Description
Изобретение относится к медицине и может быть использовано в стоматологии для выявления кариеса на видимых и скрытых поверхностях зубов. При клиническом обследовании пациентов для своевременного выявления начальной формы кариеса зубов (в МКБ-10 соответствует коду К02.0 - Кариес эмали. Стадия «белого мелового» пятна [начальный кариес]) необходимо использовать дополнительные методы диагностики. В начальной стадии кариеса эмали признаки поражения проявляются в виде очагов деминерализации - пятен от светлого до темного цвета с неровными краями различных размеров - от точечных до величины в несколько миллиметров. Пятна светлого цвета (меловидные) не всегда выявляются при визуальном осмотре. Одним из эффективных методов визуализации доклинических и ранних клинических признаков кариеса эмали является метод трансиллюминации, который проводится с использованием аппаратов с источниками света, излучающего в спектре оранжевого света. Метод реализуется различными аппаратами, которые предназначены для проведения трансиллюминационного исследования, например фотоактиваторами стоматологическими для полимеризации композитных пломбировочных материалов таких как «Эстус ЛЭД-Алладин», «Estus Light» с дополнительной диагностической насадкой «ESTUS-LED-Orange», предназначенными для выявления ранних форм кариеса зубов, скрытых кариозных полостей, дефектов, трещин эмали (АО «Геософт Дент», Россия).The invention relates to medicine and can be used in dentistry to detect caries on visible and hidden surfaces of teeth. The method is implemented by various devices that are designed for transillumination research, for example, dental photoactivators for the polymerization of composite filling materials such as “Estus LED-Aladdin”, “Estus Light” with an additional diagnostic attachment “ESTUS-LED-Orange”, designed to identify early forms dental caries, hidden carious cavities, defects, enamel cracks (JSC Geosoft Dent, Russia).

Пациент Н., 7 лет, в сопровождении своих родителей обратился в стоматологическую поликлинику с целью профилактического осмотра. Последний раз на клиническом приеме был год назад. Гигиенический уход за полостью рта выполняет регулярно 2 раза в день, под контролем родителей. Из вредных привычек отмечено повышенное употребление углеводосодержащих продуктов (4 раза в день), сладких газированных напитков (3 раза в день). Данные осмотра полости рта: интенсивность кариеса зубов по индексу КПУ равна 0, индекс гигиены (OHI-S) - 1,6 баллов, что соответствует удовлетворительному уровню гигиены рта. При визуальном осмотре и при проведении витального окрашивания патологических изменений твердых тканей зубов не выявлено. При аппаратной диагностике предлагаемым способом с помощью аппарата «Эстус ЛЭД-Алладин» с дополнительной насадкой «ESTUS-LED-Orange» (источник света размещен со стороны вестибулярной поверхности по направлению к оральной, в области межзубного промежутка в точке ниже апроксимального контакта) выявлены очаги деминерализации эмали на мезиальной поверхности зуба 1.1 в 9-м режиме яркости. Установлен клинический диагноз: К02.0, Кариес эмали 1.1. Проведено обучение правилам гигиены полости рта, рекомендованы фторидсодержащие зубные пасты для ежедневного использования. The clinical diagnosis was established: K02.0, Enamel caries 1.1. Training in oral hygiene rules was provided, and fluoride-containing toothpastes were recommended for daily use. A preventive examination is recommended after 6 months.

