201138861 六、發明說明: 【發明所屬之技術領域】 本發明係關於一種眼科手術溶液,尤指一種眼科手術 沖洗液。 【先前技術】 沖洗液常被廣泛用於如晶體乳化術 (phacoemulsification)、玻璃體切除術(vitrectomy surgery) 及青光眼手術(glaucoma surgery)之眼科手術過程中。晶體 乳化術係一種摘除眼内混濁晶體(稱之為白内障)之手術。玻 璃體切除術係一種將部份或全部玻璃狀液(vitreous humor) 從眼睛移除之手術。青光眼手術則涉及雷射治療或於眼球 中形成切口,以降低眼壓。根據中央健康保險局報導指出, 台灣每年進行約150,000次的眼科手術。眼科手術之效果與 所使用的沖洗液息息相關。不適宜的沖洗液可能會對角膜 或晶體造成傷害,因而導致視力變差、產生盲點、甚至失 明。 理想的沖洗液應當具有接近水樣液(aqueous humor)之 組成及290 mOsm至500 mOsm間之渗透壓(osmolarity)。沖 洗液的主要功能係用來維持内皮細胞完整性、角膜厚度及 視網膜組織。此外,適宜的沖洗液應可於白内障手術期間 保存角膜内皮細胞活性,提供能量來源(即葡萄糖),維持適 當機能及電解質濃度,並保護角膜内皮細胞避免pH值波動。 201138861 平衡鹽溶液(BSS®)及BSS PLUS®已常用於眼内沖洗β BSS PLUS®的組成與水樣液組成相近jsS PLUS®基本上具 有四部份組成:1)足量緩衝劑(即碳酸氫鈉),2)能量來源(即 葡萄糖),3)於7至8間之穩定PH值(即HEPES),4)抗氧化劑(即 麵耽甘肽,glutathione)。然而,該些眼内沖洗液無法對眼 科手術中最容易受到物理性傷害的角膜(内皮)細胞提供有 效充分的保護。研究指出,複雜的眼科手術(如晶體乳化術) 可能會導致嚴重的併發症。導致併發症之部分可能因素包 括:超音波的機械性作用、未吸出之晶體碎片導致的物理 性傷害、沖洗液導致熱產生甚至滲透不正常。這些因素皆 可能對角膜内皮細胞造成傷害,甚至有導致不可回復之水 泡性角膜病變(bullous keratopathy)的風險。 因此’本領域迄今仍有上述缺失及不足尚須解決,尤 需發展一種對眼内組織,尤其是角膜(内皮)細胞,具有較佳 保護功能之眼科手術沖洗液。 【發明内容】 本發明之一態樣係關於一種含量足以沖洗患者眼部組 織之眼科手術沖洗液’其包括:a)增加該沖洗液黏度之有 效量γ-聚麩胺酸(γ-PGA)及/或其鹽類;以及b) —用於 γ-PGA及/或其鹽類之眼部可接受水性載體,用以沖洗患者 之眼部組織》 本發明之另一態樣係關於一種眼科手術沖洗液,其包 括:a)增加該沖洗液黏度之有效量γ·聚麵胺酸(γ-PGA)及/ 或其鹽類;以及b) —用於γ-PGA及/或其鹽類之眼部可接 201138861 受水性載體。 本發明之再一態樣係關於一種醫藥套組,其包括:a) 如上述之眼科手術沖洗液;以及b) —說明書(package insert) ’其包含印製成的使用指南,以說明沖洗患者眼部組 織之用法。 本發明之更一態樣係關於一種含量足以於眼科手術期 間沖洗患者眼部組織之眼科手術沖洗液,其包括:a)增加 該沖洗液黏度之有效量γ-聚麩胺酸(γ-PGA)及/或其鹽類; 以及b) —用於γ-PGA及/或其鹽類之眼部可接受水性載 趙’用以眼科手術期間降低壓力對患者眼部組織所引發之 傷害。 下述較佳實施例配合隨附圖式可使該些及其他態樣更 為清楚,但亦可於不悖離該揭露之新穎概念精神及範疇下 進行變化及修飾。 隨附圖式可說明本發明一種以上具體實施例,且配合 文字敘述,可解釋本發明原則。全部圖式盡可能使用相同 元件符號來代表一具體實施例中相同或相似元件。 