20 25 30 521 031 2 Genetiska försök innefattande konstruktion och test av möss med brist på vardera av dessa selektiner antyder att de ger överlappande, men kritiska bidrag till leukocyt rekryteringen vid inflammationer. Möss som saknar vardera av dessa selektiner uppvisar defekter i neutroñlförökning och extravasering. Vidare, uppvisar möss som har brist på både E och P selektiner extrem leukocytos, förhöjda cytokinnivåer och förändringar i haemotopoes.
Dagens behandling av inflammatoriska tillstånd inkluderar behandling med användning av steroider vilka negativt påverkar immunförsvars-systemet och leder till en total inhibition av inflammationen där det är av intresse och vikt att kontrollera det inflammations reglerande systemet.
Skälen bakom godtycklig in vitro screening Eftersom initial bindning av leukocyter till endotel initierar den inflammatoriska processen, har det använts ett in vitro system för att studera adhesionen hos olika celler till human navelsträngs endotelial celler. Förfarandet är väletablerat och har framgångsrikt utnyttjats av ett antal forskningsgrupper. Denna in vitro statiska adhesionstest har använts häri för godtycklig screening av substanser som kan blockera leukocyt-adhesion till endotelialceller.
En variation av substanser testades eftersom litteraturen har visat att ett antal, både kolhydrat och icke-kolhydratstrukturer kan blockera interaktionen av selektiner med besläktade ligander. Substanser som kan blockera interaktionen av endoteliala adhesionsmolekyler med besläktade ligander på leukocyter har potential som nya, selektiva inhibitorer av akuta och kroniska inflammatoriska reaktioner och ischemiska reperfusionstillstånd utan risk för immunosuppression.
Beskrivning av föreliggande uppfinning Godtycklig screening av olika kemiska föreningar resulterade i att tre ledande föreningar identifierades, vilka kan blockera adhesionen av neutrofiler. Föreningarna som erhölls i screnningen var: isomaltitol, uridin och 4-tiouridin.
Uppfinningen kommer att beskrivas mera i detalj i det följande med hänvisning till genomförda screeningförsök. Testresultaten illustreras också i hithörande figurer.
Resultaten av screeningen av substanser som kan blockera adhesionen av neutrofiler till L- celler transfekterade med E-selektin. 10 15 20 25 30 521 051 _. ru 3 L-celler permanent transfekterade med fullängds E-selektin erhölls från Dr. T. Kishimoto, Boehringer Ingelheim, New York. Cellema transfekterades medelst elektroporering vid Boehringer Ingelheim, New York och ytterligare kloning genomfördes i Linköping, Sverige.
Humana neutrofiler isolerades från EDTA helblod medelst densitetscentrifugering. Renhet och totalantal bestämdes medelst standard haematologi instrument. L-celler odlades i 96- brunnar mikrotiter-plattor.
L-celler och neutrofiler pre-inkuberades med respektive substanser under 45 min vid 37°C.
Neutrofilerna fick sedan fästa vid L-cellema i närvaro av olika substanser under 20 min vid 37°C. Icke-vidhäftande celler avlägsnades med upprepad tvättningar och antalet vidhäftade celler bestämdes genom att mäta myeloperoxidas innehållet. Eftersom bindning av neutrofiler till selektin är kalciumberoende, användes EGTA (som chelat-binder till kalcium) som negativ kontroll, Alla tre substansema inhiberar adhesion av neutrofiler, men i olika grad (FIG. 1-3). EGTA blockerar genomgående adhesion av neutrofiler. Det föreligger inget klart dos-svar samband för dessa substanser vilket delvis kan bero på det faktum att detta är en statisk adhesionstest och selektin-medierad bindning tycks fungera bäst under flödesforhållanden. Maximal inhibition tycktes uppträda redan på mikromolära nivåer.
Godtvcklig screening av substanser som kan blockera adhesion av coloncancer celler. Colo 201, till L-celler transfekterade med E-selektin.
Fastän protein-ligander har karaktäriserats for selektinema i genkänner de ett antal kolhydrat och icke-kolhydrat-ligander. Hypotesen är att dessa föreningar bör blockera E-selektin- SLe*-interaktion. För att bekräfta detta testades också om dessa substanser kan blockera adhesion av Colo 201 celler till L-celler. Denna interaktion har visats medieras av huvudsakligen E-selektin-SLe* molekylgruppen.
Ingen av dessa substanser kunde inhibera bindning av Colo 201-celler till HUVEC vilket antyder att detta är en specific inhibering av neutrofiler (FIG. 4-6). Åter igen blockerade EGTA adhesion av Colo 201 celler. I motsats till tidigare försök med neutrofiler, var också en monoklonal antikropp mot E-selektin mycket effektiv i att blockera adhesion av Colo 201 celler till TNF stimulerad HUVEC. 10 15 20 25 30 521 031 4 lnhibition av neutrofil-adhesion till TNF stimulerad HUVEC För att ytterligare utsträcka upptäckten från L-cellers studerades bindningen av neutrofiler till TNF aktiverat endotel (HUVEC).
