Способ закрытия культи двенадцатиперстной кишки, включающий поперечное рассечение двенадцатиперстной кишки по краю пенетрирующей язвы, циркулярную отслойку слизистой оболочки формируемой культи от серозно-мышечной "манжетки", раздельное ушивание слизистой оболочки и серозно-мышечной "манжетки", отличающийся тем, что, с целью обеспечения надежной профилактики несостоятельности культи и интраоперационного повреждения пенетрируемого органа, линия швов слизистой оболочки закрывается перпендикулярно ориентированной по отношению к ней дубликатурой, формируемой из продольно рассеченных на 2/3 высоты стенок серозно-мышечной "манжетки" путем подшивания края свободной стенки к мышечному слою противоположной стенки, разобщенной с дном язвенного дефекта, по границе мобилизованной слизистой оболочки, последующим наложением стенок друг на друга и фиксацией их в этом положении вторым рядом серозно-мышечных швов, смещенным относительно первого на величину высоты "манжетки", необходимость мобилизации кишки от пенетрируемого органа при этом полностью исключается, дно язвенного дефекта изолируется прядью сальника на ножке по типу оментопластики.A method for closing a stump of the duodenum, including transverse dissection of the duodenum along the edge of the penetrating ulcer, circular detachment of the mucous membrane of the formed stump from the serous-muscular “cuff”, separate suturing of the mucous membrane and serous-muscular “cuff”, characterized in that, in order to ensure reliable prevention of insolvency of the stump and intraoperative damage of the penetrated organ, the line of sutures of the mucous membrane is closed perpendicularly oriented in relation to it duplicate, formed from the walls of the serous-muscular “cuff”, longitudinally dissected into 2/3 of the height, by suturing the edge of the free wall to the muscle layer of the opposite wall, separated from the bottom of the ulcer defect, along the border of the mobilized mucous membrane, followed by applying the walls to each other and fixing them in this position, the second row of serous-muscular sutures, displaced relative to the first by the height of the “cuff”, the need to mobilize the intestine from the penetrated organ is completely eliminated, the bottom of the ulcer the defect is isolated by a strand of the omentum on the leg according to the type of omentoplasty.