【實施方式】 基於本發明背景及每一用詞使用之特定背景使用於 本案說明書之用詞通常具有本領域中之一般涵義。用於描 述本發明之某些用詞會於下文或說明書其他地方敘及以 額外引導實施者瞭解本發明内容。為求方便起見,文中會 使用如斜體及/或引號來標顯某些用詞。使用該些標顯並不 會對用詞範圍及涵義造成影響,無論用詞是否被標顯出 201138861 來,相同背景下之用詞範圍及涵義是相同的。相同事物可 藉由一種以上的方式來敛述。因此,此處所敘及之一或多 個用詞可能會使用另一種語言及同義詞來表示,無論是否 對用詞加以闡述或討論’其也不會有任何特別的含意。本 文會提供某些用詞的同義詞。舉出的一或多種同義詞並不 排除使用其他同義詞之可能。本案說明書任何地方所舉出 的舉例’包含此處敘及的任何用詞’只是作為說明用,不 應侷限本發明或示例性用詞之範嘴或涵義。同樣地,本發 明並不限於本案說明書所舉的各種具體實施例。 除非有其他定義,否則此處所使用之技術及科學用詞 涵義與本發明所屬技術領域中具有通常知識者一般理解之 涵義相同。若有所差異,則本文(包括定義)會載明。 此處使用之「大約」、「大略」或「接近」一般意指 所提供值或範圍之20%内,較佳為10%内,更佳為5%内。 此處所提供的數值為接近值,亦即,若未特別載明,則表 示其為「大約」、「大略」或「接近」。 當敘及數值或範圍時,本領域具有通常知識者可瞭 解’其意指包含對本發明相關特定領域而言適當合理之範 圍。 就0.32至50厘泊黏度而言,其代表本發明已具體揭露 該範圍中之所有百分之一、十分之一及整數單位值。因此, 0-32、0.33、0.34 . . ·、〇_7、0.8、0.9、1、2、3、4 . 47、 48、49及50厘泊單位值皆包含於本發明之具體實施例中。 就290至320 mOsm滲透壓而言,其代表本發明已具 201138861 體揭露該範圍中之所有整數單位值》因此,290、291、 292 . . .、317、318、319及320 mOsm單位值皆包含於本 發明之具體實施例中。 就1.330至1.344折射率而言’其代表本發明已具體揭 露該範圍中之所有千分之一單位值。因此,i 33〇、1 331、 1.332 ·..、1.340、1.341、1.342、1.343 及 1.344 單位值皆 包含於本發明之具體實施例中。 就10,000 to 2,00〇,〇〇〇道爾頓(Daltons)分子量而言,其 代表本發明已具體揭露該範圍中之所有整數單位值。因 此,10,000、10,001,10,002 、1 999 997、1 999 998、 1,999,999及2,00〇,〇〇〇道爾頓單位值皆包含於本發明之具 體實施例中。 就0.2〜1% (重量/體積,w/v)而言,其代表本發明已具 體揭露該範圍中之所有十分之一及整數單位值。因此,〇2、 〇·3、0·4 . . .、〇.7、0.8、0.9及1%單位值皆包含於本發明 之具體實施例中。 在此所使用之「γ-PGA」一般意指「γ-聚麵胺酸及/或 其鹽類」或「γ-聚麵胺酸醋(gamma-polyglutamate)」。聚楚 胺酸具有2個羧基。其中一個羧基係與α—胺基連接,形 成 PGA。 可交替使用γ-聚(麩胺酸)及γ-聚麩胺酸(γ-PGA)用詞。 在此所使用之「增加沖洗液黏度之有效量」一般意指, 相較於未添加γ-PGA之沖洗液黏度,γ-PGA之存在使沖洗 液黏度增加。 201138861 交聯聚麩胺酸係由數千萬分子量之網狀結構所組成。 相較於PGA ’交聯PGA具有較高的吸水力。交聯聚麵胺酸 分子量大於10,000,000。 在此所使用之「水性生理可接受溶液」一般意指,但 不限於,無菌食鹽或無菌緩衝溶液。 