Известно, что при хирургическом лечении острой ишемии нижних конечностей после преодоления препятствия кровообращения возникает феномен ишемии-реперфузии, обусловленный прорывом в центральный кровоток продуктов деструкции тканей, образовавшихся в процессе ишемии. Продукты распада тканей являются антигеном, усугубляющим воспаление и оксидативный стресс, сформировавшиеся в периоде острой ишемии нижних конечностей. Известен Способ профилактики реперфузионных осложнений у пациентов, оперированных по поводу хронической ишемии нижних конечностей (Заявка на изобр. №2007111078/14, 26.03.2007). Для достижения лечебного эффекта за 12 часов до операции подкожно вводят низкомолекулярный гепарин Эноксапарин натрия (Clexane) в дозе 1 мг/кг массы тела. В течение первого часа после операции инъекцию повторяют. Затем Эноксапарин натрия в той же дозе вводят каждые 12 часов в течение 5 дней после операции. Сначала проводится традиционная антикоагулянтная, антиагрегантная, антибактериальная терапия, после ее завершения проводится последовательно противовоспалительная и антиоксидантная терапия. В качестве вазоактивного препарата используют дексаметазон, который вводят внутривенно капельно в дозе 8 мг в 200 мл изотонического раствора хлорида натрия, а затем в качестве антиоксидантного препарата вводят реамберин в дозе 400 мл (раствор 1,5%, внутривенно капельно), оба препарата применяют курсом от 4 до 6 дней после реконструктивной операции.The method is carried out as follows. Заявляемый способ позволяет не только улучшить результаты послеоперационного лечения больных с острой ишемией нижних конечностей, но и снизить летальность с 31% при традиционной терапии до 4%, снизить частоту осложнений как инфекционного, так и воспалительного генеза, таких как гангрена с 30% при использовании традиционной терапии до 4%; таких как сепсис с 10% у больных с базисной терапией до 2%; нагноение ран снижается с 10% до 2%, соответственно. Лечение дексаметазоном и реамберином позволяет снизить количество ампутаций с 32% у больных с базисной терапией до 8%, сократить продолжительность лечения при выписке в среднем с 13 до 9 койко-дней.
