Изобретение относится к медицине, конкретно - к хирургии кисти, и применяется при оперативном лечении сгибательных контрактур на уровне проксимального межфалангового сустава. Существует множество причин формирования сгибательных контрактур на уровне проксимального межфалангового сустава (ПМФС), однако, обычно, причиной являются патологии ладонной пластины и ее боковых отростков, в норме предотвращающие гиперэкстензию сустава. Общеизвестно, что проксимальные межфаланговые суставы склонны к формированию сгибателных контрактур. Наиболее часто сгибательные контрактуры ПМФС при повреждениях ладонной пластины возникают после иммобилизации в согнутом положении, вправления вывихов, или при наличии переломов в области ПМФС.There are many reasons for the formation of flexion contractures at the level of the proximal interphalangeal joint (PMJJ), however, usually the cause is the pathology of the palmar plate and its lateral processes, which normally prevent joint hyperextension. Известен способ лечения посттравматических контрактур пальцев кисти на уровне проксимального межфалангового сустава [патент РФ №2138218 А61В 17/56, 1999 г.], включающий несколько этапов. Производят наложение аппарата внешней фиксации, и на 2-3 день, после стихания болей, начинают дистракцию в медленном режиме по 1 мм в сутки до устранения сгибательной контрактуры, и далее на величину 2πR⋅d/360°. Пациент, с целью профилактики рубцевания сухожилия глубокого сгибателя с ладонной пластинкой в процессе дистракции, выполняет активно-пассивные движения с нагрузкой в ПМФС, направленные на сгибание ногтевой фаланги. В растянутом состоянии палец фиксируют в аппарате в течение 12-21 дня, аппарат снимают, и осуществляют интенсивную разработку движений в пораженном суставе. Больной продолжает заниматься активно-пассивной гимнастикой, направленной на максимальное сгибание ногтевой фаланги. Данный способ широко применяется при лечении сгибательных контрактур, но результаты лечения остаются неудовлетворительными в 42% случаев. Недостатком данного способ является повторное (рецидивирующее) рубцевание глубокого сгибателя пальца кисти на уровне проксимального межфалангового сустава из-за образования дефекта на месте иссеченной ладонной пластины, требующего повторных операций в 31% случаев. Выполняют П-образный доступ по ладонной поверхности основной и средней фаланг, выделяют, и поднимают, кожно-жировой лоскут. Выделяют сосудисто-нервный пучок, сдвигают его. Выделяют ладонную пластинку, дифференцируют ее связки, находящиеся на 3 мм проксимальнее ПМФС. Пациент X., 60 лет, обратился с жалобами на ограничение разгибания 4 пальца левой кисти. Травму получил за 4 месяца до обращения в результате падения на руку тяжелого предмета. Лечился консервативно в травмпункте по месту жительства, где ему была выполнена рентгенография, гипсовая иммобилизация на 10 дней. После прекращения иммобилизации отметил уменьшение отека, сохранение незначительных болей, при этом постепенно развилась сгибательная контрактура. Получал физиотерапевтическое лечение, безуспешно.Patient X., aged 60, complained of limited extension of the 4th finger of the left hand. Движения в пястно-фаланговом и дистальном межфаланговом суставе в полном объеме. Выделили и сдвинули сосудисто-нервный пучок. Выделили ладонную пластинку, дифференцируя ее связки, находящиеся на 3 мм проксимальнее ПМФС, ориентируясь на поперечную пальцевую артерию. Рассекли ладонную пластинку на уровне ее связок, произвели редрессацию ПМФС с устранением контрактуры. Идентифицировали ветви, отходящие от пальцевой артерии к жировой клетчатке ладонной поверхности поврежденного пальца, выделили васкуляризированный жировой лоскут, провели его под сухожилием глубокого сгибателя в дефекте ладонной пластинки, и зафиксировали в реципиентной зоне П-образными швами. Рану ушили, проксимальный межфаланговый сустав на 2 недели зафиксировали шиной.Patient X. underwent surgery as described above. После операции пациент получал перевязки. Начаты дозированные движения в шине. На сроке 2 недели иммобилизация прекращена, далее получал физиотерапевтическое лечение, занимался лечебной физкультурой.