在此所使用之「抗氧化劑」一般意指,可減緩或避免 其他分子氧化之分子。抗氧化劑包括,但不限於,麩胱甘 肽(glutathione)、維他命c及維他命E。 滲透作用係溶劑分子穿過選透膜進入高溶質濃度區域 之移動’俾使兩側的溶質濃度相等。溶劑的淨移動方向係 由較低濃度溶液(低滲透溶液)往較高濃度溶液(高滲透溶 液)。可藉由使高滲透溶液之壓力相對於低滲透溶液來的 兩’以逆轉該作用。渗透壓係指維持平衡之壓力,使溶劑 無淨移動現象。滲透壓係取決於溶質的莫耳濃度而非溶質 本身。滲透作用於生物系統中是極為重要的,許多生物膜 具有半透性。 滲透壓(osmolarity)為溶質濃度之度量單位,其定義為 每公升(L)溶液之溶質滲透莫耳數(〇sm〇is, 〇sm)(即
osmol/L 或Osm/L)。溶液的滲透壓通常表示為〇sm/l^滲透壓係測 量每單位體積溶液之溶質滲透莫耳數。 本發明發現,使用黏彈性材料聚_了_麩胺酸(Y_pGA)作 為沖洗液中之額外成分,可減少眼部手術所造成的傷害。 於as體乳化及吸除(PEA)期間,分散性黏彈性材料可對眼内 組織發揮保護的正面效果^彈性材料可降低眼睛前房及 201138861 後房内之擾動,協助控制眼内組織碎片及氣泡移動❶此外, 此類黏彈性材料有助於移除晶體碎片,使外科醫生更易藉 由外科手術用手持用具之尖端找到碎片。 聚-7*-麵胺酸(γ-PGA)’ 一種麩胺酸(GA)胺基酸之天然 聚合物’可藉由數種細菌、古菌(archaea)及真核生物 (eukaryote)來合成。
聚-7 -麩胺酸具有約1〇,〇〇〇至2百萬範圍之分子量。 其可被製成符合不同應用之條件。已知有食品工業上的應 用。γ-PGA是納豆(一種日本傳統食物)黏液之主要組成份, 其於1937年首次被Ivnovics發現存在炭疽桿菌(BaciUus anthracis )的莢膜中β γ_ρ〇Α係一種特有的聚陰離子性聚合 物,其係由透過γ-醯胺鍵(於α_胺基與γ_羧基之間)連接之D form及/或L form麩胺酸殘基所構成。其為具有親水性、 黏滯性、生物可降解性及無毒的生物材料。基於其特有的 離子捕獲及高吸水特性,其已被廣泛用於各種應用中如 金屬螯合劑、吸收劑、防凍劑、老化抑制劑、藥物載體及 保濕劑。過去幾年,Y_PGA已被廣泛使用作為生物材料, 其具有細微膨脹性。其生物相容性增加於臨床領域上之實 用性’如生物膠、組織工程及藥物傳輸系統。 201138861 本發明發現γ-PGA可調整眼部可接受沖洗液之滲透壓 及黏度°眼科手術沖洗液之適當滲透壓及微黏性可降低併 發症及角膜傷害。本發明係關於使用γ_Ρ〇Α作為眼部可接 受沖洗液或手術溶液添加劑之用途,俾於需要沖洗之手術 期間保護眼睛前房及後房。 γ-PGA係作為調整滲透壓並避免組織水腫之用劑。含 有tPGA之眼部可接受沖洗液滲透壓範圍為290〜320 mOsm 〇 高分子量(1020k道爾頓)Y-PGA可調整沖洗液黏度。理 想黏度可降低晶體乳化術造成的組織傷害。 目前有許多種不同的γ-PGA載體。然而,本發明之沖 洗液載體具有較佳性質係包括電解質、緩衝劑(如碳酸氫 鹽、磷酸鹽或其組合)及能量來源。該些試劑有助於手術期 間維持角膜組織之正常功能,促進術後視力快速回復。然 而,本發明對於平衡鹽溶液或可能用來形成沖洗液之其他 電解質/營養物溶液種類並無限制。 本發明之一態樣係關於一種沖洗患者眼部組織之方 法。該方法包括下述步驟:將足量之眼科手術沖洗液引至 患者的眼部組織,以沖洗患者眼部組織,其中該沖洗液包 括·· a)增加該沖洗液黏度之有效量7_聚麩胺酸(γ_ραΑ)及/ 或其鹽類;及b)—用於γ-PGA及/或其鹽類之眼部可接受水 性載體。 