一種血管穿刺封閉器,係可以封閉血管止血及減少手術程序,其包含:一套管組,包含一內套管及一外套管,該內套管之內部具有一第一容室,該外套管之內部具有一第二容室,該內套管可滑移地設於該第二容室;及一封閉件,該封閉件結合該內套管之一端,且包含一抵接部及一膨脹部,該抵接部具有相對之二表面,該抵接部具有貫穿該相對二表面之一穿孔,該膨脹部之內部具有一囊室,該囊室連通該抵接部之穿孔。

Description

血管穿刺封閉器
本發明係關於一種醫療器材,特別關於一種針對血管穿刺後可以封閉血管止血及減少手術程序的血管穿刺封閉器。
血管介入性技術,特別是心導管介入性診斷與治療,係指經皮穿刺血管,置放鞘管。經由置放的鞘管,可以從體外進入體內血管或其他管腔,並沿血管朝醫療標的進行醫療行為的手術方法。相較於開刀治療,具有傷口較小、復原時間較短的優點。
在血管介入性技術進行醫療行為後,必須移除留置的鞘管,此時血管管壁穿刺洞口必須封閉止血,防止在穿刺傷口處出血,造成嚴重的併發症。
為解決前述問題,除了以外力直接壓迫傷口止血數個小時外,醫者可以如第1圖所示,於血管介入性技術進行醫療行為後,於傷口處置入一習知血管穿刺封閉器9,以封閉血管壁上的傷口。該習知血管穿刺封閉器9包含一導線91、一第一封閉件92、一第二封閉件93及一推抵件94,該第一封閉件92設於該導線91之一端,該第二封閉件93可滑移地設於該導線91,該推抵件94則穿設於該導線91,使該第二封閉件93位於該第一封閉件92及該推抵件94之間。
當醫者完成血管介入性治療後,可以留置手術中所使用的鞘管I,即使該鞘管I的一端經由血管壁W上的開口O伸入血管V中,續將該習知血管穿刺封閉器9置入該鞘管I中,並利用該推抵件94,使該第一 封閉件92及該第二封閉件93沿該鞘管I朝向該血管V的方向移動,直到該第一封閉件92進入血管V中,醫者接著將該導線91及該鞘管I一同往回拉,使該第一封閉件92抵接於該血管壁W的內側,即可以完成該第一封閉件92的定位。
接著如第2圖所示,醫者可以再利用該推抵件94推動該第二封閉件93,使該第二封閉件93與該第一封閉件92之間的距離縮小,直到該第二封閉件93抵接於該血管壁W的外側,即可以完成該第二封閉件93的定位。最後,醫者可以移除該鞘管I及該推抵件94,並且將多餘的導線91剪除後,即可以完成該習知血管穿刺封閉器9的設置。
如此,藉由該第一封閉件92及該第二封閉件93分別抵接於該血管壁W的內側、外側,即可以對該開口O施予適當的壓力,而可以達成止血的效果;然而,實際操作上卻不容易將該第二封閉件93確實定位於位於血管壁W的外側之預定位置,因而使該習知血管穿刺封閉器9的止血效果仍顯不佳;且醫者必須分別進行該第一封閉件92及該第二封閉件93的定位,使用上較為麻煩而導致醫者拉長施術的時間,因此該習知血管穿刺封閉器9仍有加以改善之必要。
為解決上述問題,本發明提供一種血管穿刺封閉器,係可以有效封閉位於血管壁之開口者。
本發明另提供一種血管穿刺封閉器,係於使用上較為簡便者。
本發明之血管穿刺封閉器,係包含:一套管組,包含一內套管及一外套管,該內套管之內部具有一第一容室,該外套管之內部具有一第二容室,該內套管可滑移地設於該第二容室;及一封閉件,該封閉件結合該內套管之一端,且包含一抵接部及一膨脹部,該抵接部具有相對之二 表面,該抵接部具有貫穿該相對二表面之一穿孔,該膨脹部之內部具有一囊室,該囊室連通該抵接部之穿孔,該抵接部抵接於一血管壁的內側,該膨脹部接抵於該血管壁的外側,該囊室供血管之血液經由該穿孔流入該囊室。
