Вросший ноготь (онихокриптоз) относится к одним из наиболее распространенных амбулаторных хирургических заболеваний и представляет собой актуальную проблему во всем мире. По имеющимся литературным источникам, патология встречается у 6-10% населения. Заболевание регистрируется во всех возрастных группах населения, особенно трудоспособного возраста. Заболевание характеризуется патологическим врастанием края ногтя в боковой околоногтевой валик, в котором со временем возникает хроническая рана. Присоединяется воспаление с серозным, и серозно-гнойным отделяемым. Формируются гипергрануляция и болевой синдром в дистальной фаланге пальца.An ingrown toenail (onychocryptosis) is one of the most common outpatient surgical diseases and is an urgent problem all over the world. Отсутствие единых и систематизированных подходов к лечению данной категории больных, приводят к длительному лечению, осложнениям и возникновению рецидивов заболевания, вплоть до формирования инвалидности. Несмотря на кажущуюся простоту проблемы, самодеятельность и нерациональность ряда существующих методик, которые, порой, с самого начала обречены на неудачу, ежегодно приводят к огромным экономическим потерям среди трудоспособной части населения. Это объясняется тем, что при одних способах происходит удаление ногтевого матрикса, при других иссекается часть мягких тканей вместе с участком ногтевой пластинки с наложением первичных швов при сохранении неполноценных грануляций. Шмидена, заключающийся в продольной краевой резекции ногтевой пластины со стороны врастания, отступя от края пораженного околоногтевого валика на 3-5 мм. Разрез продолжают рассечением эпонихия до 5 мм и углубляют до надкостницы. Резецированный край ногтевой пластины выкручивают зажимом и удаляют. Ногтевое ложе представляет собой лежащий под ногтевой пластиной участок эпидермиса, начинающийся вместе с ногтевой пластиной и оканчивающийся у ее дистального края. Продолжением ногтевого ложа является участок кожи, не соединенный с ногтевой пластиной и расположенный под ее свободным краем. Он называется гипонихием. Матрица или корень ногтя является ростковой зоной, откуда начинаются и пластина, и ложе ногтя. Матрикс начинается под проксимальным ногтевым валиком и продолжается до начала ногтевой пластины. Край зоны матрикса, выступающий из-под проксимального валика, выглядит как белая полоска полулунной формы и носит название луночки. Ingrown nails are found mainly on the big toes, usually on one, rarely on both feet, in right-handers more often on the right foot than on the left. Most often, the outer edge grows in, sometimes both lateral edges of the nail plate. It is much less common for an ingrown nail to develop on other toes, even less often on the thumbs and other fingers [11,12].

Оперативное лечение вросшего ногтя по предлагаемому способу осуществляют без анестезии. Далее, на границе патологического участка и участка связанного с гипонихием 1 (фиг. 3), проводят рассечение ногтевой пластины 2 с помощью ножниц с закругленным концом, в продольном направлении, по направлению к проксимальному краю ногтя 6, длиной 1-3 мм (фиг. 2). Патологический участок 8 берут хирургическим зажимом 9 с тупым концом и в направлении к проксимальному краю ногтя, легким движением удаляют. Патологический фрагмент 10 (фиг. 4) ногтевой пластинки обязательно удаляется на границе с неизмененным гипонихием 7 (благодаря демаркационной линии 11, проходящей на границе патологического и неизмененного участков ногтевой пластинки), что позволяет в дальнейшем восстановится ногтевой пластике без рубца и грануляции. Объем удаленного фрагмента не зависит от действий хирурга - «где тонко, там и рвется». Организм сам «отторгает» измененный участок ногтевой пластинки с оставлением здоровой ткани. На удаленном фрагменте ногтевой пластинки всегда определяется патологический «шип». При двустороннем поражении выполняют аналогичные манипуляции. Время проведения операции с подготовительным этапом составляет 30 минут. После проведения операции, на 1-2 суток накладывают одноразовую марлевую салфетку с раствором антисептика (гипертонический раствор натрия хлорида или хлоргексидинабиглюконат 0,05%-ный). Пример №1. Больная Н., 45 лет, обратилась к хирургу в медицинский центр «Арбат» 30.09.2020 г. с жалобами на боли в области первого пальца левой стопы, гиперемию кожи в области медиального бокового валика ногтя. Считает себя больной в течение последних 6 месяцев. Консервативные мероприятия ожидаемого эффекта не принесли. В условиях перевязочного кабинета поликлиники выполнена операция по вышеуказанной методике. Через 1 час пациентка отпущена домой. Осмотр через 3 месяца - рецидива заболевания нет, достигнут хороший клинический и косметический эффект.
Пример №2. Больной Ч., 21 год, обратился к хирургу в поликлинику МКДЦ 25.07.2020 с жалобами на болезненность в области первого пальца правой стопы при ходьбе, отек бокового валика ногтя. В перевязочном кабинете выполнена операция по описанной методике. В течение часа пациент отправился домой. Осмотр через 2 месяца -рецидива заболевания нет, достигнут хороший клинический и косметический эффект.Example No. 2.