Endotelial-celler odlades i 96-brunnars mikrotiterplattor till sammanflytning. De stimulerades med TNF under 4 h vid 37°C. lsolerade neutrofiler inkuberades under 30 min vid 37°C med respektive substanser. Endotelialcellerna tvättades efter 4 h och 2xl05 neutrofiler tillsattes till varje brunn och cellema fick vidhäfta i närvaro av respektive substanser.
Isomaltitol inhiberade vidhäftningen av neutrofiler till TNF stimulerad HUVEC med ungefär 30%. Åter igen observerades inte något klart dos-svar-samband. Faktum är att motsatsen tycks råda, ökande inhibition med minskande dos (FIG. 7). 4-tiouridin var mera potent i att inhibera adhesíon av neutrofiler till TNF -stimulerad HUVEC. 4-tiouridin reducerade adhesionen av neutrofiler med 60%. Maximal adhesíon tycktes uppträda redan på mikromolära nivåer. FIG. 8 visar ett representativt försök och FIG. 9 sammanfattar data från två separata försök genomförda i kvadruplikat. Pre- inkubering av neutrofiler med EGTA eller monoklonala antikroppar mot E-selektin blockerade bindning av neutrofiler.
Effekt av 4-tiouridin Då uppreglering av E-selektin Ovannämnda substanser bör blockera interaktionen av E-selektin med dess ligand och bör inte ha någon effekt på cytokin-inducerad uppreglering av adhesionsmolekyler. En cell- ELISA genomfördes för att bekräfta ytexpressionen av adhesionsmolekyler.
Endotel-celler odlades på mikrotiter-plattor, stimulerade med TNF eller IL-l med eller utan respektive substans under 4 h vid 37oC. Efter stimulering tvättades cellerna och fixerades med forrnaldehyd. En cell-ELISA genomfördes i analogi med reguljär ELISA med användning av primära och sekundära antikroppar.
Samtidig inkubering av endotel med olika koncentrationer av 4-tiouridin påverkade inte TNF inducerad uppreglering av E-selektin som bedömts med cell-ELISA (FIG. 10). Dessa resultat antyder att 4-tiouridin inhiberar neutroñladhesion genom att interferera med 10 l5 20 25 30 521 031 .in .ai- 5 interaktionen av E-selektin med besläktade li gander och tycks inte påverka uppregleringen av E-selektin.
Viabilitet E-selektin transfekterade L-celler inkuberades med 1 mM, 0.1 mM, 0.001 mM, 0.1 uM uridin, tiouridin, eller isomaltitol under 2 tim. Viabiliteten efter inkubering var > 90 %. På liknande sätt inkuberades neutrofiler med 1 mM, 0.1 mM, 0.001 mM, 0.1 pM uridin, tiouridin, eller isomaltitol under 2 tim.

Som belagda produkter kan de vara enbart helbelagda för att åstadkomma en mera lätt-administrerbar beredning, eller som en fördröjd frisättnings-belagd komposition, där frisättningen av den aktiva substansen sker på grund av upplösning av beläggningen vilken upplösning är beroende av vari mag-tarrn-kanalen man önskar sig en frisättning.
Sålunda kan frisättningen styras till plats och tid. Det kan också vara fördelaktigt att belägga den aktiva föreningen om denna är utsatt för degradering, såsom av magsyra, för att sedan låta föreningen passera magsäcken.

Beredningar för injektion göres vanligen för subcutan, intramuskulär, intravenös eller intraperitoniell administrering. Inj ektionslösningar är normalt försedda med ett adjuvans för 10 15 20 521 031 . , , . ., 6 att underlätta absorption av den aktiva föreningen.
Beredningar för inhalation föreligger vanligen som pulver som administreras antingen i trycksatta behållare med ett doseringsmunstycke, eller ett inhalationssystem där pulvret doseras in i systemet och då patienten inhalerar luft genom apparaten till sådan grad att pulvret blir luftburet och kommer in i andningsapparaten inklusive lungoma.
Inhalationsberedningen användes nonnalt för inflammatoriska tillstånd i andningsapparaten inklusive lungoma.
Kompositionema innehåller 0,5 till 99 vikt-% aktiv förening och återstoden är olika inerta, icke-terapeutiskt aktiva föreningar vilka underlättar administrering, beredning såsom granulering, tablettering, och lagring. Sådana inerta material kan, emellertid, en administreringen positiv effekt.
De aktiva föreningama enligt uppfinningen administreras i en mängd av l till 100 mg per kilogram kroppsvikt beroende på tillståndet hos patienten, administreringsväg, ålder och kroppsvikt hos patienten, och andra överväganden som görs av läkaren. Claims (5)
1. Användning av en eller flera av töreningama ur gruppen bestående av 4-tiouridin, isomaltitol och uridin vid framställning av terapeutiskt effektiva kompositioner mot akuta och kroniska inflammationer, och/eller problem i hemostasen relaterad till trombocytfunktionen.
2. Användning enligt krav l, vari den terapeutiskt aktiva föreningen är 4-tiouridin.
3. Användning enligt krav 1, vari den terapeutiskt aktiva föreningen är isomaltitol.
4. Användning enligt krav 1, vari den terapeutiskt aktiva föreningen är uridin.
5. Användning enligt ett eller flera av kraven 1-4, vari den terapeutiskt aktiva dosen är 1 till 100 mg per kg kroppsvikt.