本發明之另一態樣係關於一種眼科手術沖洗液,其包 括:a)增加該沖洗液黏度之有效量7聚麩胺酸G 及/ 201138861 或其鹽類;及b)—用於γ-PGA及/或其鹽類之眼部可接受水 性載體。 本發明之再一態樣係關於一種醫藥套組,其包括:a) 如上述之眼科手術沖洗液;及b) —說明書(packageinsert), 其包含印製成的使用指南’以說明沖洗患者眼部組織之用 法。 於本發明之一態樣中,該沖洗液具有〇.32至5〇、或 0.32至30、或0.32至3.93厘泊之黏度。 於本發明之另一態樣中,該沖洗液具有每升290至320 mOsm之滲透壓。 於本發明之另一態樣中,該沖洗液具有〇·32至5〇、或 0.32至30、或0.32至3.93厘泊之黏度,及每升29〇至32〇 mOsm之渗透壓。 於本發明之另一態樣中,該沖洗液具有L330至1 344 之折射率。 於本發明之另一態樣中,該沖洗液不包含交聯聚麩胺 酸’且不具額外的聚胺基酸或聚合物。 於本發明之另一態樣中,該眼部可接受水性載體包括 一含有電解質之平衡鹽溶液、緩衝劑及能量來源。 於本發明之另一態樣中,γ-PGA於該眼科手術沖洗液 中之濃度範圍為0.2〜1%或0.2〜0.8% (w/v)。 於本發明之另一態樣中’ γ-PGA具有ι〇,〇〇〇至 2’000’000道爾頓或1,〇〇〇,〇〇〇 to 2,000,000道爾頓之分子 量。 201138861 於本發明之再一態樣中,該眼部可接受水性載體更包 括一抗氧化劑。 於本發明之又一態樣中,該發明係關於一種降低眼科 手術中壓力對患者眼部組織造成傷害之方法。該方法包括 下述步驟:將足量之眼科手術沖洗液引至患者的眼部組 織,以沖洗患者眼部組織,其中該沖洗液包括:a)增加該沖 洗液黏度之有效量7-聚麩胺酸(γ-PGA)及/或其鹽類;及b) 一用於γ-PGA及/或其鹽類之眼部可接受水性載體。 於本發明之另一態樣中,該眼科手術包括玻璃體切除 術(surgical vitrectomy)、白内障摘除術(cataract extraction)、晶體吸除術(lens aspiration)、眼前段重建術 (anterior segment reconstruction)及晶體乳化術 (phacoemulsification) 〇 實施例 本發明具體實施態樣之示例性儀器、設備、方法及其 相關結果如下所述,其並不侷限本發明之範疇。需留意的 是,為方便讀者理解,實施例可能會使用標題或副標題, 但皆不應侷限本發明之範疇。此外,在此提出及揭露的某 些理論,無論對錯與否,皆不應侷限本發明之範疇,只要 本發明可據以實施即可,無需探討任何特定理論或作用機 制。 材料及方法 材料及試劑 若未特別註明,所有材料及試劑皆購自 12 201138861
Sigma-Aldrich公司(美國密蘇里州聖路易市)。聚-χ-麵胺 酸(分子量為1020k Da)係購自VEDAN企業股份有限公司。 滲透壓儀標準品係由Advanced Instruments公司(美國麻州 諾伍德市)提供。 抗生素、騰蛋白酶及胎牛血清係由Invitrogen (美國 加州卡爾斯巴市)提供。細胞培養盤及培養槽係由Orange Scientific(比利時布賴恩拉勒)提供。快速細胞增生分析套組 係購自BioVision (美國加州),而細胞毒性分析係購自 Promega (CytoTox 96 Assay kit,美國威斯康辛州)》牛角膜 内皮細胞(bCE cel丨s)及人類視網膜色素上皮細胞(hRPE cells)係來自細胞科學國家中心(台灣新竹食品工業發展研 究所)。 方法 沖洗液滲透壓評估 使用Advanced Instrument公司所生產之3D3單次樣 品渗透壓儀(3D3 Single-Sample Osmometer)測量溶液渗透 壓。