Недостатками известного фиксатора и способа его применения являются отсутствие свободного пространства в камере височно-нижнечелюстного сустава между головкой мыщелкового отростка нижней челюсти и суставной впадины, что создает риск вывиха суставного диска и развития мышечно-суставной дисфункции; отсутствие снижение гипертонуса жевательных мышц и восстановление физиологической координации их работы, обуславливающих смещение нижней челюсти при открывании рта; отсутствие позиционирование нижней челюсти в физиологичное положение центральной окклюзии; массивная и подвижная металлическая конструкция повышает риск травматизации слизистой оболочки полости рта. Наиболее близкими аналогами группы изобретений являются устройство и способ временной посттравматической иммобилизации челюстей (RU 2496453, 2012). Конструкция устройства включает кольца на молярах верхней с нижней челюсти с втулками, соединенные между собой стальной пружиной. Способ осуществляют следующим образом: получают оттиски с обеих челюстей и регистраторы прикуса. Изготавливают гипсовые модели и фиксируют их в окклюдатор. По моделям изготавливают ортодонтические кольца для фиксации на первые и вторые моляры верхней челюсти и первые моляры и первые премоляры нижней челюсти. Ортодонтические кольца для фиксации на верхней челюсти спаивают между собой. На вестибулярные поверхности колец припаивают соединительные металлические втулки. Задачей, на решение которой направлена группа изобретений является создание методики консервативной репозиции смещенных мыщелковых отростков при неполном переломе нижней челюсти, позволяющей избежать недостатков известных устройств и способов лечения и возможность ее применения у детей в период сменного прикуса в амбулаторной практике, также, возможность применения при посттравматической дефлексии. Технический результат, обеспечиваемый группой изобретений, заключается в возможности проведения консервативной репозиция смещенных мыщелковых отростков при неполном переломе нижней челюсти у детей в период сменного прикуса за счет физиологической тяги жевательных мышц, разгрузки височно-нижнечелюстного сустава, снятия спазма жевательных мышц и устранения смещения нижний челюсти в сторону перелома, как при открывании рта, так и в состоянии физиологической окклюзии.The task to be solved by the group of inventions is to create a technique for conservative reposition of displaced condylar processes in case of an incomplete fracture of the lower jaw, which allows to avoid the disadvantages of known devices and methods of treatment and the possibility of its use in children during the period of mixed dentition in outpatient practice, as well as the possibility of using it in post-traumatic deflection. The technical result provided by the group of inventions is the possibility of conservative repositioning of the displaced condylar processes in case of an incomplete fracture of the lower jaw in children during the period of mixed bite due to the physiological traction of the masticatory muscles, unloading the temporomandibular joint, relieving spasm of the masticatory muscles and eliminating the displacement of the lower jaw in the side of the fracture, both when opening the mouth and in a state of physiological occlusion.

Ортодонтические кольца 4 и 5 на моляры верхней челюсти соединены проволочным металлическим небным бюгелем 10, выполненным с вестибулярной направляющей 11, соединенной с ортодонтическим крепежным кольцом 5 на моляр верхней челюсти. Пример 1. Пациентка С., 5 лет, поступила в после травмы с жалобами на ограниченное открывание рта. Объективно: в положении физиологической окклюзии наблюдается смещение нижней челюсти вправо, правосторонняя дефлексия, ограниченное открывание рта. На рентгенограмме, выполненной в прямой носолобной проекции, выявлен изгиб мыщелкового отростка кнаружи. Диагноз: неполный перелом мыщелкового отростка нижней челюсти справа. Лечение: получены оттиски с верхней и нижней челюстей. Подбор колец на опорные зубы. Изготовление небных и язычных направляющих, а также, вестибулярных дуг на верхнюю и нижнюю челюсти. К направляющим и вестибулярным дугам, в области крепления к кольцам, приваривают, длиной 3,5 мм, концы которых оформлены пластмассой в форме шариков диаметром 2 мм, металлические крючки, чтобы на стороне смещения они располагались на язычных направляющих нижнечелюстного аппарата, а на противоположной стороне - на вестибулярной направляющей верхнечелюстного аппарата. Осуществляют консервативное лечение с помощью коррекционного аппарата. При этом аппарат зафиксирован стеклоиономерным цементом на опорных зубах посредством ортодонтических крепежных колец после предварительной механической и медикаментозной обработки. Установку эластичных съемных тяг и на крючки провели на следующие сутки. Рекомендовано постоянное ношение аппарата, снимая только во время чистки зубов.
