігали, а в контрольній групі (лікування антибіотиками і сульфаніламідними препаратами) спостереження ми виявили гнійні ускладнення у 12,5 95. Третя група спостереження складалася з 26 хворих з переломами нижньої щелепи (щілина перелому проходила в межах зубного ряду, тобто переломи були відкритими). Постраждалим проводили іммобілізацію кісткових відламків нижньої щелепи за рахунок застосування назубних дротяних шин і міжщелепної гумової тяги. Видалення зуба проводили за показаннями під місцевою провідниковою анестезією з подальшою хірургічною обробкою кісткової рани. В третій групі спостереження ми застосовували "Диклоберл Мо 75" (внутрішньом'язово) протягом 9-10 днів. Через 7-8 днів від госпіталізації та початку лікування препаратом "Диклоберл": набряк білящелепних м'яких тканин в ділянці перелому визначався як помірний у 2 хворих (7,7 95) та був відсутній у 24 хворих (92,3 95); інфільтрація слизової оболонки альвеолярного паростку в ділянці щілини перелому визначалася як помірна у З хворих (11,5 95) та була відсутня у 23 хворих (88,5 95); гіперемія слизової оболонки в ділянці щілини перелому спостерігалася у 1 хворого (3,8 95) та була відсутня у 25 хворих (96,2 95). Через 8-9 днів від початку лікування проба Шиллера-Писарєва (йодне число Свракова) складало 1,6-0,6 бали (слабовиражений запальний процес); показник термоасиметрії слизової оболонки альвеолярного паростку в бо ділянці щілини перелому та протилежному боці щелепи нормалізувався та складав 0,7-0,3 "С;The third observation group consisted of 26 patients with fractures of the lower jaw (the fracture gap was within the dental row, that is, the fractures were open). "Причинні" зуби у всіх хворих були видалені при госпіталізації в стаціонар. Через 6 днів від госпіталізації та початку лікування препаратом "Диклоберл": запальний інфільтрат м'яких тканин визначався як помірний у 7 хворих (26,9 95) та був відсутній у 19 хворих (73,1 95); інфільтрація слизової оболонки в ділянці видаленого "причинного" зуба не визначалася у всіх хворих (100 95); гіперемія слизової оболонки в ділянці видаленого "причинного" зуба визначалася у 1 хворого (3,9 95) та була відсутня у 25 хворих (96,1 95); проба Шиллера-Писарєва (йодне число Свракова) складало 1,5:2-0,7 бали (слабовиражений запальний процес); показник термоасиметрії слизової оболонки альвеолярного паростку в ділянці видаленого "причинного" зуба та протилежному боці щелепи нормалізувався та складав 0,6:20,1 "С; загальна температурна реакція нормалізувалася у 26 хворих (10095). Препарат з фармакотерапевтичної групи нестероїдних протизапальних і протиревматичних засобів з пролонгованим механізмом дії - диклофенак натрію ("Діклоберл") має виражену протизапальна та жарознижувальна ефективність. Визначено його позитивну дію на оксигенацію крові та гемодинаміку, а також протизапальний та жарознижувальний ефект.A drug from the pharmacotherapeutic group of nonsteroidal anti-inflammatory and antirheumatic agents with a prolonged mechanism of action - diclofenac sodium ("Dicloberl") has a pronounced anti-inflammatory and antipyretic effect. Результатами проведеного обстеження хворих після видалення пухлин щелеп, паротидектомій і екстирпацій піднижньощелепних залоз, у хворих з відкритими переломами нижньої щелепи, а також у хворих з одонтогенними запальними інфільтратами білялщелепних м'яких тканин і гострими посттравматичними артритами скронево-нижньощелепних суглобів, ускладненими запальними інфільтратами нами була встановлена висока протизапальна та жарознижувальна ефективність нестероїдного протизапального препарату "Диклоберл" і він може бути використаний у хворих з цією патологією з метою профілактики розвитку гнійних запальних ускладнень без застосування антибіотиків і сульфаниламидних препаратів.The results of the examination of patients after removal of tumors of the jaws, parotidectomies and extirpations of the submandibular glands, in patients with open fractures of the lower jaw, as well as in patients with odontogenic inflammatory infiltrates of perimaxillary soft tissues and acute post-traumatic arthritis of the temporomandibular joints, complicated by inflammatory infiltrates, were the high anti-inflammatory and antipyretic effectiveness of the non-steroidal anti-inflammatory drug "Dikloberl" has been established and it can be used in patients with this pathology in order to prevent the development of purulent-inflammatory complications without the use of antibiotics and sulfonamide drugs.