Данные осмотра полости рта: интенсивность кариеса зубов по индексу КПУ составила 2, индекс гигиены полости рта (OHI-S) - 1,1 баллов, что соответствует удовлетворительному уровню. Данные объективного обследования: при визуальном осмотре на зубе 1.2 на вестибулярной поверхности в центральной части коронки зуба и в пришеечной области при зондировании определяется шероховатость эмали. Витальное окрашивание составило 4 балла. В результате аппаратной диагностики с использованием аппарата «Эстус ЛЭД-Алладин» с дополнительной насадкой «ESTUS-LED-Orange» (источник света размещен со стороны небной поверхности параллельно оси зуба) были четко визуализированы очаги деминерализации эмали в 5-ом режиме яркости свечения. Установлен клинический диагноз: К02.0 - Кариес эмали 1.2. Проведены лечебно-профилактические мероприятия с применением средства «Фторлак прозрачный» кратностью 3 раза с интервалом в 4 дня. По окончании курса мероприятий был рекомендован профилактический осмотр пациента и повторное проведение лечебно-профилактических мероприятий через 4 месяца. As a result of hardware diagnostics using the Estus LED-Aladdin apparatus with an additional ESTUS-LED-Orange attachment (the light source is placed on the side of the palatal surface parallel to the tooth axis), foci of enamel demineralization were clearly visualized in the 5th brightness mode. A clinical diagnosis was established: K02.0 - Enamel caries 1.2. Therapeutic and preventive measures were carried out using the “Transparent Fluoride Varnish” product in a frequency of 3 times with an interval of 4 days. At the end of the course of measures, a preventive examination of the patient and repeated treatment and preventive measures after 4 months were recommended. It is also recommended to use fluoride-containing toothpastes at home for daily hygienic oral care.

Для выполнения ежедневных гигиенических мероприятий в домашних условиях рекомендованы фторидсодержащие зубные пасты. При аппаратной диагностике предлагаемым способом с помощью аппарата «Эстус ЛЭД-Алладин» с дополнительной насадкой «ESTUS-LED-Orange», выявлен очаг деминерализации эмали на щечной поверхности зуба 4.6 в 4-м режиме яркости свечения при расположении источника света со стороны язычной поверхности в пришеечной области по направлению к щечной под углом 45°. Также выявлен очаг деминерализации эмали на жевательной поверхности при расположении источника света со стороны щечной поверхности в пришеечной области параллельно оси зуба в 4-ом режиме яркости свечения и со стороны язычной поверхности в пришеечной области под углом 45° в 4-ом режиме яркости свечения. Установлен клинический диагноз: К02.0 - Кариес эмали 4.6. Проведено обучение правилам гигиены полости рта, рекомендованы фторидсодержащие зубные пасты для ежедневного использования. Проведено лечебное мероприятие зуба 4.6 с использованием средства «Эмаль-герметизирующий ликвид». A treatment was carried out on tooth 4.6 using the “Enamel-sealing liquid” product. A preventive examination is recommended after 3 months.

При визуальном осмотре и при проведении витального окрашивания патологических изменений твердых тканей зубов не выявлено. При аппаратной диагностике предлагаемым способом с помощью аппарата «Estus Light» с дополнительной насадкой «ESTUS-LED-Orange», выявлен очаг деминерализации эмали на мезиальной поверхности зуба 2.6 во 2-ом режиме яркости свечения при расположении источника света со стороны щечной и небной поверхностей перпендикулярно оси зуба в области межзубного промежутка в точке ниже апроксимального контакта в 2-ом режиме яркости свечения. Также выявлен очаги деминерализации на мезиальной поверхности зуба 3.6 при расположении источника света со стороны щечной поверхности перпендикулярно оси зуба в 6-ом режиме яркости свечения, а со стороны язычной поверхности под углом 45° в 5-ом режиме яркости свечения. Установлен клинический диагноз: К02.0 - Кариес эмали 2.6; 3.6. Проведено обучение правилам гигиены полости рта, рекомендованы фторидсодержащие зубные пасты для ежедневного использования. Проведены лечебно-профилактические мероприятия 2.6, 3.6 с применением средства «Фторлак прозрачный». A clinical diagnosis was established: K02.0 - Enamel caries 2.6; 3.6. Training in oral hygiene rules was provided, and fluoride-containing toothpastes were recommended for daily use. Treatment and preventive measures 2.6, 3.6 were carried out using the “Transparent Fluoride Lacquer” product. A preventive examination is recommended after 4 months.
