1239830 A7 B7 五、發明説明(5 ) 之實施例 材料及方法 對比介質 本發明利用一種極小超順磁性氧化鐵顆粒(USPIO)調配 物。該顆粒包含一氧化鐵核及一總直徑為25 nm之碳氧葡聚 糖殼層。在大鼠體内,其有效血漿半生期為56 fl7分鐘,且 LD50為35 mmol之鐵/公斤體重。在兔體内,估算之血漿半 生期大約為6小時。體積2-3 ml之該對比介質以200 //mol之鐵 /公斤體重劑量緩慢注射至健康一邊的耳靜脈,然後注射2 ml的一般生理食鹽溶液。 動物模型 本實驗之實行符合德國動物保護法。responsible national authority授與許可。使用兩種性別之Chinchilla bastard兔(2.6-3.8公斤)。所有研究以皮下注射40毫克/公斤之氯胺酮-HC1 (ketanest-50,Parke-Davis GmbH, Berlin)及 17 毫克 / 公斤之 xylazine-HCl (Rompun 2%,Bayer,Leverkusen)進行低度麻醉。 在血栓導入前,頸部靜脈先利用耳靜脈注入標準對比介質 之放射靜脈攝影兩階段記錄。以一種類似Wessler所使用的 方法,藉由血流停滯方式及高凝固性血液產生栓塞性鬱 滯。[Wessler S. Braun, Melsungen)以 1 : 1 比 例混合之栓塞物以一方面使血栓混合物黏附至血管壁,另 一方面血栓與血液可一產生之方式緩慢注射。當血管閉合 後,100單位之牛凝血酵素透過第二微導管注入。該栓塞可 以導致血管已可導通形式封閉數天。該栓塞可以使得放射 性靜脈攝影之X射線因為碘之比例而無法通過並且由於T1 加權的MRT影像在粘著性固著血液產生衰減而以高強度方 式顯影。 實驗設計 五十組帶有血栓之動物試驗隨機安排至對比介質組(D)或 控制組(K)。根據第一次MRT量測時間之血栓年齡再作相對 細分(1,3,5,7或9天)。因此,可以各別再分為5個對比介 質組(Dl,D3,D5,D7及D9)及 5 個控制組(ΚΙ,K3,K5,K7及 K9)。每一小組包括五隻實驗動物。所有實驗動物,在開始 起始MRT前先以放射靜脈攝影作記錄。起始MRT之後,對 比介質組之動物(K)接受對比介質,並在間隔24小時後,接 受測定-MRT。例如,在D3組中在D 0天血栓後,起始MRT在 第三天執行,且測定-MRT在第四天執行。對於控制組,只 有ΚΙ,K3及K5動物組以不固定時間間隔接受測定-MRT,而 在先前實驗開始時量測之K7及K9之動物組不必。

Three-dimensional Magnetization-Prepared Rapid Gradient-Echo Imaging (3D MP RAGE) Magn Reson Med 1990,15:152-7]。所使用之序 列會藉由序列特有選擇性水激發脈衝抑制脂肪訊號並藉由 延遲時間之選擇抑制血液訊號。[Moody,A. R·; Pollock,J. G·; O’Connor,A. MRT 在Tl-加權之MP-RAGE序列之血栓在3維重建術後可看見 如螺蟲狀之高強度結構(圖1及2 )。在對比介質動物組例子 中’在測定量測中發現,就剩餘血管攝影術而言,較大頸 部血管之訊號輕微增加。在不考慮血栓年齡之狀況下,κ 組起始MRT之3維-MP-RAGE序列與靜脈攝影之相對血栓長度 吻合度為0.2 ±0.3,而D組’ 0.1 ±〇·3。d組動物(n=25)中,在24 小時内測定發現一致的相對血管長度〇1 ±〇·3在對比介質組 24小Β可上升之〇·5 ±0.5月i顯著增加,ρ =〇·〇〇1 (圖1,2及4);但 控制組並無此現象(只有組ΚΙ,K3及K5),ρ = 〇·34。個別天 組之分析顯示血栓分部顯影與ΜΡ-ragE序列在血栓年齡之 相依性(圖4)。其為在ΚΙ,K3,及K5之低血栓年齡組由〇2土 0.3到0.4 ±0.5,或在K7,及K9之低血栓年齡組由〇1 ±〇1及〇。在 對比介質處理後,可見之血栓分部明顯增加顯示在下列各 組中·· D3,由〇·6 ±4至0.8 ±0.4; D5,由0.1 ±0·^ ±(u ;及〇7, 由0至0.6 ±0.4。在對比介質動物組D1&D9中沒有發現重大差 異。 中等T2*-FLASH-序列中顯示一可預測之血栓-年齡-相依 之結構,訊號強度及對比範圍的變化。一個別實施例可參 閱圖5。在對照組中,”異質性_隨機”及”異質性-集中,,血 栓結構加權至第7天(圖6)。由第7天開始,”同質型”血栓 結構在控制組中較多。在控制組動物K1中,血栓之領先訊 號強度係”中等,”意即,與肌肉等強度,但是有清晰之散 射(圖7)。直到第3天,血栓之訊號強度在控制組動物中增 -14- ^紙張尺度適财目目家鮮(CNS) A4^(21GX297公董) --—-- 1239830 A7 B7 五、發明説明(12 ) 加並傾向”強訊號”且降低直到第9天至強度定義為”小或無 机號π。新鮮血栓之對比範圍在K1組中平均為1.6而K9組中 平均為0.4(見圖8 )。 农後’只有Κ組中只有四隻動物在對比介質處理後發現 中等Τ2*-加權FLASH序列之訊號降低,其可能為對比介質 效應(圖5 )。王組或由血检年齡分組之起始量測與測定量 測之比較顯示血栓結構(圖6),血栓訊號(圖7)或血栓對比 範圍(圖8 )沒有重大改變。 在50隻動物中之6隻,在自然環境下,一部分發生訊號 衰減(n = 4)或血管壁之環形訊號衰減(n = 2)及緊接血管周 圍區域發現T2*-加權序列代表層。在對比介質處理後,這6 隻動物均無訊號存在。在U之其他動物_,只有在測定控 制發現沒有血管壁及緊接Α管周圍區域訊號,2隻動物顯 示環形及9隻動物部分顯示。在這丨丨隻動物中,一隻為控 制組。其他1〇隻均接受對比介質。在這1〇隻對比_增=實: 中,4隻為D1組,且4隻來至D3,音即,4 心即,祥脈壁之對比介質 漠度及緊接周圍血管區域在年輕之血栓時期較易發現每5 個例子中有4個。 圃玩之簡要解釋 圖1 係放射性靜脈攝影⑷顯示在血栓誘導三天後之不 頸外靜脈壁鎖。頸外靜脈進一步由中央心臟靜脈二= 面靜脈吸收。對比介質流出物在手術例子中由對側二頁
