На.голове в проекции патологического очага делают подковообразный разрез ко-, жи и кожный лоскут откидывают к основанию разреза. Делают разрез мышечно-апоневротического и надкостничного сло м гких тканей на всем прот жении границ предполагаемого костного лоскута, за исключением того участка, на котором будет сохранена непрерывность питающей ножки. Производ т выпиливание костного лоскута с максимальным сохранением м гких тканей у линии распила. У основани , прикрытого мы- шечно-апоневротическо-надкостничной ножкой , костный лоскут надламывают по обычной- методике. Затем производ т приемы по дальнейшему доступу и вмешательству на патологическом очаге головного мозга. В конце этой операции перед ушиванием кожной рамы под кожно-апоневротическим слоем головы у кра костного дефекта формируют ложе тупым способом, отслаива кожу с поверхностным листком фасции головы от подлежащих тканей. Мобилизуют мышечно- апоневротическо-надкостничную ножку костного лоскута и укладывают костный лоскут в образованное ложе и максимально освобождают дефект свода черепа от перекрыти его костным лоскутом.
