Распространенным следствием неконтролируемого диабета является гипергликемия - повышенный уровень содержания глюкозы в крови, со временем приводящая к серьезному повреждению многих систем организма, особенно нервов и кровеносных сосудов [https://www.who.int/ru/news-room/fact-sheets/detail/diabetes].A common consequence of uncontrolled diabetes is hyperglycemia - elevated levels of glucose in the blood, which over time leads to serious damage to many body systems, especially nerves and blood vessels [ https://www.who.int/ru/news-room/fact-sheets/ detail/diabetes].
2020. №3. URL: https://cyberlenirika.ru/article/n/ispolzovanie-naturalnyh-rastitelnvh-ingredientov-pri-lechenii-bolnvh-saharnym-diabetoml. 2007. №27 С. 2053, Русский медицинский журнал URL: http://www.rmj.ru/articles/endokrinologiya/Upravlenie_sahamym_diabetom_roly_postprandialynoy_giperglikemii_i_vozmoghnosti_ее_korrekcii/#ixzz7kz9sLeh 1].Currently, the number of cases of diabetes is growing rapidly. In this regard, an urgent problem in pharmacy and medicine is the development of new effective medicines for the treatment of diabetes mellitus, including herbal remedies [Use of natural herbal ingredients in the treatment of patients with diabetes mellitus // Medicine. Sociology. Philosophy. Applied research. 2020. No. 3. URL: https://cyberlenirika.ru/article/n/ispolzovanie-naturalnyh-rastitelnvh-ingredientov-pri-lechenii-bolnvh-saharnym-diabetoml. It is known that improving glycemic control significantly reduces the incidence of macroangiopathic complications in patients with type 1 or 2 diabetes, while reducing postprandial (after meals) plasma glucose has a leading role and is no less important for achieving target levels of glycated hemoglobin [Ametov A. S., Melnik A.V. Management of diabetes mellitus: the role of postprandial hyperglycemia and the possibility of its correction. RMJ. 2007. No. 27 P. 2053, Russian Medical Journal URL: http://www.rmj.ru/articles/endokrinologiya/Upravlenie_sahamym_diabetom_roly_postprandialynoy_giperglikemii_i_vozmoghnosti_ee_korrekcii/#ixzz7kz9sLeh 1].
2016. №2. URL: https://cyberleninka.ru/article/ri/ingibitorv-natriy-glyukoznogo-kotransportera-2-risk-ketoatsidoza]. Кроме того многие современные гипогликемические средства, например препарат эксенатид, являются дорогостоящими [https://www.eapteka.ru/goods/drugs/diabet/gipoglikemicheskie_preparaty/prepara_ty_usilivayushchie vydelenie_insulina_podzheludochnoy_zhelezoy/baeta_eli_lilli/_?utm_referrer=https%3a%2f%2fVandex.ru%2f].To control glycemic levels in patients with type 2 diabetes mellitus, various hypoglycemic agents are used, their main groups are: biguanides; thiazolidinediones; sulfonylurea derivatives; glinides; α-glucosidase inhibitors; incretins: dipeptidyl peptidase-4 inhibitors (DPP-4 inhibitors) and glucagon-like peptide-1 receptor agonists (arGLP-1), as well as a new group of gliflozins - sodium-glucose cotransporter type 2 inhibitors (SGLT-2, synonym SGLT2). One of the main typical disadvantages of many synthetic hypoglycemic drugs is a large list of contraindications (including renal and liver failure, severe decompensation of diabetes, pregnancy, lactation), side effects, incl. gastrointestinal discomfort, the risk of hypoglycemic conditions, and also, for example, for iNGLT-2, the risk of a serious adverse reaction - ketoacidosis is reported [Bukatina T.M., Kazakov A.S., Velts N.Yu., Darmostukova M.A. ., Kolesnikova E.Yu., Alyautdin R.N., Romanov B.K. Sodium-glucose cotransporter 2 inhibitors: risk of ketoacidosis // Safety and risk of pharmacotherapy. 2016. No. 2. URL: https://cyberleninka.ru/article/ri/ingibitorv-natriy-glyukoznogo-kotransportera-2-risk-ketoatsidoza]. In addition, many modern hypoglycemic drugs, for example the drug exenatide, are expensive [https://www.eapteka.ru/goods/drugs/diabet/gipoglikemicheskie_preparaty/prepara_ty_usilivayushchie vydelenie_insulina_podzheludochnoy_zhelezoy/baeta_eli_lilli/_?utm_referrer=https%3a%2f%2f Vandex.ru %2f].