製備含有不同濃度 7 -PGA(0.2%,0.4%,0.6%,0·8%,1%) 之沖洗液及不含7* -PGA之控制組,以評估r -PGA的影響。 使用校正標準品(每升1〇〇及1500 mOsm),對滲透壓儀進 行校正。含有T -PGA之沖洗液pH值為7.4±0.1。使用碳酸 氫鈉作為pH緩衝劑,並使用二次蒸餾水來製備沖洗液。 沖洗液黏度評估 使用具有平行板(parallel plate geometry)配件的旋轉 流變儀之 HAAKE RheoStress 600 (Thermo Fisher Scientific 13 201138861
Inc., Waltham, MA, USA)儀器評估沖洗液黏度。藉由控制單 元來控制溫度。於室溫(25°C)及體溫(37°C)兩種工作溫度下 進行評估。將上部不鏽鋼板(直徑為35mm)及底部不鏽鋼板 間之間隔高度調整為1.05 mm。藉由速控旋轉斜面模式 (controlled rate rotation ramp mode),測得沖洗液的黏度曲 線。剪切率範圍係由Is·1至500s·1。 沖洗液折射率評估 使用DR-A1折射儀(日本ATAGO)測量折射率(RI)。 於室溫下製備含有0.2、〇.4、0.6、0.8及1%(重量/體積, w/v)«r -PGA之沖洗液,並小心置放於棱鏡上。當透過接目 鏡察看時’轉動控制鈕直到陰影線位於十字準線中心。再 由數位螢幕得知折射率。 T -PGA生物相容性評估 將不同濃度的r -PGA加至細胞培養液中,以評估生 物相容性。以200毫升的細胞培養液對單層角膜内皮細胞 及視網膜色素内皮細胞作測試。以5 X1 〇3細胞/孔之細胞密 度,將細胞接種於96孔組織培養盤上,於37。(:立5%二氧 化碳培養條件下’使細胞貼附於培養盤底部隔夜後,對負 控制組(以Ah。3萃取的細胞培養液)、正控制組(含〇. 1 % Triton X-100之細胞培養液)及實驗組(含〇 2%,〇 4%,0.6%, 0.8¾及1 % γ -PGA之細胞培養液)之測試組進行六重複測 試。於37。<:下培養24小時及72小時後,使用快速細胞增 生分析套組 II(Quick Cell Proliferation Assay Kit II)及 CYTOTOX 96®非放射性細胞毒性分析(CYT〇T〇x 96® 201138861
Non-Radioactive Cytotoxicity Assay),對細胞存活率及細胞 毒性進行定量分析。 於培養72小時後,將測試培養液換掉,且每孔改換 成0.2 ml水溶性四哇氣鹽_8(tetrazolium-8,WST-8)工作溶 液,以進行細胞存活率評估。於培養2小時後’由於細胞 線粒體脫氫酶(cellular mitochondrial dehydrogenases)會使 四嗤氮鹽裂解形成甲膜(formazan),故WST-8工作溶液之 顏色會改變。藉由光譜儀,讀取450 nm位置上的訊號,以 對角膜内皮細胞及視網膜色素内皮細胞之存活率進行定量 分析》參考波長為65〇 nm。 將0·05 ml培養液移入96孔ELSA盤中,並與0.05 ml 的基質混合液(substrate mix)混合,再避光培養30分鐘,以 進行細胞毒性評估。基質混合液(substrate mix)中的四峻氮 鹽會與乳酸脫氣酶(lactate dehydrogenase, LDH)反應,以形 成紅色甲賸(formazan)產物。藉由光譜儀,讀取450 nm位 置上的訊號’對釋入培養液中之LDH進行定量評估。另外 也對未培養細胞的培養液進行分析,以作為培養液的背景 值。使用細胞溶解液(1% TRITON® X-100),對角膜内皮細 胞及視網膜色素内皮細胞進行細胞溶解,並讀取OD490值。 細胞毒性百分比如下所示:

