Изобретение относится к медицине, а именно к нейрохирургии, может быть использовано в нейрохирургических отделениях и отделениях сосудистой хирургии при оперативном лечении некоторых форм головных болей и головокружений, а именно - при декомпрессии нейроваскулярных комплексов головы и шеи.The invention relates to medicine, namely to neurosurgery, and can be used in neurosurgical departments and vascular surgery departments for the surgical treatment of certain forms of headaches and dizziness, namely for decompression of the neurovascular complexes of the head and neck.
Компрессия нейроваскулярных комплексов затылочной области среди заболеваний периферической нервной системы занимают значительное место и является сложной проблемой в плане выработки правильной стратегии лечения. Решение о необходимости проведения оперативного лечения, его сроков и объемов вмешательства до сих пор обсуждается в литературе, и четких указаний нет. Однако проведение оперативного лечения в ранние сроки достоверно имеет преимущество.Compression of the neurovascular complexes of the occipital region occupies a significant place among diseases of the peripheral nervous system and is a difficult problem in terms of developing the correct treatment strategy. В настоящее время известные следующие способы лечения: блокада, фенестрация, аутодермопластика, декомпрессивно-стабилизирующие операции, декомпрессивно-пластические операции, деструкция, абляция, электростимуляция.The following treatment methods are currently known: blockade, fenestration, autodermoplasty, decompressive-stabilizing operations, decompressive-plastic operations, destruction, ablation, electrical stimulation.

Известен способ невролиза, позволяющего хирургу освободить периферический нерв. Невролиз - выделение нерва из рубцов с целью улучшения условий его регенерации и функционирования. Операция может проходить под местной анестезией или общим наркозом. Для ее проведения используются микрохирургические инструменты и микроскоп. Нерв обнажается в пределах здоровых тканей и постепенно выделяется из рубцового комплекса. Одновременно удаляются измененные окружающие ткани. Удаляют остатки рубца, которые окружают нерв как футляр. Для освобожденного нерва создается ложе между мышцами, в которое он укладывается. Это ложе нужно, чтобы не было дальнейших рубцовых сращений, деформаций нерва и его сдавления. Нерв размещается в ложе так, чтобы мышцы покрывали его, но чтобы при этом он не соприкасался с апоневрозом, кожей, фасциями. Операционная рана ушивается. Технический результат предлагаемого изобретения - разработка способа топологической декомпрессии нейроваскулярных комплексов затылочной области, из единого доступа, позволяющего избежать повреждения мышечных и костных тканей, сохраняя основные артерии, вены и нервы.The purpose of this invention is to reduce the duration of surgery and the postoperative healing period. Выполняется дугообразный разрез мягких тканей, около 8 см, с установкой ранорасширителя, с поэтапными доступами к нейроваскулярным комплексам. Выполняется доступ к анатомическим образованиям затылочного треугольника, содержащего нейроваскулярные комплексы затылочной области. Выполняется микроваскулярная декомпрессия большого затылочного нерва и затылочной артерии, перевязывается и коагулируется восходящая ветвь затылочной артерии. Компрессия на симпатическое сплетение затылочной артерии устраняется. Послойное ушивание раны. Пример 1. Пациент М., 32 года, считает себя больным на протяжении 5 лет, когда впервые стали появляться симптомы, связанные с нейроваскулярным затылочным гиперфункциональным синдромом. На фоне проводимого консервативного лечения, положительная динамика, в виде уменьшения неврологической симптоматики, не наблюдалась, пациент стал отмечать нарастание симптомов. Применялось консервативное лечение: НПВС, антихолинэстеразные препараты, препараты витаминов группы В, миорелаксанты, блокады мест выхода большого затылочного нерва, заднего ушного нерва. Поступил в нейрохирургическое отделением с выраженным болевым синдромом в затылочной области, переходящим по всей лобно-затылочной области, а также в задней ушной области - 8 баллов по шкале ВАШ (визуально-аналоговая шкала). 21.02.2021 г. в неврологическом статусе отмечается выраженная симптоматика, обусловленная затылочным гиперфункциональным синдромом.