本發明之血管穿刺封閉器,係藉由該抵接部抵接於血管壁的內側,並藉有血流經由該穿孔流入該囊室,使該膨脹部擴張,並封閉該血管壁之開口,且接抵於血管壁的外側,如此該封閉件可以確實定位於該血管壁之開口,同時分別對該血管壁的內、外側施予壓力,可以達成有效止血的功效;並且,當血流經由該穿孔流入該囊室,該膨脹部係藉由血流經由該穿孔流入該囊室而可以對該血管壁的外側施予壓力,醫者無須進一步定位該膨脹部,可以達成減少手術程序的功效。
本發明之血管穿刺封閉器,其中,該外套管設有一定位孔,該定位孔貫穿該外套管之管壁,並連通該第二容室;如此醫者可以依據存在於該第二容室之血液,得知該血管穿刺封閉器已經朝向該伸入方向移動至該預定位置。
本發明之血管穿刺封閉器,其中,該抵接部及該膨脹部呈一體相連,如此可以提升該封閉件於使用時的結構強度,可以有效對該血管壁的內、外側施予壓力。
本發明之血管穿刺封閉器,其中,該抵接部位於該第一容室中,該膨脹部經由該內套管之一端伸入並位於該第二容室中,如此可以降低該膨脹部在膨脹形成該預定形狀時受到該抵接部的干擾之可能性。
本發明之血管穿刺封閉器,其中,該封閉件由生物可相容性材料所形成,例如可以由聚氨酯、聚四氯乙烯、鈦金屬或鈦合金所形成,如此可以防止置於生物體體內之封閉件對生物體造成毒害作用;較佳地,該封閉件可以由生物可降解材料所形成,例如可以由乳酸聚合物所形成, 如此可以使該封閉件可以於生物體內被降解,並且生成可以由生物體所自行吸收的小分子,無須為了取出該封閉件而必須再次進行手術。
本發明之血管穿刺封閉器,其中,該囊室之內表面塗佈有凝血酶或甲殼素,如此當血液流入該囊室之內部時,可以受到凝血酶或甲殼素的作用,快速凝結成為固態,使該膨脹部迅速膨脹形成該預定形狀。
本發明之血管穿刺封閉器,其中,該抵接部設有一阻滯膜,該阻滯膜位於該穿孔,該阻滯膜具有數個通孔,如此可以防止該血管中的血液過於快速地沖入該囊室中。
本發明之血管穿刺封閉器,其中,該封閉件包含一牽引繩,該牽引繩結合該膨脹部之外周壁,如此醫者可以藉由該牽引繩改變該囊室內部的壓力,促使血液可以快速流入該囊室中。

為讓本發明之上述及其他目的、特徵及優點能更明顯易懂,下文特舉本發明之較佳實施例,並配合所附圖式,作詳細說明如下:本發明所述「血管穿刺封閉器」係可以搭配血管介入性技術所使用之鞘管I共同使用,如第5圖所示,該鞘管I之一端係能夠經由血管壁W上的開口O伸入血管V之中,而經由該血管穿刺封閉器的作用,即可以封閉該血管壁W上的開口O,防止該血管V中的血液經由該開口O流出,此為本發明所述技術領域中具有通常知識者可以理解,於此不再贅述。
請參照第3及4圖所示,本發明之第一實施例的血管穿刺封閉器係可以包含:一套管組1及一封閉件2。
詳而言之,該套管組1包含一內套管11及一外套管12,該內套管11具有相對之一第一端11a及一第二端11b,該外套管12具有相對之一第一端12a及一第二端12b,該內套管11之內部具有一第一容室S1,該外套管12之內部具有一第二容室S2,該內套管11可滑移地設於該第二容室S2;又,該外套管12之外徑係可以配合搭配使用的鞘管I之內徑,使該外套管12之外徑與該鞘管I之內徑可以呈適配的關係。
於本實施例中,該外套管12較佳可以設有一定位孔121,該定位孔121貫穿該外套管12之管壁,並連通該第二容室S2,當該外套管12之第一端12a凸伸超出該鞘管I之一端,使該定位孔121裸露於該鞘 管I之外時,血管中的血液可以經由該定位孔121流入該第二容室S2,如此醫者即可以瞭解已使該外套管12定位於一預定位置;或者,在該外套管12未設有該定位孔121之狀況下,醫者則必須利用經驗調整該外套管12凸伸超出該鞘管I之一端的距離,以使該外套管12定位於該預定位置。