對於含有0.2、0.4、0.6、0.8及1%(重量/體積)γ-PGA 15 201138861 之細胞培養液中的細胞進行1天及3天的染色,分別使用 LIVE/DEAD 染色套組(Molecular Probes # L3224, Eugene, Oregon,USA)染色,並使用NIS Element軟體拍攝。 7 -聚(麩胺酸)類眼科手術沖洗液之體内研究 對三隻紐西蘭白兔(2〜3 kg)的六隻眼睛進行試驗。肌肉 注射 ketalar/Chanazine 2% (Ketamine : 22 mg/kg BW ; Xylazine : 4-6 mg/kg BW)進行全身麻醉後,執行手術。於 手術顯微鏡下,將兩支靜脈留置針(vein detained needle)小 心穿過角膜而不碰到晶體(圖8A)»將其中一支靜脈留置針 與填滿試驗眼科沖洗液的瓶子連接,而另一支則與空瓶連 接(圖8B)。使用蠕動幫浦(流速為5mL/min),於37°C下用 沖洗液對角膜内皮進行灌注60分鐘(圖8B)。右眼使用 0.4%(重量/體積)之r -聚(麩胺酸)類眼科手術沖洗液沖洗, 而左眼使用生理食鹽水沖洗。於灌注期間,每隔5至10分 鐘測量角膜厚度。 角膜厚度測量 使用具有手控變換器之超音波角膜厚度測量儀 (ultrasonic pachymeter) DGH 550 (DGH Technology),測量 中心處的角膜厚度。DGH 550係使用回音突波技術(echo spike technique )測量角膜厚度之超音波角膜厚度測量儀。 於灌注期間,超音波角膜厚度測量儀每隔5至10分鐘會測 量角膜厚度。 統計分析 至少作三次統計分析,其結果以平均值±標準差(SD) 201138861 來表示。藉由變異數分析(analysis of variance,簡稱ANOVA) 來評估γ-PGA於生物相容性上的影響。值小於0.05表示 有統計學上的明顯差異。 結果 含γ-PGA沖洗液之滲透壓 右旋糖(dextrose)及γ -PGA對沖洗液滲透壓的影響 分別如圖1Α及1Β所示。表1則顯示含右旋糖之沖洗液組成 (圖1A)及含τ -PGA之沖洗液組成(圖1B)。 表1 組成份 (mM) 含右旋糖之沖洗液 含y -PGA之沖洗液 NaCl 122 122 KC1 5.08 5.08 CaCl2 1.05 1.05 MgCl2 0.98 0.98 NaHC03 25.0 25.0 Na2HP04 3.0 3.0 HCi 或 NaOH 調整pH值至7.2〜7.4 調整pH值至7.2〜7.4 左旋糖 0〜1 - 7 -PGA - 0 〜1(0/〇) 含 5mM右旋糖及0.2、0.4、0.6、0.8及 1% «τ -PGA之 沖洗液滲透壓分別為304、309、314、3 19及325 mOsm(圖 1C)。當τ -PGA濃度由1%降至0.2%時,含5mM右旋糖之沖 洗液滲透壓由325降至304 mOsm。然而,含1、2、3、4、 17 201138861 5mM右旋糖但不含r _PGA之沖洗液滲透壓分別為295、 296、296、298、300 mOsm (圖1A)。因此,右旋糖對沖洗 液滲透壓的影響小於Y-pGA。 含r -PGA之沖洗液黏度 圖2顯示γ-PGA會使沖洗液黏度呈濃度依賴型增加, 而5 mM右旋糖並不會對黏度造成影響。含5mM右旋糖及〇, 0.2、0.4、0.6、0.8或1% r -PGA之沖洗液黏度於室溫下(25。〇 分別為0.62、1.15、1_72、2.39、3.11及3.93厘泊(〇?)(表2)。 當溫度提高至體溫(37°C)時’沖洗液黏度會下降。