Abstract

一種骨釘結構,係於釘本體上軸接定位件,並於該定位件上一體延伸出把持件,且該把持件之側面係為傾斜面,以於進行脊椎固定手術時,可將其中一把持件之弧面靠合相鄰之把持件之傾斜面,使該些把持件相疊合,因而可縮減該些把持件於皮膚表面上所佔用之面積,故能大幅縮小該開口之寬度,以達到傷口範圍最小化之需求。

Description

骨釘結構
本創作係關於一種骨釘結構,尤指一種應用於脊椎之骨釘結構。
目前人類隨著年齡增加,身體容易出現症狀,例如,脊椎壓迫神經之症狀。當發生脊椎壓迫神經時,通常醫療機構會採用腰椎脊柱微創手術,以將複數骨釘固定於特定區段之脊椎骨之間,使該區段之脊椎骨不會位移,迫使病人不能再作動該區段之脊椎骨(如不能彎腰),因而該區段之脊椎骨不會再壓迫神經。
於進行腰椎脊柱微創手術之過程中,病人係呈趴姿進行手術(如第1圖所示),再於皮膚表面8上形成一開口80,以將多個骨釘1分別植固於對應之脊椎骨8a,8b,8c上,且該些骨釘1之上側部10凸出該開口80。接著,藉由一連接桿(圖略)伸入該開口80內以串接所有的骨釘1之下側部11。之後,移除該骨釘1之上側部10,以縫合傷口(即該開口80處)。
惟,習知骨釘1之上側部10係呈圓柱狀,故需以幾乎垂直病人所趴之床面之方向進入該開口80,以避免相鄰之骨釘1之上側部10相互干涉,故該開口80需具有一定 的大小(如寬度D至少5cm),因而難以縮小該開口80於皮膚表面8上所佔用之面積,致使無法達到傷口範圍最小化之需求,且即使將該骨釘1以傾斜方向進入該開口80,該骨釘1之傾斜角度仍會受到相鄰之骨釘1之圓柱狀上側部10之干涉,而無法有效縮小該開口80或縮減傷口範圍。
因此,如何克服習知技術之問題,實為一重要課題。
為解決上述習知技術之問題,本創作遂揭露一種骨釘結構,係包括:釘本體;定位件,係具有相對之第一側與第二側,且該第一側係設於該釘本體之端部;以及把持件,係設於該定位件之第二側上,且該把持件之側面係為傾斜面。
前述之骨釘結構中,該釘本體係為螺絲釘狀。
前述之骨釘結構中,該定位件與該釘本體係能相對轉動。
前述之骨釘結構中,該定位件係具有容置空間。例如,該把持件係具有連通該容置空間的通道。
前述之骨釘結構中,該把持件係呈勺狀或瓢狀。
前述之骨釘結構中,該把持件之頂端係為封閉式或開放式。
前述之骨釘結構中,復包括連接件,係穿設於該定位件之周面上。進一步包括固定件,係用以將該連接件固定於該定位件上,例如,該固定件係螺接該定位件且壓制該連接件。
由上可知,本創作之骨釘結構中,主要藉由該把持件之傾斜面之設計,以於進行脊椎固定手術時,可將其中一把持件靠合相鄰之把持件之傾斜面,使該些把持件相疊合而縮小該些把持件之間的夾角,因而可縮減該些把持件於皮膚表面上所佔用之面積,故相較於習知技術之骨釘,使用本創作之骨釘結構可於手術部位上大幅縮小該開口之寬度,以達到傷口範圍最小化之需求。