該封閉件2結合該內套管11之第一端11a,舉例而言,該封閉件2具有一結合部21,該結合部21可以結合該內套管11鄰近該第一端11a之內周壁。又,該封閉件2包含一抵接部22及一膨脹部23,該抵接部22及該膨脹部23可以呈一體相連,藉此提升該封閉件2於使用時的結構強度,而可以有效對該血管壁的內、外側施予壓力。
於本實施例中,該抵接部22可以為一抵接板,具有相對之一第一表面22a及一第二表面22b,該抵接部22之第二表面22b朝向該內套管11之第一端11a,該抵接部22另具有一穿孔221,該穿孔221貫穿該第一表面22a及該第二表面22b;該抵接部22可以被彎折,使至少部分之第二表面22b可以朝向該內套管11之外周壁,且至少部分之第一表面22a可以朝向該外套管12之內周壁,如此該抵接部22之部分被限位於該內套管11之外周壁與該外套管12之內周壁之間。該膨脹部23結合該抵接部22之該第二表面22b,該膨脹部23之內部具有一囊室S3,該囊室S3連通該抵接部22之穿孔221,如此,該抵接部22可以位於該第一容室S1中,該膨脹部23則可以經由該內套管11之第一端11a伸入並位於該第二容室S2中。
較佳地,該封閉件2可以由生物可相容性材料所形成,例如可以由聚氨酯(polyurethane)、聚四氯乙烯(polytetrafluoroethene)、鈦金屬或鈦合金所形成,如此當該封閉件2被置於生物體的體內,以閉合血管後,該封閉件2不會對生物體造成毒害作用;較佳地,該封閉件2可以由生物可降解材料所形成,例如可以由乳酸聚合物所形成,如此該封閉件2 即可以於生物體內被降解,並且生成可以由生物體所自行吸收的小分子,無須為了取出該封閉件2而必須再次進行手術。並且,該囊室S3的內表面較佳可以塗佈有凝血酶或甲殼素等促凝血物質,因而當血液經由該穿孔221流入該囊室S3之內部時,可以受到該促凝血物質的作用,快速凝結成為固態,使該膨脹部23可以迅速膨脹形成一預定形狀。
如此,請參照第5及6圖所示,本實施例之血管穿刺封閉器於實際使用時,係可以該血管穿刺封閉器沿一伸入方向D1(即自患者體表朝向血管的方向)伸入該鞘管I,並使該內套管11之第一端11a、該外套管12之第一端12a可以穿過血管壁W並伸入血管V中。
接著如第7圖所示,醫者可以持續使該血管穿刺封閉器沿該伸入方向D1移動,直到該外套管12之定位孔121超出該鞘管I之一端,此時該血管V中的血液即可以如箭號所示,經由該定位孔121流入該第二容室S2。續如第8圖所示,在該第二容室S2中存在血液後,醫者即可以使該血管穿刺封閉器略沿一抽出方向D2(即自患者血管朝向體表的方向)移動,使該外套管12之定位孔121可以為該鞘管I所封閉,此時,該血管V中的血液將不再經由該定位孔121流入該第二容室S2,即完成該外套管12之定位。
再如第9圖所示,將該外套管12沿該抽出方向D2移動,使該內套管11之第一端11a凸伸於該外套管12之外,如此,原本部分被限位於該內套管11之外周壁與該外套管12之內周壁之間的抵接部22即可以伸展開。接著如第10圖所示,將該內套管11沿該抽出方出D2移動,使伸展開之抵接部22的第二表面22b可以抵接於該鞘管I之一端。續如第11圖所示,將該鞘管I及該血管穿刺封閉器共同沿該抽出方向D2移動,直到該抵接部22以該第二表面22b抵接該血管壁W之內側。