含5mM 右旋糖及〇、0.2、0.4、0.6、0.8或1% γ-PGA之沖洗液黏度 於37。(:下分別為0.32、0.75、1.20、1.73、2.33及2.96 cP(表 3)。 含γ-PGA之沖洗液折射率 含0_25、0.5、0.75及1% (w/v)y-PGA之沖洗液折射率 分別為1.3346、1.3350、1.3355及1.3360(圖3)。沖洗液之組 成包括 122 mM NaC卜 5.08 mM KC卜 1.05 mM CaCl2、0.98 mM MgCl2、25·0 mM NaHC03、3.0 mM Na2HP04、0〜1% (w/v)γ-PGA、及用來將pH值調至7.2〜7.4之HCl或NaOH。 表2 25°C下之黏度(cP) γ-PGA (%) 右旋糖濃度 0 mM 5 mM 0 0.59 0.62 201138861 0.2 1.11 1.15 ' 0.4 1.71 1.72 0.6 2.39 2.39 0.8 3.16 3.11 1.0 3.98 3.93 表3 37°C下之黏度(cP) γ-PGA (%) 右旋糖濃度 0 mM 5 mM 0 0.48 0.32 0.2 0.70 0.75 0.4 1.26 1.20 0.6 1.84 1.73 0.8 2.36 2.33 1.0 3.01 2.96 γ-PGA生物相容性 於含γ-PGA之培養液中培養牛角膜内皮細胞(bcE cells)及人類視網膜色素上皮細胞(hRPE cells),並於第1天 及第3天時,使用WST-8及LDH分析評估細胞存活率及細胞 毒性(圖4A-B及圖5A-B)。WST-8分析係用來測量存活細胞 的數量。以 0.2%、0.4%、0.6%、0.8%及 l%(w/v)y-PGA處理 bCE細胞,且於第1天測得取自bCE細胞之培養液〇]〇45<)謂分 別為 0.20 ± 〇.〇2、0.19 土 0.02、0.20 ± 0.02、0.19 ± 0.01 及 0.17 土 0.04 ;而第 3天分別為 0.53 ± 0.05、0.47 ± 0.04、0.46 19 201138861 ±0.10、0.44±0.(^0.41±0·03(^Α)〇α〇2%、〇4〇/〇、 0·6〇/。、0.8%及 l〇/〇(w/v)Y_PGA處理hRpE細胞,且於第1天測 得取自hRPE細胞之培養液〇D45〇nm分別為〇 47 土 〇 〇5、〇 5 i ± 0.05、0.52 ± 0.02、0.51 ± 〇.〇6及0.47 ± 〇.〇7,而第 3天分別 為 0.88 ± 0.08、0.88 ± 0.03、0.81 ± 〇.〇7、0.78 ± 0.09及 0.79
± 0.10(圖 4B)。濃度為 0.2 至 1〇/〇2γ_ρ〇Α 對 bCE 細胞及 hRpE 細胞存活率沒有影響。 對細胞膜損害或破裂進行定量分析,以測定細胞死 亡狀況。LDH細胞毒性測試係一種測定死亡及細胞膜損害 細胞之比色測定法。因細胞膜損害而存在培養上清液中之 LDH會參與偶合反應,將黃色四唑氮鹽轉變成紅色的甲臢 型染料’其可於492 nm處測得吸收峰。曱牘含量會與培養 液中之LDH含量呈正比,進而與死亡或損害細胞數量呈正 比。第 1 天所測得之0.2%、0.4%、0.6%、0.8%及 1% γ-PGA 培養液中bCE細胞毒性百分比分別為7.77 士 3.5%、8.90 士 3.5%、5.76 士 2,8%、10.86 ± 2.8°/〇及 10.65 士 2.5¾ ;而第 3天 分別為 16.11 ± 5.8%、12.11 ± 6.6%、8.66 ± 4.0%、4.61 ± 3.7〇/〇 及2.76 ± 0.4%。