In contrast to synthetic oral hypoglycemic agents, the list of known herbal preparations with hypoglycemic action for use in diabetes mellitus is very small. According to the State Register of Medicines [https://grls.rosminzdrav.ru/grls.aspx?s=&m=mnn], the number of medicinal herbal preparations with a hypoglycemic effect is only 4 items, all of them are represented by medicinal herbal raw materials for infusion - blueberry shoots, common bean leaves, collections Arfazetin and Arfazetin-E) [https://grls.rosminzdrav.ru/grls._aspx?s=&m=mnn"|. Known registered collections Arfazetin and Arfazetin-E [https:/ /www.rlsnet.ru/drugs/arfazetin-e-24565] contain rhizomes and roots of Eleutherococcus, Manchurian aralia, rhizomes with roots of zamanika, and therefore have disadvantages associated with limited use in the elderly, with hypertension , insomnia, increased excitability, and also due to the tonic effect, it is undesirable to use infusions from the above mixtures in the evening.
В качестве последнего и немаловажного недостатка можно отметить труднодоступность некоторых растений (например, галеги лекарственной).Currently, a number of compositions with hypoglycemic action have been proposed and patented in the Russian Federation, including components from medicinal plant raw materials, for example, a collection with hypoglycemic activity containing dandelion roots, yarrow herb, nettle leaves, plantain leaves, strawberry leaves, lingonberry leaves, fruit leaves common beans with their equal ratio [Patent No. 2666398 C1 Russian Federation, IPC A61K 36/73, A61K 36/185, A61K 36/28. Rutin's reduction in blood glucose levels can also be achieved by stimulating insulin secretion by beta cells and increasing glucose uptake into skeletal muscle and adipose tissue. д).The disadvantages of the above proposed hypoglycemic agents containing extracts of blue cyanosis from medicinal plant materials are the method of administration into the body that is not optimal for clinical practice (intraperitoneal), as well as the labor costs in obtaining liquid extracts and the method of obtaining the extract - the use of ethyl alcohol 70% as an extractant, as a result, not the entire amount of triterpene saponins is extracted, given that plant materials contain both water- and alcohol-soluble saponins. Ethyl alcohol helps extract related compounds, for example, chlorophyll. Also, alcohol extracts have some restrictions in use (childhood and old age, driving vehicles, inability to use alcohol-containing drugs due to concomitant therapy, etc.).
Thus, there is a known mention of the hypoglycemic effect of the purified sum of saponins from the rhizomes with roots of blue cyanosis and liquid extracts of rhizomes with roots and herb blue cyanosis. However, the use of an infusion of blue cyanosis herb for the correction of postprandial glycemia when administered orally is currently not known, which characterizes the scientific novelty and inventive step of the present invention.
(Polemonii caerulei herba). The purpose of the present invention is to develop a method for correcting postprandial glycemia, characterized by the use of an oral monocomponent hypoglycemic agent of plant origin, which uses an infusion of the blue cyanosis herb (Polemonii caerulei herba).
выше).The proposed remedy, an infusion of blue cyanosis herb, obtained by a standard method, can be used to correct postprandial glycemia, which can be clinically significant for glycemic control in the prevention and treatment of type 2 diabetes. The components of the proposed product (blue cyanosis grass), according to known data, are distinguished by a wide range of biological activity, low toxicity and, based on the composition of known components (saponins, rutin, flavonoids), can help increase tolerance to carbohydrates (see above).
The technical result consists in a pronounced hypoglycemic effect of an aqueous infusion of blue cyanosis herb with its single oral administration at a dose of 120 mg/kg body weight, manifested by a decrease in postprandial glycemia at a glucose load of at least 1.5-2 times compared with the control group, starting no later than 1.5 hours after administration of the infusion, lasting at least 2 hours and exceeding in severity the effect of the well-known hypoglycemic herbal drug Arfazetin by 1.5-2 times.
The technical result is achieved by the fact that in the method for correcting postprandial glycemia, including the use of a monocomponent medicinal herbal preparation with hypoglycemic activity, according to the invention, an aqueous infusion of blue cyanosis herb Polemonii caerulei herba is used as a monocomponent medicinal herbal preparation in the ratio of 1 g of air-dried plant material with a moisture content of 11% per 10 ml of extractant - purified water, the infusion is administered orally at a dose based on the dry residue of the infusion of 120 mg of the dry residue of the infusion / kg of body weight for 60 minutes. before glucose load.