此外,請參照第6~11圖所示,血管V中的血液可以經由該 穿孔221流入該囊室S3,使該膨脹部23可以膨脹為該預定形狀,並且如第12圖所示,封閉該血管壁W上的開口O,且同時可以接抵於該血管壁W之外側,以與該抵接部22共同作用,使血管V中的血液不致經由該開口O流出,以達成閉合血管之目的。
另請參照第13圖所示,本發明第二實施例之血管穿刺封閉器的結構大抵可以與前述之第一實施例相同,惟於該第二實施例中,該抵接部22可以另設有一阻滯膜222,該阻滯膜222位於該穿孔221,該阻滯膜222具有貫穿該阻滯膜222之數個通孔223,如此可以防止該血管V中的血液過於快速地沖入該囊室S3中。
再請參照第14圖所示,本發明第三實施例之血管穿刺封閉器的結構大抵亦可以與前述之第一、第二實施例相同,惟於該第三實施例中,該封閉件2另包含一牽引繩24,該牽引繩24結合該膨脹部23之外周壁,且位於該內套管11之第一容室S1;因而醫者可以藉由沿該第二方向D2拉扯該牽引繩24,改變該囊室S3內部的壓力,促使血液可以快速經由該穿孔221流入該囊室S3中。
綜合上述,本發明之血管穿刺封閉器,係藉由該抵接部抵接於血管壁的內側,並藉有血流經由該穿孔流入該囊室,使該膨脹部擴張,並封閉該血管壁之開口,且接抵於血管壁的外側,如此該封閉件可以確實定位於該血管壁之開口,同時分別對該血管壁的內、外側施予壓力,可以達成有效止血的功效。
再者,當血流經由該穿孔流入該囊室,該膨脹部係藉由血流經由該穿孔流入該囊室而可以對該血管壁的外側施予壓力,醫者無須進一步定位該膨脹部,可以達成減少手術程序的功效。
雖然本發明已利用上述較佳實施例揭示,然其並非用以限定本發明,任何熟習此技藝者在不脫離本發明之精神和範圍之內,相對上述 實施例進行各種更動與修改仍屬本發明所保護之技術範疇,因此本發明之保護範圍當視後附之申請專利範圍所界定者為準。

Claims (11)

  1. 一種血管穿刺封閉器,係包含:一套管組,包含一內套管及一外套管,該內套管之內部具有一第一容室,該外套管之內部具有一第二容室,該內套管可滑移地設於該第二容室;及一封閉件,該封閉件結合該內套管之一端,且包含一抵接部及一膨脹部,該抵接部具有相對之二表面,該抵接部具有貫穿該相對二表面之一穿孔,該膨脹部之內部具有一囊室,該囊室連通該抵接部之穿孔,該抵接部抵接於一血管壁的內側,該膨脹部接抵於該血管壁的外側,該囊室供血管之血液經由該穿孔流入該囊室。
  2. 如申請專利範圍第1項所述之血管穿刺封閉器,其中,該外套管設有一定位孔,該定位孔貫穿該外套管之管壁,並連通該第二容室。
  3. 如申請專利範圍第1項所述之血管穿刺封閉器,其中,該抵接部及該膨脹部呈一體相連。
  4. 如申請專利範圍第1項所述之血管穿刺封閉器,其中,該抵接部位於該第一容室中,該膨脹部經由該內套管之一端伸入並位於該第二容室中。
  5. 如申請專利範圍第1項所述之血管穿刺封閉器,其中,該封閉件由生物可相容性材料所形成。
  6. 如申請專利範圍第5項所述之血管穿刺封閉器,其中,該封閉件由聚氨酯、聚四氯乙烯、鈦金屬或鈦合金所形成。
  7. 如申請專利範圍第5項所述之血管穿刺封閉器,其中,該封閉件由生物可降解材料所形成。
  8. 如申請專利範圍第7項所述之血管穿刺封閉器,其中,該封閉件由乳酸聚合物所形成。
  9. 如申請專利範圍第1項所述之血管穿刺封閉器,其中,該囊室之內表面 塗佈有凝血酶或甲殼素。
  10. 如申請專利範圍第1項所述之血管穿刺封閉器,其中,該抵接部設有一阻滯膜,該阻滯膜位於該穿孔,該阻滯膜具有數個通孔。
  11. 如申請專利範圍第1項所述之血管穿刺封閉器,其中,該封閉件包含一牽引繩,該牽引繩結合該膨脹部之外周壁。