第2A圖係為本創作之骨釘結構2之立體示意圖。於本實施例中,所述之骨釘結構2係應用於脊椎上。
如第2A及2B圖所示,該骨釘結構2係包括:一釘本體20、一定位件21以及一把持件22,且該釘本體20、定位件21及把持件22係作為前置組件2a。
所述之釘本體20係為金屬螺絲釘狀,如尖尾螺絲型,其具有相對之頭端部20a與尖端部20b。應可理解地,該釘本體20可依需求設計成各種型式,並不限於上述。
所述之定位件21係具有相對之第一側21a與第二側21b,且該定位件21以其第一側21a接合該釘本體20之頭端部20a。
於本實施例中,該定位件21係為合金材製成大致呈圓筒狀或帽狀,其具有一容置空間S,且該釘本體20之球狀頭端部20a係軸接於該容置空間S之底部,使該定位件21能相對該釘本體20轉動(或該釘本體20能相對該定位 件21轉動)。
再者,該容置空間S之內表面係具有螺紋P1,且該容置空間S之側面係形成有一連通該第二側21b之凹口210,使該凹口210橫向(如第2A圖所示之坐標之X軸向)貫通該定位件21之兩側面,例如,該定位件21之前視或後視狀態係呈「凹」字形。
所述之把持件22係以合金材一體成形於該定位件21之第二側21b上,且該把持件22係呈勺狀或瓢狀。
於本實施例中,該把持件22之設計係由中空圓柱狀變成片體。例如,該把持件22之圓筒狀周面係具有相對之兩側,其中一側係為傾斜面22c,而另一側係為弧面22d,且於該傾斜面22c上形成凹槽221,使該把持件22變成勺狀或瓢狀片體。
再者,該把持件22之長度L係大於該定位件21之長度H。
又,該凹槽221之底部也具有螺紋P2,且於該凹槽221上形成有一連通該容置空間S(即該凹口210)的通道220(如第2B圖所示),使該凹槽221與該通道220橫向(如第2A圖所示之坐標之X軸向)貫通該把持件22之周面。
另外,該把持件22之頂端22a係為封閉式,但於其它實施例中,該把持件22之頂端22a’亦可為開放式,如第2A’圖所示。
於另一實施例中,如第2C圖所示,所述之骨釘結構2 復包括一連接件23,係穿設於該定位件21之周面21c上。例如,該連接件23係為金屬製直桿狀或彎曲桿狀,其可經由該凹口210或該通道220橫向(如第2A或2C圖所示之坐標之X軸向)穿過該定位件21或該把持件22,以沿一由該通道220與該凹口210所形成之路徑(如第2A或2C圖所示之坐標之Z軸向)而位移於該凹槽221與該容置空間S中。進一步,如第2D圖所示,該骨釘結構2復包括一固定件24,係用以將該連接件23固定於該定位件21上。例如,該固定件24係為金屬製螺絲,其沿該凹槽221之螺紋P2螺接位移至該定位件21之容置空間S之螺紋P1且壓制該連接件23於該容置空間S中。
於進行脊椎固定手術時(例如,以人工方式或機器手臂方式進行手術),如第3A圖所示,先於一欲固定位置(即該脊椎骨9a,9b,9c)所對應之皮膚表面9上形成一開口90,以令使用者(如醫師)能觀看到複數脊椎骨9a,9b,9c,再將至少兩個前置組件2a以其釘本體20分別植固於對應之脊椎骨9a,9b,9c上,其中,該釘本體20連同該定位件21可沿相對該皮膚表面9傾斜之方向進入該開口90以植入所對應之脊椎骨9a,9b,9c,故該開口90之大小不需對應該欲固定位置之脊椎骨9a,9b,9c之投影範圍,即該開口90不需太大。
如第3B圖所示,由於該定位件21與該釘本體20係能相對轉動,且該定位件21與該把持件22係相固定接合(如一體成形),故當所有預定的釘本體20(如第3B圖所 示之三個前置組件2a)已植入至適當位置(如鑽入該脊椎骨9a,9b,9c中)後,轉動該些把持件22之方位,使相鄰的把持件22中,其中一把持件22的弧面22d會靠合另一把持件22之傾斜面22c,使該些把持件22呈疊加狀態,較佳地,部分該弧面22d(或該頂端22a)會容置於該凹槽221中。
接著,如第3C至3D圖所示,將一彎曲桿狀連接件23於幾乎垂直該傾斜面22c的狀態下穿過該些把持件22之通道220,再沿著該通道220向下移動,以於部分通過該凹口210後,逐漸轉動該連接件23而使其完全進入該開口90中,再沿左右方向(如第2A或3D圖所示之坐標之X軸向)調整該連接件23至適當的位置,使其大致呈平放狀態(如沿第3D圖所示之坐標之X軸向),令該連接件23得以位於該些定位件21之容置空間S中而抵靠該些釘本體20之頭端部20a,藉此串接所有的定位件21。
之後,如第3D圖所示,將複數固定件24沿該凹槽221進入該開口90中以分別對應螺接至各該定位件21上,以下壓抵靠該連接件23而固定住該連接件23。
最後,如第3E圖所示,移除或斷開該些把持件22,以縫合(如縫線91)傷口(即該開口90處),即完成脊椎固定手術,此時,該些釘本體20、該些定位件21、該連接件23及該固定件24係構成一固定組件2b(如第2D圖所示之態樣),使該些脊椎骨9a,9b,9c不會位移,因而病患不能再作動該些脊椎骨9a,9b,9c(如不能彎腰)。
因此,藉由本創作之骨釘結構2之把持件22之傾斜面22c之設計,以於進行脊椎固定手術時,可將其中一把持件22之弧面22d靠合相鄰之把持件22之傾斜面22c,使該些把持件22相疊合以縮小該些把持件22之間的夾角,因而能縮減該些把持件22於該皮膚表面9上所佔用之面積(如該開口90之大小),故相較於習知技術,於手術部位上能形成較小之開口90(其最大寬度R約3cm,如第3E圖所示),以符合傷口範圍達最小傷害之需求。
再者,可於該定位件21與該把持件22之間配置有切口25,如第2C圖所示之該定位件21之第二側21b與該把持件22之底端22b之間,以利於移除或折斷該把持件22。
又,當手術過程中,醫師認為該開口90視線不夠清楚時,可採用第2A’圖所示之頂端22a’為開放式之把持件22,以提供更寬廣的手術視野。另一方面,亦可將該把持件22之封閉式頂端22a配置有切口(圖略),故於手術進行中,醫師可依臨場需求折斷該封閉式頂端22a而變成開放式頂端22a’,以提供更寬廣的手術視野而利於將該連接件23引入該開口90中。
上述實施例係用以例示性說明本創作之原理及其功效,而非用於限制本創作。任何熟習此項技藝之人士均可在不違背本創作之精神及範疇下,對上述實施例進行修改。因此本創作之權利保護範圍,應如後述之申請專利範圍所列。

  1. 一種骨釘結構,係包括:釘本體;定位件,係具有相對之第一側與第二側,且該第一側係設於該釘本體之端部;以及把持件,係設於該定位件之第二側上,且該把持件之側面係為傾斜面。
  2. 如申請專利範圍第1項所述之骨釘結構,其中,該釘本體係為螺絲釘狀。
  3. 如申請專利範圍第1項所述之骨釘結構,其中,該定位件與該釘本體係能相對轉動。
  4. 如申請專利範圍第1項所述之骨釘結構,其中,該定位件係具有容置空間。
  5. 如申請專利範圍第4項所述之骨釘結構,其中,該把持件係具有連通該容置空間的通道。
  6. 如申請專利範圍第1項所述之骨釘結構,其中,該把持件係呈勺狀或瓢狀。