A study of the effectiveness of the proposed monocomponent oral hypoglycemic agent - an infusion of blue cyanosis herb - for the correction of postprandial glycemia was carried out using a glucose tolerance test model in preclinical studies on laboratory animals. The studies were conducted on 30 white outbred conventional male mice weighing 17-23 g, 3 groups, 10 animals each. Before the experiment, all animals were subjected to food deprivation for 12 hours with free access to water. Animals of the first experimental group were given a single dose orally through a gastric tube over 60 minutes. Before the introduction of glucose, an aqueous infusion of blue cyanosis herb was administered, prepared according to the method described above (see above). The animals of the second experimental group were administered a comparison drug, which was used as a well-known hypoglycemic drug of plant origin "Arfazetin", prepared in accordance with the instructions on the packaging [https://www.rlsnet.ru/drugs/arfazetin-e-24565], aqueous infusion of the collection with a mass-volume ratio of 1:10, i.e. For 1 g of the drug, 10 ml of extractant is used, the resulting infusion, for ease of administration of the required volume, was diluted with water 2 times and administered once orally over 60 minutes. before administering glucose through a gastric tube. The dose of both infusions from medicinal plant materials was calculated according to standard recommendations for dosing regimens for infusions, based on the recommended daily dose for humans. For the infusion of cyanosis herb, the recommended daily dose for a person (body weight 70 kg) is 1 glass of infusion 3 times a day, which, as a result of recalculation based on the body weight of the animal, amounted to 0.2 ml of infusion per 20 g of animal weight, recalculated based on dry residue (12 mg/ml), the dose was 120 mg/kg. For Arfazetine infusion (infusion 1:10 diluted with water 2 times), based on the dry residue (12 mg/ml) and the volume of 0.12 ml per 20 g of body weight, the dose was 72 mg/kg. The control group of healthy animals was administered distilled water in a volume of 0.2 ml per 20 g of body weight, once orally for 60 minutes. to glucose. After 60 min. After administration of the studied extracts from medicinal plant materials (infusion of blue cyanosis herb or infusion of the drug Arfazetin), all animals in the control and experimental groups were given a single subcutaneous injection of a 40% glucose solution at the rate of 3 g/kg. 30, 60, 120 minutes after the administration of glucose in animals of all groups, the glucose content in capillary blood was determined using a glucometer (Satellite Express, Russia). Blood sampling was performed by distal tail resection. The results were subjected to statistical processing using well-known methods of medical and biological statistics with the calculation of average values, errors of the average, the significance of differences was assessed using parametric tests (Student's t-test) and non-parametric tests (Wilcoxon test and Mann-Whitney U test).
1).The results of the study of the hypoglycemic activity of the blue cyanosis infusion are presented in Table 1 (Fig. 1).
гликемия достигает исходных значений нормы (5,21 мМ/л).It was found that in the control group (healthy animals), against the background of a glucose load, the maximum postprandial concentration of glucose in the blood was observed after 30 minutes. after the administration of glucose, amounting to 15.79 mmol/l, which is significantly 2.93 times higher than the initial level, then a decrease in glycemia occurs, so after 60 minutes. this figure is 1.72 times (or 72.1%) higher than the initial one, and by 120 min. glycemia reaches the initial normal values (5.21 mmol/l).
Against the background, the introduction of Arfazetin infusion after 30 minutes. glycemia was 12.5% lower than in the control group after 60 minutes. approached the values of the control group, being only 2.6% less than in the control after 120 minutes. no significant further decrease in glucose concentration was detected, glycemia was 70.4% higher than in the control and 65.1% higher than the initial level, which indicates the cessation of the hypoglycemic effect of the drug.
A pronounced hypoglycemic effect was revealed against the background of the administration of an infusion of blue cyanosis herb. So, after 30 minutes. after glucose administration, glycemia was 7.35 mmol/l, which was significantly 53.5% lower (or 2.1 times) than in the control group, 36% lower than the initial level and 46.8% lower than in a group of animals that were administered Arfazetine infusion. After 60 min. a decrease in glucose levels to initial values was detected (indicators correspond to normal values - 5.53 mmol/l), which is 40.3% lower (or 1.6 times) than in the control group and 38.7% lower compared with the group receiving Arfazetine. After 120 min. a significant decrease in glycemia was revealed by 33.2% (or 1.49 times) compared to the control group, which is 35.3% less than the initial level of glycemia and 60.8% less compared to the group of animals receiving Arfazetin.
Thus, in preclinical studies using a glucose tolerance test model, a fairly pronounced hypoglycemic effect of the blue cyanosis herb infusion was revealed with a single oral dose of 120 mg/kg, manifested by a decrease in postprandial glycemia at a glucose load of at least 1.5-2 times compared with control group, starting no later than 1.5 hours after administration of the infusion, lasting at least 2 hours and exceeding in severity the effect of the known hypoglycemic herbal drug Arfazetin by 1.5-2 times.
The data obtained indicate the effectiveness of the proposed method for correcting postprandial glycemia, implemented by using a monocomponent medicinal herbal preparation with hypoglycemic activity, and characterized in that an infusion of blue cyanosis herb is used as a monocomponent medicinal herbal preparation in a mass-volume ratio per 1 g of raw material with moisture content 11% - 10 ml of extractant - purified water, administered orally at a dose of 120 mg / kg (based on dry residue) for 60 minutes. before glucose load.