第 1天所測得之〇.2〇/0、〇.40/0、0.6%、0.8% 及1% γ-PGA培養液中hRPE細胞毒性百分比分別為2.97 土 0.6%、2.74 ± 0.4%、3.95 ± 1.0〇/〇、2.99 ± 1.0%及 4.78 ± 0.9% ; 而第 3天分別為 2.85 ± 0.3%、2.77 ± 1.5%、3.74 土 1.5%、8.11 ± 1.9%及10.37 ± 0.9%。含γ-PGA培養液與負控制組間並無 明顯差異(圖5A-B)。細胞毒性百分比,以死亡細胞數量除 以總細胞數量來表示,其係如下式算得:細胞毒性(%)=(培 20 201138861 養液O.D.-空白O.D.)/(總溶解O.D.-空白〇.D_)xlOO。 細胞螢光染色 存/亡細胞染色套組(Live/Dead staining kit)係使用妈黃 綠素-AM(calcein-AM)及乙鍵(ethidium homodimer,EthD-1) 兩種螢光染劑。鈣黃綠素-AM係一種廣泛使用之綠色螢光 細胞標記且具有可透膜性。一旦鈣黃綠素·ΑΜ(非螢光分子) 進入細胞中,就會被細胞内的酯酶水解成帶負電的綠色螢 光妈黃綠素。勞光#5黃綠素會留在活細胞的細胞質中。此 為細胞存活率終點測定。藉由485 nm激發波長及530放光 波長來觀察螢光訊號。測定時所產生的螢光訊號會與樣品 中存活細胞數量呈比例關係。死亡細胞具有損害的細胞 膜。乙錠(EthD-Ι)會進入損害細胞,且與核酸鍵結時會放出 螢光。EthD-1會於損害或死亡細胞中產生亮紅色螢光。結 果顯示,含γ-PGA培養液中之bCE及Hrpe細胞幾乎全部 存活(圖6A-B及7A-B)。 含γ-PGA之沖洗液對角膜厚度的影像 透過兩支穿入兔眼之靜脈留置針進行沖洗液眼内灌 注,並於灌注期間測量角膜厚度(圖8A) ^藉由蠕動幫浦來 驅動沖洗液之流動(圖8B)。於開始灌注2〇分鐘期間,角膜 厚度增加。圖9顯示,相較於生理食鹽水測試組,使用含丫_ 聚(麩胺酸)類眼部溶液沖洗測試組之角膜厚度變化明顯較 小。生理食鹽水沖洗測試組及含〇4%(w/vh聚(麩胺酸)溶 液沖洗測試组之角膜厚度變化分別為47叫及32 μπι。插入 靜脈留置針所導致的傷害可能是_厚度—開始增加的原 21 201138861 因。之後,使用γ-聚(麩胺酸)類眼科手術沖洗液持續灌注角 膜1小時期間’角膜厚度只些微增加約36μηι(圖相反的, 使用生理食鹽水持續灌注角膜6〇分鐘期間,角膜厚度呈現 持續增加的狀況’灌注1小時後,角膜厚度增加約58 μπι, 與使用γ-聚(麩胺酸)類眼科手術沖洗液沖洗之測試組比 較’其大約增加到1.6倍。 【圖式簡單說明】 圖1Α至1C係顯示γ-PGA及右旋糖濃度對沖洗液滲透壓之影 響。 圖2係γ-PGA影響沖洗液滲透壓之圖式。 圖3係γ-PGA影響沖洗液折射率之圖式。 圖4A-4B顯示γ-PGA對細胞生長無顯著的影響’(A)牛角膜 内皮細胞,(B)人類視網膜色素上皮細胞。 圖5A-5B顯示γ-PGA對細胞無顯著的毒害影響,(A)牛角膜 内皮細胞,(B)人類視網膜色素上皮細胞。 圖6-7顯示染劑染色細胞之顯微照片,於螢光顯微鏡下同時 觀察含有或未含γ-PGA培養後之存活細胞及死亡細胞,(6a) 培養1天之牛角膜内皮細胞,(6B)培養1天之牛角膜内皮細 胞’(7A)培養1天之人類視網膜色素上皮細胞,(7B)培養3 天之人類視網膜色素上皮細胞,負控制組:氧化鋁(Al2〇3) 萃取培養液;正控制組:含Triton X-100 (0,1%)之DMEM/F12 培養液。 圖8A顯示兩支靜脈留置針插入兔眼中。 22 201138861 圖8B顯示藉由蠕動幫浦進行沖洗液灌注。 圖9顯示眼部沖洗期間之角膜厚度變化圖。 【主要元件符號說明】 無。 23
Claims (1)
