Abstract
The invention relates to surgery. The purpose of the invention is the prevention of stenosis. A collagen tubular endoprosthesis, put on the nasogastric probe and fixed to it, is introduced through the wall defect into the lumen of the esophagus. They separate the sternum-abdomen muscles with a single flap, cross them near the bone of the bone, move it to the defect area of the esophagus and sew them to its wall with two rows of interrupted sutures, closing the defect. The endoprosthesis is fixed in the desired position. Every 30 days change prosthesis and probe.

О)ABOUT)
The invention relates to medicine, and specifically to surgery of the digestive tract, and can be used for the surgical treatment of patients with extensive defects in the beggar's wall, which can occur with traumatic injuries of the esophagus, after removal of benign esophageal formations; 1 after laryngectomy, when there are large defects of the hypopharynx and the initial esophagus; to prevent esophageal stenosis after plastic surgery and burns.
I The aim of the invention is to prevent stenosis by providing the epithelium (a defect and a more complete restoration of the function of the esophagus by covering the defect with muscle flaps without prior suturing of defects on a collagen xenoprosthesis. The method is carried out as follows in a manner.
I . I. Access to the cervical esophagus is pro-; water and longitudinal dissection of the neck tissues; in front of the left sterno-mastoid muscle; A collagen tubular endoprosthesis worn on the nasogastric probe and fixed to it with anchor nylon sutures is inserted through the wall defect into the lumen of the esophagus and set at the level of the defect. The distal (lower) end of the probe is inserted: into the stomach, and the upper through the oral cavity is brought out. The sterno-abdominal muscles are separated by a single flap: they are crossed at the body of the dorsal bone (they are moved to the area of the esophageal defect) and sewn to its wall with two rows of nodal seams, closing the defect. antibiotics are injected into the surrounding tissues, a glove rubber drainage is applied to the esophagus for days, the wound is protected in layers. The rubber sleeve is put on the upper end of the probe and transferred to the outside through the nasopharynx and the nasal cavity, the rubber sleeve is mixed into the nasopharynx and mounted on the nose uro no choan. A second rubber sleeve is put on the outer end of the probe and placed at the entrance to the nose, thereby fixing the endoprosthesis in the desired position. The prosthesis and probe are replaced every 30 days under anesthesia until the mucous membrane is fully restored (epithelialization of the defect from the inside), which is controlled by esophagoscopy.
The use of the proposed method of plastic surgery for extensive defects of the cervical esophagus wall as compared with the existing ones contributes to an earlier restoration of the wall and function of the esophagus, since the plate can be produced immediately in the first hours after the occurrence of a defect in its wall; creates an opportunity not
0 только избежать трудностей, св занных с кожной выстилкой дефекта стенки пищевода, но и позвол ет предупредить р д т желых осложнений, часто возникающих при кожной пластике пищевода (хроническое воспаление , стенозирование пищевода, малигни заци лоскута), так как пластика выполн етс только лоскутом, сформированным из грудино-подъ зычных мышц на сосудистой ножке; уменьшает травматичность операции, так как она искючает большие повреждеQ ни кожных покровов шеи и тканей грудной стенки, которые наблюдаютс при пластике дефектов пищевода пекторальным кож- но-мышечным лоскутом; позвол ет создать нужный просвет пищевода, восстановить его функцию и предупредить послеоперацион5 ные стенозы пищевода, что достигаетс благодар восстановлению его стенки на кол- лагеновом ксеногенном трубчатом эндопро- тезе, фиксированном на носожелудочном зонде , который удерживаетс в нужном положении с помощью двух перемещаемых рези новых муфт и находитс в пищеводе до восстановлени его слизистой оболочки (до 2 мес.).0 not only to avoid the difficulties associated with the skin lining of the defect in the wall of the esophagus, but it also helps to prevent a number of serious complications often occurring in the skin plastics of the esophagus (chronic inflammation, esophageal stenosis, malignanosis) formed from the sternum-pozy muscles on the vascular pedicle; reduces the invasiveness of the operation, since it eliminates much damage to the skin of the neck and chest wall tissues, which are observed when plastics of the esophagus are damaged by the pectoral skin-muscular flap; allows you to create the desired lumen of the esophagus, restore its function and prevent postoperative stenosis of the esophagus, which is achieved by restoring its wall on a collagen xenogenic tubular endoprosthesis fixed on a nasal gastric probe, which is held in the desired position by means of two displaced rubber joints and is in the esophagus until its mucous membrane is restored (up to 2 months).
The method developed in the experiment, recommended for clinical trials according to the existing rules of testing.
