Изобретение относится к медицине, в частности к амбулаторной хирургии и может быть использовано при лечении вросшего ногтя (онихокриптоза), в том числе и при рецидиве заболевания.The invention relates to medicine, in particular to outpatient surgery and can be used in the treatment of an ingrown nail (onychocryptosis), including the recurrence of the disease.

Данные способы довольно широко применяются в амбулаторной хирургии, но характеризуются некоторыми недостатками:These methods are quite widely used in outpatient surgery, but are characterized by some disadvantages:

Указанные способы взяты за прототип. В основу изобретения положена задача предупреждения рецидива заболевания и сокращения сроков лечения. Указанная задача решается тем, что на вросший ноготь предлагается комбинированное хирургическое и химическое воздействие, а именно:These methods are taken as a prototype. - после экономной краевой резекции ногтевой пластины (5 мм) и удаления матрикса ложкой Фолькмана, проводится двукратно химический биолизис одним миллилитром препаратом «Веррукацид», содержащим 58,8% фенола, 3,92% метакрезола в 95% растворе спирта, обладающем прижигающим действием на ткани организма.- after an economical marginal resection of the nail plate (5 mm) and removal of the matrix with a Volkmann spoon, chemical biolysis is performed twice with one milliliter of Verrukacid, containing 58.8% phenol, 3.92% metacresol in 95% alcohol solution, which has a cauterizing effect on body tissues.
Предлагаемый способ комбинированного лечения вросшего ногтя осуществляют следующим образом. Операционное поле обрабатывают официнальным спиртовым раствором йода. Выполняют проводниковую анестезию по Лукашевичу - Оберсту 4,0 мл 2% раствором Новокаина. На основание пальца накладывают жгут для снижения кровоточивости раны. Экономно иссекают вросший край ногтевой пластины (5 мм) вместе с герминативным матриксом без надреза проксимального околоногтевого валика. При помощи зажима типа «Москит» удаляют иссеченный край ногтевой пластины. Ложкой Фолькмана удаляют матрикс и грануляционную ткань. На ложе удаленного матрикса специальным аппликатором, входящим в комплект фармацевтической упаковки препарата, наносят на 3 минуты 1 мл препарата «Веррукацид», не допуская попадания на здоровые ткани. После первоначальной коагуляции тканей ложе матрикса повторно очищают ложкой Фолькмана и производят контрольную обработку ложа 1 мл в течение трех минут препаратом «Веррукацид».The proposed method of combined treatment of ingrown nails is carried out as follows. The operating field is treated with an official alcoholic solution of iodine. Conduction anesthesia according to Lukashevich - Oberst is performed with 4.0 ml of 2% Novocaine solution. A tourniquet is applied to the base of the finger to reduce bleeding of the wound. The ingrown edge of the nail plate (5 mm) is economically excised along with the germinal matrix without an incision in the proximal periungual ridge. Using a clip of the "Mosquito" type, the excised edge of the nail plate is removed. The Volkmann spoon removes the matrix and granulation tissue. On the bed of the removed matrix, a special applicator included in the pharmaceutical package of the drug is applied for 3 minutes with 1 ml of the Verrukacid preparation, avoiding contact with healthy tissues. After the initial coagulation of the tissues, the matrix bed is re-cleaned with a Volkmann spoon and a control treatment of the bed with 1 ml for three minutes is performed with the Verrukacid preparation.

В практики медицины известно использование препарата «Веррукацид» для удаления доброкачественных опухолевидных образований кожи (папиллом). Однако использование препарата для лечения вросшего ногтя путем его воздействия на ростковую зону ногтевой пластины предлагается впервые.In the practice of medicine, the use of the drug "Verrukacid" is known to remove benign tumor-like skin formations (papillomas). • уменьшить втрое срок нетрудоспособности по сравнению с ранее проведенным лечением у представленных пациентов;• reduce three times the period of incapacity for work compared with previous treatment in the presented patients;
• улучшить качество жизни.• improve the quality of life.

Под местной анестезией по Лукашевичу - Оберсту 4 мл 2% раствора Новокаина с наложением жгута на основание I пальца правой стопы, иссечен экономно (5 мм) вросший край ногтевой пластины. Ложкой Фолькмана удален соответствующий фрагмент герминативного матрикса, ложе которого коагулировано 1 мл препарата «Веррукацид» в течение трех минут. Ложкой Фолькмана удален коагулят из ложа герминативного матрикса с повторным биолизисом в течение трех минут. К работе приступил на второй день после операции. Перевязки и наблюдение в течение семи дней. Осмотрен для контроля через 6 месяцев, рецидива заболевания нет (фиг. 5). Результат оценен как хороший.
