The venoextractor contains an annular knife 1 with a cutting edge 2 and a dielectric 3 mounted in a dielectric, an electrical wire 4 connected to an annular knife. The cutting edge of the knife is made opposite the handle 5 with a protrusion 6; a sleeve 7 of a dielectric with an electrode 8 fixed in it is mounted in the knife hole, which is asymmetrically aligned with the longitudinal axis 9 of the protrusion.
The handle is made removable with the possibility of movement along the conductor 10 and with the fixing device 11. An electrical wire and an electrode are installed in the conductor. The electrical wire and the electrode are connected to the contacts 12 and 13, separated by a dielectric sleeve 14.
The venoextractor is used as follows.
Обнажают устье больщой подкожной вены, перев зывают и пересекают. В дисталь- ном направлении в просвет вены ввод т (после сн ти руко тки 5) проводник Юкон- цом с контактами 12 и 13. Продвигают проводник 10 в просвет вены до момента прекращени передвижени и вып чивани этого конца через м гкие ткани. Делают разрез кожи, подкожной клетчатки на уровне выступающего конца проводника 10, обна- жают вену, перев зывают вблизи конца проводника 10 и пересекают ее. Подт гивают рукой за выведенный из вены конец проводника 10 до захватывани ножом 1 проксимального конца пересеченной больщой подкожной вены. Надевают руко тку 5 на проводник 10 и закрепл ют фиксирующим устройством. Подсоедин ют контакты 12 и 13 к прибору дл электроприжигани . Подт гивают за руко тку 5 веноэкстрактор в дистальном направлении, пересека режу- The mouth of the large saphenous vein is exposed, tied and crossed. In the distal direction, the conductor Yukonts with contacts 12 and 13 is inserted into the lumen of the veins (after removing the handle 5). The conductor 10 is advanced into the lumen of the vein until it stops moving and this end is protruded through soft tissues. An incision is made in the skin, subcutaneous tissue at the level of the protruding end of the conductor 10, the vein is exposed, tied near the end of the conductor 10 and intersected. Hand-pull the end of the conductor 10 removed from the vein until the proximal end of the crossed large saphenous vein is gripped by a knife. Put the handle 5 on the conductor 10 and fix it with the fixing device. Pins 12 and 13 are connected to the electric appliance. Tighten the distal direction of the handle 5 with the venextractor 5;
щей кромкой 2 кольцевого ножа 1 парава- зальную клетчатку до момента ощущени сопротивлени и по влени деформации на коже, что свидетельствует о захвате кольцевым ножом венозных притоков.by the rim edge 2 of the circular knife 1, the paravascular fiber is until sensation of resistance and the appearance of deformation on the skin, which indicates that the venous inflows are captured by the circular knife.
After that, the venoextractor is rotated by the handle 5, taking into account the position of the electrode 8 (counterclockwise) up to the stop of the protrusion 6 into the venous inflow, which is determined by the resistance to rotation, include high-frequency currents and venous inflows are cauterized. The venous extractor is advanced in the distal direction and, with an emphasis by an annular knife 1 into the tributaries of the veins, they are processed. At the same time, the venous tributaries simultaneously intersect the cutting edge of the protrusion 6 from the side of the electrode 8 and coagulate on the same side due to the asymmetric arrangement of the electrode 8 in the bore of the sleeve 7. After the intersection and coagulation of all venous tributaries is completed, the venextractor along with the vein is removed from wounds. Both wounds heal, overlay dressings. Similarly, superficial veins are also removed in the lower leg area.
